1 ORIGINAL ARTICLE DOI: /jkap 박찬용 1 전영찬 2 * 정창모 2 윤미정 1 부산대학교치과대학치과보철학교실 1 대학원생, 2 교수 연구목적 : 2 종의열가압도재 (e.max Press, Empress Esthetic) 와법랑질간의마모양상을알아보기위하여장석계도재및제 3 형금합금과비교, 관찰하였다. 연구재료및방법 : 열가압도재 e.max Press, Empress Esthetic 및금속도재관제작용장석계도재 Cermaco 3, 제 3 형금합금으로원반형시편을제작하고, 상대마모재로는발거된건전한소구치의교두를이용하여 Pin-on-disk tribometer 상에서마모시험하였다. 법랑질의마모량은마모전후치아시편의무게변화를측정하여부피로환산하였고, 도재및금합금시편의평균표면조도와마모트랙깊이를표면조도기로, 마모트랙폭을주사전자현미경으로측정하여마모양상을비교하였다. 결과 : 1. 법랑질의마모량은 Ceramco 3 군에서가장크고 Empress Esthetic 군, e.max Press 군, 금합금군순으로작았으며, Ceramco 3 군과 Empress Esthetic 군사이와, Empress Esthetic 군, e.max Press 군및금합금군사이에는유의한차이가없었다 (P >.05). 2. 마모시험후도재시편의평균표면조도는 Ceramco 3 군, Empress Esthetic 군, e.max Press 군순으로작았으며, Empress Esthetic 군과 Ceramco 3 군사이에는유의한차이가없었다 (P >.05). 3. 마모트랙의깊이는모든군사이에유의한차이는없었고 (P >.05), 마모트랙의폭은법랑질마모량순서와동일하게 Ceramco 3 군, Empress Esthetic 군, e.max Press 군, 금합금군순으로작았으며, e.max Press 군과 Empress Esthetic 군사이및 Empress Esthetic 군과 Ceramco 3 군사이에유의한차이는없었다 (P >.05). 결론 : 열가압도재는장석계도재보다법랑질을더마모시키지는않았다. ( 대한치과보철학회지 2009;47:21-8) 주요단어 : 전부도재관, 열가압도재, 법랑질마모, E-max Press, Empress Esthetic, Tribometer 서론 치과분야에서심미에대한환자의기대가증가함에따라전부도재관에대한관심이늘어나고있으며그사용도증가하고있다. 도재의물성이향상되고제작기술이발전함에따라전부도재관제작이더용이해졌고그적용범위도전치부에서구치부까지확대되고있다. 1 전부도재관을제작하는방법중열가압도재시스템은기존의금속주조수복물을제작하는과정과유사하게납형을이용하여적절한형태의수복물을일시에얻게되므로제작과정이간편하며, 변연적합성이우수하다. 2-5 열가압전부도재관은하부구조위에도재를전장하거나완전한치관형태표면을착색하여완성하는두가지방법으로만들어지고있다. 적층법의경우전장용도재를이용한정확한색조부여가가능하지만, 6 파절에취약 한코어 - 상부도재계면이존재하며전장용도재는코어용도재에비해기계적강도가약하고소성후표면결함이많다. 7-9 따라서열가압전부도재관제작에있어기계적인물성과기공과정측면에서착색법이적층법에비해유리한점을가진다할수있다. 기존의여러연구에서장석계도재의과도한자연치마모성이지적되었던것처럼열가압도재도그러한문제점을가질수있으므로, 전부도재관을시술하는치과의사는심미성, 파절저항성을고려함과동시에법랑질의마모가능성에도관심을가져야한다. 15,18 그러나전장용도재와법랑질마모에관해서는많은연구가이루어진반면, 18,19 열가압도재가법랑질마모에미치는영향에관한연구는부족한상황이다. 이에본연구에서는최근소개된 2 종의열가압도재 (e.max Press, Ivoclar Vivadent, Liechtenstein / Empress 교신저자 : 전영찬 부산시서구아미동 1 가 10 부산대학교치과대학치과보철학교실 : , jeonyc@paran.com 원고접수일 : 2008 년 6 월 19 일 / 원고최종수정일 : 2008 년 12 월 15 일 / 원고채택일 : 2008 년 12 월 30 일 이논문은부산대학교자유과제학술연구비 (2 년 ) 에의하여연구되었음. 대한치과보철학회지 2009 년 47 권 1 호 21

2 Esthetic, Ivoclar Vivadent, Liechtenstein) 와법랑질간의마모양상을알아보기위하여일반장석계도재및제 3 형금합금과비교, 관찰하였다. 연구재료및방법 1. 연구재료 마모시험을위한재료로최근소개되어임상에이용되고있는열가압도재인 e.max Press 와 Empress Esthetic 및금속도재관제작용장석계도재 Ceramco 3 (Dentsply, USA), 제 3 형금합금 (Goldenian C-66, Shinhung, Korea) 을사용하였으며 (Table I), 상대마모재로는발거된건전한소구치를사용하였다. 2. 