PAP devices are divided into four basic types depending on their pressure delivery system: (1) continuous positive airway pressure (CPAP), (2) bi-level positive airway pressure (BPAP), (3) automatic self-adjusting positive airway pressure (APAP), and (4) adaptive servo-ventilation (ASV). Manual titration of CPAP or BPAP under full attended polysomnography is currently the gold standard for selection of the effective PAP pressure in patients with OSA. The use and titration of PAP are reviewed and recommendations of the PAP Titration Task Force of the AASM are introduced in this report. The goal of PAP titration is acquisition of the pressure, which eliminate obstructive respiratory related events. It should be understood and practiced the guideline of PAP titration for patients with OSA. Key Words: Obstructive sleep apnea. Continuous positive airway pressure, Guideline 서 론 폐쇄성수면무호흡 (Obstructive sleep apnea, OSA) 은성인에서 2~4% 발병률을보이며나이가들수록증가하는흔한수면과관련된호흡장애이다. 1,2 수면중상기도의완전또는부분적폐쇄로인하여공기흐름의제한, 산소포화도감소, 정상수면의단절등을야기시킴으로써주간삶의질을저하시키며인지기능의저하및심뇌혈관계통과대사성장애의원인이된다. 3-5 OSA 진단은검사실에서시행되는야간수면다원검사 (attended full night laboratory polysomnography, PSG) 가표 * Address of correspondence Kwang-Ik Yang Department of Neurology, College of Medicine, Soonchunhyang University, Cheonan Hospital Bongmyeong-dong, Cheonan-si, Chungnam, , Korea Tel: Fax: kiyang@sch.ac.kr 준화된진단검사법이며 Continuous positive airway pressure (CPAP) 및 Bi-level positive airway pressure (BPAP) 적정을위한근거가된다. 6 양압 (Positive airway pressure, PAP) 치료는가장널리사용되는 OSA 의기본치료법이다 년 Sullivan 등 7 이처음으로고안한지속성양압장치 (CPAP) 는부피가상당히크고무거우며시끄러웠다. 하지만이후양압장치가지속적으로발전하여과거보다작고가벼우며소음면에서도조용해졌다. 양압장치는압력을전달하는방법에따라 4가지종류가있다. 지속적으로한가지고정된양압을전달하는 CPAP, 호기양압 (Expiratory PAP, EPAP) 과이보다높은압력인흡기양압 (Inspiratory PAP, IPAP) 의다른두양압을전달하는 BPAP, 자동적으로 CPAP, BPAP (IPAP/EPAP) 을조절하는 auto-titrating positive airway pressure (APAP) 가있으며, 마지막으로울혈성심부전환자에게서관찰될수있는 Cheyne- Stokes 호흡같은중추성수면무호흡에적용할수있 Vol.6, No.1 / June,

