ILK modulates the transcription of β-catenin and is implicated in cell migration and invasiveness. Recently, premalignant colon polyps were found to express ILK and β-catenin. Therefore, we investigated the expression of ILK and β-catenin in colon polyps according to the gross morphology and pathologic type. Methods: Based on morphology, colon polyps (62) were classified as being a pedunculated polyp (Ip, 16), sessile polyp (Is, 22), or laterally spreading tumor (LST, 24). The colon polyps were classified pathologically as tubular adenomas (TAs, 47) and hyperplastic polyps (HPs, 15). The expression levels of ILK and β-catenin in colon polyps and normal colon (6) were evaluated with immunohistochemistry. Results: In normal colon, ILK was not expressed, and β-catenin stained in the cell membrane only. Based on the gross morphology of the colon polyps, no significant difference was seen in the expression of ILK and β-catenin (p>0.05). The expression of both ILK and β-catenin in TAs was greater than that in HPs (p<0.01): the greater the dysplasia in TAs, the more both ILK and β-catenin were expressed (p<0.05). The grade of expression of ILK was correlated with that of β-catenin in colon polyps (p<0.01). Conclusions: The expression of ILK and β-catenin did not differ according to the morphology of colon polyps, but was expressed more in TAs than in HPs, especially in severe dysplasia. 2 - The Korean Journal of Medicine: Vol. 77, No. 3, 서 론 대상및방법 대장암을비롯한여러악성질환에서 integrin-linked kinase (ILK) 는순수한종양유전자 (oncogene) 로여겨지며 Ser/Thr kinase의일종으로서세포부착수용체 (cell adhesion receptor) 와연결되어있다 1). ILK는 integrin, 세포성장인자 (growth factors) 들과연관이있는세포내연결기활성화효소 (adaptor kinase) 로서악성질환의생성과진행에관여하는여러세포전달단백질 (AKT, GSK3, NF-kB, PDK1 등 ) 의활성과직접적인관계가있다 1-3). 따라서 ILK의발현을억제할경우에항종양효과가있어악성질환의치료목적으로이용되고있다 2,4,5). ILK의발현은 beta-catenin의세포사이부착경계 (adherence junction) 에서의감소와세포핵내에서의증가와도연관이있는데이는 ILK가 E-cadherin의전사를억제하여핵에서 beta-catenin의증가를일으키기때문이다 1,5,6). 대장암에서 ILK의발현은발생초기에일어나며암의병기나림프절전이와관계가있다. 최근에발표된연구들에의하면가족성샘종용종증 (FAP) 환자의용종과산발성대장용종 (sporadic colon polyp) 에서도발현이증가되어있다고한다 7-9). 그러나대장용종의육안적모양이나병리학적차이에따른 ILK, beta-catenin의발현에대한연구는거의없다. 이에저자들은대장용종에서육안적모양과조직학적차이에따른 ILK와 beta-catnin의발현양상의차이를살펴보고자하였다. 1. 