Therefore we can compare the degree of drug-drug interactions between tacrolimus and itraconazole against tacrolimus and voriconazole. Methods: Patients who were admitted and had lung transplants in a territory referral hospital from September 2012 to May 2015 were analyzed retrospectively. The effects of itraconazole and voriconazole on the plasma concentrations of tacrolimus were analyzed. Results: Mean tacrolimus concentrations was 10.49±2.35 ng/ml vs ±2.98 ng/ml (p=0.722), and mean concentration of tacrolimus over the dose of tacrolimus per day was 8.510±5.890 (ng/ml)/(mg/d) vs ±28.47 (ng/ml)/(mg/d) (p=0.947) in itraconazole vs. voriconazole group each. The ratio of the number of the results out of target tacrolimus concentrations to the total number of tacrolimus concentration results was 18.0±13.3% vs. 24.4±18.5% (p=0.185). Conclusion: There were no significant differences between itraconzaole and voriconazole to have influences on mean concentrations of tacrolimus over tacrolimus dose per weight per day. However voriconazole tended to raise tacrolimus plasma concentrations more than itraconazole. Safer and more effective drug management to prevent and treat fungal infections should be done by therapeutic drug monitoring not only of tacrolimus but of itraconazole and voriconazole in lung transplant patients. KEY WORDS: Lung transplant, tacrolimus, itraconazole, voriconazole, drug interactions 국내에서처음으로폐이식이행해진것은 1996년 7월로 1) 해마다한자리수의이식건수를보이다 2010년이후로는지속적으로증가하여 2014년 55건의수술을포함하여 2000년부터 2014년까지총 233건의수술이진행되었다. 폐이식환자의 5 년생존율은 41.57% 로 90% 가넘는신장이식에비해매우낮 은편이다. 2) 폐는지속적으로외부환경에노출되어있고, 숙주의면역을강하게자극시킬수있는심한동종이형의기도편평상피가있으므로고형장기이식환자중폐이식환자는감염에가장취약하다고알려져있다. 3) 특히고형장기이식환자중폐이식환자가침습성아스페르길루스폐렴 (invasive *Correspondence to: Min Jung Chang, College of Pharmacy, Yonsei University, 85 Songdogwahak-ro, Yeonsu-gu, Incheon 21983, Republic of Korea Tel: , Fax: mjchang@yonsei.ac.kr 306

