But its incidence may rise with the emergence of antibiotic-resistant pathogens, most notably, Streptococcus pneumoniae. This disease still remains as a potentially dangerous condition. Significant and life-threatening complication may occur including subperiosteal abscess, facial paralysis, and intracranial complications, etc. We have experienced 10 cases of ACM. The purpose of our study was to assess the clinical features, diagnosis, isolated bacteria and therapeutic results. Materials and MethodNine patients with acute coalescent mastoiditis who had been treated from September 1999 through July 2002 were retrospectively analyzed with charts and X-ray reviews. We identified 10 cases of ACM in 9 patients. One patient suffered right ACM at 12 months of age and left ACM at the age of 14 months, respectively. The symptoms, diagnosis, methods of treatment, isolated bacteria and results of follow-up were analyzed. Seven patients were children, 2 patients were adult. Results The most common symptoms and signs were post-auricular swelling or tenderness, otalgia and bulging of tympanic membrane. All but one patient 90% complained of post-auricular swelling or tenderness. Two patients 20% complained of otorrhea. Seven patients 70% complained of otalgia. Eight patients 80% showed bulging of tympanic membrane. Temporal bone CT showed 3 cases 30% of ACM only, 6 cases of ACM with subperiosteal abscess and 1 case 10% of suppurative labyrinthitis and cerebellar abscess. We carried out medical treatment in 3 cases, and therapeutic surgery in 7 cases. In the isolated bacteria, the most common pathogen was Streptococcus pneumoniae, but there was no growth of bacteria in 3 cases 30%. Conclusion ACM is a disease that affects mainly infants and young children. In the early diagnosis of ACM, otalgia and bulging of tympanic membrane that persist more than 2 weeks are the most important symptoms and signs, apart from post-auricular swelling and mastoid tenderness. With the increasing trend of resistant Streptococcal species, treatment should include anti-streptococcal pneumoniae antibiotics to prevent complications. Conservative treatment with intravenous antibiotics only is the appropriate initial management in patients who don t exhibit any signs of subperiosteal abscess or intracranial complications. 관찰되지 못하였다(Table 1). 발생률이 현저히 감소하였다고 알려져 있다.1)2) 그러나 최 근 들어서는 페니실린에 내성을 가진 세균의 증가로 인하 고 찰 여 급성 융합성 유양돌기염의 발생률 및 그에 따른 합병증 이 증가하고 있는 실정이다.

유돌봉소들간의 융합이 일어나면서 농양동(abscess cavity) 이 형성된다고 알려져있다(Fig. 1). 특히 이 염증이 유양 돌기의 외측으로 진행되어 추제인상열(petrosquamous suture)이 개방된 경우 중이내의 농이 이를 통하여 침투하 여 골막하 농양이 형성되어 이개 후부의 심한 종창과 이개 의 전방 돌출을 일으키게 한다(Fig.

