It is an important pathway for anterior drainage which can cause inflammation in obstructed conditionstherefore, it needs appropriate procedure to eradicate the lesions without recurrence. We aimed to classify types of the anterior attic bony plates on the temporal bone CT and to find correlation between the types of the anterior attic bony plates and the development of chronic otitis media and cholesteatoma. Materials and MethodsWe reviewed 264 cases of the temporal bone CT of the normal and the chronic otitis media patients, of which 64 cases were excluded as lesions could not be identified due to severe destruction and obscurity of the figure. ResultsIn normal ears, the incidence of the anterior attic bony plate was 67% in the plate type, 28% in the pyramid type, and 5% in the anterior type. In the diseased ear, the incidences were 40%, 44%, 16%, in the plate type, pyramid type, and anterior type, respectively and in the cholesteatoma ear, 30%, 43%, 27%, respectively. The difference of incidence was statistically significant. ConclusionWe assumed that the development of the chronic otitis media is related to the type of the anterior attic bony plate in some degree. Especially, the anterior type was more prominent in cholesteatoma than in noncholesteatoma with a statistical significance. Further studies are needed to figure out the correlation between the type of the anterior attic bony plate and it s clinical importance. 2 이철희 외 고실 골판의 형태를 분류하고 정상인에서의 분포 및 만성 중 평선에서 두위를 30 숙인 축상면에서 관찰하였고 1 mm 간 이염군 사이의 분포를 알아 봄으로써 전 상고실 골판의 형태 격으로 촬영하였으며 전형적인 supratubal recess가 잘 관찰 와 특징 및 그에 따른 병변 발생의 상관 관계에 의미가 있는 되는 안면신경의 슬신경절 부위에서 cog 형태를 결정하였다. 지를 알아보고자 하였다. 통계학적 분류로는 Chi-square 분석을 사용하였다. 대상 및 방법 결 과 1995년 3월부터 2000년 1월까지 경북대학교 병원 이비 측두골 CT에 나타나는 각 분류별 특징을 보면 판형은 두 인후과에서 만성 중이염으로 유양동 삭개술을 시행 받은 환 께가 얇고 긴 골판이 있고 supratubal recess가 잘 발달되 자 및 정상인을 대상으로 하여 임의로 대상을 선정 후 측두골 어 있는 것이 특징이며(Fig. 1) 피라미드형은 두터운 골이 피 CT를 분석하여 진주종 및 비진주종성 중이염등 병변이 있는 라미드형태로 있는 것이 관찰되며 supratubal recess는 작 귀와 병변 반대측 귀 및 병변이 없는 정상인의 귀를 분류하 거나 거의 발달되어 있지 않다(Fig. 2). 전방형은 상대적으로 고 서로 비교하였다. 길이가 짧고 판형과 피라미드형과는 달리 추골 두부의 전방 132명의 측두골 CT(264측의 귀)를 분석하였으며 그 중 이 아니라 전하방에 위치하며 본 연구에서 분류에 사용된 축 만성 중이염으로 인한 골 파괴로 분류가 불가능하였던 64예 상면 CT에서는 축상면에 평행하게 위치하여 형태가 관찰되 를 제외하고 200예의 측두골 CT를 분석하여 만성 중이염이 지 않았다(Fig. 3). 있는 100예와 병변이 없는 100예를 대상으로 하였다. 중이 100예의 병변이 없는 군 내에서 비교한 결과 어느 쪽도 병 염이 있는 군 100예 중 비진주종성 중이염의 경우가 67예, 변이 없던 군(18예)과 만성 중이염 반대측 군(82예) 사이의 진주종성 중이염의 경우가 33예였으며 병변이 없는 100예 중 cog 분포는 판, 피라미드, 전방형이 각각 67%, 28%, 5%와 양측 어느 쪽에도 만성 중이염 병변이 없는 정상 군이 18예 66%, 28%, 6%로 서로 유사하여 차이를 알 수 없었고 병변 였고 만성 중이염의 반대측 귀가 82예였다(Table 1). 이 없는 군과 만성 중이염 군을 비교해 본 결과 병변이 없는 저자들은 T. This type of the anterior attic bony plate is not visible on axial CT. 형의 분포가 적었고 피라미드형과 전방형의 분포가 상대적으 로 많았으며 만성 중이염 군과 병변이 없는 군간의 분포형태 의 차이는 통계학적으로 유의하였다(p<0.01)(Fig. 4). 만성 중이염 군 사이에서 비진주종성 중이염 군과 진주종 성 중이염 군을 비교해 본 결과 비 진주종성 중이염 군(67 예)에서는 각각 45%, 45%, 10%로 분포하였으며 진주종성 중이염 군(33예)에서는 각각 30%, 43%, 27%로 나타나 중 이염 군내에서도 진주종이 있을 경우에는 판형이 더욱 적었 고 전방형이 더욱 많은 분포를 보였으며 이 두 군간의 분포 Fig. 4. Distribution of cog in Nondiseased group, COM group, COM without cholesteatoma group and COM with cholesteatoma group. 형태의 차이도 통계학적으로 유의하였다(p<0.05)(Fig. 4). 또한 진주종성 중이염 군(33예)과 병변이 없는 군(100예)을 유양동의 환기에 중요한 역할을 하는 부위로서 진주종 및 만 비교해 본 결과 각각 30%, 43%, 27%와 66%, 28%, 6% 성 중이염 등 병변 발생에 관여하는 것으로 알려져 있으며 수 로 뚜렷한 분포의 차이를 보였으며 이 또한 통계학적으로 유 술시에도 병변 제거 및 구조물 보강 등의 적절하고도 다양한 의하였다(p<0.01)(Fig. 4). 술기가 필요한 중요한 구조물이다.