OSA 치료를위한 CPAP 또는 BPAP (CPAP/BPAP) 장치사용에대한 American Academy of Sleep Medicine (AASM) 의권고사항은 1) 중등도내지중증 OSA (Standard), 2) 경증 OSA (Optimal), 3) 주간졸음을호소하거나주간삶의질을향상을위한경증이상의 OSA (Standard), 4) 고혈압이동반된 OSA에서혈압하강을위한부수적요법 (Option) 등이다. 8 괄호안은 levels of recommendations을의미한다 (Table 1). 2004년 APAP 처방에대한미국 National Survey 에의하면전체처방되는양압장치중에서 4% 가 APAP 이다. 9 이후 APAP 에대한타당성및신뢰성에대한다양한연구가진행되고있으며 APAP 사용이증가하고있는추세이다. 10,11 그럼에도불구하고, 현재까지는검사실내에서 PSG 감시하에서 CPAP/BPAP 의수기적정이최적압력결정에있어서 gold standard 라고할수있다. 최적의양압에도달할때까지낮은압력에서부터서서히증가시키는데각단계별압력에서환자의폐쇄성호흡관련사건을평가할수있는충분한시간이필요하다. 양압치료시행은적용대상의선택, 치료양압의적정, CPAP/BPAP 기기사용의순응도등크게 3단계로나눌수있는데본원고에서는양압적정에대한부분만다루고자한다. 양압적정의목적은폐쇄성호흡관련사건인무호흡, 저호흡, 호흡노력관련각성 (Respiratory effort related arousal, RERA), 그리고코골이를제거할수있는압력을얻는것이다. 2007년 4월에 The AASM Board of Directors 는양압적정에대한권고개발을승인하였으며 PAP Titration Task Force 를구성하였다. 이전까지약 4반세기동안수기양압적정이수행되어왔지만표준화된지침이없었으며이런부재는각검사실마다다른 protocol 에의존하였고표준화된양압적정지침의필요성을느끼게되었다. 12 본론에서는 2008년 Positive airway pressure titration task force of the AASM 에의해제시된지침 13 을중심으로 OSA 환자에게서최적의양압적정을위한술기를소개하고자한다. 본론 CPAP CPAP 압력은폐쇄성호흡사건-무호흡, 저호흡, RERA, 코골이-가완전히사라질때까지또는허용최대압력에도달할때까지압력을올린다. 시작최소압력은소아와성인에서같이 4cmH 2 O로시작하여 12세이하소아에서최대 15cmH 2O, 12세이상에서는최대 20cmH 2O까지압력을올린다. 물론체지방지수 (body mass index, BMI) 가높을수록또는재적정 (retitration) 인경우에서는시작압력이높을수있다. 14,15 하지만이런경우에서는시작압력을결정하는데있어서경험적인것외에는뚜렷한방법은없다. 최소 5분이상간격을두고 1cmH 2O씩압력을올린다. 12세이하소아인경우는적어도 1개이상의무호홉또는저호흡, 3개이상의 RERA, 1분이상의확실한코골이를보이는경우압력을올린다. 12세이상인경우는 2개이상의무호흡, 3개이상의저호흡, 5개이상의 RERA, 3분이상의확실한코골이를보이는경우압력을올린다. 이렇게압력을단계별로올려서폐쇄성호흡사건들이나타나지않고조절이되었다면 Exploration 기간을갖는다. 무호흡또는저호흡이제거할수있는압력이선택되었다하더라도상기도저항은정상보다 4배가클수있고 16 이런잔존해있는증가된상기도압력이각성과불면을야기시킬수있다. 17 즉, 호흡기류제한 (flow limitation) 곡선이정상이될때까지압력을 2cmH 2O씩올리며총 5cmH 2 O 이상올리지는않는다. 만약모든폐 30 수면

3 폐쇄성수면무호흡에대한양압적정의지침 쇄성호흡사건들이적어도 30분이상관찰되지않는다면꼭필요하지는않지만압력을내려보는 Down 적정을시도해볼수있다. 양압적정시압력을올리는동안에호흡기류제한이사라지는압력이압력을내리는과정에서호흡기류제한이다시나타나는압력보다 2~5cmH 2 O 보다높은 Hysteresis 현상을보일수있다. 18 즉, 적어도한번의 up-down 적정을시도할수있는데최소 10분이상의간격을두고 1cmH 2 O씩폐쇄성호흡사건들이나타날때까지압력을내려본다. BPAP 환자가높은 CPAP 압력에적응을못해불편감을호소하며견디기가어렵거나 CPAP 압력이 15cmH 2 O 이상에서지속적으로폐쇄성호흡사건이나타나는경우에는 BPAP 으로전환할수있다. BPAP 은 IPAP/EPAP 압력을폐쇄성호흡관련사건이사라지거나최대허용 IPAP 압력에도달할때까지압력을올린다. 