Herein, a case of infant hemorrhagic colitis of verotoxin-producing E. coli O157:H7 diagnosed by multiplex PCR is reported. A nine-month-old boy was admitted to our hospital with bloody diarrhea for the previous two days. Multiplex PCR using Seeplex R Diarrhea ACE Detection Kit (Seegene, Seoul, Korea) was directly applied to the stool specimens. Amplified bands specific for verotoxin, O157, and H7 indicated the presence of O157:H7 EHEC. The stool specimens were inoculated on sorbitol-macconkey agar (SMA) and tryptic soy broth containing mitomycin C (TSB-M). Colorless colonies on sorbitol-macconkey agar were O157-positive. TSB-M enrichment cultures of the stool specimen and the isolates were positive for verotoxin according to an enzyme immunoassay (EIA). The prepared ingredients of baby foods for the patient including ground meat, chopped carrot, chopped cabbage, and white rice porridge showed no EHEC on TSB-M and SMA. The patient s parents and three-year-old sister did not recently have any gastrointestinal symptoms. Cefdinir was administered for one day and was ceased after diagnosis of EHEC colitis. The stool culture and verotoxin assay were negative on the second day of hospitalization. Application of multiplex PCR and verotoxin EIA directly to diarrheal stool warrants the rapid diagnosis and appropriate treatment of EHEC colitis. (Korean J Clin Microbiol 2010;13:85-89) Key Words: Eshcherichia coli O157:H7, Enterohemorrhagic Eshcherichia coli, Verotoxin, Multiplex PCR 서 대장균은정상적인인체의대장에존재하는정상균무리중하나지만, 일부병원성대장균은장염을유발한다 [1,2]. 1982년미국에서 Escherichia coli O157:H7에의해출혈성설사의집단발병이발생하여처음보고된이래 [3] 이균은세계여러나라에서집단발병과산발적인감염을일으키는원인으로보고되고있다 [4-6]. 이대장균은 Shigella spp. 의출혈성독소 (Shiga toxin 또는 verotoxin) 를가지고있어서 [7] 전형적인출혈성설사를일으키는기전이규명됨으로써 Enterohemorrhagic E. coli (EHEC) 또는 Verotoxin-producing E. coli (VTEC) 라고명명하였다. 한국에서는 EHEC 감염이 1994년최초로보고된이후 [8] 2000년제1군법정전염병으로지정되어관리하고있으며, Received 15 October, 2009, Revised 23 February, 2010 Accepted 25 March, 2010 Correspondence: Mi-Na Kim, Department of Laboratory Medicine, University of Ulsan College of Medicine and Asan Medical Center, 86, Asanbyeongwon-gil, Songpa-gu, Soeul , Korea. (Tel) , (Fax) , ( ) mnkim@amc.seoul.kr 론 2003년과 2004년집단감염이발생하였고, 최근매년 40 50건씩보고되고있다 [9,10]. 국내문헌고찰상설사환자의대변배양에서 Sorbitol MacConkey agar로 EHEC를 1년간검사한논문에서는한주도검출하지못해서통상적으로검사할필요가없다고하였고 [11], 이후통상적으로배양검사를실시하여빈도를보고한연구는더이상없어서 EHEC 장염에대한정확한빈도와역학에대한자료는부족한실정이다. 또한 EHEC 장염의호발연령이소아이지만영아에서는드물다 [5,6]. 저자들은지역사회획득성설사를주소로내원한 9개월된영아의대변검체를다중 PCR로검사하여 verotoxin 양성 E. coli O157:H7에의한출혈성장염을진단하여임상적, 미생물학적, 역학적특성을조사하였다. 증례이전까지건강하였던 9개월된남아가혈변을주소로소아과외래를통해입원하였다. 환아는입원일주일전부터기침과콧물이있었고, 입원이틀전부터섭씨 38도이상의열과구토, 85

하루 2차례의점액성혈변이있었다. 입원하루전외래에서 cefdinir를처방받고귀가하였으나호전이없었다. 그후묽은변에피가섞인양상으로하루 5차례혈변을보았고온몸에발진이발생하여입원하였다. 입원당시급성병색을보였으며, 활력징후는혈압 97/76 mmhg, 맥박 160회 / 분, 호흡 36회 / 분, 체온 38.6 o C이었다.

입원 1일째대변배양이의뢰되었다. 설사변에대해 Seeplex R Diarrhea ACE Detection Kit (Seegene, Seoul, Korea) 로세균성설사의원인균을찾기위해다중 PCR 검사를실시하여 E. coli O157 특이밴드, E. coli H7 특이밴드와 verotoxin 특이밴드에양성결과를얻었다 (Fig. 1).

