Tel: , Fax: , Background: Remifentanil could require an appropriate pain strategy to prevent unacceptable pain in the postoperative period. We investigated the effect of perioperative ketamine for nociception and remifentanil hyperalgesia after gastrectomy when we used intraoperative remifentanil infusion for hemodynamic control. Methods: Eighty patients scheduled for gastrectomy were randomly included in this trial. In control group (C), patients received normal saline. In ketamine group (K), patients received ketamine perioperatively. In remifentanil group (R), patients received remifentanil during operation. In remifentanil and ketamine group (RK) patients received remifentanil intraoperatively and ketamine perioperatively. Hemodynamic indexes were recorded during anesthesia. Visual analogue scale (VAS), cumulative analgesic requirement, and side effects were recorded during 36 h after operation. Results: The requirement of vasodepressants was significantly increased in C and K group. Morphine consumption and agitation were significantly increased in R group at the postanesthetic recovery room. The analgesic demand of R group was greater than that in other groups. The VAS scores of K group were significantly lower than those of other groups at the ward. Conclusions: Intraoperative use of remifentanil with sevoflurane may be related to increased pain during early postanesthetic period. Perioperative low-dose ketamine decreased intraoperative remifentanil use and postoperative morphine consumption without increasing the incidence of side effects. (Anesth Pain Med 2013; 8: 91-98) Key Words: Hemodynamics, Hyperalgesia, Ketamine, Nociception, Remifentanil. 서 1988년에 10만명의마취증례에서사망률을조사하였을때약물을병용투여하여마취를하는경우한두가지의약물만을사용한경우보다더안전하다는결과가나타났다 [1]. 따라서현대의마취방법은흡입마취제를포함한여러약물을같이사용하는균형마취방법이주류를이루고있다. 그러나전신마취를위해흔히사용되는흡입마취제외에안전한균형을위해사용될수있는약제에대한선택의폭은넓지않으며주로아편유사제가많이쓰이고있다. 최근에 remifentanil은흡입마취제의최소폐포농도감소효과, 자극에대한혈역학반응의약화를포함한여러가지이유로전신마취의보조약제로임상에서널리사용되고있다 [2,3]. 특히관상동맥우회수술 [4], 복강경수술 [5], 갈색세포종수술 [6] 같은혈역학적조절이중요한수술에서유용성이입증되어왔다. 균형마취시마취심도는흡입마취제로, 혈역학적변화는 remifentanil로조절하는것이바람직하다는보고도있다 [7]. 그러나전신마취중 remifentanil 사용시수술후급성내성과통각과민유발에대한다양한논문들이보고되면서 [8-11] 수술중 remifentanil 사용의유용성에대한의문이제기될수있다 [12]. 이에본연구자들은위절제술시수술중 remifentanil을투여한경우 ketamine 병용유무에따라수술후의통증에미치는영향을알아보고자하였다. 론 대상및방법 병원내임상연구심의위원회의승인을얻은후피험자동의서를받고단일외과의사에의한위절제술예정인환자 80명을대상으로하였고모든환자는미국마취과학회협회 91

