1 부인종양제 19 권제 1 호 2008 Korean J Gynecol Oncol DOI: /kjgo 자궁내막암의예후인자로서의 MMP-2,-9, TIMP-1,-2 발현 최종렬ㆍ이태화ㆍ오영림ㆍ이천준ㆍ김원규 고신대학교의과대학산부인과학교실 목적 : 세포외기질의항상성유지는 MMP (matrix metalloproteinasese) 와 TIMP (tissue inhibitors of metalloproteinases) 로알려진단백질분해효소시스템의작용에의하여주로조절된다. MMP 와 TIMP 시스템은결합조직재구성부위와정도를조절함으로써세포외기질의분해및합성을조절한다. 병적인상태에서 MMP 와 TIMP 시스템은기저막과세포외기질분해기전에주요역할을함으로써종양의증식, 침습, 전이에참여한다. 이논문의목적은자궁내막암에서 MMP 와 TIMP 의발현그리고이들과임상병리학적예후인자들과의상관관계및생존율과의관계를분석함으로써이들발현이어떠한예후적의의가있는지를알아보기위함이다. 연구방법 :55 명의자궁내막암환자를대상으로면역조직화학적염색을통한 MMP-2, MMP-9, TIMP-1, TIMP-2 의발현을조사한뒤, 이들과자궁내막암의임상병리학적예후인자들과의상관관계및생존율과의관계를통계학적으로분석하였다. 결과 : 자궁내막암환자의 MMP-2,-9, TIMP-1,-2 발현율은각각 71.7%, 54.9%, 41.2%, 76.5% 이었다. MMP-2,-9, TIMP-1,-2 발현과개별적인임상병리학적예후인자들과의상관관계분석에서는림프절전이양성군에서통계학적으로유의한 MMP-2 발현증가를나타내었고 (p=0.04), 복강세포진양성군에서는통계학적으로유의하지는않았으나 MMP-2 발현이증가하는경향을나타내었다 (p=0.07). 그리고특히림프절전이양성군에서는 MMP-2 와 TIMP-2 동시발현하는성향이매우의미있게높았으며 (p<0.01), 복강세포진양성군에서도 MMP-2 와 TIMP-2 동시발현하는성향이매우의미있게높았다 (p=0.03). 자궁내막암환자 55 명의전체 5 년생존율은 88.8% 이었으며, MMP-2,-9, TIMP-1,-2 발현여부에따른생존율분석상에서는통계학적으로의미있는차이가없었다. 결론 : 이번연구에서는 MMP 와 TIMP 의발현이전체 5 년생존율과직접연관성은없었지만, 림프절전이양성군과복강세포진양성군에서통계학적으로유의하게 MMP-2 및 TIMP-2 가동시발현되는상관관련성이있었다. 중심단어 : 자궁내막암, MMP, TIMP, 예후인자 서 론 폐경기이후의여성에서주로발생하는자궁내막암은현재서구선진국에서는가장흔히발생하는여성생식기암으로서, 2006년미국에서는약 41,200명의신환이발생하고 7,350명정도가사망하였다고보고되었다. 1 국내에서는자궁경부암, 난소암에이어 3위의발생빈도를보이는여성생식기암이며, 1991년부인암등록사업을 논문접수일 :2008 년 2 월 15 일채택일 :2008 년 2 월 28 일 교신저자 : 김원규, 부산광역시서구암남동 34 번지고신대학교의과대학산부인과학교실전화 :051) ㆍ전송 :051) kwg@kosinmed.or.kr 시작한이후꾸준히증가하여 2002년에는 5배이상의가장현저한증가를보이고있으며, 한국여성의점차서구화되고있는생활행태, 호르몬노출, 임신횟수, 초경연령, 폐경연령등의변화와관련되어향후급속히증가할것으로예상되는여성생식기암이다. 2 국내문헌보고상우리나라여성의자궁내막암발생평균연령은 52-56세이었으며, 가장흔한증상은질출혈로서환자의 % 에서발생하였다고보고되었으며, 병리조직학적분류상자궁내막양선암이 % 로가장많이발생하였으며, 세포분화도 1이 % 로가장흔하였고, FIGO 수술적병기상으로는제 1기가 % 로가장많았다고보고되었다. 자궁내막암환자의 70-80% 정도는진단시병변이자궁에국한된초기 57

