Materials and MethodNine symptomatic patients 7 males and 2 females, aged 23 to 66 years with nasal septal perforation were included in this study. They were all refractory to conservative treatments and they underwent surgery. Preoperative symptoms, etiology, size and location of septal perforations, and postoperative improvement of symptoms and perforations were retrospectively evaluated with a mean follow-up period of 2.7 months. ResultsPreoperative symptoms included nasal obstruction, crust formation, rhinorrhea, epistaxis and foul odor. Septal perforation was assumed to have resulted in three patients from septal surgery, trauma and idiopathic causes, respectively. In all patients, the perforation size was 20 mm or less mean mm in diameter and located at the cartilagenous portion, not near the nasal dorsum. The perforations were completely repaired in 6 of 9 patients after surgery. Perforations of and 3 mm in diameter remained in 2 and of the 3 patients showing incomplete closure, respectively. However, clinical symptoms disappeared in 8 of % patients, postoperatively. ConclusionThis technique may be an effective method for the repair of nasal septal perforation when the perforation is not located near the nasal dorsum and that its size is 20 mm or less in diameter, as the technique offers a cosmetic advantage and produces a satisfactory postoperative outcome by obviating difficulty in intranasal suture. 3 홍순관 외 중격만곡이 있는 경우에는 오목한 면에 반관통절개를 하여 위치하게 하고 반대쪽 비강으로 이개연골막 면이 위치하도 접근하는 것이 편리하고, 또한 그 만곡이 교정을 필요로 할 록 한다. 비중격천공 부위에 이식편이 올바르게 위치하게 한 정도인 경우 다음 과정 전에 통상적인 비중격성형술을 하여 후 비배부 바깥으로 꺼낸 두 개의 실을 서로 묶어 비배부에 만곡을 교정하는 것이 좋다. 비중격 점막을 박리할 때 천공 고정한다. 이때 완충 목적으로 실과 비배부 피부 사이에 작 변연부에 육아 또는 반흔 조직이 있을 경우에는 그 부분을 은 거즈를 넣는다(Fig. 2). 박리된 비중격 점막을 원래 자리 다듬는다(trimming). 비중격천공 부위의 연골 결손 크기에 로 위치시킨 후 5-0 vicryl 봉합사로 반관통절개를 봉합하 맞추어 이미 준비된 이개연골편을 적절히 다듬고 측두근막 고 비중격 양측에 silastic 판(두께 0.02인치)을 고정 봉합 도 그 가장자리를 부드럽게 다듬은 후 5-0 vicryl 봉합사로 시킨 다음 통상적인 비강 패킹을 한다. 이개연골편을 측두근막 중심부에 고정시켜 자유 복합 이식 술 후 4주간 항생제(levofloxacin 수술 당일은 정맥주사 편을 완성한다(Fig. ). 이때 이개연골편의 연골막 없는 면 로 750 mg/day, 그 다음날부터 첫 2주간은 경구용 제재로 이 측두근막 쪽에 위치하도록 봉합한다. 양쪽으로 곧은 바늘 600 mg/day, 다음 2주간은 경구용 제재로 300 mg/day)와 이 달린 4-0 나일론 봉합사를 자유 복합 이식편의 측두근 비점막혈관 수축제(pseudoephedrine 80 mg/day)를 투 막 한쪽에 봉합하는데, 이때 양쪽으로 실의 길이가 비슷하게 여하고 비강 패킹은 수술 3일 후, silastic 판은 3주 후에 제 되도록 한다. 박리된 비중격 점막 안으로 바늘을 넣은 후 비 거한다. 이개 주변의 압박 붕대는 수술 2일 후, 배관은 2 3 배부를 향해 관통시켜 바늘을 비배부 바깥으로 꺼내고 다른 일 후, 두피와 이개 후면의 절개선 봉합사는 7일 후 제거한 하나의 바늘도 처음 바늘 약간 옆으로 하여 같은 방법으로 다. 비배부의 고정 봉합사는 수술 7일 후 제거하는데 비배부 관통시킨다. 충분히 지혈을 하여 좋은 시야를 확보한 후, 두 겉으로 나온 부분을 살짝 잡아당기면서 가위로 피부를 부드 개의 바늘에 달린 두 개의 실을 비배부 바깥으로 잡아당기 럽게 누르면서 봉합사를 절단하면 봉합사 아래 부분이 피부 면서 박리된 비중격 점막 안으로 자유 복합 이식편을 집어 밑으로 들어가 봉합사 끝이 피부 겉으로 보이지 않게 된다. 넣는다. 이때 접근한 비강 쪽으로 이식편의 측두근막 면이 주일에 약 2회, 4주간 외래 통원 치료를 하고 그 이후 는 치료 경과에 따라 통원 간격을 늘린다. 결 과 수술 전 증상으로는 비폐쇄(9명), 가피 형성(5명), 비루 (2명), 비출혈(2명), 악취감(명)이 있었고 천공의 원인으 로는 비중격수술(3명 비중격 점막하절제술 2명, 비중격성 형술 명), 비중격 외상(3명 비배부 가격에 의한 외상 명, 빈번한 코후빔 명, 비출혈에 대한 전기소작 명), 원인불 명(3명)이었다(Table ). 천공의 크기는 장경이 5 20(평 균 2.2) mm, 단경이 5 5(평균 8.4) mm이었는데 9명 중 5명에서는 0 0 mm 이하였고 4명에서는 0 0 mm보다는 컸으나 mm 이하였다. 천공은 9명 모두 에서 비중격 연골부에 위치하였는데 6명에서는 연골부 중 후방에, 2명에서는 하방에, 명에서는 전방에 위치하였다(Fig. Table.
