In this study, we investigated the clinical characteristics of LRTI, particularly in low birth weight children. Methods: We reviewed medical records of children at ages 0 6 years with LRTI in Korea University Anam Hospital between January and December of Clinical data including age, sex, birth history, viral pathogens, blood test results, and clinical courses were collected. Results: In the 828 eligible cases, 617 (74.5%) were pneumonia and followed by bronchiolitis 180 (21.7%) and bronchitis 31 (3.7%). The median age of the subjects was 17 months (interquartile range [IQR], 7 28 months), the median gestational age was 39.0 weeks (IQR, weeks) and the median birth weight was 3,200 g (IQR, 2,900 3,480 g). Sixty-four children (7.7%) were low birth weight ( < 2,500 g) and their median gestational age and birth weight were 33.0 weeks (IQR, weeks) and 2,045 g (IQR, 1,565 2,300 g), respectively. The rates of oxygen supplement (17.2% vs. 4.6%, P< 0.001) and systemic steroid use (20.3% vs. 4.7%, P< 0.001) were significantly higher in low birth weight children than normal birth weight children. Respiratory viruses were identified in 82.6% (519 of 628 subjects); RSV was detected in 240 subjects (38.2%), followed by rhinovirus 168 (26.8%) and adenoviruses 75 (11.9%). The distribution of respiratory viruses was not different between normal birth weight children and low birth weight children. Conclusion: Low birth weight children show more severe clinical manifestations than normal birth weight children during hospitalization for LRTI, although respiratory viral pathogens were not different. Clinicians should be aware that the severity may be increased when low birth weight children were hospitalized due to low respiratory tract infection. LRTI in low birth weight children 으나, 바이러스의종류에따른임상경과, 중증도등에대한보고는아직부족하다. 또한, 2008년 13종호흡기바이러스감염에대한임상적특징에대한국내보고 8 이후, 최근 10년이내의통합적보고는부족한상황이다. 