To obtain more evidence supporting home-based auto-titration, APAP titration was performed at home for patients who were presented with OSA on laboratory-based diagnostic PSG followed by CPAP titration. A total of 79 patients were included in the study. They all underwent split-night PSG with CPAP titration, and APAP titration for more than 7 days. The patients with successful titration at both situations were selected. The optimal pressure and apnea-hypopnea index (AHI) of CPAP and APAP titration were compared. The optimal pressure for CPAP and APAP titration were 7.0±1.8 cmh2o and 7.6±1.6 cmh2o (P 0.001), whereas the corresponding AHI were 1.3±1.5/h and 3.0±1.7/h (P 0.001). As a result, the achievement rates of optimal pressure for CPAP and APAP titration were 96.2% and 94.9% (r=-0.045, P=0.688), respectively. The results of this study did not differ with regard to the optimal pressure between CPAP and APAP titration. Overall, CPAP and APAP titrations should be chosen depending on a required situation. Comparison between CPAP and APAP Titration 호흡, 호흡노력관련각성 (respiratory effort related arousal, RERA), 코골이를제거할수있는압력을측정하는것으로, 표준화된방법은수면다원검사 (polysomnography, PSG) 를시행후 2주이내에검사실내에서수면다원검사감시하에 CPAP 을이용하여측정하는것이다 [9]. 그러나이방법은 2회에걸친검사로시간적, 경제적비용이환자의부담이될수있다. 따라서최근에는다른방법을통한적정양압측정이제시되고있다. 한가지방법은 1회검사를통해서첫 3 4 시간동안수면다원검사를하고, 이후 3 4 시간을수면다원검사감시하에 CPAP 을이용하여적정양압을동시에측정하는분할수면다원검사 (split-night polysomnography, split-night 수면다원검사 ) 방법이다 [10]. 다른하나는자동화양압기 (auto-adjusting positive airway pressure, APAP) 를이용한측정법이다 [11]. 자동화양압측정방법은수면다원검사후환자가장비를대여하고, 최대와최소양압을설정하고집에서직접장비를착용하고일정기간사용하고난후데이터를분석하는방법이다. 검사자가검사실에서실시간으로환자의상태를모니터링하면서측정하는 CPAP 측정법과달리, 센서에의해서압력의수치가자동제어된다. 따라서중추성수면무호흡 (central sleep apnea, CSA) 의구별, 정확한마스크착용확인, 공기누수와같은불확실성이있다. 그러나 APAP과 CPAP 측정법을비교한이전의연구결과를보았을때비용적인면이나, 적정양압에도달하는시간등다양한이유에서효율성이확인되었다 [12, 13]. 하지만이들연구에서는동일인을대상으로진행된것이아니고무작위적으로두그룹을분류하여수치들을비교한것이라는제한점을가지고있다. 