Although most head and neck defects can be reconstructed using a single free tissue transfer, the use of two free flap is indicated for massive tissue defects or for composite tissue requirement that cannot be met with a single free flap. In this paper, we report two patients with composite defects. They underwent single-stage, double free flap reconstruction utilizing a fibular osteoseptocutaneous flap to reconstruct the mandible and floor of the mouth, as well as an innervated radial forearm fasciocutaneous composite flap with the palmaris longus tendon and the latissimus dorsi free flap for the total tongue reconstruction. Reconstruction state using radial forearm free flap and fibular osteoseptocutaneous flap of postoperative 5 months after operation. 골동맥의 근위부는 다시 경부의 상갑상동맥에 연결하였다. 정 맥은 전완의 요골쪽 피부정맥(cephalic vein)을 비골정맥에 리피판으로 재건된 혀의 모양도 위축이 없이 유지하고 있을 연결하고 비골정맥의 근위부를 외경정맥에 연결하였다. 신 뿐 아니라, 기능적으로도 발음 장애는 있으나 연하장애나 경은 전완의 외측 아래팔피부신경(lat. antebrachial nerve) 흡인은 거의 없이 생활하고 있다(Fig. 4). 를 설신경에 연결하였다(Fig. 3). 술 후 2일째 설부 피판이 울혈이 생겨 재수술을 시행하 증 례 2 였다. 수술 중 소견은 요골쪽 피부정맥(cephalic vein)에 혈 36세 남자환자로 좌측 구강저부암(T2N2M0)으로 본원 전이 관찰되어, 혈전을 제거한 후 요골쪽 피부정맥(cephalic 에서 설유리접근법을 이용하여 구강저와 설부조직을 절제하 vein)을 좌측 외경정맥에 비골정맥을 우측 외경정맥에 연결 고 양측 경부청소술을 시행한 후 요골 전완유리피판술을 시 하였다. 행하고 방사선치료 시행 후 퇴원하였다. 외래에서 경과관찰 술 후 1개월부터 약간의 흡인은 있지만 유동식의 섭취가 중 술 후 6개월 째 우측 설 경계부에 재발소견이 나타나 설부 가능하였고 술 후 2개월부터는 특별한 연하곤란이나 기도흡 전체를 절제하고 우측 경부청소술과 우측 하악골 분절절제술 인없이 고형식도 섭취가능하였고 술 후 18개월이 지난 현재 (segmental mandibulectomy)을 함께 시행하였고(Fig. 5) 재건된 하악의 골융합에 문제점이 없으며 전완부의 감각 유 결손부위에 대해서는 역시 두 개의 유리피판을 연결하여 재 776 Korean J Otolaryngol 2001 ;44 :775-9

술 후 환자는 턱밑에 창상감염으로 인해 구강 섭취가 늦어 Fig. 6. Case 2.

Harvested double free flap using latissimus dorsi myocutaneous flap and fibular osteoseptocutaneous flap. 졌으나 술 후 45일째부터 유동식섭취가 가능하였다(Fig. 8). 술후 12개월째인 현재까지 환자는 고형식섭취가 불가능하고 발음장애가 남아있는 상태지만 재발소견없이 지내고 있다. 건하였다. 광배근피판을 이용해서 설부를 재건하고 하악골 부위는 비골 유리피판을 이용하여 재건하였다. 혈관연결은 고 찰 동맥은 광배근피판의 흉배동맥을 비골동맥의 원위부에 연결 하고(Fig. 6), 비골동맥의 근위부는 경부의 횡경동맥에 연결 진행된 병기의 구강암은 대부분 수술과 함께 술 전 혹은 하였다. 정맥은 광배근피판의 흉배정맥의 근위부를 비골정 술 후 방사선치료와 필요시 항암치료 등을 병합하게 된다. 777
