1 세부전문학회발표 고령환자를위한전정맥마취 고려대학교의과대학마취통증의학교실 이동규 의학의발달과생활문화의변화에따라최근고령환자를수술실에서마주치게되는일이과거에비하여흔한상황이되었다. 환자의마취를시행하는데있어서건강한 20 대환자와건강한 70세환자, 건강한 90세환자는어떠한차이가있을까? 고령환자를대하는데있어서 65세이상의노인환자와 20대환자의차이점을단순히노인, 혹은젊은환자로나누는이분법적인사고로마취관리계획에접근하는것은자칫혼란스러운상황에처하게되는지름길이될수있다. 즉, 나이가증가함에따라발생되는약리학, 생리학적변화를이해하고마취관리에적용하는것이고령환자관리에있어서기본이된다. 물론전신마취에사용되는흡입마취제도연령에따라변화가많이나타나게되지만, 정맥마취제도약역학적변화뿐아니라약동학전변화도많이발생하게된다. 과거에비하여마취장비의발달로전정맥마취를시행하는것이많이간소화되었으나, 기계조작이나여러가지감시장비들의사용에있어서흡입마취제에비하여익숙하지않은경우, 기저질환이동반되어있는고령의환자를마취하는데있어서쉽게적용하지못하는경우가종종있게된다. 따라서연령의증가에따른정맥마취제의약동학과약리학적차이를이해하고, 장단점을고려하여전정맥마취를시행한다면흡입마취제와차이없는안정적인마취를제공할수있을것이다. 일반적으로고령의환자에대하여마취전평가를하는데있어서반드시고려해야할점은환자의생물학적연령 (biologic age) 과생활연령 (chronologic age) 을고려하여야한다. 예를들면, 같은 70세의환자라할지라도, 고혈압, 당뇨와같은질환이나흡연, 음주의경력이환자의생활연령에비하여생물학적연령을증가시키는요인이되기때문에, 예상하는정맥마취제의반응에비하여더욱심하고더욱오래지속되는저혈압이나타날수있다. 따라서고령환자의마취전평가에는연령증가에따른영향, 생활습관, 동반된질환을모두함께고려하여야한다. 정맥내로투여되는약물들은연령의증가에따라 volume of distribution과 circulation time의변화에의하여효과의발 현이늦어지는게보통이다. 또한신장이나간의기능의저하에따른약물의효과가지속되는시간도길어지게된다. pharmacokinetic 측면에서살펴본다면, 약물의제거 (clearance) 가연령의증가에따라감소하게된다. 약물의제거가감소한다는의미는어떤일정양의약물을투여하였을때, 고령의환자일수록젊은환자에비하여 area under the curve가더크게나타나며, 결과적으로혈장내약물의농도가상대적으로높게올라간다는뜻이다. 또한약물을점적주입하게되는경우에는평형상태의약물농도 (steady-state plasma concentration) 는약물의제거에반비례하기때문에젊은화자에비하여고령의환자일수록같은속도로약물을주입하게되면더높은평형상태의약물농도를나타내게된다. 또한 phramacodynamic 측면에서볼때연령이증가할수록약물에대한 sensitivity가변하게된다. 흔히사용되는마취영역에서의 sedative/hypnotics의경우에는젊은환자에비하여연령이증가할수록 sensitivity가증가하며, 이런 sensitivity 의변화는연령에따른 pharmacokinetics의변화와별개로나타난다. 따라서고령의환자에대하여 conscious sedation 을시행하거나마취유도를하는경우지금까지언급된특성들에의하여반응이느리게나타나고, 오랫동안지속되며, 투여한용량에비하여더많은효과가나타날수있기때문에주의를해야한다. 현재시행되는대부분의마취방법은마취유도시 sedative/ hypnotics를사용하고, 마취유지중에는 narcotics를사용하는방법이다. 이렇게흔하게사용되는정맥마취약제들에대한고령환자들의반응은앞서언급한것과같이나이에따라늦게효과가나타나며, 오랫동안효과가지속되게된다. 이런문제점들을극복하기위하여최근에개발된정맥마취에사용되는약제들은좀더 potency를증가시키되, 그지속시간이짧은것이특징이다. 이런면에서보면이와같은신약은노인환자에게서더욱그장점을발휘하게되는것이다. 다음에는전정맥마취시흔하게사용되는마취제인 propofol과 midazolam, remifentanil에대하여고령환자에게있어서차이점을정리하였다. 85

2 Propofol Propofol은 1977년처음소개되면서현재전신마취의영역뿐아니라여러가지목적으로 conscious sedation을시행하는데있어흔하게사용되는 hypnotics이다. 이렇게흔하게사용되는만큼, 고령환자에서 pharmacokinetics와 pharmacodynamics가어떻게변화하는지인지하고있는것이중요하다. Propofol은체내에서광범위한 tissue distribution을가지게되며, plasma protein에 97 99% 가량 bind된상태로머물게된다. 또한어느정도의 liver cirrhosis나 renal failure, 성별과 obesity는 pharmacokinetics에큰영향을미치지않는것으로알려져있다. 일반적으로마취유도용량에해당하는 2.0 mg/kg는건강한젊은성인에서 4분가량의마취상태를유지하게된다. 