3 김동주 외 서는 세균이 배양되지 않았다. 10예중 7예는 추적관찰 중 기염은 항생제의 개발 이전에는 모든 예에서 수술이 필요 현재까지 특별한 재발의 소견은 보이지 않고 있으며, 소뇌 하다고 하여 외과적 유양돌기염(surgical mastoiditis)이라 농양을 동반한 1예는 치료도중 신경외과적인 문제로 사망 고 불렸으나8-10) 1930년 후반에 항생제의 개발로 인하여 하였으며, 1예는 추적 관찰되지 못하였다(Table 1). 발생률이 현저히 감소하였다고 알려져 있다.1)2) 그러나 최 근 들어서는 페니실린에 내성을 가진 세균의 증가로 인하 고 찰 여 급성 융합성 유양돌기염의 발생률 및 그에 따른 합병증 이 증가하고 있는 실정이다.4) 급성 중이염의 합병증으로 발생하는 급성 융합성 유양돌 급성 중이염으로 인하여 급성 융합성 유양돌기염 및 골 막하 농양이 형성되는 기전은 중이강내의 점막 및 골막에 염증성 변화로 인한 부종과 증식을 가져와 유양동구(aditus ad antrum)가 막히면서 주변 골의 탈무기질화가 일어나고, 유돌봉소 격벽의 골염과 단백성 기질의 파괴가 일어나 인접 유돌봉소들간의 융합이 일어나면서 농양동(abscess cavity) 이 형성된다고 알려져있다(Fig. 1). 특히 이 염증이 유양 돌기의 외측으로 진행되어 추제인상열(petrosquamous suture)이 개방된 경우 중이내의 농이 이를 통하여 침투하 여 골막하 농양이 형성되어 이개 후부의 심한 종창과 이개 의 전방 돌출을 일으키게 한다(Fig. 2A and B). 이 단계가 되면 수술적 치료를 요하게 된다. 또한 농양동의 염증이 파 급되는 경로에 따라 미로를 통한 내이염, 안면신경 마비, 그리고 및 뇌막염과 뇌 농양과 같은 두개내 합병증을 유발 시킬 수가 있다.6)8)9) 저자들이 치험한 급성 융합성 유양돌 기염 10예중 6예가 골막하 농양을 형성하였고, 1예는 화 농성 미로염 및 소뇌 농양 소견을 보였다(Fig. 3A and B). 급성 융합성 유양돌기염의 진단은 2주이상 지속되는 이 루, 지속적인 이통, 그리고 유양돌기 부위의 부종을 보이면 Fig. 1. B 화단층촬영상 유돌봉소들의 융합 소견을 보이면 확진을 내 5) 아에서 급성 융합성 유양돌기염이 있는데도 정상 소견으로 릴 수 있다. 이러한 소견들 중에서 진단 전의 전구 증상들 잘못 판독되는 경우가 약 32%까지 보고하고 있어 많은 도 (prodromal symptoms)이 급성 중이염 시작부터 6주까지 움을 주지 못한다.9) 그러나 측두골 전산화단층촬영은 유돌 다양하게 나타날 수 있다. 그 중에서도 이개 후부의 종창 봉소 격막의 미란으로 인한 인접 유돌봉소들간의 융합소견 또는 발적외에 2주이상 지속되는 이통 및 발열이 급성 융 으로 급성 융합성 유양돌기염의 진단을 내릴 수 있을 뿐만 9) 합성 유양돌기염의 진단에 가장 중요하다는 보고가 있다. 아니라 S자형 정맥 혈전성 정맥염(sigmoid sinus throm- 저자들의 경우에도 2주이상 지속되는 이통과 함께 고막의 bophlebitis)과 두개내 합병증을 확인하는데 필수적인 검 팽륜된 소견이 전체 10예 중 8예(80%)에서 관찰되는 것 사로 알려져 있다.13) 저자들의 증례에서도 소아에서 촬영 으로 보아서 비록 저자들이 치험한 증례의 수가 적지만 이 한 유양동 단순 방사선 소견상 유돌봉소들에 연조직 음영 루보다는 오히려 이통이 더 중요한 증상이라고 생각되며 은 보였으나 유돌봉소들간의 융합 소견은 명확하지 않았으 고막의 팽륜이 함께 관찰되면 이 질환을 반드시 의심하는 나 측두골 전산화단층촬영에서는 3예에서 유돌봉소들간의 것이 좋을 것으로 사료된다. 또한 급성 융합성 유양돌기염 융합 소견을 보였으며 6예에서는 이러한 소견과 함께 골 의 가장 일정한 이학적 검사 소견이 유양돌기 부위의 종창 막하 농양 소견을 보였다. 나머지 1예는 급성 융합성 유양 및 발적이다. 그리고 이러한 발적의 위치는 연령에 따라 돌기염과 함께 화농성 미로염 및 소뇌 농양 소견을 보였다. 다르다. 소아들은 유양돌기 함기화의 불완전한 발달로 인 따라서 유양동 단순 방사선촬영은 진단에 많은 도움을 주 하여 유양동 근처의 유양돌기와(fossa mastoidea) 상방에 지 못하였고 측두골 전산화단층촬영으로 환자에 치명적인 생기고, 성인의 경우는 유양돌기첨(tip of mastoid process) 합병증을 확인하여 조기 치료를 함으로써 환자의 이환율과 에 생긴다고 한다.11)12) 저자들의 경우에서도 소아 8예 모 사망율을 감소시킬 수 있었다고 사료된다. 두 유양돌기와 상방인 이개의 후상방, 성인 1예는 유양돌 세균배양검사에서는 급성 융합성 유양돌기염의 가장 흔 기첨에서 종창 및 발적 소견을 보였다. 따라서 급성 중이염 한 균주는 Streptococcus pneumoniae로 약 30%를 차지 환자의 이학적 검사 시에 고막과 함께 연령에 따라 종창 및 하는 것으로 알려져 있다.9) 다른 저자들의 보고에 의하면 발적 부위가 다르다는 것을 알고 이러한 부위를 항상 관찰 Staphylococcus aureus, Hemophilus influenza 등도 주 하는 것이 바람직하다. 요한 균주이다.4-6)14) 그러나 Hemophilus influenza의 경 방사선학적으로는 유양동 단순 방사선촬영과 측두골 전 우는 뼈에는 침범하지 않으므로 급성 중이염에는 흔한 균 산화단층촬영을 이용하여 진단할 수 있다. 단순 방사선촬영 주이지만 유양돌기염에서는 흔하지는 않다. 원인 균주가 의 소견들로서는 유돌봉소들의 혼탁, 유돌봉소 미세골구조 발견되지 않는 경우도 33%까지 보고되고 있으며 특히 입 의 탈회 및 액화로 개개의 유돌봉소가 없어지면서 융합된 원 전 경구 항생제 치료를 받았던 환자에서는 90%까지 이 소견을 보인다. 그러나, 급성 염증기에 있다 하더라도 단순 른다는 보고도 있다.2)7)11) 최근에 페니실린에 저항을 가진 방사선촬영상 명확한 골 파괴 소견이 없을 수 있고 특히 소 Streptococcus pneumoniae 균주가 6~38%로 그 비율이 24 Korean J Otolaryngol 2003;46:21-6
