1 2002 년도대한쉐담도연구회학술대회口 LECTURE 口 춰 l 장질환의약물요법 대구가톨릭대학교의과대학내과학교실 김 호 각 억제제인 gabexate rnesilate 에관해그엄상적유 서 론 용성을알아보고자한다. 급성춰l장엽은 80% 이상이경증으로대부분자연치유되지만, 10-20% 에서춰l 장괴사가동반되는중증의형태로진행하며 I 이러한중증의급성춰l장염은춰l 액성복수, 춰1장가성낭종등의합병증을일으킬수있으며사망률이 10-20% 로매우높은데, 현재까지급성혜장염에대한다양한치료법이시행되고있지만고식적인치료외에는사망률을낮추거나합병증을줄일수있는효과적인약물치료법은없는실정이다만성춰l장염은엄상적으로춰l장의염증, 춰l장주변의신경의염증, 춰l장실질혹은춰l관의압력의증가, 총수담관및십이지장의협착등에의해유발되는통증과혜장실질이파괴되어서일어나는설사, 체중감소, 소화불량등의외분비춰l장기능부전 (exocrine pancreatic insufficiency) 이문제가된다. 급성춰l 장엽의약물요법으로는손상된춰l장에서분비되는활성효소에의해춰l장과주변조직으로염증이진행하는것을감소시키기위한쉐장외분비억체제, 춰l 장의선방세포 (acinar cell) 내단백분해효소의활성화에의한자가소화 (autodigestion) 를막기위한단백분해효소억제제 (proterase inhibitor) 가있으며, 만성춰l장엽의약물요법으로는통증과혜장성흉수장애에의한설사의치료를위한춰l 효소제 (pancreatic enzyme) 가있다. 여기서는 Jipase, protease, arnylase를함유한춰l 효소제요법, 춰l 장외분비억제제인 somatostatin 및그유사체인 ocπeotide, 그리고단백분해효소 춰 효소제 1. 휘 장기능부전에서의춰 효소제요법 1) 치료기전 : 쉐장에서분비되는소화효소 중 lipase 의 결핍이있으면식이중의중성지방 산이분해가안되어설사, 지방변 (steatorrhea) 이 생긴다. 춰 l 효소에의한지방가수분해에는 lipase 가필수이며가수분해된지방산이소장에서흉 수가되기위해서는유리지방산이담즙산과 rnicelle 을형성하여야하며, rnicelle 형성을위해서는 십이지장의 ph 가춰 l 장에서분비된 bicarbonate 에 의해알칼리쪽으로높아져있어야한다 즉쉐 장성지방변의치료를위해서는 lipase 의투여와 함께섭이지장의 ph 가 4 이상으로유지되어야한 다. 쉐효소중 tripsin 과 chymotripsin 이부족하면 단백변 (azotorrhea) 이초래되며 tripsin 과 chymotripsin 은 lipase 에비해위산에강하며, 경구용춰 l 효소투여로쉽게단백변이소설될수가있다. 춰 l 장부전에서는타액의 amylase 가보상적으로증 가하며경구용춰 l 효소투여로쉽게해결될수가 있어서 amylase 부족에의한탄수화물의흉수장애는별문제가되지않는다. 춰l효소제요법의목적은외분비춰l장기능부전을보전해주고외분비기능의소실에의한증상의치료에있다. 2) 어느정도의지방번에서훼효소제요법을시작하나? 경한지방변은증상이별로없고체 11

2 년도대한혜담도연구회학술대회 중감소만경하게일으킬수있어서치료가필요없다. 지방변이어느정도심할때에쉐효소제요법을시작하느냐가문제인데일반적으로하루지방소설이 15 gm 이상혹은체중이감소되고있거나, 설사혹은소화불량증세가있으면치료를시작한다. 3) 약제필요량 흉수장애를치료하기위해서는십이지장으로전달되는쉐효소의양이쉐장의최대분비능의 5-10% 정도가되어야하며춰l효소가위나십이지장에서변성이없다면매식사마다약 30,000단위의 lipase를섭취하여야한다. 이는하루약 5-10 g의 pancreatin에해당한다상품화된 pancreatin은대부분돼지춰l 장추출물로만든것이데제조공정에따라차이가있으나 1 g의 pancreatin에는 30,000 FIP (Federation Internationale Pharmaceutique, 1 FIP= 1 United States Pharmacopoeia) 의 lipase 활성도, 1,200 FIP (IFIP=62.5 USP) 의 protease 활성도, 24,000 FIP (1 FIP=4.15 USP) 의 amylase 활성도가포함되어있다. 