따라서, 본연구에서는동일인을대상으로수면다원검사시행후 CPAP 과 APAP 측정법의적정양압과무호흡-저호흡지수 (apnea-hypopnea index, AHI) 를비교함으로써두방법에대한차이를확인하고, 2018년 7월이후시행된수면다원검사보험으로폐쇄성수면무호흡환자들에게좀더정확하고효율성있는치료방법을제시하고자한다. 행하였다. 2009년 4월부터 2014년 12월까지양산부산대학교병원수면클리닉에수면무호흡및코골이를주소로내원한환자중, 수면다원검사후폐쇄성수면무호흡으로진단받고치료를위해가정에서 APAP 양압측정을진행한 116명의환자를대상으로후향적으로분석하였다. 양산부산대학교병원임상시험심사위원회 (IRB) 에규정된인간대상연구와관련하여심의면제사항인 연구대상자등에대한기존의자료나문서를이용하는연구 에부합됨을확인하고진행하였다. 연구대상자중이동식수면다원검사 (potable-psg) 를시행한환자 9명, 분할수면다원검사중 CPAP 양압측정을실패한환자 22명 ( 측정시간부족 10명, CPAP 의부적응 12명 ), APAP 양압측정을실패한환자 6명은제외하였으며, 검사실에서분할수면다원검사후가정에서 APAP 을 10일이상, 하루 4시간이상사용한환자 79명을분석하였다 (Figure 1). 본연구에서폐쇄성수면무호흡은최근개정된국제수면질환분류 3판 (International Classification of Sleep Disorders-3, ICSD-3) 의분류기준에의해정의하였다 [14]. 폐쇄성수면무호흡은복부와흉부의호흡노력이관찰되는데도최소 10초간구강과비강에서무호흡또는저호흡이시간당 5회이상발생하는상태로정의하였다 [15]. 폐쇄성수면무호흡의단계별구분은시간당발생하는무호흡또는저호흡횟수에따른무호흡-저호흡지수를기준으로하여지수가 0 4 ( 상기도저항 ), 5 14 ( 경증도 ), ( 중등도 ), 30 이상 ( 중증도 ) 으로분류하였다 [16]. 수면관련호흡장애는무호흡과저호흡으로구분하는데, 재료및방법 본연구는 2018년 7월부터 OSA 환자들의부담을경감하고자건강보험행위비급여가급여로변경되어수요가급증함에따라 OSA 환자들의치료법인양압기사용의단점을개선하고적절한적정양압을측정하기위한방법을찾고자진행되었다. 또한양산부산대학교병원임상시험심사위원회에규정된인간대상연구와관련하여심의면제사항인 연구대상자등에대한기존의자료나문서를이용하는연구 에부합됨을확인하고진 Figure 1. Vol. 51, No. 2, June 미국수면학회 (American Association Sleep Medicine, AASM) 의기준으로, 무호흡은공기흐름이 90% 이상감소된상태가지속적으로 10초이상관찰될때로정의하였고, 저호흡은공기흐름이 50% 90% 로감소된상태가지속적으로 10초이상관찰되며혈중산소포화도가 4% 이상감소한경우로정의하였다. 호흡노력관련각성은저호흡의기준에는부합하지않지만호흡기류제한이있고혈중산소포화도가 3% 로감소한경우로정의하였다 [17]. 무호흡-저호흡지수의경우시간당무호흡과저호흡이관찰되는횟수를계산하여산정하였다. 수면다원검사는 Remlogic (Embla, San Carlos, CA, USA) 을사용하였으며, 표준화된방법으로각종감지기와전극을부착하였다. 검사항목은수면단계를확인하기위한 6채널 (F3, F4, C3, C4, O1, O2) 의뇌전도 (electoencephalogram, EEG), 좌우 2채널의안구전도 (electrooculogram, EOG), 1채널의턱근전도 (chin electromyogram) 를사용하였고, 호흡을측정하기위한비강케뉼라 (nasal cannula), 구비강열전대 (oronasal thermistor), 흉부와복부의유도체적변동기록기 (inductive plethysmography) 를사용하였다. 또한산소포화도측정기 (oximeter), 1채널의심전도 (electrocardiogram), 1채널의다 리근전도 (anterior tibialis electromyogram), 코골이감지기 (snoring sensor), 자세위치센서 (body position sensor) 등을포함하였다. CPAP 을이용한적정양압측정은분할수면다원검사로시행하였다. 