2 부인종양제 19 권 1 호 최종렬외 상태로발견되지만, 자궁내막암환자의전체 5년생존율은약 75% 로서자궁경부암의생존율보다낮아예후와관련된인자를규명하는것은임상적의의가크다고하겠다. 3-5 자궁내막암의예후와관련된위험인자들로는환자의연령, 병리조직학적유형, 조직학적분화도, 자궁근층침범도, FIGO의수술적병기, 림프절전이, 복강세포진, 술전 CA-125, 종양의크기, 림프혈관침습, 자궁부속기전이, 호르몬수용체, DNA ploidy, 암유전자발현및종양억제유전자소실등이있다. 3-6 최근들어서는이러한종양의임상병리학적예후인자와생존율과의연구뿐만아니라, 분자생물학적인자들의예후와의관련성연구가많이진행되고있다. 종양이성장하여침습, 전이를일으키기위해서는암세포침습에일차적인장벽역할을하는기저막층및세포외기질의분해과정이필수적인데, 7 최근종양의단백질분해효소발현의변화를연구함으로써종양세포의생물학적특징및예후추측에관련된연구가시도되고있다. 특히 matrix metalloproteinases (MMP) 는종양의침습과전이과정중에서중요한역할을하며, 이들의억제인자인 tissue inhibitors of matrix metalloproteinases (TIMP) 는 MMP에의한기저막파괴및세포외기질분해작용을조절한다. 8,9 MMP는아연또는칼슘의존성의효소군으로서현재까지 23종류 (MMP-1, MMP-23) 가발견되었으며, 이들은각각이분해하는기질의종류에따라서 interstitial collagenases (MMP-1,-8,-13,-18), gelatinases (MMP-2,-9), stromelysins (MMP-3,-10), elastases, membrane-type MMP의 5군으로나뉜다. 10,11 이들중 gelatinases (MMP-2, MMP-9) 는실험실상에서기저막의중요한구성성분인제 4형 collagen과 fibronectin을분해하고신생혈관형성에도관여하여종양의침습을유도하는것으로알려져있다. 12 특히 MMP-2는종양세포의악성도와밀접한연관성이있다고알려져있으며결장선암, 췌장암, 전립선암, 유방암, 난소암, 구강암, 두경부암, 간암, 뇌종양등에서 MMP-2의발현증가를보이는경우주위조직으로의침습과림프절전이의빈도가증가하였다는보고도있다 TIMP는 MMP에의한기질분해작용을조절하는억제인자인데, 정상조직에서는 MMP와 TIMP의분비가균형 을이룸으로써기저막의분해대사가적절히조절되고있으나악성종양에서는이들간의불균형으로인해암의침습과전이가조장될수있다. 19,20 현재까지 TIMP는다양한조직에분포하는 TIMP-1, TIMP-2, TIMP-3와심장에만국한되어존재하는 TIMP-4가알려져있다. 21,22 TIMP-1은활성형의 interstitial collagenases, stromelysins, gelatinases (MMP-2, MMP-9) 들과화학적으로 1:1 복합체를형성하여이들의분해능을억제하며, TIMP-2는 TIMP-1과밀접한연관성을가지며활성형의 MMP-2와가장강력한결합력을보인다. 특징적으로 TIMP의기능중에는경우에따라 MMP 기능을활성촉진역할을하기도하는데, 예를들어 TIMP-2의기능중에는비활성화 Pro-MMP-2를활성화 MMP-2로전환촉진시키는기능도있다. 23 TIMP-1과 TIMP-2는암조직의간질세포에서주로발현되는데, 보고에따르면이들의발현은암세포의침습에대한생체방어반응으로서불량한예후를의미한다는주장이제기되기도하고, 24 다른연구에서는 TIMP의생산저하시에 MMP의분비조절에이상이생기거나과다하게활성화하게된다고보고되고있다. 25 또다른연구에서 MMP와 TIMP의발현은암의종류에따라각각의발현정도와발현세포가다르다고보고되어있다. 12,26,27 그리하여 MMP와 TIMP는매우복잡한기전에의해조절되고있음을시사하고있기에, 본저자는자궁내막암에서 MMP-2,-9 및 TIPM-1,-2의발현이주요임상병리학적예후인자들과어떤관련이있는지, 그리고생존율과어떤관련이있는지를연구하였다. 연구대상및방법 1. 연구대상 1995년 1월부터 2003년 12월까지본원산부인과에서자궁내막암으로진단받고자궁적출술을시행한환자들중병리학적재검토가가능하였던 55명의환자를대상으로임상병리학적특징을조사하였으며, MMP-2,-9 및 TIMP-1,-2에대한면역조직학적검사를시행하였다. 이를토대로상관관계분석및생존율분석을하였으며, 생존율분석을위한대상환자의평균추적기간은 61개월이었다. 58