경구섭취한 pancreatin은위소장을지나면서활성도를많이소실하게되는데이중 tripsin 은 22%, lipase는 8% 만이 ligament of Treitz에서그활성도를유지할수있다결국쉐장효소제요법의효과의가장중요한요소는 lipase의투여양이다. 시중에서시판되는쉐장효소제는 pancreatin이 mg 이상이되는제품의경우매식사마다 3정정도, pancreatm양이적은경우는 5-6정의투여가필요하다 (Table 1). 4) 건강보험인정범우 1: 시판되는제재에따라차이가있지만 pancreatin이 400 mg이포함된제재인경우식사때에 2-4 캡슐, 간식때에 1-2 캡슐을투여하면된다. 그러나우리나라의경우 lipase, protease, amylase의소화력을같이포함하고있는춰l효소제는몇가지만쉐장성소화기능부전, 지방변증에서적응증으로인정받고있으며 pancrelipase만포함하고있는쉐효소제는춰l 외분비장애, 만성쉐장엽, 춰l장암으로인한춰l 담관폐쇄, 위장관수술후및춰l장절제술후소화장애에적응증으로인정받고있다 ( 참조 line.co.kr)(table 1). 나머지의 대부분의춰 l 효소약 제는의료보험에쉐장기능부전의치료제로효능, 효과가등록되어있지않고소화제로등록되어 있으며 2002 년 4 월 1 일부터는일반약으로재분 류되어보험적용을못받게된다. 5) 약제효과 웨효소제치료의효과는환자에 게서체중의증가, 설사, 복통그리고복부팽만 의호전을확인함으로서판정할수있다 혜효소제투여로단백변은지방변보다더 게호전된다그이유로는첫째, 만성춰 l 장염의 경우 protease 분비능이 lipase 분비능보다더오 래보존되며, 둘째, lipase 보다 πypsin 이십이지장 에서비활성화가적다 쉽 춰 l 장기능부전에서쉐효 소를투여해도반응이없는경우는쉐효소의처 방및투약양이적은경우나투여된춰 l 효소가 위산에의해비활성화되었거나위에서십이지 장으로음식과동시에넘어가지못한경우등이 다. 웨장 lipase 는비가역적으로 ph 4 미만의위 산에의해비활성화된다춰 I 장기능부전의경우 춰l 액중 bicarbonate의분비가줄어서십이지장의 ph 가산성쪽으로떨어지게되어투여된춰 l 장효 소제가활성을잃게된다. 6) 부작용 : 대부분의상용화된 pancreatin 은돼 지의혜장추출물이므로돼지단백에과민성이 있는경우는투여를피해야한다. 쉐장추출물을 투여받은외분비쉐장기능부전환자나, 투여한 가족이나간호사에서 즉시과민성반응 (immediate hypersensitivity reaction) 이보고된바가있다 8,9 최근의몇몇보고에의하면 cystic fibrosis 가있 는소아에서많은양의고단위쉐효소를투여한 경우대장협착을볼수있었다 IO 량의 소아에서는과 춰 l 효소투여는주의를요해야하나성인에 서는장협착이보고된바가없다 경구용웨장 추출물의투여에의한엽산흡수장애및철분흉 수장애도보고된경우가있다 11, 12 끼약제의제형 : 시판되고있는춰 l 효소제에는 분말, 정제, 캡슐등의여러가지종류의제형이 있으며캡슐에는미죽 (chyme) 과잘섞이게하기 위해 microsphere 혹은 microtablet 등의 형태로

4 년도대한혜담도연구회학술대회 때문에일반혜효소정제보다우수하지만 20 코멍의 ph 민감도에따라, 위산의분비량에따라, lipase의함유량에따라차이가있다. 최근의조사에서는세균에서추출한 lipase가흔히쓰이는돼지에서추출한 lipase보다사람의위십이지장내용물에서더활동도가우수하다고하여 21 개발이진행되고있다. 8) 휘 효소제에대한식이영양소의영향 지방분해능은식이중의단백성분이많을경우높고탄수화물성분이많을수록낮다. 따라서춰l장기능부전이있는경우식이는고단백식, 저탄수화물식이추천되고있다. 식이중의섬유질은쉐효소제의효과를낮추기때문에만성쉐장염에서고섬유질식이는흉수장애를악화시키므로혜효소요법중인경우는섬유질은금하는것이좋다 22 배출되어야하며, 4) 십이지장의알칼리환경에서빠르게유리되어야한다. 