따라서, 우리나라소아청소년입원의약 1/3을차지하는호흡기감염에서연중호흡기바이러스분포와연령별, 질환별특징및경과를분석하여임상양상을파악하는것은중요하다. 한편, 최근한국사회의결혼연령및불임의증가로시험관아기시술이많아지고미숙아출생또한증가하고있다. 9 우리나라통계청조사에따르면이번연구기간인 2014년도에태어난 2,500 g 미만저체중출생아의빈도는 5.7% 에달한다. 10 저체중출생아의경우, 폐용적의감소및기도기능저하로인하여호흡기감염의유병률과사망률이증가하는것으로알려져있다 특히저체중출생아는 RSV 감염의유병률이높고, 극소저체중출생아들은바이러스감염에의하여합병증과재입원비율이정상체중출생아에비하여유의하게높은것으로보고되었다. 14,15 현재까지우리나라의 60 개월미만의소아에서저체중출생아와정상체중출생아에서 RSV 단일바이러스감염의임상경과에대한분석은있으나, 16 급성하부호흡기감염입원환자중저체중출생아에서 20종호흡기바이러스검출률및임상양상에대한보고는없다. 이연구의목적은단일 3차의료기관에서 1년동안급성하부호흡기감염으로입원한 6세이하소아들의바이러스검출률, 계절에따른변동양상, 혈액검사및임상적양상등특징을관찰하고, 특히정상체중출생아에비해저체중출생아에서하부호흡기감염으로입원시바이러스검출빈도및임상양상의차이가있는지알아보고자하였다. 대상및방법 1. 대상 2014년 1월 1일부터 12월 31일까지고려대학교안암병원소아청소년과에서입원치료를받은환아중폐렴, 기관지염, 세기관지염의하부호흡기감염질환진단코드로작성된 6세이하 828명에대해입원당시의의무기록, 혈액검사, 치료경과등의무기록을후향적으로수집하였다. 천식, 심장질환, 신경학적질환등의동반질환이있는경우는분석대상에서제외하였다. 이연구는고려대학교안암병원기관생명윤리심의위원회의승인을받았다 (ED15317). 2. 방법 1) 대상자의특성입원당시의환아의성별, 나이, 재태연령, 출생체중, 임상증상, 혈액검사소견및치료와그에따른경과등에대한의무기록을후향적으로수집하였다. 6세이하대상군의연령을 2세미만과 2 6 세로나누어분석하였고, 특히출생체중을 2,500 g을기준으로정 상체중출생군과저체중출생군으로나누어임상적특징을비교하였다. 2) 혈액검사대상환아들의입원당일시행한혈액검사중, 백혈구수, 호산구분획, 적혈구침강속도, C 반응성단백, 간기능검사등에대한결과를수집하였다. 3) 호흡기바이러스및 Mycoplasma pneumoniae 항체검사비인두분비물에서호흡기바이러스검체를수집하였고, 입원당일멸균된흡인용튜브를이용하여의료진이비인두분비물을채취하였다. 이후역전사중합효소연쇄반응검사 (RV16 detection version 1.1, Seegene, Korea) 법으로 human adenovirus, coronavirus 229E/OC43/NL63, rhinovirus A/B/C, RSV A/B, PIV 1/2/3, bocavirus 1/2/3/4, influenza virus A/B, metapneumovirus 의아형을포함한총 20종호흡기바이러스를검사하였고, 아형을묶어 10개의바이러스로분류하여분석하였다. 마이코플라스마항체검사는간접효소면역검사법을이용하여 IgG와 IgM 항체를정량적으로측정하였으며 (RIDASCREEN Mycoplasma pneumoniae IgM; R-Biopharm AG, Darmstadt, Germany), Mycoplasma pneumoniae IgM이양성 (>71.0 U/mL) 으로측정된경우마이코플라스마감염으로정의하였다. 17 4) 하부호흡기질환의진단대부분환아에서기침, 가래등호흡기증상을호소하였고, 이중청진에서수포음이들리거나흉부방사선사진에서폐침윤을보이면폐렴으로진단하였으며, 청진에서호기성천명이있거나흉부방사선사진에서과팽창소견이보이면세기관지염으로진단하였다. 기침, 가래또는발열등호흡기증상이있으나흉부방사선사진에서뚜렷한폐침범이없는경우기관지염으로구분하였다. 18 5) 하부호흡기질환의치료입원기간동안환아의임상경과에따라보존적치료를시행하였고, 맥박산소계측기로측정한산소포화도가 92% 이하인경우에산소를공급하였다. 스테로이드제는폐렴의경우보존적치료에도불구하고발열이 5일이상지속되거나흉막액이동반되었을때, 세기관지염은보존적치료에도천명음이지속되거나호흡곤란이호전되지않았을때사용하였다. 기관지염은보존적치료에도발열이 5일이상지속되었을때사용하였다. 19 6) 통계분석조사된자료의값은빈도 (%) 또는중위수 ( 사분범위 ) 로표기하였다. 정규분포를따르지않는경우두군사이의연속변수의비교는 212

