IBD는평생관리해야하는질환이어서환자의삶의질에큰영향을미친다. 위장관의염증을조기에발견하고적절하게모니터링하는것이 IBD 의관리에있어매우중요하다. 그러나복통, 설사, 소화불량등의위장관증상은장염증상태를반영하는하나의수단이되지만매우주관적이고비특이적이며 C-reactive protein (CRP), 적혈구침강속도 (erythrocyte sedimentation rate, ESR) 등과같은혈청표지자들도널리사용되기는하나 IBD 를진단하는민감도와특이도가낮다. 현재로서는내시경이장염증을가장잘나타내는검사법이지만다른검사에비하여비싸고침습적이라는단점이있다. 따라서간단하고빠르고덜침습적이며동시에효과적인검사법이필요하며최근여러연구에서 fecal calprotectin (FC) 의효과가보고되고있다. 본고에서는장질환, 특히 IBD에있어 FC의사용법과성적에대하여알아보고자한다. Calprotectin Calprotectin은 1980년에과립백혈구혹은과립구에서처음으로분리되었는데주로중성구에서발견되는칼슘과아연결합단백이다 [1]. 세포가사멸되거나파괴되었을때분비되므로대변에서발견되는경우는중성구들이장관벽으로이동해서침윤되었다는것을의미한다. 따라서위장관에염증이있는경우에대변에서의 FC 농도가증가하게되는것이다 [2]. Calprotectin은대변에서세균에의하여잘분해되지않기때문에실온에서도거의 1주일가량안정적으로유지된다 [3]. 대변 calprotectin 의정상범위임상에적절하게사용하기위해서는 FC의표준화된 cut-off 값이중요하다. 대부분의연구에서는 50 μg/g을 cut-off 값으로설정하지만 IBD 환자에서는이보다더높은값이적절하다는의견들이제시되었다 [4]. 2 - Eun Soo Kim. Fecal calprotectin for intestinal diseases - 성인과비슷하게됨을주지해야한다. 한메타분석에서 FC는 cut-off 값을 50 μg/g으로하는경우에건강한성인에서 95% 민감도와 91% 의특이도를보인다 [5]. IBD 진단에있어서는 50 μg/g보다는 100 μg/g으로설정할경우, 더높은정확도를보였다 [6]. 반면, 질환의활성도확인은 FC를 250 μg/g으로설정하였을때민감도와특이도가각각 80-90%, 75-82% 로연구마다다양하게나타났다 [7,8]. IBD의진료에있어적절한 cut-off 값에대해서는추가연구가필요하다. 대변 calprotectin 결과에영향을주는인자 FC는대변에서안정적이지만실제환자에서는하루동안에도다양한농도로측정된다. 18명의활동성궤양성대장염환자들에서하루동안의 FC 변화를측정하였는데, 특히 FC 농도가높은군과심한활동성환자들에서변화가심하였다 [9]. 또한배변사이의주기가긴경우에농도가높게나와서주로아침첫배변에서검사할것을권장한다. 혈변보다는점액변이나무른변에서 FC 농도가높았다. FC 결과에영향을주는특정음식은없으나음주는높은 FC를보이는환자에서결과에변화를줄수있어검사전에삼가해야한다. 비스테로이드진통소염제, 아스피린과양성자펌프억제제복용은 FC 농도를높일수있어서결과해석에주의를요한다 [10]. 기능성질환과기질성질환의감별에서대변 calprotectin 의역할과민성장증후군과같은기능성질환에서 IBD 환자를감별하는것은매우중요하며, 대장내시경이중요한역할을한다. 덜침습적인검사법이있다면감별진단을위하여유용하게쓰일수있을것으로생각되며, 특히대장내시경을쉽게시행하지못하는소아에서는더욱유용할것이다. 대장내시경시행을위하여내원한 870명환자를대상으로한연구에서 FC는종양질환이나 IBD를감별하는데 89% 의민감도와 62% 의특이도를보였다 [11]. 2,475명의환자를분석한메타연구에서는 FC는기질성질환을감별하는데 83% 의민감도와 84% 의특이도를보였다 [4]. 특히 FC는다른염증마커인 CRP, ESR보다더우수한정확도를보여기능성질환환자에서불필요한대장내시경을피할수있는유용한검사임을증명하였다. FC를선별도구로사용하였을때성인에서불필요한대장내시경을 67% 감소시켰고소아에서도 35% 감소시켰다 [12]. 내시경활성도모니터링 IBD에서 FC는내시경및조직염증활성도와잘일치한다는연구들이발표되었다. CD의내시경활성도점수인 simple endoscopic score for CD (SES-CD) 와 FC가잘일치하며 cut-off 값을 70 μg/g으로하였을때내시경활동성을보이는 IBD 환자를 89% 의민감도, 72% 의특이도와 87% 의정확도로감별하였다 [13]. 또한 FC는 UC의내시경활성도점수인 UC endoscopic index of severity (UCEIS) 와도잘일치하였다 [14]. 최근 13개의연구를분석한메타연구에의하면 FC 는 CD보다는 UC에서더우수한결과를보였다 [7]. 다른메타연구에서도 FC가내시경활성도를진단할수있는정확도는 CD보다 UC에서높게나타나내시경활성도를보이는 IBD, 특히 UC를진단할수있는민감한바이오마커로제시되었다 [15]. 질환의재발예측높은 FC를보이는 IBD 환자의 90% 가향후 1년내에재발한반면, 낮은농도의 FC를보이는환자는 10% 에서만재발하였다 [16]. FC의 cut-off 값을 50 μg/g으로한경우에 90% 의만감도와 83% 의특이도로재발을예측할수있었다. FC는 ESR과 CRP에비하여재발예측의높은정확도를보여 [16], 이는 FC가위장관염증을직접적으로반영하는지표임을알수있다. 한연구에서 FC 300 μg/g 이상은 IBD 재발의독립적위험인자였다 [17]. IBD 환자에서 tumor necrosis factor 억제제를중단하고 1년간추적하였을때내시경재발이확인되기 2개월전부터 FC 농도가증가하기시작하였고관해를유지한환자에서는 FC 가정상으로유지되었다 [18]. 또한 infliximab을유지요법으로사용한환자에서도 FC는첫 2개월내재발을예측할수있었다. FC는 CD에비하여 UC에서우수하게재발을예측하였다. 다른연구에서 FC는 UC와대장침범 CD의재발과연관이있었으나회장 CD에서는일치도가높지않았다 [19]. 치료반응모니터링 FC는치료반응에따른염증상태를잘반영한다. UC 환자들을 18주간추적하면서연속적으로 FC를측정하였을때 FC 농도의감소는내시경, 조직학, 임상활성도의호전과관련이있었다 [20]. Infliximab과 adalimumab 을사용하고임상

다른연구에서도 FC 200 μg/g 이상은수술 1년후내시경재발을민감도 63% 와특이도 75% 정도로예 측할수있어 FC가수술후재발예측에유용한표지인자임을제시하였다 [24]. 그러나 FC가높은경우에도재발유무는추가적인내시경검사로확인해야한다. 증례 28세여자가 2개월간의복통으로방문하였다.