외분비쉐장기능부전이있으면서위산분비가정상인경우는장용코탱제제를사용하고, 저위산증이있거나위절제술을받은경우는과립형태의제제를투여하는것이이상적이다. 지방변이있을때혜효소제의추천용량은매식사와함께 25,000-40,000 단위의 pancreatin을투여하며, 효과판정은설사가줄고, 복통이나복부팽만감이줄거나, 대변의양과지방이줄어드는것으로알수있다. 만일증상호전이충분치않거나치료에실패시에는효소제양을 2-3 배증량하거나, 환자가충분히약을복용하고있는지확 9) 훼효소제의십이지장으로의전달 : 식후에경 구투여한춰l호소제의위에서의배출정도는효소제의효능과관련이있다. 1 mm 미만의과립은 2.4 mm의과립보다빨리위에서십이지장으로배출이된다 23 웨효소체의업자가 l.4 mm 인경우에미죽 (chyme) 과가장완전히섞이며위에서음식과같은속도로십이지장으로배출된다. 입자가큰경우는음식이내려간뒤에도위에남아있어서흉수장애의치료에효과적이지못하다. 5 김호각웨장질환의약물요법 15 인 ô~ 고, blind loop syndrome, bacterial overgrowth, terminal ileal disease 등의다른질환으로인한지방변은아닌지를확인해보고, 마지막으로하루 40-60g 미만의저지방식을 권유하거나혹은 H 2- 수용체걸항제나 proton pump inhibitor를추가해본다 (Fig. 1) 만성쉐장염중에서도우세한증상에따라효소제를차별해서다르게선택할수있다. Protease 특히 chymotrypsin은 lipase를비활성화시킬수있기때문에흉수장애가저명한경우는 lipase가많고 protease가적게포함된제재를, 통증이저명한증상일경우는 lipase가적고 protease가많이포함된제재를선택하는것이좋다 2. 통증에서의훼효소제요법 1) 치료기전. 경구용쉐효소제가만성쉐장염에의한통증을경감하고악화를방지하는기전으로는춰l 액분비의 feedback regulation이있다. 즉만성쉐장염에의한십이지장내의낮은 protease 활성도가십이지장에서 ch이 ecystokinin (CCK) 분비를자극하고 CCK는쉐장에서효소분비를자극한다는것이다 (Fig. 2).25 통증이있S며십이지장내 protease 활성도가냥은만성혜장염의경우경구로투여한쉐효소제는십이지장내의 protease 활성도를증가시키며, 이는 CCK에의한춰l 분비자극을감소시켜서춰l 관내의압력을낮추고통증을경감시킬수있다. 최근까지많은연구에서통증에대한조사가시행되었는데일부의연구에서는통증감소가있었고 26 장용코탱정이나미세과립의춰l 효소제를투여한경우에는통증치료에효과가없었다 27 이는활성이있는춰I 효소의유리가십이지장보다하부에서분비되 었기때문으로보고있다. 따라서십이지장에서쉽게유리되는효소제를사용하여야한다. 또한특발성웨장염및경도의알콜성춰l장염의통증에서는경구용춰l 효소제는통증치료에효과가있으나많이진행한알콜성춰l장염에서는효과가없었다 28,29 통증경감의가장좋은성적으로는환자스스로가통증이없어질때까지용량을증가시키면서복용하는방법이며이는상용량의복용보다통증감소의효과가좋았다 30 2) 훼장성통증메서의훼효소제치료결론 : Medline을통한 meta-analysis에서경구용춰l효소 요법은통증해소에효과가없다고하나 31 통증이있는만성웨장염의경우에고용량의 protease가많이함유된비장용코멍정 (nonenteric coated tablet) 을식사와함께수주간투여하여본다. 비알콜성쉐장염, 재발성이아닌꾸준한통증, 경도미만 의춰l 장기능부전의춰l장염에서가장좋은효과가기대된다 32 Nonnal Pancreas: Basal Secretion Chronic Pancreatitis Chronic Pancreatitis+ Oral Enzyme Therapy 4>> = CCK-releasing enzyme Pancreatic enzymes (proteases) Fig.2. πle mechalùsm by which oral pancreatic enzyme therapy may reduce the pain of chrolùc pancreatltls.