대상 2000년 3월부터 2005년 3월까지의정부성모병원에서가족성용종증의가족력이없는연속적인환자들을대상으로하였고, 이들중에서무작위로선택하였고, 이들의평균연령은 58.8±9.6세였으며남자 42명, 여자 25명으로전체환자는 67명이었으며, 대장용종절제술을시행하여얻어진 69개의용종의평균크기는 1.28±0.64 cm였다. 대장용종의모양에따른구분은 2003년 Paris Workshop 의분류 10) 에따라시행하였으며용종양병변 (polypoid lesion) 중에서그높이가병변주변점막두께의두배이상이며용종의기저부가좁아진경우를유경성용종 (Ip 16), 용종의기저부와상부가같은경우를무경성용종 (Is 22) 으로하였다. 비용종양병변 (non- polypoid lesion) 중에서정상점막에비해약간융기되어있으며그직경이 10 mm를넘는경우를측방발육형종양 (laterally spreading tumor, LST 24) 로하였다 ( 표 1) 10). 병리학적분류는대장암 (2), 관샘종 (tubular adenoma, TA 47), 융모샘종 (villous adenoma, VA 1), 관융모샘종 (tubulovillous adenoma, TVA 4), 과형성용종 (hyperplatic polyp, HP 15) 으로구분하였다. TA는이형성 (dysplasia) 의정도에따라약한이형성 (mild dysplasia 15), 중등도 (moderate dysplasia 23), 심한이형성 (severe dysplasia 9) 으로구분하였다 ( 표 1). Table 1. Integrin-linked kinase (ILK) and β-catenin expression in colon polyps - A C E B D F 1) ILK, beta-catenin 염색방법면역조직화학염색은제작한파라핀슬라이드를자일렌으로파라핀을제거하고알코올 (100~50%) 에서항원을제거하기위해 10 mm citrate 완충액 (ph 6.0) 에넣어서마이크로파로 15분동안가열하였다. 실온에서어느정도식으면 TBST (TBS - 0.1% tween 20) 에서세척후내인성과산화효소 (peroxidase) 활성을저지시키기위해 3% H 2O 2 에서 10분간처리하였다. TBST 완충액으로세척후결합항체의비특이적반응을억제하기위하여정상억제혈청 (Zymed, CA, USA) 으로 10분간반응시킨후일차항체를처리하였다. beta-catenin 에대한단일클론항체 (LabVision, CA, USA) 는 1:600 농도로실온에서 30분간반응하고 ILK에대한단일클론항체 (Santa Cruz, CA, USA) 는 1:100 농도로 4 하루밤동안반응후이차항체 biotinylated antibody (Zymed, CA, USA) 와 10분반응후다시세척하여 streptavidin (Zymed, CA, USA) 으로 10 분반응후에다시세척하였다. DAB chromogen (Zymed, CA, USA) 으로실온에서 2~3분간반응시켜발색하여광학현미경으로관찰하였다. 2) 조직판정두명의병리의사가 Bravou 등 6) 의방법에따라 ILK에대한면역염색은세포질에염색된정도에따라서음성 ( 등급 0), 약한세포질염색 ( 등급 1), 중등도염색 ( 등급 2), 강한염색 ( 등급 3) 으로판정을하였고, beta-catenin은핵혹은세포질에대한염색정도를음성 ( 등급 0), 약한세포질염색 ( 등급 1), 중등도염색 ( 등급 2), 강한염색 ( 등급 3) 으로구분하였다 ( 그림 1). 3. 통계분석 G Figure 1. The staining patterns for β-catenin included weak (F), moderate (G), and strong (H) expression in the cytoplasm or nucleus of the TA. 2. 방법 H 대장용종에서 ILK, beta-catenin의발현정도에대한통계는 Chi-square test로하였고, p<0.05인경우에통계적유의성이있는것으로하였다. 모든통계분석은 SPSS (ver. 10.0, Chicago, IL, USA) 를이용하였다. 결과 1. 대장용종에서 ILK 의발현 1) 용종의모양에따른발현 Is에서 ILK의발현등급 0, 1, 2, 3은각각 10, 10, 2, 0개였으며 Ip 용종에서는 2, 9, 5, 0개였고, LST에서는 10, 9, 5, 0 개로서모양에따른통계학적차이는없었다 (p>0.05, 표 1). 2) 병리학적소견에따른발현 TA에서 ILK 발현등급 0, 1, 2, 3은각각 11, 24, 12, 0개였으며, HP에서는 15개중발현하지않는경우가 13개, 등급 1 이 2개, 등급 2, 3의경우는없어서 HP보다 TA에서통계학적으로유의하게발현율이높았다 (p<0.01, 표 1). 또한 TA 이

