One day prior to admission he had received chemotherapeutic agents comprising 130 mg cisplatin and 5,200 mg 5-Fluorouracil for nasopharyngeal carcinoma. EKG showed ST elevations in the leads II, III and avf. The levels of cardiac enzymes were elevated [creatine kinase (CK) 1129 U/L, CK-M 180 U/L, troponin T 1.23 ng/ml and troponin I ng/ml]. Urokinase was administered at the emergency room, but the patient s chest pain continued with persistent ST segment elevations. Urgent coronary and renal angiograms revealed thrombotic occlusive lesions in the distal right coronary and right renal arteries. Percutaneous transluminal renal angioplasty using mm balloon was performed for the renal artery. However, filling defects and distal renal flow were not improved and so bciximab (ReoPro ) was administered. Follow-up coronary and renal angiograms on the fifth hospital day showed no filling defects with good distal flow in both right coronary and renal arteries. antibody 음성이었다. 신동맥에 관찰되었으며, 5회 반복하여 풍선확장술을 시 심전도에서 율동은 심방 세동이었으며, Ⅱ, Ⅲ, avf 행하였으나 혈류는 개선되지 않아서 bciximab(reopro ) lead에 ST분절 상승이 있었다(Fig. 1).

3 antibody 음성이었다. 신동맥에 관찰되었으며, 5회 반복하여 풍선확장술을 시 심전도에서 율동은 심방 세동이었으며, Ⅱ, Ⅲ, avf 행하였으나 혈류는 개선되지 않아서 bciximab(reopro ) lead에 ST분절 상승이 있었다(Fig. 1). 혈전용해요법을 17 mg을 bolus로 정주하고 12시간 동안 400 mg을 정 150만 Unit urokinase를 각각 2회 bolus 정주하여 시행 주하였다. 받았으나 증상 및 심전도상 ST 분절 상승 지속되었고 시술 후 환자는 저분자량 헤파린인 dalteparin 7500 흉통과 복통이 지속되어 관상동맥 조영술을 시행하였다. Unit를 1일 2회 정주하였고 aspirin 200mg, clopido- 관상동맥 조영술에서 우관상동맥은 비정상적인 기시를 grel 75 mg, simvastatin 20 mg을 투여 받았으며, 입원 보였고 전반적으로 매우 느린 혈류와 함께 원위부 우측 2일째부터 흉통과 복통이 소실되었다. 입원 5일째 추적 관상동맥에 심한 99%의 협착을 보였고 TIMI Ⅱ 혈류를 검사한 심전도에서 Ⅱ, Ⅲ, avf lead에 ST분절의 상승 보였다(Fig. 2). 신동맥 조영술에서 우측 신동맥의 완전 은 소실되었고 신기능은 정상이었고 추적 관상동맥 조 폐쇄가 관찰되어(Fig. 3), 6.0±20 mm 풍선도자를 영술(Fig. 2) 및 우측 신동맥 조영술에서 혈전성 동맥 이용하여 풍선확장술을 시행하였다(Fig. 4, ). 그러나 폐쇄가 완전히 소실되었다(Fig. 3). 우측 신동맥의 혈류는 개선되지 않았고 혈전이 원위부 Fig. 3. : right renal angiogram showed abrupt filling defect in the proximal right renal artery. : a follow-up right renal angiogram after 5 days showed no filling defects in right renal artery with good flow. 4 고찰 Cisplatin 은폐암, 생식세포암, 비인두암등고형암치료를위하여널리사용되며효과적인약제이나, 오심, 구토, 신독성, 말초신경독성, 이독성, 골수억제와같은여러가지부작용과 Raynaud 현상, 고혈압, 심근경색증, 뇌혈관질환과같은혈관손상이 cisplatin 과관련하여보고되고있다. 3-5) Cisplatin 이혈관독성을일으키는기전은명확하지않으나, 저마그네슘혈증, 혈소판응집의변화, 응고기전의변화, thromboxane-prostacyclin 작용이상, 혈중 von Willebrand 인자의상승, 고콜레스테롤혈증, 자동능의이상, 자가면역반응등이주요인자로생각되고있다. 저마그네슘혈증은혈압과말초혈관의저항성을상승시키며혈관의칼슘농도를올리며레닌, 안지오텐신, 세로토닌, 노아에피네프린, 아세틸콜린, 칼륨과같은관상동맥의혈관수축을유발하는물질을활성시킴으로써관상동맥의혈관경련과심근허혈을가져올수있다. 또한혈중 von Willebrand 인자의상승, 혈관조영술에서폐색성혈관염, 혈관내혈전은혈관내피손상후이차적혈전형성의가능성을암시하였다. 14) 5-Fluorouracil 부작용으로심장독성은 1.6 내지 8% 에서발생된다고알려져있으며, 협심증, 부정맥, 심근경색증, 심부전, 심인성숔으로나타나며예측이어렵고치명적이다. 5-fluorouracil 의심장독성에직접적심근허혈, 관상동맥경련, 급성심근염이관여하며약물농도증가에따라독성이증가될수있다. 6-10) 5-Fluorouracil 과 cisplatin 병합요법은심장독성의부과적위험인자이며 5-Fluorouracil 이독성에주로관여할것으로여겨진다 ) 방사선치료와기존관상동맥질환자는심장독성의위험이더높이고흉부방사선조사는심근의 5-Fluorouracil 를감작시켜심장독성을일으킬것으로알려져있으나, 본환자의경우흡연가이었으나최근금연하였고관상동맥질환의과거력이없었으며두경부암에대한국소방사선조사력도없었다. 혈소판응집의마지막단계를차단하는현재까지알려진가장강력한혈소판응집억제제인혈소판당단백 Ⅱb/Ⅲa 수용체차단제인 bciximab(reopro ) 는급성관상동맥증후군에효과적으로사용되고있으며, 고위험군에서관상동맥중재술시술결과를향상시킬뿐만아니라관상동맥내혈전을성공적으로치료할수있 다고여러연구를통해밝혀지고있다 ) 본증례에서는 bciximab(reopro ) 을사용하여풍선확장술에반응하지않는급성심근경색증과신동맥의혈전성폐쇄증을효과적으로치료하였던예로서, 혈전의 spontaneous thrombolysis와감별이어려운제한점이남아있으나혈소판당단백 Ⅱb/Ⅲa 수용체차단제가항암제에의해유발된동맥혈전에도치료효과가있음을알수있었다. 요 비인두암에대하여 5-Fluorouracil 과 cisplatin 병합항암요법을받은 55세남자환자에서 1일후에발생한우관동맥과신동맥급성혈전성폐쇄병변에대한경피적풍선확장술과 bciximab(reopro ) 등의항혈소판제제로성공적으로치료하였음을보고한다. 약 중심단어 : 심근경색증 ; 혈소판 ; 혈관성형술.