3 폐이식환자에서 tacrolimus 와 itraconazole 혹은 voriconazole 병용시 tacrolimus 의혈중농도변화에미치는영향 / 307 Aspergillus pneumonia, IAP) 의발병률이가장높은데, 4) 침습성아스페르길루스폐렴은평균치사율이높아폐이식환자에서침습성아스페르길루스폐렴의예방은필수이다. 5) 침습성아스페르길루스폐렴의조기진단은 galactomannan 검사를통해이루어지는데폐이식환자에서특이도는 95%, 민감도는 30% 에불과해진단이정확하지못하므로 6) 이를예방하기위해모든폐이식환자에게이식후초반에일정기간동안진단없이 6개월동안항진균제복용이권장되며 itraconazole 복용이일반적이다. 7) 미국식품의약품안전청 (Food and Drug Administration, FDA) 의침습성아스페르길루스폐렴치료에허가를받은약제는 deoxycholate amphotericin B, itraconazole, voriconazole, posaconazole, caspofungin등이나, 침습성아스페르길루스폐렴의 1차치료제는 deoxycholate amphotericin B 와 voriconazole이고, 8) 이중 voriconazole은 amphotericin B 보다생존율을향상시키고 (71% vs. 58%), 치료성적도우수하였음이 (53% vs. 32%) 밝혀진바있다. 9) 그러므로실제임상에서침습성아스페르길루스폐렴이의심또는진단시에는 voriconazole을처방할것이권장되며 8,10) 실제로미국의폐이식센터에서침습성아스페르길루스폐렴진단시에는 92.6% 가경구 voriconazole을처방하고있다. 11) Itraconazole과 voriconazole은모두 azole계항진균제로진균의세포벽합성에필수적인 14 alpha-lanosterol demethylation에관여하는 cytochrome P450 (CYP) 를억제하여독성methyl sterols을축적시킴으로써약리작용을나타낸다. 12) 폐이식환자에게거부반응을예방하기위한면역억제제는필수적이며 tacrolimus가주로처방되는데치료영역이좁고, 체내약물농도가낮을시적절한면역억제효과를기대하기어렵고체내약물농도가높은경우독성발현가능성이높으므로 therapeutic drug monitoring (TDM) 을시행한다. Tacrolimus 의급격한혈중농도변화는이식후시기와상관없이거부반응과밀접한연관이있다는것은여러연구들로증명되고있다 ) Tacrolimus의주된대사경로는 CYP 3A4 이므로이에영향을주는약제들과동시복용시 tacrolimus의대사에영향을줄수있으므로, 이들의상호작용을주의깊게관찰할필요가있다. 16) Azole계항진균제는 CYP450의대표적인억제제이므로이를시작하거나중단할때에는 tacrolimus의 TDM 을주의해서시행해야하며필요시항진균제의 TDM도수행해야한다고보고되었다. 17) 침습성아스페르길루스폐렴을예방하는목적으로초반에는 itraconazole을 6개월간처방하고있으며침습성아스페르길루스폐렴이진단또는의심될때는 voriconazole로교체하여투여되고있다. Itraconazole은침습성아스페르길루스폐렴을예방하기위해통상 1회 200 mg를 12시간간격으로투여할것을권장하며, 침습성아스페르길루스폐렴을의심또는진단된환자에게투여되는 voriconazole 은정맥주사로는부하용량으로 6mg/kg를 12시간간격으로 투여받은후유지용량으로 4 mg/kg를 12시간간격으로투여받거나경구약제로 40 kg 이상인환자에게 1회 200 mg를 12 시간간격으로투여하거나 40 kg 미만인환자에게 1회 100 mg를 12시간간격으로투여할것을권장하고있다. 18) 그러나폐이식환자에서항진균제를사용하여 tacrolimus와 azole계항진균제와의상호작용에따른 tacrolimus의농도변화를국내에서연구한적이없으므로, 본연구는 tacrolimus를투여받고있는국내폐이식환자에게항진균제가투여될때의 tacrolimus 혈중농도변화의정도를분석하여항진균제와동시복용시필요한 tacrolimus의용량예측의근거를마련하고자한다. 연구방법 2012년 9월 1일 ~ 2015년 5월 31일, 33개월동안연세대학교세브란스병원에입원한전체환자중 electronic medical record (EMR) 에폐이식진단명 (Z94.2) 이입력된환자를대상으로후향적관찰연구를진행하였다. Tacrolimus를복용하는폐이식환자중입원기간동안 7일이상연속적으로 itraconazole 또는 voriconazole이처방된환자들을연구에포함하였다. 항진균제투여기간동안환자의체중과하루 tacrolimus의용량대비 tacrolimus의평균혈중농도를 1차결과로정하고, 총 tacrolimus의혈중농도검사횟수대비목표농도 (5~15 ng/ml) 를벗어나는 tacrolimus의혈중농도결과횟수비율을 2차결과로분석하였다. 본원에서 tacrolimus의혈중농도는입원중에는주 3회이상, 외래관찰기간에는방문일마다 1주일 ~ 1달간격으로검사하게된다. 이에입원기간이 7일미만인환자의처방은비교할수있는검사횟수가적으므로제외하였다. 또한항진균제처방일수가입원중 7일미만인환자인경우에도 3회미만의 tacrolimus의혈중농도로상호작용을분석하기에는제한점이있어제외하였다. 