3 자궁내막암의예후인자로서의 MMP-2,-9, TIMP-1,-2 발현 2. 연구방법 1) 임상병리학적특징환자의임상병리학적특징은의무기록을통하여연령, 병리조직학적분류, 조직학적분화도, 자궁근층침범도, FIGO의수술적병기, 림프절전이유무, 복강세포진유무, 그리고술전 CA-125치로나누어확인하였다. 환자의병리조직학적분류및조직학적분화도는수술후병리조직학적소견을기준으로하였으며, 환자의병기분류는 FIGO의수술적병기분류를따랐다. 환자의질병상태및생존여부확인을위하여의무기록지와전화연락을통하여확인하였으며, 최초진단일부터마지막생존여부확인일까지를생존기간으로하였다. 2) 면역조직화학적검사면역조직화학적표본의제작, 염색및판정은다음과같이하였다. 파라핀에포매된조직을 4μm 두께의절편 aminoprophyltri-ethoxysilane (APEOS, Sigma, USA) 이도포된유리슬라이드에부착시키고 60도에서 1시간파라핀을녹인후 100% xylene에서 10분간 2회탈파라핀과정과 90%, 85%, 80%, 70%, 60%, 50% 계열알코올로 10 분간함수과정을거친후증류수로세척하였다. 항원성회복을위하여 1% zinc sulfate (Sigma, USA) 가포함된 10 mm citrate 완충액 (ph 6.0) 에슬라이드를넣고 microwave 오븐을이용하여 5분간 3회가열하였다. 항체에대한비특이적 Tris buffer에 30분간실온에서반응시켰다. 메탄올과 30% 과산화수소수가 9:1 비율로섞인용액에 10분간처리하여내인성 peroxidase을차단한후 Tris buffered saline (10 mm Tris, 150 mm NaCl, ph 7.2) 으로 3회수세하였다. 이후의과정은 DAKO 회사의 LSAB (Labeled streptavidine biotin) kit를이용하여시행하였다. 15분간 normal gout serum을처리한후단클론일차항체인 MMP-2, MMP-9, TIMP-1, TIMP-2를희석하여실온에서 60분간반응시켰다. TBS로 10분간 3차례수세한후이차항체인 hoarse biotinylated anti-rabbit IgG (1:50 in TBS, sigma) 을실온에서 30분간반응시켰다. TBS로다시 3차례수세한후 peroxidase-conjugated streptavidin (1:500 in TBS, DAKO) 과실온에서 30분간반응시킨후 TBS로수세하고 AEC (3-amino-9-ethylcarbazole) 로 10-20분간실온에서발색시켰다. 다시 TBS 로세차례수세한후 Mayer's hematoxylin으로대조염색 하고 Crystal mount를사용하여봉입한후검정하였다. 사람의태반조직을양성대조표본으로사용하였으며, 음성대조표본을일차항체대신 TBS를사용하여위의동일한방법으로염색하였다. MMP-2, MMP-9, TIMP-1 및 TIMP-2의발현에대한판독은종양세포의세포질이나종양세포주변기질세포질에서의염색이주변비종양성림프구나선상피조직의염색정도보다유사하거나더강하게세포질에염색될때, 그발현범위가검색범위의비특이적인국소발현을제외한 5% 이상일경우를양성으로판독하였다. 3) 통계학적분석통계학적분석은 SPSS 12.0 프로그램을이용하여구하였다. 자궁내막암의각각의주요임상병리학적예후인자들에대한 MMP-2,-9, TIMP-1,-2와의상관관계분석에서는 Pearson의카이제곱검정을이용하였으며, 전체 5년생존율분석에서는 Kaplan-Meier법과 Log-Rank법을이용하였으며, 통계학적유의수준은 p-value <0.05 미만으로하였다. 