It has been proved to be the most common primary neoplasm of the middle ear. The aim of this study is to evaluate the clinical characteristics and the outcome of treatment among the patients with glomus tumor in the temporal bone. Materials and MethodsFrom Dec to Dec. 1998, six patients with glomus tumor underwent surgical removal at Asan Medical Center. The medical records were retrospectively reviewed with regard to sex, age, signs and symptoms, audiometric and radiologic evaluation, treatment modalities, and complications. ResultsThis study group consists of 1 man and 5 women. Their ages ranged from 34 to 70, an average of 51.9 years old. Pulsatile tinnitus was the most common symptom. Five of 6 patients were positive for Brown's sign. The patients showed conductive hearing loss or mixed hearing loss with a mean airbone gap of 29.2dB. The main feeding vessels for the glomus tumor were the ascending pharyngeal artery 67%, and the middle meningeal artery 33%. The treatment modalities were 1 transcanal approach, 4 posterior tympanotomy, and 1 subtotal petrosectomy. The postoperative complications consist of 1 case of wound infection, 1 case of facial palsy, and 2 cases of deafness. There was no recurrence. ConclusionGlomus tumor has to be considered to differentiate the etiology of middle ear mass with pulsatile tinnitus. After embolization for feeding vessels, we could achieve complete removal of mass without severe bleeding. Korean J Otolaryngol 1999;42: KEY WORDSGlomus tumor Pulsatile tinnitus Embolization. 1370

W.I., Deaf 2 M/34 17/53 60/100 종괴를 관찰할 수 있었으며, 그 5예 모두에서 Brown 소견 3 F/47 28/60 18/ 48 APA,MMA ICWM 을 관찰할 수 있었다. 나머지 1예의 경우는 하부에 청적색 4 F/70 40/63 37/ 63 APA,MMA ICWM 고막을 관찰할 수 있었으나, 박동성 종괴는 관찰할 수 없었 5 F/68 30/38 23/ 28 MMA,APA 고, Brown 소견 또한 음성이었다. 술전 청력검사 소견상 4예는 혼합성 난청을 가지고 있 었으며 2예는 전음성 난청을 가지고 있어 모든 예에서 전 음성 난청의 요인을 가지고 있었으며, 기도골도청력차는 8 db에서 43 db까지 평균 29.2 db였다. 술후 3개월째 시행 A Cx FNP T.C. 6 F/36 10/43 3/ 20 MMA,CTA ICWM PTA pure tone average V. vessel Op operation Cx complication Preop. 이러한 연부조직 음영의 력 호전을 보였고, 3예는 술전청력과 차이가 없었으며, 2예 감별진단 및 병변의 범위를 알기 위해 측두골 자기공명영 는 술전에 비해 청력이 악화되었다(청력이 악화된 2예의 상촬영을 실시한 결과, 병변은 T1 및 T2-weighted영상 술전 청력은 각각 30/73, 17/53 db이었으며, 술후 청력은 에서 중등도의 신호강도를

B After embolization, it shows scanty tumor-staining (arrow). 한 청력검사상 1예에서 술전 기도청력에 비해 22 db의 청 골파괴가 관찰된 경우는 없었다. 이러한 연부조직 음영의 력 호전을 보였고, 3예는 술전청력과 차이가 없었으며, 2예 감별진단 및 병변의 범위를 알기 위해 측두골 자기공명영 는 술전에 비해 청력이 악화되었다(청력이 악화된 2예의 상촬영을 실시한 결과, 병변은 T1 및 T2-weighted영상 술전 청력은 각각 30/73, 17/53 db이었으며, 술후 청력은 에서 중등도의 신호강도를 보였으며, Gadolinium-enh- 골도 청력은 각각 scale out이었고 기도 청력은 각각109 anced T1-weighted영상에서 높은 신호강도를 보여, 이 db, 100 db이었다). 청력이 악화된 경우중 1예는 수술 중 차적으로 발생한 고실 및 유양동의 저류액과 감별할 수 있 종괴 제거를 위하여 추골 및 침골을 제거한 후 고실성형 었다(Fig. 1). 종괴의 침범범위는 직경이 약 4 mm정도로 (tympanization)을 시행한 경우이고, 나머지 1예는 종괴에 중고실에만 한정된 경우가 1예였고, 상고실, 중고실, 하고 의해 추골 및 침골의 손상이 있었으며, 또한 유돌봉소 및 실 및 고실협부까지 침범한 경우가 2예였으며, 고실내의 후안면신경 봉소의 심한 육아조직형성으로 인하여 추체아전 종괴가 유골동구(aditus ad antrum)까지 침범한 경우가 3 삭개술을 실시한 경우였다. 그리고 전례의 경우에서 술전 예였다. 또한 유골동구까지 침범한 3예 중 1예에서 유골봉 골도 청력은 평균 22.1 db였으며, 술후 3개월 째 시행한 골 소 및 후안면신경 봉소 등에 사구체 종괴와 구별하기 곤란 도 청력은 2예에서는 전농이 되었으며, 나머지 4예에서는 한 육아조직이 발견되었다. 술전 골도청력과 비교하여 큰 차이가 없었다(Table 2). 방사선학적 검사는 모든 예에서 측두골 단층촬영과 측두 골 자기공명영상촬영을 실시하였으며, 6예 모두에서 측두 종괴의 기능성 여부를 알기 위해, 경대퇴동맥 경동맥 조영 술을 하기 전에 24시간 요중 vanillylmandelic acid(vma) 양을 조사하였는데 6예 모두에서 정상범위였다. 골 단층촬영상 고실 및 유양동에 연부조직 음영이 관찰되 수술하기 1일에서 3일 전에 시행한 경대퇴동맥 경동맥 었고, 이소골의 미란이 관찰된 경우는 2예였으며, 측두골의 조영술상 4예에서 상행인두동맥(ascending pharyngeal ar Korean J Otolaryngol 1999;42:1370-5