CRP는 1.16 mg/dl으로약간증가되어있었으며, BUN/Cr은 10/0.3 mg/dl로참고치이내였다. 대변로타바이러스항원검사와기생충충란검사는음성이었다. 입원 1일째대변배양이의뢰되었다. 설사변에대해 Seeplex R Diarrhea ACE Detection Kit (Seegene, Seoul, Korea) 로세균성설사의원인균을찾기위해다중 PCR 검사를실시하여 E. coli O157 특이밴드, E. coli H7 특이밴드와 verotoxin 특이밴드에양성결과를얻었다 (Fig. 1). Salmonella spp., Shigella spp., Campylobacter spp., Clostridium spp. 등에는음성이었다. 대변검체를 sorbitol-macconkey 한천배지에접종하여 35 o C에서하룻밤배양하였을때분홍색집락과투명한집락이섞여자라났다 (Fig. 2). 투명한집락을 MacConkey 한천배지에계대배양했을때대장균속의전형적인분홍색집락으로자랐고, oxidase 음성이었으며, 도말염색에서그람음성의통통한막대균이관찰되었다. 이균은 MicroScan NegCombo 44 패널 (Siemens, West Sacramento, CA) 에서 E. coli로동정되었고, amikacin, aztreonam, ceftriaxone, ceftazidime, cefazolin, ciprofloxacin, cefepime, cefuroxime, cefotetan, imipenem, levofloxacin, meropenem, moxifloxacin, piperacillin/tazobactam, tobramycin 등에감수성, ampicillin, ampicillin/sulbactam, trimethoprim/sulfamethoxazole 등에내성이었다. 이균을질병관리본부에서공급하는 O157, O26, O111 항혈청 ( 중겸, 안산, 한국 ) 으로검사하였을때 O157 항혈청에만응집반응이일어났다. Verotoxin 검사를위해대변검체를 tryptic soy broth (TSB) 와 mitomycin C를첨가한 tryptic Fig. 2. TSB 와 TSB-M에서얻은균액으로 RIDASCREEN R verotoxin enzyme immunoassay (R-Biopharm, Darmstadt, Germany) 를시행한결과, TSB-M 에서얻은균액만 verotoxin이양성이었다. 이상의검사결과를종합하여 E. coli O157:H7 에의한출혈성장염으로최종진단하였다. 환아는입원당일 E. coli O157:H7 장염으로진단하여항균제치료를하루만에중단하고대증요법을시행하였으며, 입원 2일째부터열이떨어지고설사등의증상은호전되었다. 입원 2일째와 4일째동일한방법으로대변배양과독소검사를실시하였으나더이상균이배양되지않고 verotoxin 효소면역검사에서도음성이었다. 환아의가족으로부모와 3세된누나가있었으며모두장염증상은없었다. 냉장고에보관되어있었던환아의이유식재료인흰쌀죽, 양배추, 당근, 소고기간것을수거하여재료를분쇄한후 TSB-M에증균하고, sorbitol-macconkey agar에접종하여배양하였으나모두음성이었다.