결과 1. 환자의임상병리학적특징 55명의자궁내막암환자들의연령분포는 34-82세, 평균연령은 51.8세이었다. 50세를기준으로나눈연령별분류상 50세이상의환자 34명 (61.8%), 50세미만의환자 21명 (38.2%) 이었으며, 연령대별발생빈도는 50-59세가 23명 (41.8%) 으로가장많은빈도를보였고, 다음으로 40-49세 15명 (27.3%), 60-69세 9명 (16.4%) 의순이었다. 병리조직학적분류상자궁내막양선암 (endometrioid adenocarcinoma) 이 51명 (92.7%) 으로대부분을차지하였으며, 그외비자궁내막양선암 (non-endometrioid adenocarcinoma) 이 4명 (7.3%) 이었다. 조직학적세포분화도분류상세포분화도 1은 24명 (43.6%), 세포분화도 2는 23명 (41.8%), 세포분화도 3은 8명 (14.6%) 을나타내었다. 술후병리조직학적검사상자궁내막에국한된환자가 7명 (12.7%), 자궁근층 1/2 미만침범한환자가 20명 (36.4%), 자궁근층 1/2 이상침범한환자는 28명 (50.9%) 이었다. FIGO의수술적병기상분포를보면병기 I이 40명 (72.7%) 으로서대부분을차지하였고, 병기 II는 8명 59

4 부인종양 제19권 1호 최종렬 외 (14.6%), 병기 III은 6명(10.9%), 병기 IV는 1명(1.8%)이 3. 임상병리학적 예후인자들과 MMP-2,-9, TIMP-1,-2 었다. 개복 직후 복강세포진 검사에서는 음성 환자가 46 발현과의 상관관계 명(83.6%)으로 대부분이었으며, 양성 환자 7명(12.7%), FIGO의 수술적 병기에 따른 MMP-2,-9, TIMP-1,-2의 불명 2명(3.6%)이었다. 수술시 골반림프절 절제술을 시 발현과의 상관관계 분석에서는 모두 통계학적 유의성이 행하여 검사한 결과에서 림프절 전이 음성 환자 45명 없었으나, 특징적으로 병기 3기와 4기 환자군 7명 모두 (81.8%), 림프절 전이 양성 환자 9명(16.4%), 불명 1명 가 TIMP-2 발현 증가되는 경향을 나타내었다(p=0.11). The correlation between the coexpression of MMP-2 and TIMP-2 and the clinicopathologic factors in endometrial cancer Clinicopathologic factors Coexpression of (-) MMP-2 and TIMP-2 (+) p-value Surgical stage Stage I, II (n=45) Stage III, IV (n=7) Peritoneal cytology Negative (n=43) Positive (n=7) Lymph node metastasis Negative (n=43) Positive (n=8) CA-125 level <35 U/ml (n=30) 35 U/ml (n=19) < 경향을나타내었다 (p=0.12). 림프절전이유무에따른 MMP-2,-9, TIMP-1,-2의발현과의상관관계분석에서는림프절전이가존재하는군 9명모두에서 MMP-2 발현이통계학적으로유의하게증가하는상관관계를나타내었으며 (p=0.04), TIMP-2 발현은통계학적으로유의하지는않았지만 8명모두에서증가하는경향을나타내었다 (p=0.08). 술전 CA-125치에대한 MMP-2,-9, TIMP-1,-2의발현과의상관관계분석에서는모두통계학적으로유의성이없었으나, 술전 CA-125치 35 U/ml 이상인군에서 MMP-2 발현이증가하는경향이있었다 (p=0.08) (Table 3). 그외연령, 병리조직학적유형, 세포분화도, 그리고자궁근층침범정도에대한 MMP-2,-9, TIMP-1,-2의발현과의상관관계분석에서는모두통계학적유의성이없었다. 