외래진료시에측정된 tacrolimus의혈중농도또한환자의복약순응도를정확히확인할수없으므로분석에서제외하였다. Itraconazole은침습성아스페르길루스폐렴을예방시에는 1회 200 mg를 12시간간격으로투여하며, 침습성아스페르길루스폐렴을의심또는진단된환자에게투여되는 voriconazole은정맥주사로는부하용량으로 6 mg/kg를 12시간간격으로정맥투여후유지용량으로 4 mg/kg를 12시간간격으로투여하거나경구약제로 40 kg 이상인환자에게 1회 200 mg를 12시간간격으로투여하거나 40 kg 미만인환자에게 1회 100 mg 를 12시간간격으로투여하였다. 18) 이식후 tacrolimus의목표농도는거부반응의위험도가높은이식초반보다감소되나지속적으로변화하고, 환자별로상이할수있어본연구에서는일반적인농도범위인 5~15 ng/ml를기준으로하였다. 19)

4 308 / Korean J Clin Pharm, Vol. 26, No. 4, 2016 Itraconazole 과 voriconazole 을사용한두그룹간의 tacrolimus 의일평균용량, 체중대비 tacrolimus 의일평균용량, 평균 tacrolimus농도, 체중과일평균용량대비 tacrolimus의농도, 목표농도범위 (5~15 ng/ml) 를벗어난 TDM 횟수의백분율비교는 SPSS 23.0 (SPSS Inc., Chicago, IL, USA) 을이용하여분석하였다. p 값이 0.05 이하일때통계적으로유의한것으로정하고정규분포를따른다고가정할수없는경우 Wilcoxon rank sum test를시행하였다. 본연구는피험자의정보보호에관한대책등을포함한연구계획을연세대학교세브란스병원기관윤리심의위원회 (Institutional Review Board, IRB) 로부터승인 (IRB No ) 받았으며, 후향적관찰연구로써피험자동의는생략하였다. 연구결과 환자의기본적인특징관찰기간동안폐이식진단명이입력된총 82명의환자중입원기간 7일미만인 20명과항진균제사용기간이 7일미만인 5명의환자를제외한 57명을분석하였다. 폐이식을받은환자들은 tacrolimus를기반으로 mycophenolate mofetil, prednisolone 또는 deflazacort로면역을억제하여거부반응을예방하고있었다. 평균나이는 51.2세였으며 17세부터 71세까지다양했으며, 평균체중은 54.1±8.8 kg, 평균신장은 163.8±7.6 cm, 평균체질량지수 (body mass index, BMI) 는 20.2±3.5 kg/m 2 였다. 남성이 34명 (59.6%), 여성이 23명 (40.4%) 이었으며대부분양쪽폐이식을시행받았다 (52명, 91.2%). 이식전단일진단명으로는특발성폐섬유증 (idiopathic pulmonary fibrosis, IPF) 가 28명 (49.1%) 로가장많았으며폐쇄기관지염 (bronchiolitis obliterans, BO) 가 8명 (14.0%), 간질성폐질환 (interstitial lung disease, ILD) 가 7 명 (12.3%), 림프관평활근종증 (lymphangioleiomyomatosis, LAM) 이 3 명 (5.3%), 폐동맥고혈압 (pulmonary arterial hypertension, PAH) 가 1명 (5.3%), 기타 3명 (4.7%) 순이었다. 진단명이 2개가중복된환자는 7명으로 IPF와만성폐쇄성폐질환 (chronic obstructive pulmonary disease, COPD) 가중복진단인경우가 2명이었으며 IPF와 PAH, IPF와 LAM, COPD와 BO, COPD와 PAH, BO 와 ILD를동시에진단받은환자는각각 1명씩있었다. 2015년 5월까지생존한환자는 42명 (73.7%) 였고, 주요사망원인은패혈증으로인한다발성장기부전이었다. 평균입원기간은 55.4±47.7일이었으며, 입원기간동안항진균제의평균사용일수 29.0±24.7일로 itraconazole은 31.1±26.7일, voriconazole은 23.4±17.3일이었다. Itraconazole만사용한환자는 37명 (64.9%), voriconazole만사용한환자는 3명 (5.3%), itraconazole로투여받다가침습성아스페르길루스폐렴이의심또는진단되어 voriconazole로교체하여사용한환자는 17명 (29.8%) 였다 (Table 1). Itraconazole를사용한군과 voriconazole을사용한군간의혈중 tacrolimus 농도비교입원기간과항진균제사용기간이모두 7일이상인 57명의환자중 itraconazole, voriconazole이처방된환자를각각중복계수했을때 itraconazole을사용한환자는 63명, voriconazole을사용한환자는 