3 Hae-Sun Cho, et al. : Verotoxin-producing E. coli in an Infant 87 고찰본증례는 9개월된남아로우유와이유식으로만식이를하고있었다. 연령별분리율이 5 9세는 0.9% 인데비해영아의경우 0.06% 으로 5 9세소아연령이 EHEC의호발연령임에비해영아에서의발병은매우드물다 [5]. 국내에서는 30일된영아에서감염된증례가유일하다 [12]. 본증례에서 EHEC가발생한역학적원인을찾고자이유식재료를검사하였으나, 모두음성으로이유식이전파원이라는증거는찾지못했다. EHEC 는소나양같은반추동물의장에서식하며사람에서는덜익힌고기, 멸균하지않은우유나유제품, 균에오염된과일이나채소, 물을섭취함으로써감염이발생할수있다 [13,14]. 그러나 EHEC는 개의균만있어도감염이가능하기때문에사람대사람전파도가능하여단기간에확산되어유행을일으킬수도있다 [1,15]. 따라서본증례에서음식외의전파원으로환아의가족들을생각해볼수있다. 증상이있었던가족은없었으나, 균배양을실시하지는못했기때문에가족들중무증상감염자를통해서전파되었을가능성은배제할수없다. 소아연령, 특히 4세이하에서는무증상일때도 53% 에서 3주이상균이배출된다는보고가있다 [16]. 영아들처럼식이가제한되어있는경우에는가족들로부터의전파가중요한역할을할것으로추측할수있다. 국내에서 2004년광주에서 EHEC의집단감염이발생했을때최초발현자 2명을제외하고는나머지 72명모두무증상감염이었던사례가있어서 [10] 무증상감염이의외로흔함을알수있다. 국내에서산발적으로보고된 EHEC 장염증례들중전파원인이규명된경우는없었으며, 유일하게영아에서발생한 EHEC 설사환자 1명또한원인을밝히지못했다 [12,17]. 본증례는설사변에에서직접핵산을추출한후여러가지세균성설사의원인균을검출하기위한다중 PCR 검사를적용함으로써발견되었다. 국내한대학병원에서 1990년에서 1996 년사이설사환자에서분리된대장균 7주에대해 stx1과 stx2를다중 PCR로검사하여모두 stx1만양성이고혈청형도다양하여국내에서 EHEC를진단하려면 verotoxin 을세포독성검사나유전자를검출하는검사가필요함을보여주었다 [18]. 혈성설사변에서직접 stx1과 stx2를검출하는 PCR 검사를실시해서진단한방법은 1998년처음소개되었고 [19], 국내에서도 1999 년소아환자의설사변을증균한다음적용하여 O157:H7 EHEC를처음검출하였다 [20]. E. coli O157:H7은 sorbitol 발효음성인특성이있어서선택적으로분리배양할수있지만 [20] 그외의 EHEC는배양에서검출하기가어렵다. 특히국내에서는 O26, O91, O111 등다른혈청형이더우세해서배양으로검출하기힘들다 [9,10,18]. E. coli O157:H7이 EHEC의대부분을차지하는미국에서 EHEC 감염이매년인구 10만명당 8명정 도로보고되는데 [14] 비해국내에서 EHEC가드문것은진단율이낮은데기인할수있다. 초반에 EHEC 감염임에도비혈성설사가있는경우에는다른감염성설사와구분하기어려워 EHEC가감별진단에포함되지않을수있다. 따라서직접검체에서여러가지설사원인균을감별할수있는다중 PCR 검사를사용하면 EHEC 진단율을높일수있을것이다 [21]. EHEC의증상이호전된뒤에도대변을통해 E. coli O157:H7 이배출될수있다고하지만 [22], 급성설사가일어나는동안균의전파력이가장크다고알려져있다 [7]. E. coli O157은증상발현후 2일이내에배양을할경우에대부분검출할수있었지만, 1주일이경과한후에는약 1/3에서만검출되었다는보고가있다 [23]. 본증례에서도환아가입원한첫날접수된대변검체에서 E. coli O157:H7이검출되었지만, 증상이호전되기시작한그다음날부터더이상검출되지않았다. 이처럼균이검출될수있는기간이짧을수있어서출혈성장염환자에서 EHEC를검출하는면역학적또는분자생물학적독소검사나배양을증상발현초기에실시하는것이진단율을높일것이다 [14]. 본증례에서환아의검체를 TSB-M 배지에증균한데서 verotoxin이검출되었다. TSB 증균에서는독소검출에실패하고, TSB-M 증균한것에서만독소가검출된것은 mitomycin C 가 EHEC의 toxin 생성을촉진시키기때문이다. 이에대한명확한기전은알려져있지않지만 mitomycin C가박테리오파지증식을촉진해서세포당독소유전자수를증가시키기때문으로추정한다 [24]. 본증례는 multiplex PCR을사용하여처음진단하였기때문에다른확인검사가필요하였다. 또한 O157:H7 형이아닌 EHEC 감염에서 stx만검출한다면 Shigella 감염과감별이안되기때문에확진을위해분리주에서독소를검출하는것이필요할것이다. Mitomycin C가첨가된배지에서증균하여 verotoxin 을효소면역법으로검출하는방법은 EHEC 장염을진단하는데민감도를높일수있는방법으로생각되었다 [25]. EHEC 감염증의 5 15% 정도에서 HUS가발생한다고알려져있다. E. coli O157:H7 감염, 5세미만의영아에서 EHEC가발병한경우, 백혈구수의증가, 항균제와장운동억제제의사용등이 HUS의발생위험을높인다 [7]. 본증례는입원당일대변검사에서 EHEC를진단함으로써항균제사용을하루만에중단하고대증요법으로만치료하였고, HUS가발병하지않았다. 이처럼조기에감염성설사의원인균을규명하는것이설사환자의치료와예후에결정적인역할을한다. 소아의급성신부전의가장중요한원인중하나는 HUS이며, 국내연구에서 HUS 의 71.8% 가 5세미만에서발병하고, 설사가선행되는경우가 70.2% 로가장흔한전구증상이었다 [26]. 따라서소아연령에서혈성설사가발생했을때 EHEC에의한설사를감별해주는것이필요하다. 본증례는국내에서최초로영아의설사변에직접다중 PCR

4 88 Korean J Clin Microbiol 2010;13(2):85-89 를적용하여 verotoxin 양성 E. coli O157:H7에의한 EHEC 감염을진단하였다. 효소면역법으로 verotoxin을검출하였고, E. coli O157:H7을배양에서분리함으로써 EHEC 감염으로확진할수있었다. 향후감염성설사의다양한원인균을한번에선별할수있는분자생물학적검사법이설사원인의감별진단과치료에유용할것이다. 참고문헌 1.