CD 수술후재발예측 CD는수술후높은재발률을보이므로재발을예측할수있는유용한지표가요구된다. 회장과대장을절제한후증상이없는 86명의 CD 환자에서내시경재발소견을보인군이관해군보다의미있게높은 FC 수치를보였고, 이를가장잘예측하는 cut-off 값은 100 μg/g이었다 [23]. 수술후재발을예측할수있는민감도, 특이도와음성예측도는각각 95%, 54% 와 93% 였다. 다른연구에서도 FC 200 μg/g 이상은수술 1년후내시경재발을민감도 63% 와특이도 75% 정도로예 측할수있어 FC가수술후재발예측에유용한표지인자임을제시하였다 [24]. 그러나 FC가높은경우에도재발유무는추가적인내시경검사로확인해야한다. 증례 28세여자가 2개월간의복통으로방문하였다. 주로배꼽주위에쑤시는양상의통증이었으며 1-2시간지속되었고배변이나식사와관련없이발생하였다. 1년전에동일한증상으로다른병원에서위내시경및대장내시경을시행하였으나특이소견이없어과민성장증후군으로치료받은병력이 Figure 1. Considerable single-day intra-individual variation in patients with active UC has been reported. No validated cut-off value in FC assays exists to define active IBD or remission. Several organic diseases, such as infectious gastroenteritis, colorectal cancer, diverticulitis, microscopic colitis, liver cirrhosis, and food allergy, are associated with elevated levels of FC. FC cannot be used to distinguish CD from UC. FC is less useful for children than adults. The accuracy of FC for detecting disease activity is higher in UC than in CD. 4 - 김은수. 장질환에서대변칼프로텍틴의사용 - 있다. 무른변양상으로하루 1-2회배변하였고혈변이나체중감소는없었다. 8년전항문농양및누공으로치료받은과거력이있었다. 신체진찰에서혈압 110/62 mmhg, 맥박 86회 / 분, 체온 36.4 였고복부는부드럽고압통은없었다. 혈액검사에서혈색소 9.1 mg/dl로낮았고 CRP를포함한다른검사에서는특이소견이없었다. 다시시행한대장내시경에서도말단회장을포함한전체대장점막에서특이소견이없었으나항문누공치료병력과빈혈소견등알람징후를보여주고있어기질적장질환을감별하기위하여 FC 검사를시행하였고수치는 2,000 μg/g ( 정상 < 50) 을초과하였다. 소장의염증을확인하기위하여시행한전산화단층소장조영술에서회장부위에다발성으로장관벽이두꺼워져있는소견이관찰되었고협착병변은보이지않았다 (Fig. 1). 캡슐내시경에서는전산화단층촬영에서보이는병변과동일한부위에서다발성궤양소견이관찰되어 (Fig. 2) 소장을침범한 CD로진단하였다. 소장만을침범하는 CD는서양과비교되는국내 CD의특징중하나이므로장기간의복통을호소하는환자에서대장내시경이정상이라고과민성장증후군으로진단하는것은매우위험하며본증례와같이 FC가기능성장질환과기질적장질환의감별에 FC 검사가큰도움이될수있다. 결론 Calprotectin은손상된중성구에서분비되는안정된단백물질로위장관염증을직접적으로반영해주는표지자이다. 최근많은연구들은 FC가 IBD의진단과활성도모니터링, 재발예측과치료반응예측등에효과가있음을보고하면서 IBD 진료에서유용한검사법임을제시하였다. 그러나현재까지명확한 cut-off 값이정의되지않았고소아및소장 CD 등에서효용성이낮은점등의제한점 (Table 1) 을잘알고임상에서적절하게사용하려는노력을기울여야할것이다. 특히비특이적인위장관증상을보이는환자에서혈변, 빈혈, 체중감소알람징후가있는경우는 FC 값과관계없이적극적인추가검사를시행해야한다. 중심단어 : 대변칼프로텍틴 ; 염증성장질환 REFERENCES 1.
