However, the morbidity and mortality are high. Pancreatic injuries rarely occur alone and common associated with other intra or extra-abdominal injuries. An accurate diagnosis of pancreatic injury is difficult preoperatively due to the retroperitoneal location of pancreas and the lack of early clinical signs. Therefore, early diagnosis and interventions are important to manage pancreatic injuries. There are several therapeutic options for pancreatic injuries. Recently, the adoption of non-operative management for pancreatic injury is increasing. To minimize the morbidity and mortality, the important thing is to choose the optimal time and method to treat the pancreatic injuries. (J Acute Care Surg 2013;3:44-48) Key Words: Pancreas, Injuries, Pancreatic trauma 서론 췌장손상은복부외상환자의약 3 12% 로드물게발생하지만, 유병률은 30 80%, 사망률은 10 30% 로매우높고치료가쉽지않은질환이다. 1-3 특히췌장은후복막에위치하고있어진단이쉽지않고, 다발성손상이동반되는경우가많아치료에주의를요한다. 게다가교통사고나낙상등의복부둔상으로인한췌장손상일때, 초기에췌장손상을진단하기가쉽지않다. 4-7 따라서복부손상환자에서췌장손상이의심되는경우에는진단을위한적극적인노력이필요하다. 여기에서는췌장손상의진단과치료방법에대해고찰해보고자한다. 본론췌장손상의분류 췌장손상에대한분류는미국외상학회 (American Association for the Surgery of Trauma) 지침을주로참고하여사용하고있다 (Table 1). 8 손상정도에따라수술등을포함한치료방법들이달라지며, 이에따른합병증이나사망률도달라진다. 특히췌관의손상여부에따라치료예후가달라지므로췌장손상이있는 Table 1. 2 이승환 외: 외상 췌장 손상의 치료 과 십이지장 등 후복막 장기뿐만 아니라 간과 비장 등과 같은 경우에는 췌관의 손상 유무를 확인하여야 한다. 고형장기의 손상 여부를 확인할 수 있어 가장 좋은 진단 방법 중의 하나이다(Fig. 1). 췌장 손상의 진단에 있어 CT의 민감도와 진단 췌장 손상에 의한 증상은 비특이적으로, 초기 증상만으로 진 특이도는 약 85 90% 정도로, 비교적 정확하게 진단할 수 있 단을 내리기는 어렵다. 특히 복강 내 다른 장기의 손상이 동반된 다.1,9-11 그러나 췌장 손상의 진단이 늦어지면 환자의 경과에 악영 경우가 많고, 후복막에 위치하고 있어 임상 증상만으로는 확진할 향을 줄 수 있으므로 초기 CT 검사에서 음성으로 나왔더라도 수 없다. 일반적으로 혈중 아밀라제 또는 리파제가 상승된 경우에 췌장 손상이 의심된다면 반복하여 CT를 촬영하여야 한다. 췌장 췌장의 손상을 의심할 수 있다. 그러나 아밀라제나 리파제는 췌장 손상에서 췌관의 손상이 동반되는 경우는 약 15% 정도로 보고되 손상 환자의 약 30%에서는 정상으로 나타날 수 있으며, 손상이 고 있으며, 췌관 손상의 유무는 CT로 정확하게 진단하기 어렵다. 심한 경우에도 정상으로 나타날 수 있어 진단 시 주의하여야 한 따라서 췌관 손상이 의심되는 경우에는 췌관조영술을 시행하여야 9-11 하며, 자기공명영상을 이용한 자기공명 담췌조영술(magnetic re- 췌장 손상이 있는 환자에서 치료 방법을 결정하고, 예후를 sonance cholangiopancreatography) 또는 내시경을 이용한 내시경 결정하는 중요한 요소는 췌관의 손상 유무와 췌장의 손상 부위이 적 역행성 담췌조영술(endoscopic retrograde cholangiopancrea- 다. 