4. 임상병리학적예후인자들과 MMP-2, TIMP-2 동시발현군과의상관관계자궁내막암의임상병리학적예후인자들과 MMP-2,-9, TIMP-1,-2 발현과의개별적상관관계분석에서는단지림프절전이양성인군에서 MMP-2 발현이통계학적으로유의하게증가하였다. 나머지상관관계분석에서는통계학적유의성이없었으나, 특징적으로림프절전이양성군, 복강세포진양성군, 수술적병기 3-4기, 술전 CA U/ml 군에서 MMP-2 발현및 TIMP-2 발현되는성향이높았다. TIMP-2는 MMP-2의억제조절인자 이면서 Pro-MMP-2의활성촉진인자이기때문에 MMP-2, TIMP-2 동시발현군에대해서만다시조사하였다. 림프절전이양성군은통계학적으로매우유의하게 MMP-2, TIMP-2 동시발현성향을나타내었으며 (p <0.01), 복강세포진양성군에서도통계학적으로유의한상관관계를나타내었다 (p=0.03). 수술적병기와 CA-125 에대한상관관계분석에서는통계학적으로유의하지않았다 (Table 4). 5. 생존율분석자궁내막암의임상병리학적예후인자들과 MMP-2,-9, TIMP-1,-2의발현여부가환자의생존율에있어서의미있는예후관련성이있는지를단변량분석으로조사하였다. 이연구에서 55명의자궁내막암환자의전체 5년생존율은 88.79% 이었으며, 개별적인예후인자에대한자궁내막암과의생존율분석상에서는수술적병기에대해서만통계학적으로유의한전체 5년생존율감소를보였고 (p<0.01), MMP-2,-9, TIMP-1,-2 발현여부를비롯한나머지임상병리학적예후인자들에따른생존율분석상에서는통계학적으로유의한차이가없었다 (Table 5, 6). 고찰세포외기질의항상성은 MMP 시스템, 즉 matrix metalloproteinases (MMP) 로알려진단백분해효소와그 62

MMP 시스템은신체의결합조직재구성을섬세하게조절하는데, 태아발달, 장기형성, 신생혈관형성, 연골개조, 뼈성장, 각막복구, 상처치료등다양한과정에참여한다. 그러나종양, 관절염, 심혈관질환과같은병적인상태에서는 MMP 시스템의미세한조절기능이붕괴되어광범위한세포외기질의분해, 재구성이발생한다. 7-9 종양세포가침습, 전이하는기전은매우복잡한과정을거치게되는데, 암세포침습에일차적인장벽역할을하는기저막과세포외기질단백질을분해하는과정이중요한요소가된다. 7 특히 gelatinase (MMP-2, MMP-9) 는실험실상에서기저막의중요한구성성분인제 4형 collagen과 fibronectin을분해하고신생혈관형성에도관여하여종양의침습, 전이를유도한다. 12 정상조직에서는 MMP와 TIMP의분비가균형을이룸으로써기저막의분해, 재구성이적절히조절되고있으나악성종양에서는이들간의불균형으로인해암의침습과전이가조장될수있다. TIMP-1과 TIMP-2는암조직의간질세포에서주로발현되며, 특히 TIMP-2는활성형의 MMP-2와가장강력한결합력을보인다. 특이한것은 TIMP의기능이항상 MMP 기능을억제하는것만있는것이아니라는것인데, 예를들어 TIMP-2의기능중에는 MMP-2 활성화과정을촉진시키는기능도있어비활성화 Pro- MMP-2를 TIMP-2와 MT1-MMP가관여하여 MMP-2로활성화시킨다. 8,9 최근보고에따르면이들의발현이암세포의침습에대한생체방어반응으로서불량한예후를의미한다고제기되기도하고, 24 다른연구에서는 TIMP의생산저하시에 MMP의분비조절에이상이생기거나과다하게활성화하게된다고보고되기도한다. 25 그리하여 MMP와 TIMP는매우복잡한기전에의해조절되고있음을시사하고있기에, 본연구에서는자궁내막암에서 MMP-2,-9 및 TIPM-1,-2의발현이주요임상병리학적예후인자들과어떤관련이있으며전체생존율에는어떠한영향을미치는지를연구하게되었다. 본연구에서는림프절전이양성군 9명모두에서 MMP-2의발현이나타나통계학적으로유의한상관관계를나타내었다 (p=0.04). 