Authors performed conservative treatment for displaced distal femoral epiphyseal fracture detected on 5th day after cesarean section delivery. At 10 years of follow-up, satisfactory functional recovery was observed. Herein, we report on a case of displaced distal femoral epiphyseal birth fracture with literature reviews. Key words: femur, epiphysis, distal, fracture, birth, neonate 분만중발생하는신생아의대퇴골골절은쇄골및상완골골절에비해드물고특히분만중발생한대퇴골원위부의골단판골절은몇차례증례보고정도만있을정도로매우드물게발생한다. 환자가나타내는증상은환부의부종, 발적, 환측하지의부동및운동시의동통등이며단순방사선상에서쉽게발견되지않아세균성골수염이나화농성관절염으로잘못진단될수있다. 1) 대부분의증례보고에서는보존적치료로좋은결과를보고하고있으나치료시기및방법에대한객관적지침은여전히확립되어있지않다. 저자들은제왕절개분만후 5일째발견된대퇴골원위부골단판골절환자에대하여보존적치료를시행하였고 10년간추시관찰한결과를문헌고찰과함께보고하는바이다. Received July 13, 2015 Revised November 10, 2015 Accepted November 16, 2015 Correspondence to: Dong-Ju Shin, M.D. Department of Orthopedic Surgery, Daegu Fatima Hospital, 99 Ayang-ro, Dong-gu, Daegu 41199, Korea TEL: FAX: aabga@hanmail.net 증례보고 재태기간 40주, 몸무게 4,020 g인여아로둔위체위로제왕절개를통해분만하였으며모체는 29세의기저질환이없는건강한초산모였다. 생후 5일경, 우대퇴부의부종이확인되어패혈성관절염의임상적진단하타의료기관에서본원으로전원되었다. 당시시행한이학적검사상열은없었으며우측대퇴부의부종및슬관절의완전신전제한이관찰되었고, 혈액검사상백혈구수치는 /μl ( 정상치 : /μl), C 반응성단백시험 (C-reactive protein) 수치는 0.40 mg/μl ( 정상치 : μl) 로임상적으로의미있는변화를보이지않았다. 우측대퇴골의단순방사선사진에서우측대퇴골원위골단이후방으로전위된 Salter-Harris 1형골절을보였으며 (Fig. 1), 시행한초음파에서는건측에비해약 60 도가량후방으로전위되어있는대퇴골원위골단이관찰되었다 (Fig. 2). 환아의치료로는골절의정복없이하지의거상및솜붕대와압박붕대를이용하여보존적치료를시행하였다. 생후 8일경시행한단순방사선사진에서후방골막반응의소견을보였으며 (Fig. 3), 생후 1개월후단순방사선에서대퇴골간 The Journal of the Korean Orthopaedic Association Volume 51 Number Copyright 2016 by The Korean Orthopaedic Association This is an Open Access article distributed under the terms of the Creative Commons Attribution Non-Commercial License ( which permits unrestricted non-commercial use, distribution, and reproduction in any medium, provided the original work is properly cited.

생후 3년에서 각 변형은 약 20도 정도였으며 생후 10년 최종 추 시에서는 각 변형이 15도 정도로 관측되었고 우측 대퇴골은 건측 에 비해 약 1.

