There are a few cases regarding initial presentation of a patient with distant metastasis leading to the diagnosis of follicular thyroid carcinoma. Most follicular thyroid carcinomas present as asymptomatic thyroid nodules, but the first sign of the disease is occasionally lymph-node metastases or in rare cases lung or bone metastases. Thirteen percentage of patients had distant metastasis at presentation, and bone metastasis constituted the majority. Vertebrae, pelvis, ribs, and femur were common sites of bone metastasis. Thus, case of metastasis to the femur only from follicular thyroid carcinoma is very rare. We recently experienced a case of follicular thyroid carcinoma with metastasis to the femur who presented with bone pain and pathologic fracture. We present this case with a review of the literature. Key Words: Follicular thyroid carcinoma, Femur, Metastasis 서론 갑상선여포암은갑상선암중두번째로흔한유형이다. 갑상선여포암은대부분무증상으로갑상선에국한되어발현되는경우가흔하지만경부림프절전이또는드물게폐나골전이로인한증상이첫징후일수있다. 1,2) 원격전이는진단당시에약 13% 의환자에서관찰되고골전이가흔하며주로척추, 골반, 갈비뼈및장골순으로보고되고있다. 3) 따라서, 대퇴골과같은장골에단독으로골전이된소견을첫임상증상으로갑상선여포암이진단되는경우는아주드물다고할수있다. 저 자들은원위부대퇴골의통증및병적골절에대한검사과정에서 1 cm 미만의작은갑상선여포암이진단된 1예를경험하였기에문헌고찰과함께보고하는바이다. 증례 58세여자환자가내원수개월전부터발생한오른쪽허벅지통증및절뚝거림을주소로타병원을내원하였고, 일반방사선검사에서대퇴골의전이성종양이의심되어수술적치료위해본원방문하였다. 환자의과거력및가족력에서특이소견없었으며내원시전신상태는양호하였다. 말초혈액검사에서백혈구 4110/mm 3, 혈색소 13.4 g/dl ( g/dl), 혈소판 논문접수일 : 2012 년 4 월 26 일 / 심사 ( 수정 ) 일 : 2012 년 5 월 22 일 / 심사완료일 : 2012 년 5 월 23 일교신저자 : 김보현, 부산시서구구덕로 305, , 부산대학교의학전문대학원내과학교실 Tel: , Fax: , pons71@hanmail.net 73

2 김상미 외 ,000/mm (140, ,000/mm )이었다. 갑상선 기 서 높은 신호강도를 보이는 종괴가 관찰되었다(Fig. 능 검사에서 T ng/dl ( ng/dl), 유리 T B). 정확한 진단을 위해 종양 소파술(tumor curettage) ng/dl ( ng/dl), 갑상선자극호르몬 2.15 uiu/ml 과 골이식(bone graft)을 시행하였다. 조직병리학적으 ( uiu/ml)로 정상이었으며, 혈청 갑상선글로불 로 Hematoxylin & Eosin 염색 조직표본에서 콜로이드 린은 ng/ml (0-50 ng/ml)로 증가되어 있었다. 항 를 포함하는 여포들로 구성되어 있었고 면역조직화학 갑상선글로불린항체 U/ml (0-60 U/ml), 갑상선자 염색에서 thyroglobulin에 양성으로 나타났다. 이상의 극호르몬 수용체항체 nondetectable (0-1.5 IU/L)로 음성 소견으로 갑상선에서 대퇴골로 전이된 여포암으로 확 이었으며, 항갑상선과산화효소항체는 U/ml (0-60 진되었다(Fig. 2A). 갑상선여포암의 진단을 위해 추가 U/ml)로 증가되어 있었다. 일반방사선사진에서 오른쪽 적인 검사를 시행하였다. 갑상선 스캔에는 경도의 종 대퇴골 부위에 경계가 불명확한 골 흡수 병변이 관찰 대와 균일한 섭취 소견을 보였고(Fig. 3A), 갑상선 초음 되었다(Fig. 1A). 파에서는 양엽에서 다수의 결절이 보이고 좌엽 하부에 대퇴부 자기공명영상에서 오른쪽 대퇴골 골간부위 서 미세 석회화 소견이 동반된 0.8 cm의 결절이 보여 에 T1-강조영상에서 낮은 신호강도를, T2-강조영상에 갑상선 세침흡인을 시행하였다(Fig. 3B). 세침흡인 검 Fig. 1. The bone scan shows the cold defect with surrounding increased uptake in the right proximal femur. 사결과는불충분한세포소견으로나왔다. 종양소파술후 3일째되는날, 낙상으로대퇴골전이부위에골절이발생하여비개방교정술및내고정술을시행하였다 (Fig. 1C).

