In the present study, the usefulness of QFT-G was evaluated for the diagnosis of extra-pulmonary tuberculosis (EP-TB). Methods: From June 2006 to February 2009, we evaluated the usefulness of QFT-G in patients (n=65) suspected with EP-TB, retrospectively. The diagnostic sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of the QFT-G assay were analyzed. Results: EP-TB was diagnosed in 33 (51%) participants. The overall sensitivity, specificity, PPV, and NPV of the QFT-G assay for EP-TB were 78%, 79%, 81%, and 77%, respectively. Of the 33 with EP-TB, 14 (42%) were diagnosed with TB pleurisy, 7 (21%) with TB lymphadenitis, 7 (21%) with intestinal TB, and 5 (15%) with EP-TB in other sites. In subgroup analyses according by site of infection, the QFT-G showed 86% sensitivity, 64% specificity, and 78% NPV in TB pleurisy. On the other hand, the sensitivity, specificity, and NPV of the assay were 71%, 83% and 71%, respectively in TB lymphadenitis, and 86%, 100% and 88%, respectively in intestinal TB. Among the patients with suspected alternative site EP-TB, the sensitivity, specificity, and NPV of the assay were 50%, 80% and 67%, respectively. Conclusion: The QFT-G assay showed moderate diagnostic accuracy in EP-TB. However, negative QFT-G assay does not exclude EP-TB because of the low NPV of this assay. 2 HM Lee et al: Usefulness of interferon-gamma release assay for extra-pulmonary tuberculosis ency virus, HIV) 감염환자및면역억제제사용환자가늘어남에따라관심이증가하고있다. 폐외결핵은폐이외의모든장기에침범한결핵으로써흉막결핵이가장흔하고, 림프절결핵이두번째이며, 복부결핵, 골관절결핵, 비뇨기계결핵, 신경계결핵의빈도로보고되고있다 3. 폐결핵은객담검사와영상의학적검사등의간단한방법으로진단되는데비해폐외결핵은환자의분비물이나조직을얻기위한침습적인방법이필요한경우가많다. 또한, 임상양상이비전형적인경우가많고, 검체의항산균도말검사와배양검사의민감도가낮으며결핵균배양에는 2 7주정도가소요되어진단에어려움이많다 4,5. 또한, 결핵을진단하는데도움이되는투베르쿨린피부검사 (tuberculin skin test, TST) 는우리나라처럼 bacilli Calmette-Guérin (BCG) 예방접종을필수적으로하는경우투베르쿨린피부검사의특이도에대한제한점이있다 6, 년이후전혈인터페론감마측정법 (interferongamma releasing assay, IGRA) 이결핵을진단하는데사용되고있다 8. 최근에는활동성폐결핵에서유용성에대한연구가보고되었으나 9-11, 폐외결핵에서의진단적유용성에대한연구는많지않으며, 폐외결핵의진단시 IGRA의민감도가 14% 에서 80% 까지다양하게보고되고있다 6,7,10, 이에본연구에서는폐외결핵에서 IGRA의진단적유용성에대해알아보고자하였다. 대상및방법 1. 대상환자 2006년 6월부터 2009년 2월까지중앙대학교병원에서임상적으로폐외결핵이의심되는환자중 IGRA를시행한환자 65명을후향적으로분석하였다. 