Methods : The precision as measured by the NCCLS protocol and a comparison of the method were done for each TSH, FT3, FT4, and E2 assay. The functional sensitivity and linearity tests were performed for both TSH and E2. The free hormone validity test was performed for both the FT3 and FT4. Results : All four analytes showed an acceptable precision. The functional sensitivities of TSH and E2 were miu/l, and 87 pmol/l, respectively. TSH and E2 showed excellent linearity up to 78 miu/ml, and up to 7,700 pmol/l, respectively. The free hormone validity test showed acceptable results demonstrating accurate free hormone determination. The E2 showed a significant proportional bias requiring an adjustment of the reference range, However, the other analytes showed good agreement with a slight proportional bias. Conclusions : The TSH, FT3, FT4, and E2 assay by Vitros ECi exhibited excellent performance overcoming the drawbacks of a conventional radioimmunoassay. 2 96 용동은 선영규 이건수외 2 인 상의민감도를보이는화학발광면역검사법 (electrochemiluminescence immunoassay) 를이용한여러가지자동화기기가소개되어임상검사실에서사용되고있다 [2]. 이중에서 Vitros ECi Immunodiagnostic System은전자동임의순응식화학발광면역분석기기 (full automated random access electrochemiluminescence immunoassay) 로서특히 free thyroxine (FT4) 검사의정확성이우수한것으로평가되고있다 [3-5]. 국내에서는안등 [6] 이이미 Vitros ECi 를이용하여여러종목검사에대한평가를한바있으나, 본연구에서는 thyroid stimulating hormone (TSH), free triiodothyronine (FT3), FT4 및 estradiol (E2) 검사를평가하여보고하는바이다. 재료및방법 1. 재료 차례 (run) 에걸쳐환자 3명의희석검체와원래검체를 TSH와 E2에대하여반복하여측정하였다. 이러한검사차례간 %CV와 TSH 및 estradiol의농도간에 X의누승관계추세선을그려 20%CV인 TSH 및 E2 농도를기능적민감도로정하였다. 3) 직선성 TSH 및 E2의직선성을검사하였다. 검체는고농도와저농도의 TSH 검체를서로의양을달리해혼합하여각각 3회반복하여측정하였다. 계산상예측치와반복검사상의평균값을그래프에나타내고, 회귀방정식을구하였다 [8]. 4) 유리호르몬검정 (Free hormone validity) 평가기간중하루를정해시행하였다. 갑상선기능이정상인 (euthyroid) 검체를 HEPES (Sigma Chemical; Cat. No. H3375, St. Louis, MO, USA) 완충액 (10 mmol/l, ph 7.4) 으로각각 4회 serial dilution 하여 FT4 와 FT3 를반복하여측정하였다. 기기는 Vitros ECi (Ortho-Clinical Diagnostics, Raritan, NJ, USA) 을사용하였고, TSH, FT3, FT4 및 E2를평가종목으로하였다. 