응급실에 다발성 손상으로 입원한 환자에서 복부 CT는 췌장 tography, ERCP) 등으로 진단할 수 있다(Fig. 2). 3 J Acute Care Surg Vol. 3, No. 2, Oct 손상에대한진단뿐만아니라췌관내배액관삽입술또는췌관손상부위에스텐트등을삽입할수있어진단외에치료적인목적으로사용할수있다. 그러나 ERCP를시행하는경우오히려진단및치료가늦어질수있으므로신중히선택하여야한다. 10,12 췌장손상이동반되어수술을시행하는환자에서췌관손상이의심되는경우에는췌관조영술을시행하여췌관손상의유무를확인할필요가있다. 치료복부손상이있는환자에서혈역학적으로불안정하거나, 복막염이의심되는경우에는별다른진단검사없이바로수술적인치료를시행하게된다. 따라서응급개복하에췌장의손상이진단된경우에는손상통제수술의원칙 지혈및오염원제거 에맞춰수술을시행한다. 췌장손상에따른손상통제수술은손상부위지혈, 괴사조직제거, 그리고가능한경우에는해부학적절제술을시행하고폐쇄성배액관을삽입하여손상부위의염증 및출혈을조절하여야한다. 이와더불어십이지장의손상여부를확인하고, 혈역학적인안정여부에따라수술범위를결정하며, 필요시에는수차례에걸쳐수술적치료를시행한다. 췌장손상에대한수술적치료방법은손상정도에따라수술법을다르게적용할수있다 (Table 2). 그러나수술적치료의범위는집도의의숙련도, 동반손상의유무, 손상정도및환자의상태등을종합적으로판단하여결정하여야한다. 최근에는췌장의손상정도가 I/II인경우수술적치료보다는비수술적인치료가더많이시행되고있다. 비수술적인치료가수술적치료와비교해합병증이나사망률에차이가없음이밝혀지면서췌관손상이없는저등급의췌장손상은경과를관찰하면서환자의임상양상의변화에따라경피적배액관삽입술또는수술적괴사조직제거술및배액관삽입술등을시행할수있다. 2,13-16 췌관손상을동반한환자에서도혈역학적으로안정적인경우수술적치료와 ERCP를통한비수술적인치료가비슷한합병증과사망률을보인다는보고들이점차증가하고있다. 따라서환자상태가안정적이고복막염증상과같은응급수술의적응증이아닌경우에는 ERCP를시행하여췌관내배액관삽입술을통한췌장액의배액과함께보존적치료를시행해볼수있다 (Fig. 3). 췌장손상후발생하는주된합병증은췌장주위농양과가성낭종으로이는수술적인배액술을시행할수있으며, 많은경우에서경피적배액술이나내시경을이용한경위배액술등을시도하기도한다. 17 Fig. 2. CT: computed tomography, ERCP: endoscopic retrograde cholangiopancreatography, MRCP: magnetic resonance cholangiopancreatography, ICU: intensive care unit. 췌장손상에대한치료방침은환자의상태에따라일차적인치료방침을정하고, 수술또는비수술적치료의적응증을면밀히살펴환자에게맞는치료를제공하는것이다. 이에따라여러기관에서췌장손상에대한치료프로토콜을이용하여치료를하고있으며, 이의유용성에대해서는이미밝혀져있다 (Fig. 4). 13,18,19 합병증췌장손상환자에서합병증의발생률은매우높으며 30 80% 정도에서보고하고있고, 국내의보고에서도약 38 77% 정도로보고하고있다. 3,9 가장흔한합병증은췌장루로약 10 20% 정도에서발생하며, 대부분보존적치료로호전되지만수술적인치료를요하는경우도있다. 9 췌장주위농양도비교적흔한합병증으로 6 32% 정도발생할수있으며, 방사선중재시술을통해배액시키거나수술적으로배액술을시행하게된다. 10 췌장가성낭종은약 5 30% 정도에서발생할수있으며, 대부분비수술적인치료로호전되고, 내시경을이용한위-낭종조루술 (gastrocystostomy) 등을시행하여치료할수있다. 그러나비수술적인치료에호전되지않는가성낭종은수술적으로위-낭종조루술이나회장-낭종조루술을시행하기도한다. ERCP를이용한췌관스텐트삽입술이나괄약근절개술등도췌장루의치료에도움을줄수도있다. 결론 췌장손상은초기진단이중요하며, 최종적인치료방법은혈역학적인상태, 췌관손상의유무, 비수술적치료의적용가능성등을종합적으로판단하여결정하는것이바람직하다. 또한비수술적인치료방법을선택한경우에는환자의상태를면밀히관찰하여합병증발생여부, 손상부위의악화소견이있을때는경피적배액술이나수술적인배액술, 췌장절제술등을고려하여야한다. 참고문헌 1.