전반부에수면다원검사를시행하여총수면시간 (total sleep time, TST), 무호흡, 저호흡, 호흡노력관련각성, 무호흡- 저호흡지수, 최소산소포화농도 (minimum saturation, Min SaO 2 ) 를확인하였다. 후반부에시행한적정압력측정은미국수면학회의가이드라인 [9] 을따라진행하였으며, 무호흡, 저호흡, 호흡노력관련각성의기록은숙련된검사자가하였다. CPAP 을이용한적정양압측정은분할수면다원검사후반부인새벽 2시 30분에서 3시사이부터진행되었으며, 최소압력은 4 cmh 2 O 로시작하여최대 20 cmh 2O까지를범위로하였다. 압력사이의간격은최소 5분으로, 무호흡 2개이상, 저호흡이 3개이상, 호흡노력관련각성 5개이상, 지속된코골이 3분이상중적용되는항목이있으면 1 cmh 2O씩압력을올렸다. 최종적으로무호흡-저호흡지수가 5 이하인상태로 15분이상수면상태를유지하면, 적정양압으로판단하였다. Comparison between CPAP and APAP Titration 후, 1개월이내에가정에서 APAP 장비를대여하여적정양압의압력범위를설정하고일정기간사용하면서적정압력을측정하였다. 일정기간사용후기록된데이터를분석하여무호흡-저호흡지수, APAP 사용일수, 1일당사용시간을확인하였고, 수면전문의에의해평균압력, 95th percentile, 최고압력을확인한후적정양압을판단하였다. 본연구의통계처리는통계프로그램 (SPSS version 22.0, SPSS, Chicago, USA) 을사용하였다. 연속형변수의표현은평균과표준편차로나타내었으며, 대응표본 T검정및 Wilcoxon s 부호순위검정을시행하였다. 범주형변수는빈도와비율 (%) 로나타내었으며, 교차분석 ( 2 ) 혹은 Fisher의정확한검정분석을시행하였다. 상관분석은양측검정 Pearson 검정및 Spearman 의 Rho 검정을수행하였다. 통계적유의수준은 P<0.05 로하였다. 시간이상사용비율은 70.6±18.1% 이었다 (Table 1). 수면다원검사후 CPAP 과 APAP 에서측정된적정양압과, 무호흡-저호흡지수는 Figure 2와같다. CPAP 의적정양압은 7.0±1.8 cmh 2O로 APAP 의적정양압의 7.6±1.6 cmh 2O보다통계적으로유의하게낮았다 (P<0.001). 또한적정양압상태에서의무호흡-저호흡지수는 CPAP 이 1.3±1.5/h, APAP 은 3.0±1.7/h 로보다통계적으로유의하게낮았다 (P<0.001). Figure 3에서평균무호흡-저호흡지수가 5/h 이하로감소되는적정양압도달비율은 CPAP 측정법이 96.2% (76명), APAP 측정법이 94.9% (75명) 으로유의미한차이를보이지않았다 (r= 0.045, P=0.688). 고찰 결과본연구에서는수면다원검사후폐쇄성수면무호흡으로진단받은총 79명의환자를분석하였다. 남성은 59명 (74.6%) 이었으며, 대상자의평균나이는 51.5±10.2세, 체질량지수는 26.6±3.2 kg/m 2 이었다. 대상자중고혈압을동반한자가 30명 (37.9%), 고지혈증의경우는 8명 (10.1%) 으로확인되었다. 64 명 (81.0%) 의대상자가카페인을섭취하였으며, 음주력은 45명 (56.9%), 흡연력은 16명 (20.2%) 에서확인되었다. 수면과관련된임상증상은수면중각성 55명 (69.6%), 아침두통 23명 (29.1%), 불면증 13명 (16.4%), 수면마비 2명 (2.5%), 수면발작 1명 (1.2%) 의순으로나타났다. 수면다원검사결과는수면중대상자의 REM 수면비율이 17.2±7.0% 이었으며, 비 REM 수면의 N1, N2 및 N3는각각 29.3±19.4%, 41.8±14.8%, 14.2±9.8% 이었다. 대상자의 50명 (63.2%) 이중증도폐쇄성수면무호흡, 17명 (21.5%) 이중등도폐쇄성수면무호흡으로나타났으며, 평균무호흡-저호흡지수는 41.9±23.7/h이었다. 또한수면중혈중최소산소포화도는 80.0±6.7% 로나타났다. 분할수면다원검사후대상자중 51명 (64.5%) 이 CPAP 사용의불편함을호소하였다. APAP 의사용일은 14.8±6.2일, 1일당 4 이연구를요약하자면 : 1) CPAP 과 APAP 의적정양압측정법을비교하였을때, CPAP 측정법의경우적정양압이더낮게측정되며, 무호흡-저호흡지수도낮게유지하므로, 보다효율적으로적정양압을측정할수있는방법으로보인다. 