1 KISEP Abstract 산탄총에의한두경부손상 1 례 이대원 김종규 김형진 최득록 A Case of Shotgun Wound on Head and Neck Dae Won Lee, M.D., Jong Kyu Kim, M.D., Hyung Jin Kim, M.D., Deug Rok Choi, M.D. Department of Otolaryngology, Hanil General Hospital, Seoul, Korea Shotgun injury compared to gunshot injury has been reported to have tendency to make less severe damage due to its characteristic low velocity and fragmentation. Recently, we encountered with a patient who survived from shotgun injury. She shot herself for the purpose of suicide with a hunting gun. The multiple fragmented bullet particles were found on the nasal septum, inferior turbinate, near the sphenoid sinus, and especially in the esophagus. They were removed safely with nasal endoscope, C-arm, and esophagoscope. We report this case with review of literature. 격 중앙부위, 하비갑개 후하부위)을 제거하였으며 접 형동 전면에 박혀있는 납탄 파편이 발견되었으나 출 혈이 더욱 심하여질 것으로 판단되어 제거하지는 않 았으며 지혈목적으로 좌측 비인강 부위에 후방팩킹을 시행하였다. 좌측 연구개에 박혀있는 다른 파편을 제 거하였고 설저부의 창상을 봉합하였다.

2차수술(97년 5월 20일) 잔여 파편의 정확한 위 치를 알기위해 부비동 전산단층촬영을 시행한 후(Fig. 4) 전신마취하에 비내시경과 C-arm을 이용하여 접 형동주변과 하비갑개, 연구개에 남아있던 납탄 파편을 게거 하였으며 연구개부위의 창상을 봉합하였다.

There is no atelectactic finding on both lung field, so it is estimated esophageal foreign body. 격 중앙부위, 하비갑개 후하부위)을 제거하였으며 접 형동 전면에 박혀있는 납탄 파편이 발견되었으나 출 혈이 더욱 심하여질 것으로 판단되어 제거하지는 않 았으며 지혈목적으로 좌측 비인강 부위에 후방팩킹을 시행하였다. 좌측 연구개에 박혀있는 다른 파편을 제 거하였고 설저부의 창상을 봉합하였다. 그후 식도경하 에서 식도내 이물을 제거하였다(상악절치로부터 27cm 에 위치). 제거한 이물은 납탄파편이었으며 통과경로인 구인두, 하인두 및 식도 등의 점막손상은 보이지 않았 다. 5월 17일 후방팩킹을 제거하였으며 출혈은 없었으 나 혈색소치가 9.4로 전혈 2pint 수혈을 시행하였다. 2차수술(97년 5월 20일) 잔여 파편의 정확한 위 치를 알기위해 부비동 전산단층촬영을 시행한 후(Fig. 4) 전신마취하에 비내시경과 C-arm을 이용하여 접 형동주변과 하비갑개, 연구개에 남아있던 납탄 파편을 게거 하였으며 연구개부위의 창상을 봉합하였다. 수술후 경과 5월 25일 퇴원 후 외래 경과 관찰중 수상시 있었던 개구장애가 서서히 개선되었으며, 연구 개 봉합부위의 일부가 결손되어 유동식의 역류가 있 Fig. 2. Skull AP view showing multiple metallic foreign body shadows. 변과 상악 치조골 주변에 다수의 금속성 이물 음영이 었으나 재봉합하여 해결되었으며, 수상 2개월후 시행 한 혈중 납농도는 12.8ug/dl(정상치 <10ug/dl)이었 고 말초혈액도말검사 소견은 정상소견을 보였다. 보였고(Fig. 2), 단순흉부 X-선 촬영에서 금속성 이 고 물 음영이 식도부위에 관찰되었다(Fig. 3). 찰 수술소견 및 경과 1차수술(97년 5월 13일) 전신 마취하에 비내시경을 이용해 좌측 비강내 이물(비중 비강을 제외한 안면부 이물의 원인은 대부분 총기, 1687
