1 Case Report Neonatal Med 2018 February;25(1):44-48 pissn eissn A Case of Platyspondylic Lethal Skeletal Dysplasia Torrance Type Department of Pediatrics, Chosun University College of Medicine, Gwangju, Korea ABSTRACT Platyspondylic lethal skeletal dysplasia, Torrance type (PLSD-T), is one of the phenotypes of type II collagenopathy and is characteristic of severe bone growth disorder. This phenotype may limit the growth and expansion of the lungs, which is known to cause death from respiratory failure during or shortly after birth, but in few less severe cases, patients have been reported to have survived to adulthood. We have experienced a case of PLSD-T in a preterm infant who was delivered via cesarean section at the gestational age of 29 weeks 3 days, with a birth weight of 1.15 kg. Physical examination of the infant revealed characteristic findings of short arms and legs, small thorax, distended abdomen, and cleft palate. On the basis of the subsequent genetic testing, the patient had a heterozygous mutation in the encoded c-propeptide region of collagen, type II, alpha 1 (COL2A1), c.4335g>a (p.trp1445*) in exon 52. This is the first case of PLSD-T diagnosed in Korea, and we hereby report the case. Collagen, type II, alpha 1 (COL2A1), 유전자는연골및눈의유리체에서발견되는섬유소교원질인제2형 Homotrimeric 콜라겐의 alpha 1 사슬을암호화한다. 많은임상표현형들은대부분 COL2A1 유전자의이형접합돌연변이에서발생하게되며, COL2A1 유전자의돌연변이로인한질환을통틀어제 2형교원질병이라부른다. 임상표현형은 Stickler dysplasia type I (STD-1), Kinest dysplasia (KND), achondrogenesis type II (ACG II), hypochondrogenesis(hcg), several spondyloepiphyseal dysplasia (SED), SED congenital (SEDC), SED late onset (SEDT), PLSD-T, spondyloperipheral dysplasia (SPPD), vitreorntinopathy and phalangeal epiphyseal dysplasia (VPED) 등이있으며 STD-1과 SED가그중에잘알려진편이다 6). PLSD-T는 1979년 Horton 등에의해처음기술된 6) 이래 2012년까지전세계적으로 5건의증례에서 18명의환자가보고된매우드문질환이다. 그리고대부분의임상표현형들은 COL2A1 유전자의삼중나선도메인의과오돌연변이 (missense mutation) 혹은 in-frame derangement와관련되어있으나 3,7), 흔하지않은임상표현형들 (PLSD-T, SPPD, VPED) 의경우는 C-propeptide 도메인의변이와연관되어있으며, 과오돌연변이 (missense mutation) 를통해일반적으로상염색체우성유전을나타낸다 1,8-10). PLSD-T는 1979년 Horton 등에의해처음기술되었으며드문골격이형성증으로편평척추, 매우짧은사지, 심각한폐의저형성, 그리고단지증을특징으로한다. PLSD-T는특징적으로영상학적검사에서다양한형태의편평척추, 짧은갈비뼈, 짧은늑골, 하부장골의저형성, 방사형골간단을보인다 6,11,12). 일반적으로주산기에치명적이지만출생시동반된기형의정도에따라서장기생존자가보고된바있으며, 환자의 2세에서같은변이가발견된사례역시함께보고되었다 2,4,5). 