Stronger expression is seen in severe dysplasia than in mild dysplasia. 형성의정도에따른발현은약한이형성 (mild dysplasia) 의경우발현등급 0, 1, 2, 3이각각 7, 7, 1, 0개였고, 중등도이형성 (moderate dysplasia) 의경우 4, 13, 6, 0개였고, 심한이형성 (severe dysplasia) 의경우 0, 4, 5, 0개로서이형성이심할수록정도에따라 ILK의발현이유의하게높았다 (p<0.05, 그림 2A). 이외에통계분석에서제외한상피내암 (2) 은모두등급 3을나타내었고, TVA 4개중에 3개는등급 1, 1개는등급 2 를보였으며 1개의 VA는등급 1을나타내었다 ( 표 1). 2. 대장용종에서 beta-catenin 의발현 1) 용종의모양에따른 beta-catenin 의발현 Is에서 beta-catenin의발현등급 0, 1, 2, 3은각각 8, 10, 4, 0개였고, Ip에서는 3, 9, 5, 0개였으며 LST에서는 9, 12, 3, 0 개로서모양에따른통계학적차이는없었다 (p>0.05, 표 1). 2) 병리학적소견에따른 beta-catenin 의발현 TA에서발현등급 0, 1, 2, 3은각각 7, 28, 12, 0개였으며, HP에서발현하지않는경우 15개중 13개, 등급 1인경우가 2개였고, 등급 2, 3은없어서 HP보다 TA에서발현율이높았다 (p<0.01, 표 1). 또한 TA에서이형성에따른발현을보면약한이형성 (mild dysplasia) 의경우발현등급 0, 1, 2, 3이각각 5, 10, 0, 0개였고, 중등도이형성 (moderate dysplasia) 은 2, 13, 8, 0개였으며심한이형성 (severe dysplasia) 의경우 0, 5, 4, 0개로서이형성이심할수록정도에따라 beta-catenin의발현이유의하게높았다 (p<0.05, 그림 2B). 이외에통계분석에서제외한상피내암 (2) 은모두등급 3을나타내었고, TVA 4 개중에 3개는등급 1, 1개는등급 2를보였으며 1개의 VA는 Figure 3. Comparison of the expression levels of ILK and ß-catenin showing the good correlation between them. 등급 1 을나타내었다. 3. 대장용종에서 ILK 와 beta-catenin 발현과의관계 또한전체 62개의용종에서 ILK의발현정도에따른 beta-catenin 발현의정도를보면 ILK의발현이높은경우에 beta-catenin의발현이높아서두표지자간의발현이밀접한관계가있었다 (p<0.01, 그림 3). 고 대장암발생은단계적으로발생한다고알려져있으며정상점막에서여러단계의유전자이상에의해샘종을거쳐암으로진행한다고알려져있으나작은편평형병변이나함몰형병변에서생기는새로운경로 (de-novo pathway) 도알려져있다 11,12). 대장암의발생에관여하는여러유전자중에 APC (adenomatous polyposis coli) 유전자가가장많이알려진 찰

5 - 채현석외 10 인. 대장용종에서 ILK 와 beta-catenin 의발현 - 문지기유전자 (gatekeeper gene) 로서 APC 유전자의돌연변이가생길경우 beta-catenin이세포내에축적되고핵내의 Tcf 의전사인자 (transcription factor) 가활성화되어암의발생과진행에관계하는단백질들을합성한다 13). beta-catenin은대장암의발생과정의초기에관여하여암세포의세포질이나핵내에축적되어암의발생과진행에관여하는 MMP (matrix metalloproteinase) 등의물질을생성하며암의진행과정에관여한다 13). 한편세포표면에서 integrin은여러고정의존 (anchorage dependent) 물질인섬유결합소 (fibronectin), 아교질 (collagen) 이나성장인자등과결합하여이합체화 (dimerization) 되고이로인해서 ILK가활성화되어하류신호전달 (down stream signal) 에관여한다 3). 악성질환에서 ILK의역할은 integrin 과액틴세포뼈대 (actin cytoskeleton) 를연결하고액틴의중합반응 (polymerization) 에관여하며 alpha-parvin, beta-parvin 을비롯한여러세포내단백질과직접관계하여국소유착 (focal adhesion), 세포확산 (cell spreading), 침범 (invasion), 이동 (migration), 신생혈관형성 (angiogenesis) 에관여한다 1,3,14,15). 또한 ILK는 beta-catenin의발현과밀접한관계가있어서 beta-catenin의활성체로작용하며 beta-catenin의세포질이나핵내에서의발현과밀접한관계가있다 6,16,17). 