2 256 Young-Soo Byun, et al. 단부 주변으로 신생골 형성이 확인되어 유합된 것으로 판단하였 고 각 변형은 약 60도로 유지되고 있었다(Fig. 4A). 생후 5개월 단순 방사선 사진에서 전방 각 변형은 약 40도(Fig. 4B), 생후 1년 5개월에는 각 변형이 30도(Fig. 4C)로 점진적인 재 형성을 보였다. 생후 3년에서 각 변형은 약 20도 정도였으며 생후 10년 최종 추 시에서는 각 변형이 15도 정도로 관측되었고 우측 대퇴골은 건측 에 비해 약 1.5 cm 정도 단축되어 있었으나 환자의 보행은 정상적 이었으며 일상생활에서 불편감을 호소하지 않았다(Fig. 5). 3 257 Fracture of Distal Epiphysis of Femur during Childbirth 골골절은태아가매우크거나작은경우, 둔위, 외태아회전술 (external version), 난산, 쌍태아, 불완전골형성증을가진환아의경우에서증가한다. 3) 역사적으로대부분질식분만을했을때장골골절의발생이더증가한다고이해해왔으나최근의연구에따르면둔위일경우질식분만보다제왕절개를선호하게되며이경우장골의골절이더자주발생한다고보고되었다. 4) 분만시의대퇴골의골절은특징적으로둔위에서난산중에하지를견인해야할때발행하는것으로알려져있다. 5) 본증례에도태아는둔위였으며제왕절개를통한분만으로수상하였다. 분만중대퇴골의골절은대부분대퇴골간부에발생하며대퇴골원위부골단판골절은매우드물어현재까지몇예의증례보고만이보고되어있다. 1,6-9) 신생아의성장판골절은희귀한손상 Figure 5. Radiographs of the right femur and standing long bone view at 10 years after delivery. 이고단순방사선사진으로진단이어려워종종탈구및패혈성관절염, 골수염으로잘못진단되어치료받기도하였다. 과거에는관절조영술만이추가적으로성장판골절의진단에사용할수있었으나최근에는자기공명영상검사및초음파를이용하여조기에비교적정확한진단을내릴수있게되었다. 9) 본증례에서는단순방사선사진및초음파를이용하여진단을하였으며특히초음파는신생아에게시행하기쉽고안전하다는점에서이점이있다. 분만중에발생한대퇴골원위부골단판골절을보고한대부분의증례보고에서는보존적치료로좋은결과를보고하고있으나 (Table 1) 최초골절의각변형의정도및허용범위, 손상의발견시기, 장기추시등이기술되어있지않아치료방법에대하여객관적지침은여전히확립되어있지않다. Riseborough 등 10) 에의하면신생아의재형성잠재력은뛰어나기때문에대퇴골원위부골단판골절은상당한전위가있어도받아들일수있으며자연적인재형성으로인해변형의교정이예상되면도수정복은필요없다고하였다. Mangurten 등 1) 은생후 22시간에발견된대퇴골원위부골단판골절에대하여골절정복을시도하였고도수정복이실패하자생후 50시간에전신마취하에서도수정복및핀고정술을시행하여좋은결과를보고하며, 분만후조기에발견된불안정성이심한골절에대해서는도수정복및핀고정술을고려해야한다고하였다. 본증례에서는첫진단시약 60도의심한각변형을보여도수정복이필요하다고판단하였으나진단시기가수상후 5일째로도수정복을할경우추가적인성장판손상이우려되어정복술을시행하지않고단순고정으로경과관찰을계획하였다. 보존적치료이후지속적인각변형의재형성을보였으나최종적으로 10 년추시에서도약 15도의각변형, 1.5 cm의단축이남아있었다. 각변형의위치는대퇴골의성장에의해근위부에남아있었으며 Table 1. 4 258 Young-Soo Byun, et al. 기능적으로환자는전혀불편감을호소하지않았다. 이러한결과는 Hägglund 등 5) 이보고한분만중에발생한대퇴골간부골절에대하여보존적치료후 16년동안장기추시한결과와유사하다. 최초 45도의각변형을가진골절은별다른기능소실없이잘치유되었으나최종추시에서 14도의전방각형성을남겼다. 따라서대퇴골의분만골절의보존적치료결과는임상적으로는만족스러우나초기의각변형은완전히재형성되지않을수있음을시사하며비록치료방법에대하여객관적지침은확립되어있지않지만본증례와는달리보다조기에진단이된심한각변형, 전위가동반된골절에대해서는초기에도수정복을시행하면좀더해부학적인정렬로회복될수있을것이라생각하며이에대한좀더많은증례를통한연구가필요하다고생각한다. 본증례에서는분만후 5일째발견된대퇴골원위부골단판분만골절에대하여단순방사선사진및초음파를통해비교적조기에진단을내릴수있었으며수상후 5일이지나비교적늦게발견된골단판골절에대해더이상의성장판손상을피하기위하여도수정복술은시행하지않고보존적치료를시행하였고 10 년간장기추시관찰하여만족할만한각변형의재형성과기능적으로큰문제없이회복된결과를보여문헌고찰과함께보고하는바이다.