저자들은아직국내에보고되지않은 COL2A1 gene mutation 이확인되어 PLSD-T 로진단된재태연령 29주 3일미숙아를경험하였기에보고하는바이다. 사에서미숙아망막병증 (retinopathy of prematurity, ROP) 은보이지않고정상이었다. 몸통에비해짧은팔다리와작은흉곽, 튀어나온배, 구개열등의선천기형평가를위해일차적으로가장흔한원인인연골무형성증여부확인위해 FGFR3 유전자검사를시행하였으나변이는발견되지않았으며, 골격이형성증 (Skeletal dysplasia) 등의제 2형교원질병이의심되어 Diagnostic Exome sequencing (DES) 을검사하였다. DES에서 COL2A1 유전자의 1445번째아미노산인 Tryptophan이 stop codon으로치환되는 nonsense variation을보였으며 (Figure 2), 이는 PLSD-T 환자에서 pathogenic variant로보고된바가있었다 2). 부모에서는유전자변이를보이지않아산발적 A 증례 재태연령 29주 3일, 출생체중 1.15 kg으로제왕절개로태어난둘째아이로여아였으며, 첫째아이는만 3세여아로건강하였고, 가족력에서다른근골격계이상질환은없었다. 임신 25주에시행한태아초음파에서 Femur의길이는각각 2.5 cm였으며, 이는 18주태아의평균길이와비슷한수준이었다 13). 출생시아프가점수는 1분에 3점, 5분에 5점이었으며, 호흡곤란이지속되어기관내삽관후계면활성제를투여하였다. 말초혈액검사에서혈색소 15.7 g/dl, 백혈구 9,120/mm 3, 혈소판 292,000/mm 3 이었으며, 정맥혈가스분석에서 ph 7.342, pco mmhg, po mmhg, HCO mmol/ L, SPO2 80.5% 였다. 신체검사에서몸통에비해짧은팔다리와작은흉곽, 튀어나온배, 구개열이관찰되었고 (Figures 1A,1B), 체중 1.15 kg (10-25 p), 신장 32.5 cm (<10 p), 두위 28 cm (50-75 p) 였다. 환자는신생아호흡곤란증후군으로기계환기치료를지속하였으며, 교정연령 36주에도 FiO2 35% 이상요구하며, 기계환기가필요한상태로중증기관지폐이형성증으로진단하였다. 출생 3일째시행한뇌초음파에서뇌실확장및뇌실내출혈소견은확인되지않았고, 9일째시행한심장초음파에서이상소견은보이지않았다. 19일째양쪽의 1단계뇌실내출혈이관찰되었으며, 75일째양쪽뇌실의확장을보였다. 출생 66일째인재태연령 38주 6일에시행한안과검 B Figure 1. B 인경우 (sporadic case) 로판단하였다. 상대적으로짧은팔다리, 편평한척추몸통, 갈비뼈의성장미숙과이로인한작은흉곽, 비전형적인골간단, 노뼈의휘어짐등의임상양상 (Figures 3A, 3B) 및제 2 형교원질의 carboxyl propeptide의변이를통해 PLSD-T로진단하였다. 기계환기치료중출생 38일째점차기계환기치료에서이탈하여비침습적양압환기 (Non invasive positive pressure ventilation, NIPPV) 로변경하였으나함몰호흡및고탄산혈증이반복되어기계환기와비침습적양압환기로교대치료를반복하였고호흡곤란이지속되어출생 55일째부터다시 FiO2 35% 로기계환기치료를시작하였다. 기계환기치료중에산소요구량이점차증가하고출생 85일째부터 FiO2 100% 로산소치료를함에도산소포화도잘유지되지않는모습반복되었다. 출생 136일째부터심박수및산소포화도가함께떨어지는양상으로저산소증이반복되어심폐소생술을시행하였고, 3분이내의심폐소생술후회복되는양상이었으나, 출생 151 일째 30분동안강직간대발작이지속되었으며, amplitude integrated electroencephalography (aeeg) 에서도경련양상이지속되어항경련제 (Phenobarbital) 를투여하였다. 이후경련조절은잘되었으나, 출생 170일째심박동수 60회이하및산소포화도 60% 이하지속되 어약 6분간심폐소생술을시행후회복되었다. 171일째움직임감소되고, 자발호흡이없고통증에반응보이지않았으며, aeeg에서전반적으로최저와최대진폭모두 5 μv 이하로저하되어있었고 raw EEG에서도폭이일정하며, 비활성화된양상확인되고동공반사보이지않아반복되는저산소증으로인한뇌손상으로판단하였으며, 보호자에게환자의상태설명후연명치료중지에대한동의를얻었다. 환자는출생후 174일, 교정연령 3개월 10일째사망하였다. 고찰 PLSD-T는제 2형교원질병의하나로 12번유전자에존재하는 COL2A1 유전자의이질성돌연변이에의한다 7,9). 제 2형교원질병의임상증상은제 2형교원질의기능과연관되어있는데, 제 2형교원질은연골의주구성성분으로연골의기본구조를형성하는것이주기능이며, 연골내골형성에있어서도중요한역할을한다. 장골과추체의연골형성장애로저신장을초래하고, 편평척추, 골반의골화지연, 골간단골격지연등골간단의이상을보

