T h es e ce lls in v a d e th e lim it in g p la t e a n d pe n e tr a te in t o t h e pe rip ort a l s in u s oid( H & E s t a in, 2). 4. 간조직 전후의 임상적 진단과 조직 진단과의 차이 도 경계가 불분명한 침윤성 질환으로 관찰되었고(F ig. 4) 1예는 경한 담즙울체가 동반된 S h im a d a 분류 2번지 간조직 검사후 임상적 진단과 조직학적 진단이 다른 의 만성 간염으로 관찰되었다. 육안적으로 유착과 백색 경우가 12예( 31% )에서 관찰되었다. 골수이식을 시행한 결절로 관 찰 되 었 던 3 예 중 1예 에 는 간 의 육 아 종 이 관 군에서는 6예에서 진단명을 바꿨는데 조직학적으로 이 찰 되 었 고 2예 는 간 농 양 으 로 진 단 되 었 다 ( F ig. 5). 조 직 식편대숙주반응이 진단된 2예에서는 스테로이드 충격요 으 로 담 관 외 폐 쇄 로 진 단 된 1예 에 서 는 육 안 적 으 로 심 법을 할 수 있었고 임상적으로 이식편대숙주반응을 의 한 담 즙 울 체 와 S h im a d a 분 류 2 번 지의 간염 소견을 심하였으나 약물 유인성 간염으로 진단된 3예는 원인 약 보였다. 제를 줄였고 임상적으로 간정맥 폐쇄질환이 의심되었으

C M L: ch r on ic m y e loid leu k em ia, A M L: a c u te m y e loid leu k e m ia, G V H D : g r a ft v e rs u s h os t d is e a s e 나 비특이성 반응성 간염으로 진단된 1예는 보존적 치료 다양한데 골수이식을 받지 않았을 경우 악성신생물의 를 하였다. 골수이식을 시행하지 않은 군에서는 6예에서 간침윤, 약제, 감염 및 바이러스 간염등이 원인이 될 수 임상진단과 조직학적 진단에 차이가 있었는데 임파종의 있고 골수이식을 시행한 경우는 골수이식 전처치에 사 간침윤으로 진단된 예에서는 전신 항암치료를 하였고 용되는 여러 강력한 항암제5)나 전신 방사선 조사, 이식 약물유인성 간염으로 진단된 예에서는 의심되는 약물을 편대숙주반응의 예방을 위해 사용하는 면역억제제에 의 중단하였다( T ab le 3). 해 간손상의 위험이 더 높아지며 간정맥 폐쇄질환, 이식 편대숙주반응, 재발한 혈액종양세포의 간침윤 등도 원인 고 찰 이 될 수 있다6-8). 복강경하 간조직검사 결과 골수이식을 본 연구에서는 혈액종양 환자와 재생물량성 빈혈 환 받은 혈액종양환자에서 간기능의 이상이 있을 때 이식 자에서 간기능 이상 소견을 보일 때 가능한 진단을 복강 편대숙주반응과 약물유인성 간염이 주요 원인이었고 골 경하 간조직검사로 알아보았다. 간기능 이상의 원인은 수이식을 시행하지 않은 경우는 바이러스 간염과 약물