폐외결핵은감염부위에따라흉막결핵, 장결핵, 림프절결핵, 신경계결핵, 골관절결핵및기타결핵으로나눠살펴보았으며, 각각폐외결핵으로의심되는환자는다음과같이정의하였다. 1) 흉막결핵은흉부방사선촬영에서편측성의흉막액이있고폐렴을의심할경결은관찰되지않으며흉막액의세균배양검사에서음성인경우에흉막결핵을의심하였고, 2) 장결핵은복통, 설사, 발열, 체중감소, 오심등의증상이있고대장내시경소견에서주로회맹부를침범하는궤양또는점막비후가있는경우장결핵을의심하였으며, 3) 림프절결핵은단단하고경계가명확한무통성림프절종창이특징인경우의심하였고, 4) 신경계결핵은두통, 발열, 경부강직등의뇌막자극증상이있으면서, 뇌척수액 검사에서림프구우세를보이는경우신경계결핵을의심하였다. 5) 골관절결핵, 간결핵등의기타폐외결핵으로의심되는경우는전산화단층촬영 (computed tomography, CT) 과같은영상의학적검사에서결핵이의심되었던환자를대상으로하였다. 이환자 65명을대상으로결핵의과거력이있는환자와기저질환 ( 당뇨병, 간경화, 만성신부전, 악성종양, HIV 감염, 면역억제제를최근 3개월이내복용한경우 ) 동반여부에대해분석하였다. 폐외결핵은폐이외의부위에감염된결핵으로폐외결핵의진단은 1) 감염부위의조직에서건락육아종 (caseous granuloma) 이증명된경우, 2) 조직검사에서항산막대균 (acid-fast bacillus) 이발견된경우, 3) 조직배양에서결핵균이증명된경우, 4) 진단적흉강천자를시행하고흉막액이삼출액이며, 아데노신탈아미노효소 (adenosine deaminase, ADA) 40 U/L 이상과림프구 / 호중구비가 0.75 이상일때진단하였다 1,15,16. 흉막결핵을진단할때사용한흉수아데노신탈아미노효소활성도는자동화기기로분석하였다 (model 7180; Hitachi, Tokyo, Japan). 이연구는중앙대학교용산병원의학연구심의위원회승인을받았다. 2. IGRA IGRA는 QuantiFERON-TB Gold (QuantiFERON-TB Gold [QFT-G]; Cellestis, Carnegie, Victoria, Australia) 로시행하였다. QFT-G는헤파린처리된용기에환자의전혈을채취하여배양용기에각각 1 ml씩분주하고각 well에결핵균특이항원 early secreted antigenic target-6 kda (ESAT-6) 과 culture filtrate protein-10 kda (CFP-10), 양성대조항원 (mitogen) 및음성대조항원 (nil) 을첨가한뒤 16 24시간배양한다. 배양후생성되는인터페론감마를효소면역측정법 (enzyme-linked immunosorbent assay, ELISA) 으로측정하여판독하였다. 인터페론감마농도가 ESAT-6 또는 CFP-10에서 0.35 IU/mL 이상일경우양성으로판정하고, 환자의면역상태와관련된양성대조항원이 0.5 IU/mL 이하인경우에는미정 (indeterminate) 인경우로하였다 8, 통계 QFT-G의진단적유용성은민감도, 특이도, 양성예측도, 음성예측도를통하여분석하였다. 양측검정 95% 의신뢰수준은 Wilson score method 을사용하였다. 폐외결핵환자와폐외결핵이아닌환자군간의비교는연속변수의 332

EP-TB: extra-pulmonary tuberculosis; TB: tuberculosis; HIV: human immunodeficiency virus. 경우 Mann-Whitney U-test를, 범주형변수의경우 Fisher s exact test를시행하였다. p값이 0.05 미만일때통계적으로유의하다고판정하였다. 통계분석은 SPSS for windows 12.0 (SPSS Inc., Chicago, IL, USA) 프로그램을이용하였다. 결과 1. 대상환자의특성폐외결핵이의심되어 QFT-G 를시행한환자는 65명이었고그중폐외결핵환자가 33명 (51%), 폐외결핵이아닌환자가 32명 (49%) 이었다. 전체환자의나이의중앙값은 46세 ( 범위 13 98세 ) 이고남자가 38명 (59%) 이었으며, 중앙추적관찰기간은 6개월 ( 범위 ) 이었다. 결핵치료의과거력이있는환자는 7명 (11%) 이었으며기저질환을가진환자는중복된질환을포함하여 15명 (23%) 이었다. 폐외결핵환자와폐외결핵이아닌환자군사이에나이, 성별, 추적관찰기간, 결핵치료의과거력, 면역억제질환의동반여부는차이가없었다 (Table 1). 