2 임동준 윤 관 정용구 김봉룡 정흥섭 이기찬 이훈갑 서중근 종과 뇌척수액 또는 경막혈관내 혈액의 삼투압 차이의 결과 대상 및 방법 로 발생한다는 이론이 제시되고 있다1)5)7). 뇌 전산화단층촬영 (Computed Tomography, CT)상에서 만성 뇌경막하혈종의 음영은 여러 형태의 다양한 밀도로 관찰되는데, 이러한 다양성 1991년 1월부터 1998년 3월까지 만성 뇌경막하출혈로 은 재출혈의 시기와 혈종막의 투과성과 관련되어 재출혈된 혈 본원에 내원하여 천두폐쇄배액술을 시행한 166명의 환자 종과 액화된 혈종의 혼합의 결과로 발생한다12). 수술적 치료방 중 양측성으로 발생한 36명(21.7%)의 환자를 포함하여 모 법으로는 천공술을 이용한 배액술과 개두술을 통한 혈종막제거 두 202례를 대상으로 하였다. 전형적인 신생 혈종막과 혈종 술의 두가지로 나누어지며, 효과와 안정성에서 저자들간에 의 내에 전형적인 액화된 혈액의 존재가 수술동안 확인되고, 급 견의 차이가 있으나 술기가 간단하고 효과적인 면을 고려하여 성기의 뇌경막하혈종이 있었던 경우에는 적어도 3주 이상 시 전자의 천두폐쇄배액술이 보편적으로 시행되고 있다4)6)10)14)15). 간이 경과된 경우 만성 뇌경막하혈종으로 진단하였다. 수술전 천두배액술후 재발되어 재수술이 필요한 경우는 저자마다 약간 151명(184례)의 환자에서는 컴퓨터단층촬영을, 15명(18례) 3)9) 의 차이가 있으나 5%에서 24%까지 보고되고 있다. 의 환자에서는 뇌자기공명촬영(Magnetic Resonance Image, 저자들은 천두폐쇄배액술을 시행한 만성 뇌경막하혈종환자 MRI)을 수술당일 혹은 수술 하루 전에 시행하였다. 만성 뇌 에서 술전 CT음영과 혈종막의 투과성과 관련되는 술후 배액 경막하혈종을 술전 CT소견에 따라 회백질의 음영을 기준으 량 및 재발과의 연관성을 비교 연구하여, 술후 배액량과 만성 로 비교하여 저 음영, 등 음영, 층 음영, 고 음영, 혼합 음영 뇌경막하혈종의 재발과의 연관성과 천두폐쇄배액술이 술전 의 5가지로 분류하였고, 술전 MRI소견에 따라 T1강조영상 CT상의 다양한 음영의 형태에 관계없이 모든 예에서 유용한 (Weighted Image, WI)의 고 음영과 나머지의 2가지로 분 수술방법인지에 대하여 관찰하고자 본 연구를 시행하였다. 류하였다(Fig. 1). 수술방법은 CT 소견과 관계없이 모든 환 A B D C E Fig. 1.