하지만 2) 미국수면학회의가이드라인에서정하는적정양압 ( 무호흡-저호흡지수 5/h이하 ) 의도달비율은 CPAP 과 APAP 측정법모두통계적차이를나타내지않았다. 폐쇄성수면무호흡은삶의질이높아짐에따라수면의중요 Figure 3. Vol. 51, No. 2, June 성이증가하고다양한질환과의연관성이밝혀져정확한진단및적절한치료가필요하다 [18, 19]. 폐쇄성수면무호흡의진단은수면다원검사가표준화된방법으로, 이검사의결과에따라치료방법이결정된다. 현재폐쇄성수면무호흡의치료는무호흡-저호흡지수로나타나는폐쇄성수면무호흡과저호흡의정도에따라달리하고있다. 무호흡-저호흡지수를기준으로중증도미만 ( 무호흡-저호흡지수<30) 에서는수술적인방법의치료성공률이 40.7% 로낮지만, CPAP 사용의불편함및적응증때문에널리시행되고있다 [20]. 반면중증도이상 ( 무호흡-저호흡지수 30) 은미국수면학회의가이드라인에따라 CPAP 을가장기본적인치료방법으로권고하고있고 [21, 22], 이에대한효과도많이보고되었다 [23, 24]. CPAP 을사용한폐쇄성수면무호흡치료에서가장중요한것은적정압력을측정하는것이며, 측정방법에는검사실에서측정하는수동화양압측정법과가정에서자가로측정하는자동화양압측정법으로분류할수있다. 앞선연구들에서 APAP 을이용한적정양압측정이 CPAP 보다환자가부담하는검사비용뿐만아니라, 적정양압을측정하기위한제반비용 [12], 적정양압에도달하는시간 [13] 에서효율성이높았다. 그러나기존의연구들은 CPAP 과 APAP 의측정법을동일한대상자가시행한결과를비교한것이아니고두군으로분류하여분석한것이기에대상자들사이의차이로인한결과의오류를배제할수없고, 두측정법에따른적정압력과무호흡-저호흡지수를비교하는데제한점이있었다. 이에본연구에서는동일인을대상으로 CPAP 과 APAP 의측정법을비교하여실제로환자에게적용하는적정양압의차이를확인하였다. 그결과적정양압의수치가 CPAP 에서 APAP 보다낮게측정되어효율적인압력을주는치료라고판단되었다. 이는양압치료에서압력이올라갈수록환자가느끼는불편감이커지고그에따라양압기사용의순응도가떨어지기때문이다. 이러한결과는수면다원검사감시하에 CPAP 을사용하는측정법이표준방법이라는것을뒷받침할수있는근거이다 년 7월부터폐쇄성수면무호흡의진단및치료의중요성을인정받아, 폐쇄성수면무호흡의진단및치료의보험급여가시행되었다. 이로인하여적정양압을측정할때시간부족과비용적인문제로인한분할수면다원검사로시행되거나가정에서 APAP 을사용하는측정법보다는표준방법을권고할수있게되었다. CPAP 을사용중이더라도체중변화에따른상기도폐쇄정도의변화를예상할수있고, CPAP 을꾸준히사용하면무호흡-저호흡지수의변화 [25] 도관찰되었다. 이런변화는적정양압을 수정해야될수있는원인이되므로양압에대한지속적인관찰과꾸준한관리가필요하다. 그러나국가재정에의한보험급여는진단시 1회, CPAP을사용한적정양압측정시 1회만인정하기때문에비용적인면에서제약이따른다. 이러한문제는미국수면학회가이드라인에따라무호흡-저호흡지수 5/h이하인적정양압으로의도달률에서두측정법의유의한차이가없다는결과 (Figure 3) 를보면 APAP 의사용으로도가능하다고사료된다. 폐쇄성수면무호흡을치료하기위한양압기사용의중요한사항은올바른마스크사용과양압에대한순응도이다 [26, 27]. 본연구에서도분할수면다원검사이후 CPAP 양압측정을성공한 79명중 51명 (61.5%) 이 CPAP 의순응도에서불편함을호소했다. 특히 CPAP 양압측정을실패한 22명중적정양압측정시간의부족으로인한 10명을제외한 12명의대상자가 CPAP 양압측정을실패한이유가마스크의불편함과양압의부적응때문이었다. 검사실에서 CPAP 을사용하여 1회의검사로측정하는것보다는가정에서 APAP 을사용하여수일간측정하는경우, 환자가보다여러번양압기를사용하면서그동안자신에맞는마스크를찾을수도있는장점이있다. 따라서환자가비교적쉽게양압에적응할수있는환경이조성되므로 APAP 을이용한양압측정이양압기사용의순응도를높일수있는방법중의하나라생각된다. 