폐외결핵으로진단된 33명의감염부위별빈도는흉막결핵 14명 (42%), 장결핵 7명 (21%), 림프절결핵 7명 (21%) 및기타폐외결핵의순서였다. 폐외결핵이아닌 32명의감염부위별질환은흉막질환 12명 (38%) [ 농흉 2명, 부폐렴성흉수 3명, 악성흉수 3명, 심부전에동반된흉막액 2명, 흉막유착 1명, 베게너씨육아종 (Wegener s granulomatosis) 1명 ], 장질환 7명 (22%) [ 크론씨병 (Crohn s Table 2. EP-TB: extra-pulmonary tuberculosis. disease) 5명, 장염 2명 ], 림프절질환 6명 (19%) [ 기쿠치씨병 (Kikuchi s disease) 4명, 사르코이드증 (sarcoidosis) 1 명, 림프종 1명 ], 기타질환 7명 [ 수막염 6명 (19%), 간세포성암 (hepatocellular carcinoma) 에동반된악성복수 1명 (3%)] 이었다 (Table 2). 2. QuantiFERON-TB GOLD 의진단적유용성 폐외결핵이의심되었던환자에서 QFT-G양성인환자 31명 (48%) 중폐외결핵으로진단된환자는 25명 (81%) 이었으며, QFT-G 음성인환자 30명 (46%) 중폐외결핵은 7명 (23%) 이었다. QFT-G결과가미정 (indeterminate) 인 4 명 (6%) 을제외하고, 폐외결핵을진단하는데 QFT-G 의민감도, 특이도, 양성예측도, 음성예측도는각각 78%, 79%, 333

QFT-G 검사가음성이었던 9명 (33%) 의환자중폐외결핵은 2명이고, 미정 1명은폐암에동반된악성흉수로진단되어, QFT-G 의민감도, 특이도, 양성예측도, 음성예측도는각각 86%, 64%, 75%, 78% 이었다. 림프절결핵으로의심된환자 13명중 7명 (54%) 에서림프절결핵으로진단되었다. QFT-G 양성인 6명 (46%) 중 5명은림프절결핵으로진단되었고, 1명은기쿠치씨병으로진단되었다. QFT-G 음성인환자 7명중 2명 (29%) 이림프절결핵으로진단되어 QFT-G의민감도, 특이도, 양성예측도, 음성예측도는각각 71%, 83%, 83%, 71% 였다. 장결핵으로의심된환자는 14명이었으며, 그중 7명 (50%) 이장결핵으로진단되었다. 장결핵이의심된환자 14명중 QFT-G 양성 6명 (43%) 은모두장결핵으로진단되었고, QFT-G 음성인환자 8명중 1명이장결핵으로진단되어장결핵에서 QFT-G의민감도, 특이도, 양성예측도, 음성예측도는각각 86%, 100%, 100%, 88% 였다. 기타폐외결핵질환으로의심된경우는신경계결핵, 골관절결핵으로분류되었으며기타폐외결핵으로의심된환자 12명중폐외결핵으로진단된환자는 5명 (42%), 다른질환으로진단된환자는 7명 (22%) 이었다. 기타폐외결핵으로의심되었던환자 12명중 QFT-G 양성인환자 3명중 2명 (67%) 이폐외결핵으로진단되었으며, QFT-G 음성인환자 6명중 2명 (33%) 이폐외결핵으로진단되었고, 미정이 3명 (25%) 이었다. 기타폐외결핵에서민감도, Table 4. QuantiFERON-TB GOLD 위음성의위험인자 폐외결핵으로진단된 33명중 7명 (21%) 이위음성의소견을보이고있었다. QFT-G 양성인환자와 QFT-G 위음성인환자에서나이, 성별, 면역억제질환의빈도그리고폐외결핵의감염부위는통계학적으로유의한차이를보이지않았다 (Table 4). 334

No. 4, Oct 고찰본연구의결과를보면, QFT-G 검사법은폐외결핵이의심되는환자에서중등도의정확도를 ( 민감도 78%, 특이도 79%) 보임을알수있었다. 하지만 77% 의음성예측도를보이고있어, 이검사법을폐외결핵을배제하는데사용하는것에는제한점이있음을알수있다. 우리나라에서결핵신고신환자중폐외결핵은 14% 빈도로보고되고있다 2,16. 또한스테로이드등의기존면역억제제외에종양괴사인자 (tumor necrosis factor-α) 에대한단일세포항체 (monoclonal antibody) 제제가류머티즘, 크론씨병등의치료에사용되면서폐외결핵의빈도가증가되고있다 17. 따라서이런면역억제환자에서다른질환으로오인되거나진단이늦어지면치명적이거나심각한후유증을초래하기도한다 18. QFT-G 검사는환자의전혈과결핵특이적인항원 (ESAT-6, CFP-10) 을이용하여결핵에감염되어있는지를검사하는방법이다 19. 이검사법은투베르쿨린피부검사와달리 BCG 접종을하는군에서위양성률이낮은장점때문에우리나라와같이 BCG 접종을하는나라에서도결핵의감염을진단하는데도움이될것으로판단되고있다 20,21. 