The Kyoto classification of gastritis is a classification that comprehensively describes the association between an individual s H. pylori infection status and endoscopic findings. Characteristic endoscopic findings in uninfected individuals include a regular arrangement of collecting venules, fundic gland polyps, and red streaks, among other such features. Characteristic endoscopic findings in patients with current H. pylori infection include diffuse and spotty mucosal erythema, atrophy, intestinal metaplasia, enlarged or tortuous folds, secretion of sticky mucus, mucosal nodularity, foveolar hyperplastic polyps, and/or xanthomas. Characteristic endoscopic findings in previously infected individuals include patchy and map-like mucosal erythema. This classification can reflect the risk of gastric cancer and can benefit primary care physicians, as well as expert endoscopists owing to its easy applicability in routine clinical practice. (Korean J Helicobacter Up Gastrointest Res 2019;19:88-93) Key Words: Classification; Gastritis; Helicobacter pylori; Stomach 서 론 위염의내시경진단은 1922 년에 Schindler 1 가위경을이용하여내시경위염의존재를보고한뒤 Schindler 의위염분류를바탕으로여러가지의위염분류가제시되었으며, 위축의범위진단에대해서는 Kimura-Takemoto 분류가이용되어왔다 년 Warren 과 Marshall이 Helicobacter pylori (H. pylori) 균을발견하고 H. pylori 감염이위염의원인이될수있다는것이밝혀진후 1990년에원인, 조직소견, 내시경소견을체계화한 Sydney system 이제창되었다. 3 그후조직학적분류는 updated Sydney system 으로개편되어국제적인분류로서지위를확립하였다. 4 Sydney system 은국제적으로통용되어왔지만, 내시경분류에있어서는몇가지문제점이있어일반임상에서는널리보급되지못하였다. Sydney system 에서사용되는내시경소견과의차이점은미만성발적, 지도상발적, 장상피화생 (intestinal metaplasia, IM), 백탁점액 (sticky mucus), 다발성백색편평융기 (multiple white flat elevated lesion), RAC 가새로운내시경소견으로 Kyoto 분류에추가되었고, 반면삼출물 (exudate) 과점막취약성 (friability) 은 Kyoto 분류에서제외되었다. 1. H. pylori 감염에따른내시경소견 1) H. pylori 미감염내시경소견 (Fig. 1) H. pylori 미감염의위저선점막에는규칙적으로배열하는미세한붉은점들이관찰된다. 이소견을 RAC 라고하며, 정상위점막에서보이는소견이다. 염증이없는위저선점막에서는원형의선관구조가배열하고선관주위를에워싸는미세혈관들이벌집모양의네트워크를형성한다. 이러한미세혈관들이융합하여집합세정맥 (collecting vein) 이되고점막하층의정맥 A B C D E F G H I Fig. 2. pylori 미감염에서도 RAC 는보이지않는다. 위저선용종 (fundic gland polyp) 은일반적으로 H. pylori 미감염또는 H. pylori 제균후관찰되며, 위저선용종의존재는 H. pylori 음성을시사한다. 특히양성자펌프억제제 (proton pump inhibitor) 의장기복용에의하여위저선용종의발생이증가될수있다.

pylori 현감염내시경소견 (Fig. 2) H. pylori 감염시염증을동반한위점막에서는점막표층에존재하는모세혈관의울혈및확장에의하여점막전체가발적을띄게되는데, 이를미만성발적이라고한다. 이러한점막의색조변화는 hemoglobin index 가 H. pylori 위염일때상승하는것과비슷한의미이다.