연령에따라더욱영향을받겠지만 65세이상의환자군에서투여된 2.0 mg/kg의 propofol은젊은환자군에서 2.5 mg/kg의 propofol을투여하였을때보다더높은혈장농도를보인다는보고처럼, 고령환자에게서는삼구획모형에서의 central compartment가점차작아지게되고, total body clearance가감소하기때문에주의를하여야한다. 또한고령환자가질병과여러가지원인으로 plasma protein이감소되어있는상황에서는 unbound propofol의농도가상대적으로증가하여, 심혈관계억제등의효과가더욱과장되어서나타날수있다는점도반드시기억해야한다. 또한점적주입을시행하는경우에는연령에따라 clearance가감소하여건강한젊은환자에비하여높은혈장농도가유지될수있다. 고령환자에대하여연구된결과에따르면, propofol의체내에서각구획간의분포는연령에의하여영향을받으며, metabolic clearance는체중과키, lean body mass에영향을받게된다. 이에따라고령환자는초기혈중농도는빠르게감소하게되나, 뇌의 propofol에대한 sensitivity가연령에따라증가하기때문에작은용량을투여하더라도 peak effect는더욱오래지속되게된다. 또한점적주입시에 75세환자가수면상태를유지하기위하여필요한 steady-state plasma concentration이 25세환자의 steady-state plasma concentrion에절반정도라는결과에서도유추할수있듯이 propofol의 sensitivity는나이에따라상당히증가하게된다는점을기억해야한다. Midazolam Midazolam은수용성의 benzodiazepine으로정맥내투여나근육주사가가능하며, 그작용시간이다른 benzodiazepine에비하여짧아서마취영역에서많이사용되고있다. Midazolam의 일차적인대사는간에서 hydroxylation을통하여이루어지며, 주된대사산물인 1-hydroxymidazolam은약효가미미한것으로알려져있으나, 신장을통해배설되기때문에신기능이저하된환자에게서는체내에축적되어진정작용을부가시킬수있다. 고령환자에게있어 midazolam은성별에따라 pharmacokinetics가차이를보인다. 고령의남자환자의경우젊은남자에비하여 midazolam의 elimination half-time (T 1/2β) 이길고 clearance가감소되어있는반면, 고령의여자환자에게서는이런변화가관찰되지않는다. 또한고령환자들은구두명령에반응하지않을정도의반응을보이는 steady-state plasma midazolam concentration은매우낮은것으로도알려져있다. 이런결과들에대하여정확하고신뢰할만한자료는아직충분하지않으나, 연령의증가에따른용량을감소해야하는것에대한기본적인정보를제공해주고있다. Remifentanil Remifentanil은기존의 μ-opioid receptor agonist들과다르게 nonspecific esterase에의하여빠르게대사되기때문에다양한임상상황에서예측이가능한 pharmacokinetic advantage 를가지고있다. 이런특성은장시간점적주입을하더라도상황민감성반감기 (context-sensitive half-life time, CSHT) 가약 4분정도로거의일정하게유지되기때문이다. 물론이와같은장점은마취종료시빠른각성을유도할수있다는특징을가지게하지만, 반대로급격한통증을유발할수있다는단점이되기도한다. 이와같이빠른효과발현과빠른효과소실을주된장점으로가지고있는 remifentanil도연령의증가에따라 elimination half-life가증가하며, 고령환자의경우혈액과뇌의농도평형이더천천히이루어지기때문에초기에상대적으로높은혈중농도를보이더라도, 일회정주후효과처에최고농도에도달하는시간은젊은환자에비하여늦게나타나지만그농도는거의유사하게된다. Pharmacodynamics의측면에서고려할때는고령환자의경우다른 opioid들과마찬가지로 sensitivity가증가되어있다. 따라서일회정주시에는고령환자에게 pharmacokinetics의변화때문이아니라 pharmacodynamics의변화때문에용량을줄여서사용하는것이추천된다. 이와같은상황을점적주입시에고려해본다면, 고령환자는젊은환자에비하여뇌의 sensitivity의증가에따라같은효과를위하여약 50% 정도의효과처농도가요구되며, clearance가젊은환자에비하여 66% 가량되기때문에, 고령환자는젊은환자에비하여주입속도를약 1/3 수준으로유지하여야한다. 86