3 윤윤선외 저체중출생아에서하부호흡기감염 Mann-Whitney test를, 세군이상의연속변수에대한검정은 Kruskal-Wallis test를이용하였고, 각군간의차이는 Bonferroni 방법을이용하였다. 스테로이드제투여, 산소치료, 항생제치료유무등범주형변수에대한빈도분석은 chi-square test를사용하였고, 기대빈도가 5보다작은셀이전체의 20% 이상인경우에는 Fisher exact test를이용하였다. 통계분석은 IBM SPSS Statistics ver (IBM Co., Armonk, NY, USA) 을이용하였고 P값이 0.05 미만인경우를유의한것으로해석하였다. 결과 1. 대상환아군의분포및특징 2014년 1년동안급성하부호흡기감염으로입원한 6세이하환아중총 828명에서의무기록분석이가능하였다. 연령의중위수 ( 사분범위 ) 는 17.0개월 ( 개월) 이었으며, 남아가 476명 (57.5%) 으로여아보다약간많았다. 총대상자의출생체중의중위수 ( 사분범위 ) 는 3,200 g (2,900 3,480 g) 이었고, 출생체중 2,500 g 이상의정상체중출생아는 764명 (92.3%), 저체중출생아 ( 출생체중 2,500 g 미만 ) 는 64명 (7.7%) 이었다. 재태연령중위수 ( 사분범위 ) 는 39.0주 ( 주), 37주미만출생미숙아는 76명 (9.2%) 이었다. 질환별로는폐렴 617명 (74.5%), 세기관지염 180명 (21.7%), 기관지염 31명 (3.7%) 의빈도를보였다. 총대상자 828명중 628명 (75.8%) 에서호흡기바이러스 PCR 검사를시행하였고, 그중 519명 (82.6%) 에서바이러스가검출되었다. RSV 240명 (38.2%), rhinovirus 168명 (26.8%), adenovirus 75명 (11.9%) 과 metapneumovirus 62명 (9.9%) 의순이었다 (Table 1). 2. 질환별임상양상및혈액검사결과비교대상아의질환별연령의중위수 ( 사분범위 ) 는세기관지염 5.0개월 ( 개월), 폐렴 21.0개월 ( 개월), 기관지염 19.0개월 ( 개월) 로세기관지염에서가장낮은연령을보였다 (P < 0.001). 발열기간은폐렴 5.0일 ( 일), 기관지염 2.0일 ( 일 ), 세기관지염 1.0일 ( 일) 로폐렴에서가장길게나타났다 (P < 0.001). 혈액검사결과적혈구침강속도가폐렴에서 17.0 mm/hr ( mm/hour) 로가장높았고, C반응성단백은기관지염에서 12.9 mg/l ( mg/l) 로가장높았다. 폐렴에서는 RSV 152명 (30.8%), rhinovirus 120명 (24.3%), adenovirus 59명 (12.0%), 세기관지염에서도 RSV 86명 (52.4%), rhinovirus 39명 (23.8%), adenovirus 14명 (8.5%) 의순으로바이러스가검출되었다. 기관지염에서는 rhinovirus 9명 (56.3%) 이가장높은빈도였다. 항생제치료는기관지염 10명 (32.3%), 폐렴 83명 (13.5%), 세기관지염 10명 (5.6%) 순서였고, 전신스테로이드제사용은세기관지염 28명 (15.6%), 기관지염 3명 (9.7%), 폐렴 18명 (2.9%) 순서로높은빈 Table 1. Demographics of study subjects (n= 828) Variable 도를보였다. 산소치료율은세기관지염 29 명 (16.1%), 폐렴 17 명 (2.8%) 의빈도였다 (Table 2). 3. 