24명이었다. Itraconazole 사용환자군과 voriconazole 사용환자군에서 tacrolimus 의일평균용량은 1.77±1.04 mg/d vs. 2.39±2.76 mg 으로 voriconazole 사용환자군이좀더많은용량의 tacrolimus 를복용하였으나통계적으로유의하지않았으며 (p=0.864), 환자의체중대비 tacrolimus 일평균용량차이도유의한차이는없었다 (0.034±0.021 mg/kg/d vs ±0.060 mg/kg/d, p=0.924). Tacrolimus 의목표농도 (5~15 ng/ml) 를벗어나는비율간에도유의한차이는없었다 (18.0±13.3% vs. 24.4±18.5%, p= 0.185) (Table 2). 침습성아스페르길루스폐렴을예방하기위해폐이식초반에 itraconazole을처방하다가침습성아스페르길루스폐렴이의심또는진단되어 voriconazole로전환한 17 명의환자중두항진균제모두 7일이상투여한환자는14명으로 tacrolimus TDM 결과가목표혈중농도를벗어난회수의비율이 itraconazole이 voriconazole 보다높은환자는 5명, voriconazole이 itraconazole 보다높은환자는 9명이었다 (Supplement 1-14) #. 고 본연구에서는통계적으로유의하지는않았지만 itraconazole 을투여받은군에비해 voriconazole 을투여받은군의 tacrolimus 의일평균용량, 체중대비 tacrolimus 의일평균용량, 평균 tacrolimus농도, 체중과일평균용량대비 tacrolimus의농도, 목표농도범위 (5~15 ng/ml) 를벗어난비율이더높은것을알수있었다. 이는환자의개별상황을고려하여 tacrolimus 의농도를설정하고이에따른용량조절을함에도 voriconazole을투여받은군에서 itraconazole을투여받은군보다임상적으로 tacrolimus의용량조절과목표농도안에도달하기가더어렵다고해석할수있었다. 이러한결과는침습성아스페르길루스폐렴이진단될때 voriconazole을투여받기때문에예방적으로 itraconazole을투여받을때와달리환자의상태가더악화되어있는것도한이유일수있다. 대부분의환자들에게두항진균제사용기간모두에서 tacrolimus 농도의급격한변화들이관찰되었으나환자별로 찰 각기다른양상을보였다. 9명환자의경우, voriconazole이 itraconazole보다농도변화폭이더커서목표농도범위를벗어난비율이더컸다. 특징적인환자로폐이식직후부터 itraconazole 을투여받다가 galactomannan 검사양성으로 voriconazole을투여받던 45세여환 (Patient 9) 의경우관찰기간동안총 voriconazole 투여중에는지속적으로높아지던 tacrolimus의혈중농도가 voriconazole을중단한이후에는급격히감소되었으며두번째 voriconazole을중단후에는 tacrolimus를증량하고 tacrolimus의농도에영향을줄수있는다른원인들이없음에도 tacrolimus의농도가지속적으로감소하여면역억제효과를볼수가없었다. Voriconazole 중단후에 tacrolimus 용량을증가시켰음에도불구하고 tacrolimus의혈중농도가급격히감소하여결국 basiliximab을투여하게된사례였다. Voriconazole과 tacrolimus 병용시 tacrolimus는기존용량의 3배를감량하라는권고 20,21) 가있지만오랜기간동안 voriconazole을투여했을때는 voriconazole을중단후 tacrolimus의증량이권고사항인 3 배로는부족할수있다고보고한연구도있었으므로 22) 본사례는 voriconazole 중단후 tacrolimus의증량이더필요한상태였을수있다. 다른연구에서도 tacrolimus의농도가안정적이었던간이식환자에게 voriconazole과 tacrolimus의병용시 tacrolimus의농도가 10배까지상승한환자를보고한바가있었다. 23) 또한 Mori 등에의하면 25명의조혈모세포이식환자에게 tacrolimus와 voriconazole을병용했을때체중과일평균용량대비 tacrolimus의농도가 (range ) (ng/ ml)/(mg/kg) 에서 (range ) (ng/ml)/(mg/ kg) 로유의하게 (p < 0.01) 상승하였으나농도범위의변화가매우크므로개별상황을고려한처방이필요하다고주장하였다. 24) 반면, 환자 1, 5, 10, 13의경우, itraconazole 병용시 voriconazole 병용시보다목표혈중농도를벗어난비율이더컸다. 이는 itraconazole의투여기간이 voriconazole 의투여기 # : Supplement 자료제공요청시교신저자에게문의요망.