6 년도대한혜담도연구회학술대회 훼장외분비억제제 1. 춰 장외분비억제제의개요이제까지쉐장염치료에이용된춰l 장외분비억제제에는 H2 -수용체걸항제, aπopine, calcitonin 그리고 glucagon과같은항 choline제등이있었으나 33,34 급성춰l장염의치료에는임상적유용성이없는것으로판명되어왔다. 그러나 1973년에 somatostatin이발견이되고3 5 뒤이어그유사체인 ocσ'eotide가개발되어실험동물과사람의급성 만성쉐장염과합병증의치료에쉐장외분비억제제로써의임상적적용가능성이보고되고있 다 았다. (SMS , Sandostatin@, 스위스 Novartis 사 ) 가 1908 년대초기에처음소개되었는데반감기가 2 시간 이상 (72-98 분 ) 이며피하주사후에 6-12 시간동 안생물학적효과가지속되어피하주샤가가능 하고 somatostatin 에비하여약효가 20 배이상 강하며당대사장애가적은장점이있다. Somatostatin 과 octreotide 의 십이지장유두부팔약근에 대한작용은서로상이하게나타나는데 somatostatin 은팔약근이완호르몬을분비하여팔약근 을이완시키나 octreotide 는오히려유두부팔약근 의기저압및위상파의수축빈도를상승시키거 나 있다 45 경련을일으켜춰 l 장염을악화시킬수가 2. Somatostatin 과 octreotide 의임상적응용 Somatostatin은성장호르몬유리촉진인자의연구중에발견되었는데 14 amino acid peptide로이루어져있고 3 5 쉐장외분비억제기능은인체에서도증명되었으며억제기능이투여양과비례하여관찰되며 39,40 동물실험에서춰l 장혈류를 30% 정도잠소시킨다고알려져있다 41 그러나발갑기가 2분정도로매우짧아실제임상에서는지속적무로정맥주사를해야하는불편함이 1) 급성훼장염과그합병증의치료 : 급성웨장염에서현재까지많은동물실험과임상연구가진행되어왔 나 somatostatin과 octreotide의치료효과는아직까지충분한합일점이없는상태이다. 많은동물실험에서는 somatostatin과 octreotide는실험적으로유발된급성춰l장염에서생존율을높이고혜장염의심한정도와조직학적손상을감소시키는효과가있었다 특히조기에대량 Table 2. 7 D~ati~n 깅호각웨장질환의약물요법 17 의 ocπeotide를정맥주사할경우에더좋은성적을얻을수있었다 년에여러엄상연구들을 meta-analysis한결과를보면심한급성쉐장염에서치사율은 somatostatin의경우 22.7% 에서 9.8% 로, octreotide의경우 25.6% 에서 14.4% 로의의있게줄었으나쉐장엽의합병은두약제모두의의있게줄지는않았다고보고되었다 (Table 2, 3).49 과거무작위, 이중맹검연구 % 혹은무작위, 대조임상연구에서 51 ocπotide는급성혜장엽의치료에효과가없다고하였a나최근의무작위연구에따르면 Ranson 기준이나전산화단층촬영소견에서심한쉐장염으로판단된경우에 ocπeotide 를피하주사한경우가비교군에서보다패혈증, ARDS 등의합병증도적었고사망률도적었음이보고되었다 52 여러연구의투여용량과기간은 somatostatin의경우대부분 250μ에l로정맥주사를 3-7 일정도로하였고, octreotide 경우는 μg을하루 3회 7-10일정도피하주사하였다. 투여양과의관계는투여양이많은경우흑은대량으로투여한경우가쉐장염의치료효과가더좋았다고 보고된경우가있다 5 3,5 4 동물시험과임상시험이차이가있고임상실험도방법에따라차이가있는것은실험동물과쉐장염환자의질병경과의차이, 치료투약시점의차이, 그리고사망의평가문제의차이등으로추정된다. 실제임상에서는쉐장엽이어느정도진행한이후에약물치료가가능하므로치료시점이동물실험과는다를수밖에없다. 실지임상시험에서도 ERCP 후춰l장염이나수술후혜장염에서 somatostatin이나 octreotide를시술흑은수술전에투여하여춰l 장염이나합병증예방에효과가있음이보고되어있다 ) 훼장염혹은훼장수술후의합병증의치료 : Sandostatin과 octreotide는급성쉐장염자체의치료뿐만아니라쉐장염혹은춰l장수술로인한가성냥종, 춰l 장성복수, 춰l장성흉수, 춰l루등의치료에서도춰l장외분비를억제하여효과가있어수술을피할수있는경우가많이보고되었다 국내에서도 ocπeotide를이용한혜액성복수와종격동내가성낭종의치유경험이보고된적이 있다 61,62 Table 3. 