연구방법 1) 치아시편의제작치아시편은교정치료를위해최근발거되어생리식염수에보관해온건전한소구치를가공하여교두노출 3 mm, 지름 8 mm, 높이 6 mm 의원추형태로 pin-on-disk tribometer (Triboss PD-102, R&B, Korea) 의 jig 에맞도록투명레진 (Orthodontic resin, Dentsply, USA) 에포매하였다. 2) 열가압도재시편의제작인레이왁스로지름 20 mm, 두께 1.5 mm 의납형을만들어매몰하고 e.max Press 와 Empress Esthetic 을제조사의지시에따라제작한다음원반형시편을자동연마기 (Metpole, R&B, Korea) 에서 #600, 1200, 2000 SiC 로연마하고마모시험기의 jig 에맞도록투명레진으로포매하였다. 각제품별 5 개씩모두 10 개의열가압도재시편을제작하였다. 3) 장석계도재시편의제작소성후지름 20 mm, 두께 1.5 mm 의원반형태가되도록 silicone mold 를제작하고, 이를이용하여금속도재관 Table I. Schematic diagram of pin-on-disk tribometer with enamel and test specimen in wet environment. 가. 법랑질의마모무게측정에는 10-5 gram 단위까지측정가능한정밀전자저울 (DualRange XS205, Mettler-Toledo, Switzerland) 을사용하여, 마모시험전후증류수에보관하였던치아시편을 5 분간진공건조기에서건조시킨뒤무게를측정하였다. 법랑질의마모량은무게변화와법랑질의밀도 (2.8 mg/mm 3 ) 를이용하여산출하였다. 교두첨부의마모된양상을관찰하기위하여마모시험전후치아시편을광학현미경 (Eclipse LV150, Nikon, Japan) 으로관찰하였다. 22 대한치과보철학회지 2009 년 47 권 1 호

나. 도재와금합금시편의마모도재와금합금시편을표면조도기 (Surftest SV-3000, Mitutoyo, Japan) 를사용하여마모시험전후의평균표면조도와마모트랙의깊이를측정하였고, 주사전자현미경 (JSM-6480LV, JEOL, Japan) 을사용하여마모시험전후의시편표면을관찰하고마모트랙의폭을측정하였다. 다.

3 나. 도재와금합금시편의마모도재와금합금시편을표면조도기 (Surftest SV-3000, Mitutoyo, Japan) 를사용하여마모시험전후의평균표면조도와마모트랙의깊이를측정하였고, 주사전자현미경 (JSM-6480LV, JEOL, Japan) 을사용하여마모시험전후의시편표면을관찰하고마모트랙의폭을측정하였다. 다. 통계분석법랑질의마모량, 도재와금합금시편의비마모면평균표면조도및마모트랙의깊이와폭의차이를 SPSS 통계프로그램을사용하여일원분산분석법 (One-way ANOVA) 으로분석하고다중범위검정법 (Sheffe s test) 을이용하여유의성을검정하였다. 연구성적 1. 법랑질의마모 법랑질마모량의평균값과표준편차는 Table II 와같으며, 법랑질교두의마모양상은 Fig. 2 와같이관찰되었다. 법랑질마모량은 Ceramco 3 군에서가장크고, Empress Esthetic 군, e.max Press 군, 금합금군순으로작았으며, Ceramco 3 군, Empress Esthetic 군사이와 Empress Esthetic Table II. Mean values and SDs of wear track width of restorative specimens ( mm ) Test material Wear track depth e.max Press ± a Empress Esthetic ± ab Ceramco ± b Type III gold alloy ± c Identical superscripted small letters indicate that values are not significantly different (P >.05). 마모시험전주사전자현미경으로각시편의연마상태를관찰할수있었다. e.max Press 와 Empress Esthetic 시편에서내부구조가치밀하고기포가적은열가압도재의특징을관찰할수있었고, Ceramco 3 시편에서는큰결정상과기포가관찰되었다. 마모시험후주사전자현미경으로각시편에형성된마모흔적을관찰할수있었다. 금합금의마모양상과는달리도재시편에서는피로마모양상을관찰할수있었으며 Ceramco 3 에서가장뚜렷한마모흔적을보였다. 총괄및고찰 법랑질의마모는생리적또는병적인상태모두에서발생될수있다. 서로대합되는치아의점진적인마모는정상적인현상으로간주되지만, 마모가일어나는정도는식생활, 이갈이같은습관및대합치교합면수복물의마모특성에의해서달라질수있다. 15 A B A B C D C D Fig. 3. 따라서심미적결점은있으나기능성, 마모성을고려한측면에서는적절한마모저항성을가지면서대합치를적게마모시키는금함금이가장안정적인수복재료로여겨진다. 10,12,14 도재는치과용수복재료중심미성이가장우수한재료이며내구성및생체적합성이뛰어나전치부뿐만아니라구치부수복시심미적인수복재로가장일반적으로사용된다.

