거대뇌하수체선종의 외과적 치료 직경이 4.0 9.cm으로 평균 5.5cm 이었다. MRI에서 종양 의 진전 방향에 따른 분류를 보면 suprasellar 5예, suprasellar and parasellar 4예, preor retrosellar 5예, intrasphenoidal 6예로 비슷한 분포를 보였다(Table 4).

TCA로 수술한 경우는 5예 중 예에서 만 아전적출을, 3예에서는 부분적출이 가능하였다(Table 5, No. of cases Direction 되었다. Suprasellar only 5 Fig. ).

수술 후 방사선 치료를 시행한 경우는 7예였다. 수술 후 합병증으로는 TSA로 수술한 경우, 일시적 요붕증이 예에 Table 5.

3 거대뇌하수체선종의 외과적 치료 직경이 cm으로 평균 5.5cm 이었다. MRI에서 종양 의 진전 방향에 따른 분류를 보면 suprasellar 5예, suprasellar and parasellar 4예, preor retrosellar 5예, intrasphenoidal 6예로 비슷한 분포를 보였다(Table 4). 또 10예 에서 종양내 낭포(cyst)를 형성하거나 종양내 출혈이 관찰 Table 4. Direction of growth extension 3. 수술 결과 TSA로 수술을 시행한 15예 중 9예에서 아전적출 이상 의 종양제거가 되었고, 부분적출을 시행한 경우는 6예였다 (Table 5, Fig. 1). TCA로 수술한 경우는 5예 중 예에서 만 아전적출을, 3예에서는 부분적출이 가능하였다(Table 5, No. of cases Direction 되었다. Suprasellar only 5 Fig. ). 수술전 시력장애가 있었던 18예에서 TSA로 수술 Suprasellar & parasellar 4 한 13예에서 9예가 호전되었고, TCA로 수술한 경우는 5예 Preor retrosellar 5 중 예가 호전되었으며, 1예에서는 오히려 악화되었다(Table Intrasphenoidal 6 6). 수술 후 방사선 치료를 시행한 경우는 7예였다. 수술 후 합병증으로는 TSA로 수술한 경우, 일시적 요붕증이 예에 Table 5. 이렇게 거대 뇌하수체 선종의 치료시, 수술적 접근법을 선택하는데 있어서 서로의 장단점과 그 적응증에 있어서 비 교하기가 어렵지만 환자의 연령, 임상증상, 종양의 진전방향, 침습정도, MRI소견, 합병증 등을 고려하여 적절한 수술법을 선택하는 것이 중요하다.