저체중출생아의임상적특징 정상체중출생아 764 명의재태연령의중위수 ( 사분범위 ) 는 39.0 주 ( 주 ), 출생체중의중위수 ( 사분범위 ) 는 3,250 g (3,000 3,500 g) 이었고, 64 명의저체중출생아는재태연령의중위수 ( 사분 범위 ) 가 33.0 주 ( 주 ), 출생체중의중위수 ( 사분범위 ) 가 2,045 g (1,565 2,300 g) 이었다. 정상체중출생아의입원시연령은 17.0 개월 ( 개월 ), 저체중출생아의입원시연령은 20.0 개월 ( 개월 ) 로정상체중출생아와저체중출생아사이의입원 시연령의차이는없었다. Preterm (gestational age < 37 weeks). 두군에서백혈구수, 적혈구침강속도, C 반응성단백, 간기능검 사, 호산구분획의혈액검사및호흡기바이러스검출빈도는차이 가없었다. 그러나정상체중출생아군에서 36 명 (4.7%) 이전신스테 로이드제치료를받은반면, 저체중출생아군에서는 11 명 (17.2%) 으 로저체중출생아군에서유의하게스테로이드제치료율이높았다 (P<0.001). 또한산소포화도저하로산소치료를받은경우는저 213

P< 0.05, com-pared with pneumonia (Bonferroni correction). 체중출생아군에서 11명 (17.2%) 으로정상체중출생아군의 35명 (4.6%) 보다유의하게많았다 (P<0.001). 반면, 항생제치료빈도는정상체중출생아군과저체중출생아군간의유의한차이가없었다 (Table 3). 저체중출생아 64명중에서 1,500 g 미만극소저체중출생아는 14명이었고, 스테로이드제사용이 1,500 g 미만출생아에서 3명 (21.4%), 1,500 g 이상 2,500 g 미만출생아에서 10명 (20.0%) 이었고, 2,500 g 이상정상체중출생아에서는 97명 (12.7%) 으로출생체중이적을수록스테로이드제사용빈도가유의하게높았으며 (P<0.001), 산소치료에서도 1,500 g 미만출생아 4명 (28.6%), 1,500 g 이상 2,500 g 미만출생아 7명 (14.0%), 그리고 2,500 g 이상정상체중출생아에서 35명 (4.6%) 으로산소치료빈도가출생체중이적을수록유의하게더높았다 (P<0.001). 4. 연령에따른임상양상및혈액검사결과비교 Table 4에서는 2세미만과 2 6세대상아의급성하부호흡기감염의임상양상및혈액검사결과를비교하였다. 2세미만에서저체중출생아는 37명 (6.9%), 2 6세의저체중출생아는 27명 (9.4%) 으 로두군간에유의한차이는없었다 (P = 0.195). 폐렴은 2세미만에서 357명 (66.1%), 2 6세에서 260명 (90.3%) 으로 2 6세에서많았으나, 세기관지염은 2세미만에서 166명 (30.7%), 2 6세 14명 (4.9%) 으로 2세미만에서높은빈도를보였다. 기관지염은 2세미만에서 17 명 (3.1%), 2 6세 14명 (4.9%) 으로연장아에서높은비율이었다. Mycoplasma pneumoniae IgM 양성빈도는 2세미만 6명 (1.1%), 2 6세 24명 (8.3%) 으로 2 6세연령에서비율이높았고 (P<0.001), RSV는 2세미만에서 172명 (31.7%), 2 6세에서 68명 (25.7%) 으로 2 세미만에서유의하게비율이높았다 (P = 0.026). Metapneumovirus는 2세미만 28명 (5.2%), 2 6세 34명 (12.8%) 으로 2 6세에서유의하게비율이높았다 (P < 0.001). 항생제치료는 2세미만 64명 (11.9%), 2 6세 39명 (13.5%) 으로통계적으로유의한차이가없었고 (P = 0.472), 스테로이드제치료는 2세미만 38명 (7.0%), 2 6세 11명 (3.8%) 으로 2세미만에서약간많았다 (P = 0.062). 산소치료는 2세미만 42명 (7.8%), 2 6세 4명 (1.4%) 으로 2세미만연령에서유의하게높은빈도를보였다 (P<0.001)