본연구에서확인된 Figure 3의결과에서보았을때에도 APAP 을사용한측정법이순응도를높이기위한좋은대체방법이될수있겠다. 본연구에서 CPAP 을이용하여측정하였을경우에 APAP 을이용한경우보다적정양압 (7.0±1.8 cmh 2O, 7.6±1.6 cmh 2O) 과무호흡-저호흡지수 (1.3±1.5/h, 3.0±1.7/h) 가유의하게낮게측정되었다. 물론두측정법의적정양압차이가압력을올려주는단위인 1 cmh 2O 이내에있고, 무호흡-저호흡지수도미국수면학회가이드라인의기준인무호흡-저호흡지수 5/h 이하로, 두방법모두충분히교정하고있다고볼수있다. 하지만좀더낮은압력에서좀더적은빈도의무호흡-저호흡지수를보이는것은의미가있으며 APAP 측정법에서몇가지개선을통해이를보완할수있을것이다. 첫째로부적절한양압의상승을제어하기위해공기누수에대한통제가필요하다. 이는환자에게적합한마스크선택과올바른마스크착용법을통해서해결할수있을것이고, 이로인해적정양압의공급으로무호흡- 저호흡지수의수치도감소될것으로기대된다. 둘째로이전의연구에서확인된 APAP 장비종류에따라적정양압의차이가있었다는점은 [28], 각회사의기술력의차이때문이라고볼수있다. 향후지속적인연구와기술력의발전으로 APAP의정확성과효율성을높일수있을것이라기대된다. 특히 APAP 장비에서

Comparison between CPAP and APAP Titration 중추성무호흡과폐쇄성무호흡을구분하지못하고양압을상승시키기때문에직접 CPAP 을통해서적정양압을측정하는것과차이를보이기도한다. 이두종류의무호흡은복부와흉부의호흡노력을통해구분할수있으나현재까지의양압기에서는구분이불가능하나, 차후시스템개선이이루어지거나추가적인센서를부착할수있게된다면 APAP 에서도정확한적정양압을구하고무호흡-저호흡지수도보다효과적으로감소시킬수있게될것이다. 이번연구의결과를토대로폐쇄성수면무호흡의치료를위한제언을하자면, 병원에내원하는무호흡환자들을수면다원검사를통해폐쇄성수면무호흡과중추성수면무호흡을구분한다. 폐쇄성수면무호흡으로진단된환자들은보험을적용시켜수면다원검사감시하에 CPAP 을사용하여적정양압을측정하게되면, 적절한마스크의사용과공기누수에의한압력증가를제어할수있을뿐만아니라양압으로인한중추성수면무호흡이발생하는것도확인할수있겠다. 이후가정에서 CPAP 을사용하면서주기적으로 APAP 을통해양압적정을확인하면보험재정이되기전보다폐쇄성수면무호흡환자들에게적응성을높인적절한치료방법이될것이다. 이번연구는국내에서진행되지않았던 CPAP 과 APAP 의적정양압측정법을비교한것으로온전한하루밤동안의 CPAP 적정양압측정이아닌분할수면다원검사중적정양압측정을시행한결과라는점과대상자들의주간졸림증, 우울척도, 삶의질과같은주관적인데이터를비교하지않았다는점은아쉬운부분으로남는다. 또한 case 부족으로인한통계적인부족성은향후개선되어야할부분이라고사료된다. 요약폐쇄성수면무호흡환자들의양압기사용을위한적정양압측정은편리함과경제성뿐만아니라다양한원인으로 CPAP 측정법보다는 APAP 측정법을더선호한다. 그러나 PSG 감시하에진행하는 CPAP 측정법보다정확성에대한의문이아직까지남아있다. 이에본연구에서는동일한대상자에대한 CPAP 과 APAP 측정법의적정압력과무호흡-저호흡지수를변수로서두방법간의효율성과정확성을비교하였다. 대상자는분할수면다원검사중 CPAP 양압측정과 7일이상 APAP 양압측정에성공한 79명이모집되었다. CPAP 과 APAP 측정법의적정양압은 7.0±1.8 cmh 2 O, 7.6±1.6 cmh 2 O 무호흡-저호흡지수는 1.3±1.5/h, 3.0±1.7/h 로 CPAP 측정법이두변수모두통계적으로유의하게낮게나타났다 (P<0.001). 그러나미국수면학회 가이드라인무호흡-저호흡지수 5/h이하의적정양압도달비율은 CPAP 측정법이 96.2% (76명), APAP 측정법이 94.9% (75 명 ) 으로유의미한차이를보이지않았다 (r= 0.045, P=0.688). 본연구를요약하면, CPAP 은 APAP 측정법보다좀더효율적인측정법으로나타났다. 그렇지만적정양압에도달하는비율은두방법모두통계적차이를나타내지않아정확성을가진다. 따라서 CPAP 과 APAP 측정법중요구되는상황에따라적절한방법을선택하여적정양압을측정할수있겠다.