4 Gwang Ha Kim: Endoscopic Findings of Kyoto Classification of Gastritis 으로이어진다. 내시경검사시집합세정맥이위점막표면에투영되는형태가 RAC 이다. 전정부근위부와위체부에서 RAC 가관찰된다면 H. pylori 미감염점막을시사한다. RAC 는멀리서는미세한붉은점들로인식되지만근접시불가사리형태의혈관들을확인할수있다. 전정부에국한된 H. pylori 감염위염에서는위체부점막에 RAC 가관찰될수있으므로, H. pylori 미감염과현감염여부를감별하기위해서는 RAC 의판정은위각부와하부체부소만에서실시하는것이좋다. 8 한편유문선점막에서는선관의구조형태가다르기때문에 H. pylori 미감염에서도 RAC 는보이지않는다. 위저선용종 (fundic gland polyp) 은일반적으로 H. pylori 미감염또는 H. pylori 제균후관찰되며, 위저선용종의존재는 H. pylori 음성을시사한다. 특히양성자펌프억제제 (proton pump inhibitor) 의장기복용에의하여위저선용종의발생이증가될수있다. 헤마틴 (hematin), 선상발적 (red streak), 융기형미란 (raised erosion) 은 H. pylori 미감염자에서좀더많이관찰되지만, H. pylori 감염자에서도보일수있으므로이러한소견만으로는 H. pylori 미감염을단정지을수는없다. 2) H. pylori 현감염내시경소견 (Fig. 2) H. pylori 감염시염증을동반한위점막에서는점막표층에존재하는모세혈관의울혈및확장에의하여점막전체가발적을띄게되는데, 이를미만성발적이라고한다. 이러한점막의색조변화는 hemoglobin index 가 H. pylori 위염일때상승하는것과비슷한의미이다. 또한미만성발적으로인하여좀더깊은곳에위치한집합세정맥이점막표면에서관찰되지않게되는데, 이를 RAC 의소실이라고한다. H. pylori 감염이장기간지속되면고유위선의감소로인한위축성변화가발생하여내시경검사시다양한정도의점막주름의위축, 혈관투견등의위축소견이나타난다. 장상피화생은위축이진행함에따라나타나며, 내시경검사시회백색조 의편평융기가관찰되면장상피화생에특이적인소견이라할수있다. 하지만장상피화생이이렇게관찰되는경우는일부에불과하며, 정확한진단을위해서는 methylene blue 염색이유용하다고알려져있으나우리나라에서는거의사용되지않는다. 최근에는위점막의융모상변화나 indigo carmine 색소내시경에서위소구의크기와모양이불규칙한경우가장상피화생진단의유용한지표가된다고알려져있다. 9 H. pylori 감염과관련된기타내시경소견으로는점막종창 (mucosal edema), 위체부주름의종대 (enlarged fold) 와사행 (tortuous fold), 위저선영역의점상발적 (spotty redness), 백탁점액, 황색종 (xanthoma), 10 닭살모양의결절성변화 (nodularity), 11 선와상피- 과형성성용종 (foveolar-hyperplastic polyp) 12 이있다. 반상발적 (patchy redness) 과함몰형미란 (depressive erosion) 은 H. pylori 감염상태와관련없이나타나는소견으로, 비스테로이드성항염증약물 (non-steroidal anti-inflammatory drug) 등으로인한점막손상과관련이많다. 3) H. pylori 기감염 ( 제균후 ) 내시경소견 (Fig. 3) H. pylori 의제균치료가이루어지면위점막의염증이개선된다. 조직학적으로제균성공직후다핵구침윤은저명하게개선되어거의사라지며, 단핵구침윤도제균직후크게감소하고남아있는단핵구침윤은제균후 4~5 년에걸쳐점진적으로개선된다. 그러므로제균후내시경소견의변화는제균당시의위염의상태와관찰시기에따라차이가있다. 따라서제균후에는 H. pylori 감염시내시경소견과 H. pylori 미감염시내시경소견이같이관찰되는경우가흔하다. 제균성공후단기간내에점상발적, 미만성발적, 점막종창, 위주름의종대와사행, 백탁점액은개선되는경우가많다. 13 한편제균후장기경과시에는 RAC 가회복되거나일부에서위축, 장상피화생도개선되기도하지만개선의속도와정도는개인차가있다. 제균후특징적인내시경소견으로는지도상발적이있다. 5 Korean J Helicobacter Up Gastrointest Res: Vol 19, No 2, June 2019 H. pylori 양성인만성위축성위염에서는흔히혈관이투영되는위축영역이퇴색ㆍ백색으로, 위저선이남아있는비위축영역이발적으로관찰된다. 하지만제균후에는위축영역의장상피화생을가진점막이발적으로, 반대로비위축영역의위저선점막은미만성발적이소실하기때문에백색조로나타나게된다. 이처럼제균후나타나는위축영역의발적을지도상발적으로부르며, 이선경계에서의색조의역전은현저하여색조역전현상 (reversed phenomenon on mucosal borderline) 이라고하기도한다. 지도상발적, 색조역전현상의존재가확인된경우나위축점막과동시에미감염소견인 RAC 와위저선용종이관찰되는경우에는제균후점막이라고진단하기에는별로어려움이없다. 