8 년도대한혜담도연구회학술대회 3) ERCP 후의취 장엽의예방. 내시경적역행성담혜관조영술 (endoscopic retrograde cholangiopancreaticography, ERCP) 에수반되는웨장염이나합병증이늘어나면서 ERCP 후의급성춰l장염을대상으로하는약물적예방요법에대한연구가많이시행되었다. Octreotide의경우대부분의연구에서 ERCP 후의고아밀라제증이나혜장염예방에효과가없었으며 55,63,64 십이지장유두부팔약근절개술후의쉐장염예방에도효과가없었다 65 그러나시술전 3일간의장기간투여한경우고아밀라제증을예방할수있었던연구도있었다 66 최근의 meta -analysis 연구결과에따르면 somatostatin은춰l장염, 고아밀라제, 춰l장성동통의예방에유의한효과가있으나, octreotide는고아밀라제의위험성은감소하였으나혜장염이나쉐장성동통에는효과가없었다 67 이러한차이점은 ocσ'eotide가십이지장의 Oddi 팔약근을자극하여수축시키기때문으로추측된다. 그러나 ERCP 에연관된춰1장엽은대부분경증a로특별한치료가필요없고적정한투여기간이명확하게규정되자않아 somatostatin을이용한 ERCP 후의혜장염예방은 ERCP를시행하는모든환자에게적용하기보다는 ERCP 후혜장염발생의고위험군을대상으로선별적으로시행하는것이필요하다 67, 잃 3) 만성훼장염에서의통증치료 : ocπ'eotide는혈중 CCK치를낮추고쉐장외분비를억제하기때문에혜장을쉬게만들고혜관의압력을낮추어서만성쉐장염의통증치료에효과가있을것으로 예측되어왔다 69 미국에서시행된다기관공동연구의초록발표에서 octreotide를하루 3회 200μg 피하주사한경우에통증은 44-65% 에서감소하 였고 31% 에서는통증이소설되었었다 70,71 그러나소수의환자를대상으로하였지만임의로시행한이중맹점연구에서는 octreotide를매우단기간투여한경우에통증감소를볼수없었다 72 실지엄상에서통증이있는만성춰l 장염에서주춰l관이확장이보이고쉐효소제의투여로효과가없는경우쉐관에도관을삽입하거나수술적요법을시행하기전에 ocπeotide를장기간투여하여효과를기대하여볼수있으나비용이고려의대상이된다. 4) Somatostatin과 octreotide의부작용 Somatostatin이나 octreotide를주사할경우주사부위의작열감, 알레르기성발진이생길수있고전신작 용으로오심, 설사, 혈당조절의장애가생길수있고,73 ocπeotide의경우급성춰l장엽의악화가보 고되고있다 45 그외에도담낭의수축기능저하로담석발생이가능하다 74 그러나이런부작용은장기간투여한경우에발생하며, 춰l 장염의치료는대부분 2-4주이내로짧기때문에임상적으로유의할만한합병증의가능성은매우적다. 4. 9 김호각웨장질환의약물요법 19 장 쉐장계내분비성종양의증상경감, 위 식도정맥류출혈의응급환자및춰l장관련소화기수술후발생하는합병증의예방등에보험적용이되나급성 만성춰l 장염이나합병증에대하여서는아직보험적용이되지않고있다 ( 참고 WWW. casemanager.co.kr). 현재국내에서는몇몇종류의 somatostatin과 octreotide가시판되고있다 ( 참조 co.kr)(table 4). 5. Somatostatin과 octreotide의임상적사용의결론급성춰l장염의치료에서 somatostatin과 octreotide 의임상적유용성은아직논란이있으나중증의급성춰l 장엽의초기에비교적대량의 somatostatin 이나 octreotide를사용할경우치사율을줄일수있으며급성혜장염의합병증으로생긴복수, 흉수, 쉐루의치료나혜장수술후의합병증이예방에사용할수있으며 somatostatin의경우 ERCP 후의춰l 장엽의예방에상용할수있으나투약방법, 투약기간및시기, 용량등에일관성있는기준을정하기위한대규모의전향적연구가필요하겠다단백분해효소억제제 1. 