4 Neonatal Med 2018 February;25(1): 인다. 특징적인얼굴모습으로는함몰된비교로인해편평한안면과짧은목을보이고제 2형교원질이발생학적으로주된역할을하는구개, 초자체액, 시신경망막세포및내이세포에장애를초래하여구개열이나망막병증, 망막의박리, 유리막병증과난청의조기발병을초래한다 7,14). 그리고선천적소하악증, 설근의침하와구개열을특징으로하는 Pierre Robin sequence 가동반되었을경우에도제 2 형교원질병을의심해볼수있다. 대표적인제 2형교원질병의임상표현형으로 ACG II, HCG, SEDC, SPPD, Kniest dysplasia, Skickler syndrome type I, PLSD-T, VPED가있으며 3) 간단히살펴보면, ACG II는지연된연골내골화로인해짧은팔과다리, 짧은늑골과작은흉부미성숙한폐와돌출된이마, 작은턱이특징적이며구개열을동반하며상대적으로손과발모양은정상적이다. HCG는 ACG와비슷하나보다경하며, 목과골반의일부인척추골이부분적으로골화가되지않으며, 얼굴은편평하고타원형이며넒은미간과작은턱이특징적이고구개열을동반하기도한다. SEDC는장골원위부의골화지연, 치골및근위골의골화결절을특징으로하며시력과청력에도영향을미친다. SPPD는골화의장애로편평한척추뼈, 비정상적으로짧은손발가락이특징적이며, 근시및청력소실, 정신지체를동반하기도한다. PLSD-T는드물고치명적인골격이형성증으로편평척추, 매우짧은사지, 심각한폐의저형성이특징적이다. COL2A1 유전자의돌연변이형태와임상표현형사이의상호관계는명확하지않으나유전자의돌연변이형태에따라분류를하면, ACG/HCG/SEDC는 COL2A1 유전자의 3중나선형영역에서의과오돌연변이 (missense mutation) 혹은 in-frame derangement로발생하며 3), Kniest dysplasia는 Exon skipping 에의하며, Stickler dysplasia 는단상부족 (Haploinsufficiency) 에의해주로발생하는것으로알려져있다. 반면에 PLSD-T/SPPD는 COL2A1 유전자의 C-propeptide 영역에서의변이에의해발생한다 8,10). 대부분의 Type II collagenopathy는 C-peptide영역에서의변이는드물며주로 3중나선구조의이상에서발현되므로다른아형들과 SPPD, PLSD-T를감별하는가장중요한감별점이될수있다 8-10). PLSD-T는대부분이상염색체우성유전을따른다고알려져있으나정상표현형의부모에서발생하는산발적인경우도드물게보고되어있다. 산발적인발생의경우는대부분이새로생긴돌연변이 (de novo mutation) 에의한경우이나형제에서같은질환이동반되는경우에는생식세포모자이크현상 (germline mosaicism) 도있을수있다 11). 본증례의환자의경우는부모의유전자검사에서이상을보이지않았고, 형제에서도질환이동반되지않은산발적인증례 (sporadic case) 로생각된다. SPPD와 PLSD-T 는둘다중심부저형성증, 요추및대퇴부의형성부전, 짧은다리, 편평하고얇은척추가특징적이고 PLSD-T가보다드물며치명적이나, 상대적으로손발가락의변형은드물다고알려 져있다. 또한 PLSD-T 는주산기에치명적이지만출생시동반된기 형의정도에따라서장기생존자가보고된바있으며, 이를 SPPD 의 실례로보고하기도하였다. 따라서 PLSD-T 와 SPPD 는제 2 형교원질 병중에서도연속선상의스펙트럼을구성하는질환이라고볼수있 다 2). 제 2 형교원질병은출생전시기에자궁내에서저명한편평한척 추, 짧은팔다리와좁은흉곽이확인되는데이는제 1 형연골무형성 증 (achondroplasia type 1), 골형성부전증 (Osteogenesis imperfecta), 저인산증과감별이필요하다. 또한 Thanatophoric dysplasia (TD) 는편평척추뼈를동반하는치명적인골격이형성증으로과거에 는 PLSD-T 와임상증상이유사하여 PLSD-T 가처음보고되던시기 에 TD 의아형으로도생각되었다. 그러나유전자분석에서 TD 는 FGFR-3 유전자의과오돌연변이에의한것으로둘은전혀다른독립 된질환으로분류되었으며, TD 에서는제 2 형교원질병에서는보이 지않는두개골의이상이발견되기도한다. 연골무형성증은 FGFR-3 유전자의점돌연변이에의한연골내골화의장애가원인이며, 근위 부의골이주로짧아져있는것 (rhizomelic short limp) 이특징이다. 골형성부전증은제 1 형교원질을규정하는유전자의변이가원인이 고, 넓은이마와삼각형모양의두부가특징적이다. 또한공막, 치아 의상아질, 내이, 피부등에서도교원질의이상증상이동반되며, 주 증상은주로대퇴골과경골의반복되는골절이다 15-17). 국내에서는아직 PLSD-T 에대한보고는없었으며, 국외연구에 서도상당수가사후에영상검사및유전자검사가진행된것에비하 여, 본증례는재태연령 29 주 3 일로출생한미숙아에서특징적인골 격과폐의저형성, 구개열의 type II collagenopathy 의임상증상, 영 상학적검사및유전자검사를통해 PLSD-T 를진단하였다.