시작최소압력은소아와성인모두에서 IPAP 압력은 8cmH 2O 이며 EPAP 압력은 4cmH 2 O 이거나폐쇄성무호흡을사라지게하는 CPAP 압력이다. 최대 IPAP 압력은 12세이하에서는 20cmH 2O 이며 12세이상에서는 30cmH 2O이다. 상기도에 30cmH 2 O 이상의과도한압력은폐의압력상해및기타합병증을일으킬수있다. 19 IPAP 압력과 EPAP 압력의차이는최소 4에서최대 10cmH 2O 유지한다. BMI 가높은경우와재적정일경우는 CPAP 과마찬가지로경험적으로높은 IPAP/EPAP 압력으로시작할수있다. CPAP 과마찬가지로최소 5분이상간격을두고모든폐쇄성호흡관련사건이사라질때까지 1cmH 2 O씩압력을올리는것이다. 12세이하소아인경우는적어도 1개이상의무호흡이나타날경우에 IPAP 과 EPAP 압력을동시에올리고, 1개이상의저호흡, 3개이상의 RERA, 또는 1분이상의확실한코골이를보이는경우에는 IPAP 압력만을올린다. 12세이상인경우는 2개이상의무호흡이나타날경우에 IPAP와 EPAP 압력을동시에올리고, 3개이상의무호흡, 5개이상의 RERA, 3분이상의확실한코골이를보이는경우에는 EPAP 압력만을올린다. 이렇게압력을올려비정상적인호흡사건들이조절 되는압력에도달하였을때 CPAP 과마찬가지로 Exploration 기간을갖는데방법은 CPAP 과같으며 BPAP인경우에서는 IPAP 압력만으로 exploration 한다. Down 적정역시 CPAP 과같은방법이며 IPAP 압력만으로조절한다. Grading System 상기와같은방법으로양압적정을한후결정된압력에대한적용가능성에따라최적 (optimal), 우수 (good), 충분 (adequate), 부적용 (unacceptable) 으로나눌수있다. 첫째, 최적인경우는 Respiratory distress index (RDI) 가 5이하이며 SpO 2 가 90% 이상이고 supine REM 기간이최소 15분이상포함된압력을의미한다. 우수인경우는 RDI 가 10 이하인경우이거나 RDI 가 15 이하이면서진단당시 RDI 의 50% 이상감소되고 supine REM 이포함된압력을의미한다. 또한 RDI 가 10 이하로감소하지않았지만심한 OSA 경우처럼진단당시 RDI의 75% 이상감소하였거나적정압력이최적내지우수에해당하나 supine REM을포함하지않고있다면충분에해당한다. 위의세가지기준에포함되지않는다면 OSA 환자에게적용하기어려운압력 (unacceptable) 이된다. 이상적정검사에서얻은압력이최적내지우수에해당되지않으면재정적검사가필요하다. 양압적정에고려해야할요소수면단계및수면중자세에따라압력이다를수있다. Supine REM 기간에적정된압력은아주높다. 20 이런높은압력은 NREM 수면중에중추성무호흡을야기시킬수있어 NREM 수면중의환자를평가하는것이또한중요하다. 21 Supine 자세에서얻은적정압력은 lateral 자세에서얻은압력보다 2cmH 2 O 가량높다. 22 이는 REM 과 NREM, 비만한환자와비만하지않은환자, 60 세를기준으로해서이하와이상인사람에게서도마찬가지이다. 그외고려해야할요소로서는기류소실 (air leak) 이다. 여기에는마스크의구멍으로부터호기시이산화탄소배출을위한정상적인기류소실 (intentional air Vol.6, No.1 / June,

4 양광익 박형국 leak) 이있는데압력이올라가면서소실량이증가한다. 각압력에대한소실량은 liter/min 으로표시하며각마스크의종류마다다르고제조회사마다각압력에따른소실량에대한자료를제공한다. 하지만입을통한소실 (mouth leak), 마스크와안면사이를통한소실 (mask leak) 과같은비정상적소실 (unintentional leak) 은일반적으로소실량이크며적정검사중압력변화가없는데갑작스런소실량이증가하면이런비정상적소실에대한가능성을염두해두고마스크를재착용하여기류소실을최소화하도록노력해야하며구강호흡을할경우는 chinstrap 을사용하거나 full-face 마스크로교체해서적정검사를수행해야한다. 23,24 APAP 위에서언급한바와같이양압적정을위해서는검사실내에서수면다원검사의감시하에각수면단계및수면중환자의자세에따른폐쇄성호흡사건을감시하고비정상적호흡사건을제거하기위해수동으로압력을조절한다. 즉, 숙련된수면기사의감시가필요하고, 적정검사전에마스크착용과공기유출 (leak) 에대한중재가필요하며원할한적정검사를위해 CPAP 에대한첫대면을위한준비가필요하다. 