림프절전이양성군과 TIMP-2 의발현군과의상관관계분석에서는통계학적유의성이없었으나 8명모두에서 TIMP-2의발현이나타났는데 (p=0.08), 더많은모집단크기에서연구가필요할것으로사료된다. 특히림프절전이양성군과 MMP-2 및 TIMP-2 동시발현군과의상관관계분석에서는통계학적유의성이매우높은것으로나타났는데, 림프절전이양성군 8명모두에서 MMP-2 및 TIMP-2의동시발현이나타났다 (p<0.01). 복강세포진검사와 MMP-2 발현여부혹은 TIMP-2 발현여부와의상관관계분석에서는모두통계학적으로유의하지않았으나, 복강세포진양성 6명모두에서 MMP-2 의발현 (p=0.07) 및 TIMP-2의발현이나타나 (p=0.12) 더많은모집단연구가필요할것으로사료된다. 특히복강세포진양성군과 MMP-2, TIMP-2 동시발현군과의상관관계분석에서는복강세포진양성인 6명모두에서통계학적으로유의하게 MMP-2 및 TIMP-2의동시발현이나타났다 (p=0.03). 본연구의결과에서는림프절전이양성군과복강세포진양성군에서 MMP-2, TIMP-2 동시발현하는성향이통계학적으로매우의미있게나타났는데, 이는비활성화 Pro-MMP-2를 TIMP-2가 MMP-2로활성화시키는과정과관련될것으로생각된다. Maatta 등은자궁내막암에서 TIMP 발현과함께 MT1-MMP 발현시에자궁내막암의침습, 성장에중요역할을한다고보고하였고, 28 Di Nezza 등은 MT1-MMP, MMP-2, MMP-9 연구에서활성화형태가자궁내막암의침습, 전이에중요한역할을한다고보고하였는데, 29 이들역시 Pro-MMP-2의 MMP-2로의활성화과정과관련된연구들이었다. 그런데향후연구에서는이러한 TIMP-2의발현이활성화 MMP-2에대한억제인자로서증가한것인지, 아니면비활성화 Pro- MMP-2에대한활성화촉진매개인자로서증가한것인지에대한확인과정이필요할것으로사료된다. 자궁내막암과 MMP, TIMP와의관련성을다룬다른연구들을보면, Graesslin 등은세포분화도에따라서는 MMP-2 발현증가및 TIMP-2 발현감소되고, 근층침범도와림프절전이에따라서는 MMP-2 발현증가및 TIMP-2 발현감소를보고하였다. 30 Guo 등은자궁내막암에서 MMP-2, MMP-9의발현은세포분화도와침습및전이와관련있으며, MMP-2가주된형태라고보고하였다. 31 Moser 등은 MMP-1,-2, TIMP-2는자궁내막암환자의전체생존율과관련이없어예후인자로도움이안된다고하였고, 32 Honkavuori 등은자궁내막암에서 MMP-2 64

9 자궁내막암의예후인자로서의 MMP-2,-9, TIMP-1,-2 발현 가악성도와관련된예후가치를지니지만 CA-125보다못하다고하였고, 33 Aglund 등은임상경과에대한지표로서추가적인치료의필요성을결정하는데유용하다고하였다. 34 한편, 본연구에서자궁내막암의예후인자들에대한전체 5년생존율분석은단변량분석상수술적병기 3기, 4기군에서만통계학적으로유의한생존율감소를보였으며 (p<0.01), 나머지임상병리학적예후인자들과 MMP, TIMP 발현과의생존율분석에서는유의한차이를보이지않았다. MMP, TIMP 발현에대한생존율분석에서는통계학적으로유의한생존율차이가없었지만, 림프절전이및복강세포진양성군에서 MMP-2, TIMP-2 동시발현하는성향이매우강한것으로볼때좀더많은모집단의전향적연구가필요할것으로보인다. 본연구의전체적인결과로보았을때, 자궁내막암에서 MMP-2과 TIMP-2 동시발현시에자궁밖으로의전이위험성을예측하는데보조적역할을하리라기대된다. 즉, MMP-2, TIMP-2 동시과다발현이있는경우에서예후가안좋은질병의경과로진행할수있다는것을암시할수있으며, 추가적인치료결정에유용할것으로보인다. 참고문헌 1. 