6 310 / Korean J Clin Pharm, Vol. 26, No. 4, 2016 간에비해긴것에기인한것으로 itraconazole의반감기가평균 65시간으로다른 azole계항진균제에비해길어투약기간이길수록 tacrolimus의대사억제를연장시킨다는 Dodds- Ashley와 Saad의연구결과를뒷받침하였다. 21,25) Voriconazole 에의한 tacrolimus 혈중농도조절의어려움은 itraconazole과 voriconazole의용량용법차이에의한것일가능성도배제할수없다. 본연구에서 itraconazole을투여받은환자들은부작용에의한용량조절을하였으나 (100~400 mg/ day) voriconazole은모든환자에게동일한 400 mg/day가처방되어 voriconazole의약리학적특성에따른용량용법의조절이필요하다. Voriconazole은주로 CYP 2C19으로대사되는약물로한국인을포함한아시안은 CYP 2C19의대사저하자가많다고알려져있다. 26) 실제로건강한한국인 497명의약물유전형과약물대사효소활성과의상관성을 CYP 2C19으로대사되는대표적인약제인 voriconazole과 lansoprazole을통해규명한식품의약품안전평가원의임상연구에서한국인중 CYP2C19의정상대사유전형을지닌사람은전체 36% 에불과하였으며, 중간대사유전형은 47%, 대사저하유전형은 15% 으로 voriconazole의용량조절이필요한것으로나타났다고설명하였다. 27) 본연구에서 voriconazole을투여받은환자군에서 tacrolimus의혈중농도의변화의폭이눈에띄게큰환자들이있는것은한국인의 CYP 2C19 유전자다형을고려하지못하고모든환자에게동일한 voriconazole 400 mg/day를투여한데있을수있다. 즉, 한국인의유전자다형을감안할때체중 40 kg 이상에서권고되는 400 mg/day는실제한국인에게는고용량일수있고, CYP 3A4를억제하여 tacrolimus의농도를급격히증가시켜부작용을발현하게하므로 28) 개별상황을고려한용량조절이반드시필요하다. 본연구는후향적연구로 voriconazole, itraconazole의직접적인농도나환자의유전자형등을관찰하지못했으며항진균제의투여기간을동일하게통제하지못했으나폐이식환자들의항진균제사용에따른 tacrolimus의농도변화를비교한국내첫논문으로의미가있으며향후이연구를바탕으로한국인들의유전자형을기반으로한폐이식환자의상호작용연구에응용될수있을것이다. 결 폐이식환자들은감염에취약하며치사율이높은침습성아스페르길루스폐렴을예방또는치료하기위해면역억제제와상호작용이큰 azole계항진균제를사용함에따른면역억제제의용량조절이어렵다. Tacrolimus를투여받는폐이식환자에게 itraconazole과 voriconazole 의투여기간동안환자의체중과하루 tacrolimus의용량대비 tacrolimus의혈중농도와총 tacrolimus의혈중농도검사횟수대비목표농도 (5~15 ng/ 론 ml) 를벗어나는 tacrolimus의혈중농도결과횟수비율은통계적인차이가없었다. 하지만 voriconazole을투여받던몇몇환자들에게특이점들이발견되어추후대규모, 다기관의전향적인연구가필요하며 tacrolimus 농도뿐만아니라 voriconazole 의농도를관찰함으로써안전하고효과적인진균감염의예방과치료약물요법이이루어져야한다. 참고문헌 1.