14 도재의문제점을개선하고자하는노력이계속되면서신소재의개발과제작기법의향상으로, 기계적강도가증가되어구치부에서도전부도재관의제작이가능하며, 10 마모도가적은도재로심미적이면서도대합치마모를감소시키는결과를얻을수있게되었다.

B D 열가압전부도재관은코어를먼저제작한후상부도재를축성하거나전체적인외형을갖도록제작한후착색하여완성하게되는데, 6 전장용도재와법랑질마모에관해서는많은연구가이루어진반면, 19,20 열가압도재가법랑질마모에미치는영향에관한연구는부족한상황이므로, 본연구에서는임상에서사용되는열가압도재의대표적인성분인 lithium disilicate glass ceramic (e.max Press) 과 leucite reinforced glass ceramic (Empress Esthetic) 이자연치법랑질마모에미치는영향을알아보고자하였다. 마모시험은법랑질과수복재료의마모에관한연구에서사용되어온 pin-on-disk tribometer 를사용하여 #2,000 SiC 까지연마한도재와금합금시편을습식환경하에서진행하였으며, 수차례의예비실험을통하여각실험군별마모양상차이를알수있도록이동거리는 100 m 로설정하였다. 이전의도재마모연구들에서는구강내발생하는교합력에근거하여마모시험기의하중을 N 으로다양하게설정하였는데, 상대재가도재시편위를미끄러질때발생하는 microfatigue 가마모에중요한요소이며, pin-on-disk tribometer 는미끄러지는마모형태를나타내므로, 본연구에서는치아접촉시측방력이약 20 N 이라는 Graf 22 의보고에따라하중 2 kg 하에서치아시편이회전하도록하였다. 본연구결과에따르면, 법랑질의마모량은 Ceramco 3 군에서가장크고, Empress Esthetic 군, e.max Press 군, 금합금군순으로작았으며, 금합금군과열가압도재군사이에는유의한차이가없었고, e.max Press 군은 Ceramco 3 군보다법랑질마모량이작았다. 도재와금합금시편에형성된마모트랙의깊이는 e.max Press 군에서가장작았으나모든실험군사이에유의한차이는없었고, 마모트랙의폭은법랑질마모량순서와동일하게 Ceramco 3 군에서가장크고, Empress Esthetic 군, e.max Press 군, 금합금군순으로작았다. 교합과안면고경에영향을미치는수직고경과직접적인연관이있기때문에일반적으로치의학에서마모양상을평가할때깊이측정이더중요하게생각되는데, 23 본연구에서마모트랙의깊이는모든실험군에서서로유의한차이가없었고, 마모트랙의폭또한교두법랑질이마모되면서시편과의접촉면적이증가하여나타난결과라고본다면, 도재및금합금시편의마모보다는법랑질의마모를더중요하게평가하고고려해야할사항임을알수있다. 치과용도재의마모성은강도보다는미세구조, 접촉면의거칠기, 구내환경과밀접한관련이있으며, 제작과정 대한치과보철학회지 2009 년 47 권 1 호 25

6 에서발생한내부기포및표면결함에의해서도영향받는다. 24 본연구에서마모시험전평균표면조도는금합금을제외한나머지도재시편사이에서유의한차이가없었으나, 마모시험후도재시편트랙의평균표면조도가 e.max Press 에서가장작고, Empress Esthetic, Ceramco 3 순으로커지는것은마모가진행되면서각도재시편구성성분의미세구조및결정크기의차에따라도재표면에발생하는미세한요철정도가다르기때문인것으로보인다. 이러한표면조도의차이가상대법랑질의마모도에영향을끼친것으로생각된다. 마모시험전시편표면을주사전자현미경으로관찰시 e.max Press 와 Empress Esthetic 시편에서내부구조가치밀하고기포가적은열가압도재의특징을관찰할수있었고, 25 Ceramco 3 시편에서는큰결정상과제작과정에서생긴것으로보이는기포가관찰되었다 (Fig. 3). 마모시험후형성된트랙의주사전자현미경관찰시금합금의마모양상과는달리도재시편에서는피로마모양상을관찰할수있었다 (Fig. 4, 5). 일반적으로도재의마모양상은표면피로마모를보이며이것은두개물체에가해지는하중상태에서미끄러짐으로인해미세균열이발생되어파절을일으키는것으로, 보통깨지기쉬운도재에서이러한양상의마모가발생된다. 26 이상의연구결과로도재의미세구조, 접촉면의거칠기, 표면의다공성이상대법랑질의마모에영향을주었으며, 열가압도재는장석계도재보다법랑질을더마모시키지는않음을알수있었다 (Table II). 다만본연구는 pin-on-disk tribometer 를이용한실험실연구이므로교합력과저작습관, 음식의종류, 악궁에서의치아위치등의다양한요소가관여하는실제임상상황과는다를수있다. 또한치아시편의무게변화를밀도를이용하여부피로환산한법랑질마모량을비교한연구이므로무게변화측정시오차가생길가능성이있으며, 실제구강내저작운동에서와같은저작력에의한동적인압축력을부여하지못한한계가있으므로, 이후의연구에서는삼차원레이저스캔을이용하여마모량을측정하고, 구강내저작운동을재현하는마모시험기를이용한실험이필요하다고생각된다. 결론 본연구에서는열가압도재와법랑질간의마모양상을조사하기위하여최근소개된 2 종의열가압도재인 e.max Press, Empress Esthetic 과장석계도재 Ceramco 3 및제 3 형금합금을 pin-on-disk tribometer 상에서마모시험을 하였다. 