7 cm의경계가좋은종괴를보였고, 조직병리소견상국소적으로분화도가좋은갑상선여포암으로확진되었으며갑상선피막침습은있었으나주위림프절이나혈관침습은없었다 (Fig. 2B). 수술 3개월후 200 mci의 131 I 치료를시행하였고일주일뒤 131 I 전신스캔을시행하였다. 전신스캔에서는잔존갑상선조직및우측대퇴골부위의요오드섭취증가가관찰되어있었으나새로운전 Fig. 5.

The bone scan shows the cold defect with surrounding increased uptake in the right proximal femur. 사결과는불충분한세포소견으로나왔다. 종양소파술후 3일째되는날, 낙상으로대퇴골전이부위에골절이발생하여비개방교정술및내고정술을시행하였다 (Fig. 1C). 종양소파술이후시행한양전자방출단층촬영술 (positron emission tomography, PET) 및전신뼈스캔 (bone scan) 에서다른부위의원격전이소견은관찰되지않았다 (Fig. 4). 갑상선초음파에서발견된석회화를동반한결절은갑상선전절제술및중앙림프절절제술을시행하였다. 갑상선좌엽하부에 0.7 cm의경계가좋은종괴를보였고, 조직병리소견상국소적으로분화도가좋은갑상선여포암으로확진되었으며갑상선피막침습은있었으나주위림프절이나혈관침습은없었다 (Fig. 2B). 수술 3개월후 200 mci의 131 I 치료를시행하였고일주일뒤 131 I 전신스캔을시행하였다. 전신스캔에서는잔존갑상선조직및우측대퇴골부위의요오드섭취증가가관찰되어있었으나새로운전 Fig. 5. No evidence of other distant metastasis was noted. 이병변은관찰되지않았다 (Fig. 5). 환자는갑상선호르몬제로 TSH (thyroid stimulating hormone) 억제치료를유지하면서정형외과진료및외래경과관찰중이다. 6개월뒤경부초음파, 131 I 전신스캔및혈청갑상선글로불린, 대퇴골 X-ray를추적관찰하면서재발여부를평가할예정이다. 고찰 갑상선암은조직병리학적소견에따라크게유두암 (papillary carcinoma), 여포암 (follicular carcinoma), 수질암 (medullary carcinoma), 역형성암 (anaplastic carcinoma) 으로구분된다. 유두암이가장흔히발견되며, 75-85% 의빈도를나타내고, 2번째유병률을보이는여포암은 10-20% 의빈도를보인다. 1,2) 75 J Korean Thyroid Assoc

4 김상미외 여포암의정확한병인은알려져있지않으나일반적으로요오드섭취가많으면여포암의빈도가줄어드는것으로알려져있고우리나라의경우전체갑상선암중 3-7% 의빈도를보이고있다. 4) 여포암은유두암과는달리수술전진단의정확도가낮고, 림프절전이를잘하지않는대신혈행성전이를잘일으킨다. 이러한특성때문에유두암에비해공격적인양상을띠어그예후역시유두암에비해불량하다. 5) 분화갑상선암중유두암에서는 10% 에서, 여포암에서는 20-40% 에서원격전이가발생하며, 6) 분화갑상선암초기진단시에원격전이가있는경우는 1-4% 이며, 추적관찰기간중에원격전이가발생하는경우는 4-20% 이다. 7) 여포암환자에서골전이는 7-28% 에서발생하며그빈도는척추 (29%), 골반 (21%), 갈비뼈 (17%) 그리고장골 (11%) 순서이다. 3) 특히원격전이에의한증상으로갑상선암을진단받는경우는 4% 미만이며, 8) 골전이빈도로보았을때도증례와같이대퇴골단독전이를보여여포암을진단받은경우는매우드물다고할수있다. 