4 세부전문학회발표 VIMA (Volatile induction/maintenance anesthesia) 동국대학교의과대학마취통증의학교실 인준용 Halothane을사용해왔던 VIMA영역에서, 기도에대한자극이기존의흡입마취제에비해현저히적고, blood:gas solubility coefficient가낮은 sevoflurane이도입된이후로 2000 년도를거치며 halothane에서 sevoflurane으로중심이동이이루어졌다. 현재, 소아마취및당일수술전신마취등을위주로 VIMA가시행되고있으며, 이에대한연구또한지속되고있다. 본연제에서는 VIMA와관련하여최근에발표된연구들을살펴보았다. VIMA의소개 Volatile anesthetics 정맥마취로를확보하여약제를투여하는방법이나오기전까지마취는흡입마취제위주로시행되었다. Ether, chloroform, nitrous oxide가 1840년대임상적으로도입되었으며, 1934년에는 cyclopropane이, 1950년대이후부터할로겐화마취제인 halothane, methoxyflurane, enflurane, 그리고 isoflurane이개발되었다. 1990년대들어 desflurane과 sevoflurane 이선보였으며특히 sevoflurane은낮은 blood:gas solubility coefficient 및기도에대한낮은자극으로인해 VIMA의중심약제로자리잡았다. 5 인준용 :VIMA (Volatile induction/maintenance anesthesia) - LMA insertion; from starting the anesthesia until successfully inserting the LMA - LMA removal; from discontinuing anesthesia until removing the LMA - Recovery; from completion of the scan to achievement of an Aldrete score of 9 - Fig. 1을보면마취의유도, LMA의삽관및환아의회복시간이모두유의하게짧은것을볼수있다. - Movement during LMA: P (27%) vs S (0%), P < Interruption during MRI: P (10%) vs S (0%), P < Pediatric anesthesia emergence delirium (PAED) scale: P (6.1 ± 4.0) vs S (10.5 ± 3.7), P < LMA 거치시및 MRI 검사중환아의움직임은 TIVA 군에서더많이나타나지만, PAED score는낮음을알 수있다. Foreign body removal - 흡인된이물질을제거하기위한시술을받은, 1-4세 64 명의환아에서 VIMA 및 TIVA를적용하고, 이를살펴본연구로, 마취유도후 BIS score가 40에도달하였을때 fresh gas flow side port가장착된 rigid bronchoscope intubation을실시하였다. - Fig. 2를보면, 시술중, 호흡수, 심박수및평균동맥압에서 VIMA군이 TIVA군에비해잘유지또는높은상태로유지됨을볼수있다. - Table 1을살펴보면, LOC까지의시간, BIS score가 40에도달하는시간, 마취종료후각성에소요된시간모두 VIMA에서짧은것을관찰할수있다. 그외에도, procedure 중 breath holding, desaturation은 TIVA에서더높은빈도로나타나지만, procedure 후 excitement는 VIMA군에서더많이발생함을보인다. Data are mean (SD). *P < 0.05 versus the sevoflurane group [1]. - VIMA에서 TIVA에비해많이나타나는 postanesthetic agitation을줄여보고자마취는 sevoflurane으로유도한후, 마취유지를 desflurane 또는 sevoflurane으로하였다 세 38명의환아를두군으로나누었으며, 수술종료후마취에서깨어나는시간및회복과정을살펴보았다. - Table 2의결과는마취제중단후발관까지의시간, 회복실도착시환아의 modified Aldrete score, 그리고회복실에서 PAED score에서모두 desflurane 유지군이양호한것으로나타났다. Bispectral index score for pediatric anesthesia - 소아 VIMA와관련하여 adequate hypnosis level을보려는노력으로 BIS를종종사용하나 BIS가정확한지에대한의문이종종제기된다. - 20명의소아를 halothane, sevoflurane군으로나눈후 equi- MAC으로마취유도및 30분간의유지동안 BIS를관찰 하였다. - Fig. 3의좌측그림을살펴보면, sevoflurane군에서는 2분정도까지급하게떨어지다가약간상승하는모습을보이며, halothane군에서는점진적인감소를관찰할수있다. 그러나실질적으로 brain내마취제의농도는지수함수적인상승을보일것이므로, BIS의변화는마취깊이를적절히반영하지는못한다. 90