LRTI, lower respiratory tract infection; WBC, white blood cell; ESR, erythrocyte sedimentation rate; CRP, c-reactive protein; AST, aspartate transaminase; ALT, alanine transaminase; PCR, polymerase chain reaction; RSV, respiratory syncytial virus; M. pneumonia, Mycoplasma pneumonia. 5. 바이러스검출의월별분포바이러스검출의계절별분포를보면 3 5월에는 rhinovirus, 6 8 월에는 PIV, 12월에는 RSV A, 그리고 1 2월에는 RSV B와 influenza virus가높은빈도를보였다 (Fig. 1). 고찰이연구에서는 1년간급성하부호흡기감염으로입원한 6세이하소아 828명의의무기록을후향적으로분석하였다. 질환의빈도는 폐렴, 세기관지염, 기관지염순서였고, 원인바이러스는 RSV, rhinovirus, metapneumovirus, adenovirus 순서로검출비율이높았다. 계절별로는봄에는 rhinovirus, 여름에는 PIV, 가을과겨울에는 RSV의검출비율이높았다. 828명중저체중출생아 ( 출생체중 <2,500 g) 는 64명 (7.7%) 으로산소투여및스테로이드제치료비율이유의하게높았다. 호흡기바이러스검출분포는정상체중출생아군과저체중출생아군에서유의한차이가없었다. 이연구는단일 3차기관에서 1년동안하부호흡기감염으로입원했던총 828명의의무기록을분석한연구이며, 과거연구에서는 215

Monthly distribution of respiratory viruses from subjects with lower respiratory tract infections between January to December in RSV, respiratory syncytial virus. 13종미만의원인바이러스를분석하였던것에비해이연구에서는아형을포함한 20종바이러스검사가포함되었고, 특히재태연령과출생체중을포함하여출생력에따른향후하부호흡기감염으로입원시임상적특징을관찰하는데의의가있다. 이연구에서남아는 476명 (57.5%) 으로남아가약간많았는데, 이는우리나라 2014년도통계청출생자료에의하면 2014년도인구분포가남아 22만 3천 4백명, 여아 21만 2천 1백명으로남아가여아보다 1.1배정도높은점과일치하여남아가호흡기감염으로인한입원의위험인자로볼수는없었다. 10 한편소아의호흡기바이러스감염으로인한입원은 3개월에서 24개월사이에집중되었고연령이증가하면서감소함을알수있었다. 이는연령이증가할수록기도가커지고, 흉벽이견고해짐에따라저산소상태를극복하는능력이증가하기때문인것으로생각된다. 다만, 연령이높을수록 C 반응성단백과백혈구수치가높았는데, 이는높은연령에서는중증도가증가할때입원하는경향을나타낸것으로보인다. 이이연구에서의질환의빈도는폐렴, 세기관지염, 기관지염의순서였다. 국내다른연구에서도대부분폐렴이가장높은빈도를보였다. 8,20-22 이연구에서 2,500 g 미만저체중출생아는 64명으로전체의 7.7% 에해당하였으며, 출생체중중위수는 2,045 g ( 사분범위, 1,565 2,300 g), 출생주수중위수는 33.0주 ( 사분범위, 주 ) 였다. 저체중출생아군에서는폐렴 37명 (57.8%), 세기관지염 24 명 (37.5%), 기관지염 3명 (4.7%) 순으로정상체중출생아군에서폐렴이월등히많았던것과달리세기관지염의빈도가유의하게높았다. 이는저체중출생아에서말단기관지발달이정상체중출생아 에비하여견고하지못함에기인하는것으로보인다. 또한정상체중출생군에비하여저체중출생군에서스테로이드제치료와산소치료빈도가높게나타났다. 입원기간중산소포화도 92% 이하로저산소증을보이는경우산소를공급하였고, 발열, 흉곽함몰, 빈호흡등호흡곤란이보존적치료에도호전을보이지않을시전신스테로이드제를투여하였다. 즉, 산소투여와스테로이드제치료는하부호흡기감염에의한임상적경과에서중증도가높음을의미하는데저체중출생아군에서하부호흡기감염시중증도가더높음을시사한다. 이는저체중출생아의폐용적이정상체중출생아에비하여작고, 호흡기감염시중증의임상경과를가질가능성이높음을의미한다. 