법랑질의마모량은치아시편의마모전후무게변화를측정하여부피로환산하였고, 도재및금합금시편의평균표면조도와마모트랙의깊이와폭을측정하고주사현미경으로마모시험전후의시편표면을관찰하여다음과같은결론을얻었다. 1. 법랑질의마모량은 Ceramco 3 군에서가장크고 Empress Esthetic 군, e.max Press 군, 금합금군순으로작았으며, Ceramco 3 군과 Empress Esthetic 군사이와, Empress Esthetic 군, e.max Press 군및금합금군사이에는유의한차이가없었다 (P >.05). 2. 마모시험후도재시편의평균표면조도는 Ceramco 3 군, Empress Esthetic 군, e.max Press 군순으로작았으며, Empress Esthetic 군과 Ceramco 3 군사이에는유의한차이가없었다 (P >.05). 3. 마모트랙의깊이는모든실험군사이에유의한차이는없었고 (P >.05), 마모트랙의폭은법랑질마모량순서와동일하게 Ceramco 3 군, Empress Esthetic 군, e.max Press 군, 금합금군순으로작았으며, e.max Press 군과 Empress Esthetic 군사이및 Empress Esthetic 군과 Ceramco 3 군사이에유의한차이는없었다 (P >.05). 참고문헌 1. Material and methods: Intact cusps of extracted premolars were used for enamel specimens. Five disk samples were made for each of two heat-pressed ceramics groups, conventional feldspathic porcelain group and type III gold alloy group. Wear tests were conducted in distilled water using a pin-on-disk tribometer. The amount of enamel wear was determined by weighing the enamel specimens before and after wear tests, and the weight was converted to volumes by average density. The wear tracks were analyzed by scanning electron microscopy and surface profilometer to elucidate the wear characteristics. Results: 1. Ceramco 3 led to the greatest amount of enamel wear followed by Empress Esthetic, e.max Press and type III gold alloy. However, there was no significant difference between Ceramco 3 and Empress Esthetic (P >.05), and there were also no significant differences among Empress Esthetic, e.max Press and type III gold alloy (P >.05). 2. The average surface roughness of e.max Press after wear test was smallest followed by Empress Esthetic and Ceramco 3, but there was no significant difference between Empress Esthetic and Ceramco 3 (P >.05). 3. There were no significant differences among the depth of wear tracks of all the groups (P >.05). The group that showed the largest width of wear track was Ceramco 3 followed by Empress Esthetic, e.max Press and type III gold alloy. However, there was no significant difference between e.max Press and Empress Esthetic (P >.05), and there was also no significant difference between Empress Esthetic and Ceramco 3 (P >.05). Conclusion: Within the limits of this study, heat-pressed ceramics were not more abrasive than conventional feldspathic porcelain.