11 자궁내막암의예후인자로서의 MMP-2,-9, TIMP-1,-2 발현 =ABSTRACT= Expressions of MMP-2, MMP-9, TIMP-1, and TIMP-2 as prognostic factors in endometrial cancer Jong-Ryeol Choi, Tae-Hwa Lee, Young-Lim Oh, Chun-Jun Lee, Won-Gyu Kim Department of Obstetrics and Gynecology, College of Medicine, Kosin University, Busan, Korea Objective:Homeostasis of the extracellular matrix (ECM) is maintained by the action of a specific system of proteolytic enzymes known as matrix metalloproteinases (MMP) and tissue inhibitors of metalloproteinases (TIMP). The MMP/TIMP system regulates the composition and turnover of ECM to control the site and extent of connective tissue remodeling. In pathologic conditions, MMP play a key role in degradation of basement membrane and extracellular matrix, and is responsible for cancer invasion, progression and metastasis. The aim of this study is to evaluate the correlation between expressions of MMP/TIMP and clinicopathologic factors in endometrial cancer. Methods:Expressions of MMP-2, MMP-9, TIMP-1, and TIMP-2 were assessed by immunohistochemistry in a total of 55 endometrial cancers and were analyzed by the correlation between expressions of MMP/TIMP and clinicopathologic factors in endometrial cancer. Results:Expression rates of MMP-2,-9, TIMP-1, and TIMP-2 were 71.7%, 54.9%, 41.2%, and 76.5% respectively. Expression of MMP-2 was correlated with the group of positive lymph node metastasis in endometrial cancer (p=0.04). Specially, coexpression of MMP-2 and TIMP-2 was significantly more frequent in the group of positive lymph node metastasis (p<0.01) and the group of positive peritoneal cytology (p=0.03). In survival analysis, expressions of MMP and TIMP were not related to poor survival rate. Conclusion:The expressions of MMP and TIMP were not a significant difference in survival analysis, but this study was recognized that the coexpression MMP-2 and TIMP-2 is correlated with lymph node metastasis and positive peritoneal cytology.