이기기는 streptavidin이부착된작은플라스틱용기를고정상으로사용하고, biotin이표지된특이항체와 horseradish peroxidase가표지된 conjugate를부착하여넣고, luminogenic substrate를신호표지자로사용하였다. TSH, FT4, FT3는방사면역법검사 (Radim, Pomezia, Italy) 와, 그리고 E2 는역시방사면역법검사 (Biodata, Roma, Italy) 와각각비교하였다. 또한유사한화학발광면역분석기인 ELECSYS 2010 (Boehringer Mannheim, Mannheim, Germany) 을비교기종으로사용하였다. 2. 방법 1) 정밀도 (Precision) NCCLS 지침 [7] 에의거하여 TSH, FT4, FT3 및 E2에대한정밀도검사를시행하였다. 고, 중, 저농도의인혈청을모아정도관리물질로사용하였다. Calibration interval인 28일중 20일을균등한분포를가지도록선정하여검사간에 2시간이상의간격을두고, 오전과오후, 각각 2회씩정도관리물실을검사하였다. 검사차례내 (within-run), 검사차례간 (between-run) 및일간 (between-day) 정밀도는 NCCLS 평가방법을따라시행하였다. 2) 기능적민감도 (Functional sensitivity) TSH와 E2에대하여기능적민감도를측정하였다. 환자 3명의검체를 TSH-free plasma matrix와 estradiol-free plasma matrix로각각 5배희석하였다. TSH-free plasma matrix와 estradiol-free plasma matrix는각각영국 Ortho 회사로부터기증받은것으로사용하였다. Calibration interval 중 10회의검사 5) 참고방법과의상관성 TSH, FT4, FT3 및 E2에대하여 Vitros ECi, ELECSYS 2010 및방사선면역측정법사이의상관성을비교하였다. (1) TSH 총 91검체를 ECi와 Elecsys TSH assay, Radim 등으로측정하였다. 이중한쪽에서라도검사한계치를넘거나검사를시행하지않은검체를제외한, 76 및 82검체의결과에대하여각각통계처리하였다. (2) FT4 총 91검체를 ECi와 Elecsys 및 Radim으로비교측정하였다. 이중한쪽에서라도검사를시행하지않았거나 FT4가 80 pmol/l 이상인검체를제외한, 83 및 84검체의결과에대하여각각통계처리하였다. (3) FT3 총 91검체를 ECi와 Elecsys 및 Radim으로측정하였다. 이중한쪽에서라도검사한계치를넘거나혹은검사를시행하지않은검체를제외한, 78 및 82검체의결과에대하여각각통계처리하였다. (3) Estradiol (E2) 총 61검체를 ECi와 Elecsys 및 Biodata로측정하였다. 이중한쪽에서라도검사를시행하지않은검체를제외한, 59 및 61검체에대하여통계처리하였다. 6) 통계처리검사방법간의비교를위해 Deming regression, Pearson cor-

3 Vitros ECi 에의한 TSH, FT3, FT4 및 E2 검사평가 97 relation을시행하였고, 통계프로그램은 Microsoft Excel 97과 Analyse-it Software (Version 1.61, Leeds, England) 을사용하였다. 결과 1. 정밀도 NCCLS법에의해산출된정밀도는 TSH검사의경우 5.4% 이내였고, FT4는 5.8% 이내, FT3는 3.9% 이내, 그리고 E2는 7.7% 이내이었다 (Table 1). 2. 기능적민감도 (Functional sensitivity) TSH의기능적민감도는 miu/l이었고 (Fig. 1), E2의기능적민감도는 87 pmol/l이었다 (Fig. 2). 3. 직선성 TSH는직선성검증을통해 78 miu/ml까지측정가능하였다 (Fig. 3). E2는직선성검증을통해 7,700 pmol/l까지측정가능하였다 (Fig. 4). 4. 유리호르몬검정 (Free hormone validity) 갑상선기능이정상인검체를각각 4회희석하여측정한결과 FT4와 FT3 각각 >8.5 pmol/l, >4 pmol/l로감소되지않았다 (Fig. 5). 5. 참고방법과의상관성 TSH, FT4, FT3, E2에대하여 Vitros ECi 과 ELECSYS 2010 및방사면역검사법사이에 의기울기로비례오차를발견하였고, 특히 E2의경우에는 의비례오차가있었다 (Table 2). Free hormone level changes in diluted samples. 