7 인준용 :VIMA (Volatile induction/maintenance anesthesia) - 또한, EEG에미치는영향의마취제간차이 (agent-specific effects on EEG) 또한 BIS가정확하게반영하지못하는것이아닌가추측할수있다. 이는 EEG를바탕으로하는다른모니터에서도비슷한양상을보일것으로생각된다. - 우측그림에서는 halothane군에서 high BIS가 maintenance 동안관찰되고있다. 그리고환자간변이가매우큰것을볼수있는데그원인이완전히알려진것은아니나환자의나이가그중하나이다. 취학전아동 (under school age) 에서 high BIS가관찰되며, 연령에따라 EEG 의차이가있다는점은알려져있다. 또한, MAC은통증자극에대한 spinal response를반영하는반면, BIS는 brain의hypnosis를반영하므로두요소에대한마취제간차이가있음을알수있다. Ambulatory anesthesia - Moor 등은당일수술을위해내원하는 1,158명의환자를대상으로, P/P, P/I, P/S, 그리고 S/S의네군을형성하였다. P/P군을제외하고는 N 2O를사용하였으며 LMA 또는 T-tube를거치하였다. - Four groups Propofol induction & maintenance (P/P) without N 2O Propofol induction/isoflurane maintenance (P/I) Propofol induction/sevoflurane maintenance (P/S) Sevoflurane induction & maintenance (S/S) - Table 3 마취유도시를살펴보면, propofol 사용군들에서 pain on injection과 hiccough이높게나타나는반면, breath holding은 S/S군에서많이나타나는것을볼수 있다. Coughing 및 excitatory movement는 P/S군에서, laryngospasm은 P/P군에서 S/S군에비해유의하게낮음을볼수있다. - 전체적으로보면 P/S군에서 adverse events가가장적게발생했음이나타난다. - PONV에서는회복실에서나병동에서 S/S군이높은발생빈도를보인다. - 다음에수술을받는다면동일한마취방법을선택하겠느냐는질문에서는 S/S군이유의하게낮은선호도를보인다. - 그외, 퇴원후재입원빈도나퇴원후추적관찰한통증, 집중력저하, 건망증그리고정상생활로의복귀등에서는유의한차이가보이지않는다. - 따라서, 외래마취시 sevoflurane VIMA는적절한약제가아니라는점, 그리고환자에서 PONV는퇴원후 2일이내에사라지므로, 마취방법선택에는 PONV가크게영향을미치지않는것같다는점을볼수있다. Anesthesia for neurosurgery 1. Neuroanesthesia에서일반적으로고려해야할사항들에는 relatively low cerebral metabolic rate for O 2, sufficient cerebral perfusion pressure, avoidance of increased intracranial pressure 등이있다. 2. Sevoflurane이 CMRO 2 감소, Lumbar CSF pressure 및 ICP 증가를유발하는반면, propofol은 cerebral blood volume 감소를통한 ICP하강을나타낸다고알려져있다. 따라서, sevoflurane VIMA는 hypoperfusion이예상되는환자에서유리할것으로예상할수있다. 이를뒷받침하기 Table 3. 8 및 COER은전기간에서유의하게높음을볼수있다. 이상의결과에서보면, sevoflurane VIMA는 cerebral hypoperfusion이나insufficient oxygen delivery의가능성이있는환자에서더적합할것으로보인다. 맺음말소아마취및당일수술전신마취를위주로제공되는 VIMA 와관련하여지금도지속적으로연구가진행되고있다. 특히소아에서정맥로확보및수술실입실에따른스트레스를줄여주기위한일환으로 VIMA가많이시행되고있다. 그렇지만, VIMA 또는 TIVA 어느한분야만을강조하기보다는각약제의장, 단점을충분히숙지하고조합하여 balanced anesthesia 제공을위한노력이필요할것이다. 위하여 Liao 등은 jugular bulb oxygen saturation 및 jugular venous oxygen partial pressure 등을측정하여다양한지표를관찰하였다 [6]. - Fig. 4에서, 심박수는 VIMA군에서기관내삽관부터수술종료시까지높은상태보였으며, 평균동맥압의경우기관내삽관부터수술시작초반부에서 VIMA군에서높게나타났다. Lumbar CSF pressure를살펴보면 VIMA 군이기관내삽관부터수술시작초반부까지는상승하는반면, TIVA군은하강함을보인다. - CPP의경우 lumbar CSF pressure와비슷한감소양상을보이나, 기관내삽관, 기관내삽관 5분후 (T 5int), 기관내삽관 15분후 (T 15int) 에서 VIMA군이높게나타난다. - Fig. 5에서는 VIMA군에비해서 TIVA군은 jugular venous oxygen partial pressure, jugular bulb venous saturation 는수술동안유의하게낮음을볼수있고, AJDO 2 Fig. 4.