단백분해효소억제제의기전급성춰l장염은 ERCP, 특히유두부팔약근절개술후의중요한합병증이다. 급성춰l 장염, 특히 ERCP 후의급성춰1장염의시작점은아직정확히알려져있지않으나단백분해효소 (proteolytic enzyme) 의세포내활성화에의한자가소화가중요한역할을할것으로알려져있다. 그러므로이론적으로단백분해효소억제제를투여할경우에혜장선방세포내단백분해효소의활성화를차단시킴으로써급성쉐장염을예방하거나치료할수있을것으로기대할수있다. 현재까지개발된단백분해효소억제제는고분자량의 aprotinin (Trasylol~ 과저분자량의합성 단백분해효소억제제인 gabexate mesilate (Foy~, camostat mesilate (Foypan~, nafomostat mesilate 등이있다 75 Aprotonin은최초로개발되어임상에사용된단백분해효소억제제로서급성춰l장염의치료에효과가있을것으로기대하였으나분자량이 6,500 dalton으로너무크기때문에세포막을통과할수가없어서쉐장선방세포내에서 일어나는단백분해효소의활성화를차단할수없어서혜장염의치료에효과가없는것으로밝혀졌다. 이후새로개발된단백분해효소억제제인 gabexate mesilate는분자량이 417 dalton인저분자량의물질로비교적쉽게세포막을통과할 수있어서선방세포내단백분해효소의활성화 를보다효율적으로차단할수있어서 76 춰l장염 의치료나예방에효과가있을것으로기대가되어서많은연구가진행되어왔다. 2. Gabexate mesilate의임상적응용 1) 급성휘 장염에서의치료효과 : 동물실험에서 gabexate mesilate를정맥주사할경우약체에의 해유발된급성춰l장염이호전되며 77,78 혈중아빌라제치및 trypsinogen치도감소하며현미경적쉐 장조직손상도감소한다고보고되고있다 79 급성혜장염환자를대상으로한연구에서는 1991 년 gabexate mesilate를혜장염초기에정맥주사를하면심한쉐장염에의한사망률을감소시키고 중등도의춰l장염에서통증과고아밀라제혈증을 호전시킨다는보고가있은 80 이후몇몇보고에서는심한춰l장염에서사망률을줄이고심한합병증을줄여서수술의필요성을감소시킨다고 하였으며 49,81,8 2 meta-analysis에서는합병증은줄일수있으나 90일사망률은줄이지못하고비 용측면에서효과적이지못하다고하였다 83 그러나많은환자를대상으로한이중맹검연구에서는 gabexate mesilate가급성웨장엽의치료에효 과가없으며합병증이나사망률을감소시키지않는다는서로상반된결과를보여주고었다 84, 85 이러한연구결과의차이는연구자마다급성쉐 장염의원인이나중등도, 발병후치료시작까지

10 년도대한웨담도연구회학술대회 의 시간, gabexate mesilate의 투여용량및 기간 전향적이중맹검연구에서 ERCP 시행전 30- 등이서로다르기때문에비교하기는어려우며 90분부터시술후 12시간까지 gabexate mesilate 보다체계적인연구가필요할것으로생각된다 1 g을정맥주사한결과고아밀라제혈증의정도 1998년의 meta-analysis에서는 gabexate mesilate는 를완화시키고, 시술후쉐장에의한통증의빈 중증급성춰l 장염에서사망률은줄일수없고합 도가적었S며, 급성춰l 장엽의발생빈도를감소시 병증은줄여서 수술의 필요성은감소시킨다고 킨다고보고하였다. 이후조사된 meta-analysis에 하였고경한혜장염에서는경과에영향을끼치 서도 gabexate mesilate는급성웨장염의 발생의 지않는다고하였다 (Table 5).49 예방, 고아밀라제혈증호전, ERCP시술후통증 2) ERCP 후의춰 장염예방 : ERCP 후의급성 의예방에효과가있는것으로보고되었다 67 국 혜장염의예방을위한 gabexate mesilate의 역할 내에서도 1981 년윤등이처음으로 gabexate me- 에관한가장최초의체계적인연구는 1996년 silate가 ERCP 후의혈중아밀라제치증가를예 Cavallini 등에의해발표되었다 86 이들은 ERCP 방할수있다고처음으로보고하였고87 gabexate 를시행받는 418명의환자를대상으로시행한 mesilate가쉐장외분비기능중단백의분비를억 Table 5.