반대로, ILK를과발현하는암세포의부착경계 (adherent junction) 에서 beta-catenin을감소시킴으로서상피세포의 EMT (epithelial-mesenchymal transitions) 와관련이있고, 이와함께 E-cadherin 전사 (transcription) 의억제로인한 E-cadherin 소실과함께세포핵내의 beta-catenin의축적이일어난다 6). 대장암을비롯한다른장기의암에서도암의진행이심할수록 ILK의발현이많다고알려져있고 18-20), 대장암의전구병변인대장용종에서도발현이증가한다고하여 ILK의발현은대장암의초기부터관여한다고생각된다 5-10). 그러나대장암의전구병변인대장용종에서이들의모양이나이형성의정도에따른발현을연구한경우는거의없다. 따라서저자들은대장암의전구병변에서 ILK 및 beta-catenin의발현을보고자하였다. 본연구에서저자들은 ILK의발현시에세포질이나핵내에서의 beta-catenin 발현여부를동시에알아본결과두물질의발현이상당히일치함을발견할수있었고, 정상대장조직에서 ILK의발현이없고, beta-catenin은세포막에서만발현이되어대장암발생초기에이두단백질이관여함을알수있었다. 이들의발현은위에서기술한두물질간의신호전달체계와서로밀접한관계가있음을본연구에서도알수있었다. 본연구에서는 69개의용종을대상으로하였으며이중 두개는상피내암이었고, 4개는 TVA였으며 1개는 VA였고, 이들의종양에서 ILK, beta-catenin의발현이있었으나그수가많지않아통계적분석에서는제외하였고, 나머지 HP과 TA를대상으로분석하였다. 저자들은 ILK가종양의성장방향과관련이있을가능성이있다고생각하여용종의모양과성장방향이다른 Is, Ip, LST 용종간의 ILK, beta-catenin의발현의차이를관찰하였으나용종의모양과는관계가없었다. 따라서대장용종의성장방향과관련된이두물질이외의다른물질이관여하는지는앞으로더연구되어야하리라고본다. 병리학적차이에따른 ILK와 beta-catenin의발현을보면 HP보다 TA에서발현이증가하였는데이는 HP보다 TA가대장암으로진행할가능성이높고반대로 HP는그가능성이적다는종래의의견과일치하는소견이다 12). 또한 TA에서도이형성의정도가심할수록 ILK와 beta-catenin의발현이많았는데이들의소견역시대장암의발생초기와진행과정에이들이중요한역할과한다는의견과일치하는소견이다 7,9,12,21). 따라서대장암의발생초기중에정확히어느시점에이들이관여하는지는앞으로더연구되어야한다. 그러나본연구에서 HP의경우두개의용종에서 ILK와 beta-catenin에대해약양성을나타낸것은과거의보고와다른소견으로서조직염색의위양성에의한것으로생각된다. 결론적으로대장용종에서 ILK, beta-catenin과의관계를보면이들의발현은용종의모양과는관계가없었고, 이형성이심할수록발현이증가되었다. 또한이들의발현은밀접한상관관계가있었고, 앞으로이에대한연구가더필요하리라고생각된다. 요약목적 : ILK와 beta-catenin을통한세포전달은여러악성질환의시작과진행에있어서중요하다. ILK는 beta-catenin의전사를조절하며암세포에서세포이동및침습에관여한다. 최근에대장암과전구단계인샘종에서 ILK와 beta-catenin이발현한다는연구가있었다. 이에저자들은대장용종에서육안적및병리학적소견의차이에따른 ILK와 beta-catenin의발현을알아보았다. 방법 : 비악성질환으로수술한 6명의정상대장조직 (6) 과대장용종절제술로획득한용종을모양에따라유경성용종 (Ip, 16), 무경성용종 (Is, 22) 및측방발육형종양 (laterally spreading tumor LST, 24) 으로나누었다. 또한현미경적소견

6 - The Korean Journal of Medicine: Vol. 77, No. 3, 에따라관샘종 (TA, 47) 과과형성용종 (HP, 15) 으로나누고관샘종은이형성의정도에따라나누었다. 이들조직에서 ILK와 beta-catenin의발현을면역화학염색법으로비교하였다. 결과 : 용종의육안적모양에따라 Ip, Is, LST에서 ILK, beta-catenin의발현의차이는통계적으로차이가없었다 (p> 0.05). 조직학적으로 ILK, beta-catenin의발현은과형성용종보다관상샘종에서발현이증가하였다 (p<0.05). 관상샘종에서는이형성의정도가심할수록 ILK와 beta-catenin의발현이높았다 (p<0.05). 전체용종에서 ILK의발현이높은용종의경우에 beta-catenin의발현이높은경우가많았다 (p< 0.01). 결론 : 대장용종에서 ILK와 beta-catenin의발현은대장용종의모양에따른차이가없었고, 샘종이과형성용종보다발현이증가되었고샘종의이형성이심할수록발현율이높았다. 중심단어 : 대장용종 ; Integrin-linked kinase; 단백질 ; beta-catenin REFERENCES 1) Hannigan G, Troussard AA, Dedhar S.