25 고로검사비용, 불충분한적정이이루어졌을경우반복검사로인한불편감, 자신의침실과다른검사실환경에따른편차, 이로인해적정된압력의적합성, 시간이지나면서체중및동반된내과적조건의변화에따른적정압력의변화가능성, 26,27 REM 단계와 NRME 단계, supine 자세와 non-supine 자세와같이수면단계및자세에따른각각다른압력이적정될수있다. 10,11 이런점을고려한다면좀더효과적이고양압적정검사의간편성과상황에따른고정된압력이아닌변화하는압력을전달할수있는장비에대한요구가 APAP 발전의영감을불러일으켰다. APAP 은코골이진동, 공기흐름을통한무호흡, 저호흡및 flow contour, 압력변동, 상기도저항측정센서를통한되먹임을통해치료압력을변화시킨다. AASM 의 APAP 사용에대한권고사항 28 은 1) OSA 진단에사용하지않는다 (Standard). 2) 울혈성심부전, 만성폐쇄성호흡기 질환, OSA 이외다른원인에의한야간동맥내산소포화도감소, 코를골지않는경우, 그리고중추성수면무호흡등에서는사용하지않는다 (Standard). 3) Split-night 검사에서는사용하지않는다 (Standard). 4) 중등도내지심한 OSA 환자에적용할단일 CPAP 압력을정하기위해검사실내에서수면다원검사를통한 APAP 이사용될수있다 (Guideline). 5) 울혈성심부전, 만성폐쇄성호흡기질환, 중추성수면무호흡, 그리고저환기중후군등을동반하지않는중등도내지심한 OSA 환자에서사용할수있다 (Opinion). 6) 이와같은의미있는동반질환이없는중등도내지심한 OSA 환자의고정된 CPAP 압력을결정하기위해검사실외 (unattended) 에서사용할수있다 (Option). 7) APAP 적정을근거로하여고정된 CPAP 압력을사용하거나 APAP 을사용하는환자는그효과와안정성에대한추적관찰이필요하다 (Standard). 8) APAP 사용후효과가부족하거나 OSA 증상이해결되지않는다면표준검사실내양압적정이수행되어야한다 (Standard). 하지만 APAP 적정이검사실내야간 PSG 감시하에적정과직접비교한연구는아직까지없으며또한 APAP 에대한유용성연구가중등도내지중증 OSA를대상으로몇몇이루어져왔지만경증 OSA를대상으로한연구는없다. Split-Night 적정검사전반부는진단을위한검사이며후반부는적정검사로이루어져있어서편리하고비용절감효과가있다. 하지만양압적정시간이짧아올바른압력을찾지못할가능성이있다. 검사전반부에 maximum severity 를보이지않은경우의경증내지중등도 OSA 환자에서는적정압력이다소낮을수있지만, 29,30 full-night 적정검사를통한처방압력, CPAP 순응도및환자의적응도를비교하였을때의미있는차이는없었다. 31,32 AASM의 split-night 검사적응은 6 1) 진단검사기간이최소 2시간이상이며 AHI가적어도 40 이상인경우 2) AHI 가 20 에서 40 인경우는반복적으로긴폐쇄성호흡사건이동반하거나산소포화도가감소한경우이다. 하지만 AHI가 40 이하인경우는 full-night 검사를통한 32 수면

5 폐쇄성수면무호흡에대한양압적정의지침 적정압력보다다소정확도가떨어질수있다. 적정검사는잠의후반부로갈수록비정상적인호흡관련사건이증가할수있기때문에최소 3시간이상이필요하며 supine REM을포함하여야한다. 만약이조건에충족하지못하였다면 full-night 적정검사가필요하다. 결론 Sullivan 이후약 30년에걸쳐양압장치는성능및간편성면에서많은발전을해오고있다. 또한 CPAP/BPAP 을위한수기적정의비용, 편리성, 환자의내적외적요건의변화및장기적사용시간에따라첫번째적정검사에서결정된양압이지속적으로적합성을유지될수있는지에대한의문등을고려하여 APAP 역시발전하고있으며임상에서적용되고있다. 하지만현재로선 OSA 환자에게최적양압결정을위한 gold standard 는검사실내에서 PSG 감시하에이루어지는수기적정이다. 이의목적은폐쇄성호흡관련사건인무호흡, 저호흡, RERA, 코골이를제거할수있는압력을얻는것이다. 이를위해본론에서 2008 년 AASM 에서제시하는수기양압적정지침을중심으로폐쇄성수면무호흡에대한양압적정을다루었다. 이에대한이해및임상술기가필요하며향후첫번째양압적정검사를통해선택된최적압력에대한추적적정검사의유용성, OSA 의심한정도에따른 CPAP/BPAP 과 APAP 장치사용에따른각각의임상적효과에대한비교연구가필요하리라사료된다.