국내에서도추체골 (petrous bone), 9) 흉골 4) 및척추에단독전이를보인여포암의증례는보고된바가있으나대퇴골단독전이를보인예는거의없다. 특히, 본증례와같이크기가 0.8 cm 정도로갑상선여포암이아주작음에도불구하고혈행성원격전이를동반한경우는극히드물다고하겠다. 따라서, 갑상선결절이아주작더라도여포성종양이의심이되는경우는원격전이의가능성을고려한진단이필요할것으로생각된다. 여포암은보통종양내국한된피막, 혈관침범또는두가지모두를침범하는미세침윤형 (minimally invasive type) 과육안적으로피막침범소견이보이거나갑상선주변조직으로의침범을특징으로하는광범위침윤형 (widely invasive type) 으로분류한다. D Avanzo 등 10) 및 Lang 등 11) 은각각평균 7.5년, 14년간여포암환자를경과관찰한결과, 미세침윤형의재발률은평균 18.2%, 사망률은평균 13.9% 정도인반면, 10-12) 광범위침윤형의경우는예후가상대적으로불량하여재발률이평균 55.8%, 사망률은평균 50.2% 로보고하였다. 10,11) 원격전이의예후는전이된병변의크기, 위치, 개수, 방사성요오드섭취능, F18-FDG섭취능, 전이가진단된당시의나이, 종양용적 (tumor volume) 에따라다르다. 12) Schlumberger 등 13) 은폐전이와골전이가같이있는경우 10년생존율은 13-14%, 골전이만있는경우 21-27% 로보고하였으며, Orita 등 14) 은골전이만있는 경우의 10년생존율을 40% 로보고하였다. 갑상선암이전이된경우가장좋은치료는갑상선의완전한절제와전이된병소를가능한많이제거하는것이며전이병소를완전히제거하지못했을경우에는 131 I을이용한방사성동위원소치료와갑상선자극호르몬을억제하기위한갑상선호르몬의투여가최선의방법이다. 12,15) 갑상선여포암의골전이에대한치료에는방사성요오드치료, 수술, 외부조사방사선치료및항암화학요법등이있다. 증상이동반된수술로절제가능한단독골전이는수술로완전제거가일차적인치료목표로 45세이하의환자에서는적극적으로추천된다. 16,17) 이는골전이부위의절제연및다른전이부위에대한방사성요오드치료의효과를높일수있기때문이다. 18,19) 수술적치료의적응증은내과적치료에효과가없는지속적인통증이나방사성요오드치료에저항성을보이는경우, 신경압박증상유무에상관없이척추불안정이있는경우등이다. 골전이병소가다섯부위이하인경우골전이에대한수술을시행하게되면생존율상승과삶의질개선을보이므로, 수술적접근이가능하고절제가능한병소에대해서적극적인수술이필요하다. 17,20) 한연구에서는골전이에대해보존적치료를했을경우 2.4년의생존율을보인데반해, 수술적치료를받은군에서는 6.2년의생존율을보였다. 21) 골전이에대한단독방사성요오드치료는저항성을보이는경우가많이있지만요오드친화성골전이는방사성요오드치료로생존율이높아지므로치료를권하고있다. 방사성요오드용량은경험적으로 mci를투여하거나선량측정으로계산한다. Petrich 등 16) 은 45세미만의환자, 3개미만의골전이가있을때완치목적으로방사성요오드치료시 62.5% 의관해율을보고한바가있어, 이러한적응증이될때골전이대한초치료로고려해볼수있다고보고하였다. 외부방사선조사는방사성요오드의친화력이없는골전이에시도해볼수있으며, 통증과골절의위험, 척수합병증등이있을때또한증상호전을위한치료로사용할수있다. 항암치료에대한효과는치료효과가부족하여아직논란이있으며추가적인연구들이필요하다. 본증례는대퇴골단독전이로인한병적골절로발생한경우로전이암의제거를위해비개방교정술및내고정을시행하였다. 이후갑상선전절제술및중앙림프절절제술을시행하였고수술 3개월후경험적인용량으로 200 mci의방사성요오드치료를시행하여 Vol. 5, No. 1,