하지만위축점막에서는부정형의집합세정맥이관찰될수있어서제균후의 RAC 회복을평가할때에는주의가필요하다. 2. 위암위험도를고려한내시경소견점수화체계 내시경소견 ( 위축, 장상피화생, 주름의종대, 닭살모양의결절 ) 을토대로그리고 H. pylori 감염과 H. pylori 제균후를구별하는소견으로미만성발적을포함시켜 14 위암의위험도를예측할수있는점수화체계를구성할수있다 (Table 2). 7 즉, 제균에의하여염증세포침윤이소실되면위점막이미감염위점막에가깝게회복되므로내시경소견을통하여위암의위험도 Table 2. Kimura- Takemoto 분류를사용하여기술하며 7 위축은유문부에서시작하여체부소만, 이후체부전체로확장하므로범위에따라 C-1, C-2, C-3, O-1, O-2, O-3의 6단계로구분한다. C는 close 를, O는 open 을의미하며, 분문부부터유문부까지위축이연결되지않은경우를 close type, 연결된경우를 open type 이라고분류한다. 위축이관찰되지않는 C-0, C-1 은점수 0점, 위축이경한 C-2, C-3 는점수 1점, 위축이중정도부터심한경우인 O-1~O-3 은점수 2점으로계산한다. 예를들면점수 1점의경우에는 A 1 으로기재한다. 2) 장상피화생 (IM) 장상피화생은일반백색광관찰및영상강화내시경 (imageenhanced endoscopy, IEE) 을이용한관찰에서다른소견을보인다. 백색광으로는특이형장상피화생이라고불리는백색융기병변과제균후나타나는발적함몰병변의관찰만가능하다. 그러나 methylene blue 를사용한색소내시경에서는특이형이외의장상피화생도청색으로염색되어관찰된다. 협대역영상 (narrow band imaging, NBI) 에서선와상피의표면에청백색의선이보이는 light blue crest (LBC) 가관찰되면장상피화생으로진단할수있다. 또한특이형의장상피화생에서는 NBI 로관찰시점막상피에백색물질이부착되어있는 white opaque substance (WOS) 를볼수있다. IEE 관찰에서는이러한 LBC, WOS 의정도와범위를평가한다. 따라서백색광관찰및 IEE 관찰소견을구분하여기재한다. 장상피화생이관찰되지는않는경우는점수 0점, 장상피화생이전정부에국한되어있는경우는점수 1점, 장상피화생이체부까지확장되어있는경우는점수 2점이다. 백색광관찰에서점수 1점인경우는 IM 1 으로기재한뒤 IEE 관찰소견은괄호내에표기한다. 백색광관찰점수 1점, IEE 관찰점수 2점의경우에는 IM 1(2) 로기재한다. 3) 주름의종대 (H) 충분한공기를주입하여관찰시주름의폭이 4 mm 이하인경우는점수 0점, 5 mm 이상인경우는점수 1점이며 H 1 으로기재한다. 4) 닭살모양의결절 (N) 닭살모양의결절이관찰되지않는경우는점수 0점, 관찰되는경우는 1점이며 N 1 이라고기재한다. 5) 미만성발적 (DR) 위축이없는체부선영역을관찰한다. 숙련되지않는경우에 92

RAC 가관찰되는경우는미만성발적이없으므로점수 0점, RAC 가소실된경우는점수 2점, 제균후와같이일부에서 RAC 가관찰되는경우는점수 1점으로한다. 점수 2점인경우는 DR 2 로기재한다. 6) 전체점수모든인자를 A 1 IM 1(2) H 0 N 1 DR 2 로기재하며, 전체최종점수 (IEE 점수는계산하지않음 ) 는괄호내에기입하여 A 1 IM 1(2) H 0 N 1 DR 2(5) 로기재한다. 전체점수는최소 0점에서최대 8점이다. 3. 내시경소견기재방법 Kyoto 분류의내시경기재과정은다음과같다. 15 1) H. pylori 감염위염 ( 현감염, 활동성위염 ), H. pylori 감염병력, H. pylori 미감염을구분하고, 국제적으로통용하기위하여영어로기재한다. 현감염의경우는 active-gastritis, 기감염은 inactive-gastritis, 미감염은 non-gastritis 로기재한다. 2) 이후위축범위 (Kimura-Takemoto 분류 ) 를괄호안에기재한다. 예를들면, active-gastritis (O-1) 로기재한다. 3) 위염의진단에사용한소견이외의필요한소견에대해서는다음에기재한다. 또한, 각내시경소견의부위를 of 다음에기재한다. 예를들면, active-gastritis (C-3) with erosion of antrum, A 1 IM 1 H 0 N 0 DR 2(4) 로기재한다. 결론 위염의 Kyoto 분류는내시경시관찰되는소견을구체적으로정리하고이러한소견을위암의가장중요한원인인자인 H. pylori 감염여부와의연관성을포함한분류이다. 실제국내에서위내시경검사의주목적이위암의조기발견이므로, 이러한 Kyoto 분류는이전분류보다는위내시경검사시좀더많은정보를가지고체계적으로접근할수있는길을제시해줄것으로생각된다. 또한이중중요한내시경소견을토대로점수화한위암의위험도점수체계는추후임상연구를통하여검증이필요하지만, 스크리닝내시경검사기간의선별에도어느정도도움을줄수있지않을까하는기대를가져본다.