최근에는활동성폐결핵에서이검사법의유용성에대한보고들이있다 10,22. 하지만, 잠복결핵과현증결핵을감별할수없다는근본적인문제점으로인하여제한점이있다. 그러나 IGRA와 TST를병합하였을때의민감도가 99% 로높아서두검사모두음성이면결핵을배제하는데에는도움이될것으로판단되고있다 22. IGRA의폐외결핵에대한보고는하위집단분석을통한연구가대부분이었다 7,10,12,23. 최근에 Song 등 14 은 100 명의폐외결핵환자만을대상으로하여진단율을보고하였다. 이연구에서민감도와특이도는 69% 와 82% 로중등도의정확도를보였으나 78% 의낮은음성예측도를보이고있었다. IGRA가잠복결핵과현증결핵을감별할수없다는문제점을감안할때이검사의유용성은결핵을배제할수있는가가중요하다고판단된다. 하지만, 이결과를바탕으로생각하면 IGRA가음성이라고하더라도결핵을배제하기는어려움을알수있다. 그러나이연구는다양한폐외결핵중림프절결핵과척추결핵만을대상으로하였기때문에모든폐외결핵에적용하기는어려움이있을수있다. 따라서본연구에서는다양한형태의폐외결핵에서이검사의정확성을보고자하였다. 본연구의결과를살펴보면 QFT-G의민감도는 78%, 음성예측도는 77% 로 QFT-G 결과로결핵을예측하거나, 배제하는것은어려움을알수있었다. 또한감염부위로나누어살펴보았을때에도음성예측도는 67 88% 로감염부위에상관없이낮은음성예측도를보이고있었다. 최근미국질병관리본부의발표에따르면 QFT-G 음성이활동성폐결핵을배제할수없다고하였다 24. 본연구결과를보아도폐외결핵이의심되는환자에서이검사법을이용하여결핵을배제하기는어려울것으로판단된다. QFT-G 검사의위음성은면역억제환자에서흔하게나타난다고알려져있다 9,25. 본연구에서도위음성의위험인자를알아보기위하여위음성과양성인군을비교하여보았으나두군간에면역억제환자의빈도그리고폐외결핵의감염부위의차이를발견할수는없었다. 따라서본연구의결과를보면면역상태와상관없이일부의환자에서는폐외결핵에서도위음성의결과가나타남을알수있었고, 이러한결과는폐외결핵의진단에있어서면역상태와무관하게 QFT-G가음성이라고하더라도폐외결핵을배제해서는안됨을알수있었다. 폐외결핵에서의다양한보고가있지만장결핵에대한결과는많이보고되지않았다. 폐외결핵중큰비중을차지하는장결핵은다른장질환과비교할때증상이큰차이가없다. 또한, 장결핵은염증성장질환과내시경소견이유사하여감별이어려운경우가많다. 장결핵과크론씨병을감별하는것이중요한데만약, 장결핵인데크론씨병으로오진한경우면역억제치료후장결핵이전신감염으로악화될가능성이있기때문에정확한진단이필수이다 26. 확진은대장내시경또는조직학적또는조직검사의배양으로진단할수있지만, 미생물학적진단은긴잠복기로진단에어려우며, 조직검사에서도감별이어려운경우가많이있다. 본연구에서비록환자수가많지는않았으나장결핵이의심되었던 14명에서음성예측도는 88% 로내시경적소견이장결핵이의심될때비록 QFT-G 검사가음성이더라도장결핵을배제하기는어려움을알수있었으며, 특히우리나라와같이잠복결핵의빈도가높은경우에낮은민감도로인하여진단에제한점이있을것으로판단된다. 우리연구에는한계점이있다. 첫번째, 본연구에서는 TST에대한비교분석이없다. TST와 IGRA를같이사용하였을때결핵을배제할수있다는보고가있듯이 22, 두검사를같이시행하여비교하여야하나후향적인연구로이에대한분석이없다는것이가장큰제한점으로생각된다. 두번째, 이연구는후향적인연구로모든폐외결핵이 335

6 HM Lee et al: Usefulness of interferon-gamma release assay for extra-pulmonary tuberculosis 의심되는환자에서 QFT-G 검사가시행되지않았다. 따라서후향적연구에따른선택적비뚤림이발생하였을가능성이있다. 세번째로, 본연구에서는전혈인터페론감마검사로 QFT-G를사용하였으며, T-SPOT.TB (Oxford Immunotec, Oxford, UK) 이나 QuantiFERON-Gold In- Tube와비교하지않았으므로본연구결과가모든 IGRA 의유용성을반영하지는않을것으로판단된다. 마지막으로작은대상군또한중요한제한점으로판단된다. 하지만, 이러한제한점에도불구하고이번연구는다양한폐외결핵의진단에있어 QFT-G의유용성을본논문으로의미가있을것으로판단된다. 결론적으로, QFT-G 는폐외결핵에서중등도의민감도와특이도를보이고있지만폐외결핵의감염부위별로 67 88% 의음성예측도로인하여 QFT-G검사에서음성의결과를보이더라도폐외결핵을배제하기는힘들것으로판단된다. 참고문헌 1.