고 저자들은화학발광면역검사법을적용한 Vitros ECi 의 TSH, FT4, FT3 및 E2에대한검사능을평가하였다. 정밀도를볼때 찰 FT4 FT3 TSH의경우모두 8.1% 미만이었고, 저농도 FT4의검사차례내, 검사차례간정밀도와중등도 FT4의검사차례간정밀도가 6% 미만으로만족할만하였다. 전체적으로 TSH의경우 %, FT4의경우 %, FT3의경우 % 그리고 estradiol의경우 % 의정밀도를보여만족할만하였다 (Table 1). 기능적민감도는 TSH는 miu/l로 mu/l인 3세대 TSH 검사의기준 [9, 10] 을만족하였다. 따라서 2세대 TSH 검사시약보다 10배이상민감하여 thyrotoxicosis와정상인을 99% 이상구별할수있는유용한검사법 [11] 으로생각되었다. 혈청내 E2 측정은기능적불임 (functional infertility) 진단 [12] 과배란예측에유용하다 [13]. 혈청 E2는정상여성에서폐경후 51 pmol/l 이하로감소하여기존의방사면역법검사나 EIA의최소검출농도측정범위를벗어난다 [14]. 본평가에서 E2 에대한 Vitros ECi의 functional sensitivity는 55 pmol/l로서, 방사면역검사법, EIA 방법의검출한계농도 [14] 보다더낮은농도까지측정할수있었다. 혈청내유리갑상선호르몬은총갑상선호르몬농도와혈청내갑상선호르몬결합단백의농도의영향을받는다. 또생리적활

5 Vitros ECi 에의한 TSH, FT3, FT4 및 E2 검사평가 99 성형이기에총갑상선호르몬농도보다실제갑상선기능을잘반영하는것으로받아들여지고있다. 유리호르몬검정은임상적으로혈청내갑상선호르몬결합능의변화가있을수있는 familial dysalbuminemic hyperthyoxinemia, T4 혹은 T3에대한자가항체가있는환자들의실제갑상선기능을평가하는데유용하다 [15, 16]. 본평가에서 euthyroid 검체를각각 4회희석하여측정한결과, FT4와 FT3 모두감소되지않아서 free hormone 측정에신뢰성이확인되었다. 본연구에서는시행하지않았으나다른기관에서평가한바에의하면 FT4 검사의정확성이우수한것으로평가된바있다 [3-5](Fig. 1). TSH, FT4, FT3, estradiol에대하여 Vitros ECi 과 ELEC- SYS 2010의상관성을평가한결과 Table 2와같이대부분은약간의비례오차또는항시오차가있어서교체에는큰문제가없을것으로보였다. 다만, E2의경우에는참고범위재설정이필요할정도로비례오차가있었고이는사용한참고물질등에의한차이로생각되었다. 이상의결과로보아 Vitros ECi 에의한 TSH, FT3, FT4 및 E2 검사방법은임상적으로방사면역검사법등기존의방법을대체하며, 자동화할수있고, 특히유리호르몬농도검사의정확도는우수한것으로평가되었다. 요약배경 : 임상검사실에서는자동화및간편성의요구가커지는한편정확도, 민감도및직선성이우수한검사방법이요구된다. 저자들은방사선면역측정법 ( 방사면역검사법 ) 을대체할만한화학발광면역검사법을적용한 Vitros ECi Immunodiagnostic System에의한 TSH, FT4, FT3 및 estradiol 검사를평가하였다. 방법 : TSH, free triiodothyronine (FT3), free thyroxine (FT4) 및 estradiol (E2) 을평가대상으로하였다. NCCLS기준에의한정밀도를측정하였고, Elecsys 2010 및방사면역검사법을기준으로비교분석하였다. TSH와 E2에대하여는기능적민감도와직선성을측정하였다. FT4 및 FT3에대해서는배수희석법에의한유리호르몬검정을실시하였다. 결과 : 정밀도는네종목모두우수하였고, TSH 및 E2의기능적민감도는각각 miu/l 및 87 pmol/l이었다. TSH 는최소 78 miu/ml까지, E2는최소 7,700 pmol/l까지직선성이유지되었다. 유리호르몬검정에서도 FT4와 FT3 각각 >8.5 pmol/l, >4 pmol/l로감소되지않았다. 참고방법과상관성비교에서 TSH, FT4 및 FT3는기울기 의비례오차를보였고, E2의경우기울기 의비례오차가있었다. 결론 : Vitros ECi 에의한 TSH, FT3, FT4 및 E2 검사는우수한성능을보여서, 기존의방사면역검사법의단점을극복할수있는방법으로평가되었다. 감사 Ortho-Clinical Diagnostics 사의 Dr. Andrew J.