기존연구에서만삭아중저체중출생아에서낮은폐기능을보이고기도과민반응이증가하며알레르기감작의위험이증가한다는보고가있고, 23 저체중출생아와미숙아를비교한연구에서미숙아로인한폐의기능은성장과정중회복되지만저체중출생으로인한폐용적저하와과민반응은지속된다는연구결과가있어 24 저체중출생아에서하부호흡기감염시더큰영향을받을수있는것으로보여진다. 하지만이연구의저체중출생아 64명중재태연령 37주미만의미숙아가 53명 (82.8%) 포함되어있어미숙아자체에의한영향도있을것으로생각한다. 또한, Walter 등 11 은저체중출생아는정상체중출생아에비해성인이되었을때에도호흡기질환으로입원할확률이더높고만성천명을보일확률이높다고보고하였다. 또다른연구에서도극소저체중아에서 3세에천식으로진단된경우가정상체중출생아에비해월등히높았다고하며, 11,25 극소저체중아의장기관찰연구에서 216

7 윤윤선외 저체중출생아에서하부호흡기감염 도 26 20세까지성장했을때천식유병률이더높았다고하여저체중출생아에서호흡기질환의유병률의위험도가높음을시사하였다. 저체중출생아및미숙아는물리적으로좁은기도를가지며특히기계환기및산소치료를받은경우에기도손상이발생할수있고이로인해호흡기감염시정상체중출생아또는만삭아에비하여취약하다. 27 면역학적인측면에서도임신 3분기에면역글로불린이태반을통과하여태아에서의농도를유지하고신생아시기까지영향을주는데저체중출생아또는미숙아일경우면역글로불린농도가낮아바이러스감염에취약할수있다 년도통계청출생통계자료에따르면우리나라산모의평균출산연령은 32.04세로 2013년보다 0.2세상승, 10년전의 세보다상승하였고 35세이상고령산모구성비도 21.6% 로지속적인증가추세를보였다. 고령산모증가에따른고위험산모비율이증가하고, 또한 37주미만미숙아출생아의구성비는 6.7%, 2,500 g 미만저체중출생아의비율은 5.7% 로 2013년대비 0.2% 증가, 10년전 4.1% 보다 1.6% 증가하였다. 10 또한, 우리나라의미숙아와정상출생아의호흡기감염에따른임상경과비교연구에서도기관지폐이형성증을가진미숙아에서정상아에비하여기침의기간이길고천명음이호전되는기간이유의하게길었으며, 정상체중출생아에비해산소치료가필요한경우가많았고산소의치료기간도길었다. 14 따라서, 저체중출생아가하부호흡기질환감염시중증도가증가할수있다는이연구의결과는의미를가진다. 또한, 이연구에서봄에는 rhinovirus, 여름에는 PIV, 가을에는 RSV, 겨울에는 RSV와 influenza virus 빈도가높았는데이는 2014 년도질병관리본부에서발표한자료와일치하였고, 우리나라다른연구결과와도일치하였다. 28,29 Influenza virus는하부호흡기감염입원환아에서검출빈도가낮았다. 이는백신의개발로인해빈도와중증도가감소하였다는것으로해석할수있다. 30 단, 바이러스감염의연중분포가그해의기후에영향을받을수있고, 지역에따라다를수있으며, 신종바이러스출현의영향이있을수있기때문에그해의특이사항을질병관리본부감시체계에서확인하는것이필요하다. 이연구의한계점은단일기관에서시행한연구로, 대한민국서울지역에국한되어저체중출생아의인원이상대적으로작았다. 또한후향적의무기록분석연구로스테로이드, 산소치료외에도점수체계등을이용한급성하부호흡기감염증상의중증도판정의다른객관화된지표를사용하지못하였으며, 병력청취상기관지폐이형성증여부, 신생아중환자실에서의기계환기, 산소치료기간, 모유수유, 간접흡연노출등에대한구체적인정보를모든환자에서얻지는못하였다. 앞으로좀더객관화된지표를가지고장기간, 다기관의연구를통하여출생체중과임상경과의관계에대한분석이필요하다. 이연구에서연간하부호흡기감염으로입원한 6세이하환아들 의의무기록을통해특징을분석한결과질환, 연령, 원인, 출생체중 등에따라임상양상에차이가있으며, 계절에따라다른종류의원 인바이러스가검출되었다. 특히저체중출생아에서정상체중출 생아에비해전신스테로이드제사용과산소투여비율이높았다. 이연구는 1 년동안급성하부호흡기질환으로입원한환아의임상 경과를분석하였고, 특히저체중출생아에서급성하부호흡기감염 시높은임상적중증도는임상의에게주의가필요함을시사한다.