2 92 Anesth Pain Med Vol. 8, No. 2, 2013 신체등급 (ASA) I, II, III인환자들이었다. 적용대상은 20세이상 70세미만으로제외대상은만성통증, 약물이나알코올남용의과거력이있는환자, 정신질환자, 응급수술환자, 심한간질환, 신질환을동반한환자, 심한호흡기질환, 관상동맥질환, 심부전을동반한환자, 서면동의를이해하지못하거나동의하지않는환자등이다. 수술전날환자들은본연구의목적과방법에대한설명을듣고자가통증조절기의사용법과시각통증등급 (Visual analogue scale, VAS) 에대한교육을받았다. VAS는통증이없는것을 0점, 참을수없는아픈통증을 10점으로정하였다. 모든환자는전처치로 midazolam 0.05 mg/kg, glycopyrrolate 0.2 mg을근주받았다. 환자들은마취유도전밀봉된불투명봉투 80개중한개를연구에참여하지않는의료진이선택하는방식으로무작위방식으로연구군에배정되었다. 환자가수술실에도착한후기본적인비침습적혈압, 심전도, 맥박산소포화도를측정하였다. 또한이중편광수치 (Bispectral index, BIS) 측정을위해이마에 BIS (BIS TM, Aspect Medical System, USA) 감지기를부착하였다. 전신마취는 thiopental 250 mg, lidocaine 60 mg과 rocuronium 0.6 mg/kg으로유도하고기관내삽관을시행하였다. Remifentanil만투여되는군 (R군, n = 20) 과 remifentanil과 ketamine이투여되는군 (RK군, n = 20) 은마취유도시부터목표효과처농도 (Ce) 4.0 ng/ml로 remifentanil을, 대조군 (C군, n = 20) 과 ketamine만투여하는군 (K군, n = 20) 에는생리식염수가들어있는주사기를이용하여지속정주를시작하였다. Remifentanil 및생리식염수는 Orchestra Base Primera (Fresenius Vial, France) 를이용하여정주하였으며투여시농도는혈역학변화에따라조절하되가능한 4 ng/ml의농도로주입하도록하였다. 마취는 sevoflurane vol% 와 rocuronium으로유지하였다. 피부절개전 ketamine이투여되는군 (K, RK군 ) 에는 0.5 mg/kg 부하용량을정주하고이후 0.5 mg/kg/h로지속정주하였고 C, R군에는생리식염수를정주하였다. 마취와수술중평균혈압, 맥박수, BIS, sevoflurane과 remifentanil 농도를 15분간격으로측정하였다. 불충분한마취는수축기혈압이기본치보다적어도 1분이상 20% 이상넘을때와이중편광수치가 70이상증가시로정의하였다. 환자의움직임, 기침, 눈물, 땀분비역시부적절한마취의징후로여겼다. 흡기 sevoflurane 농도나 remifentanil의목표주입농도를불충분한마취시 0.5 vol% 나 1 ng/ml씩단계적으로증가시켰다. 저혈압은수축기동맥압이 80 mmhg 이하거나평균동맥압이 60 mmhg 이하일때로정하고이중편광계수가 60 이하인상태에서 sevoflurane 농도를 0.5 vol% 씩감소하였다. 서맥과지속적인저혈압 ( 평균혈압 60 mmhg 이하시 ) 이 5분이상지속시 glycopyrrolate나 간헐적인 ephedrine을투여하였다. 마취유도후혈압의심한상승 ( 수축기혈압 160 mmhg 이상 ) 이있거나빈맥이있는경우 esmolol, labetalol, hydralazine과같은혈압하강제를투여하였다. 다른아편유사제는수술동안투여되지않았다. Ketamine 투여는복막봉합시중단되었고 remifentanil 투여는피부봉합시중단되었다. 복막봉합시정맥내자가통증조절장치 (Accumate 1100, Wooyoung Medical, Seoul, Korea) 를이용하여자가통증조절법을시작하였다. C, R 군에는 morphine sulfate 100 mg과생리식염수의혼합액을, K, RK 군에는 morphine sulfate 100 mg, ketamine 100 mg과생리식염수의혼합액 100 ml를시간당 1 ml, 잠금장치시간 7분, 환자조절량 1 ml로투여를시작하였다. 신경근차단회복을위해수술종료시정맥내 glycopyrrolate 0.4 mg과 pyridostigmine 15 mg을투여하였다. 적절한자발환기의회복과눈뜨기와같은구두명령에대한반응후기관내튜브를제거하였다. 수술후환자는회복실로이송되어약 1시간동안주의깊게관찰되었다. 마취중에는수술중평균동맥압, 심박수에따른 ephedrine, glycopyrrolate, 혈압하강제의사용여부및횟수, remifentanil의총사용량및농도, ketamine 소모량, BIS, sevoflurane의시간에따른농도를기록하였다. 회복실도착후약 1시간동안 15분간격으로 VAS, 진통제소모량, 오심, 구토, 호흡저하같은부작용을측정, 기록하고오심과구토가심한경우 metoclopramide 10 mg을먼저투여하고지속되는경우 palonosetron HCl 75 μg을투여하였다. 회복실에서의진정측정은 Riker 진정-흥분점수 (Riker sedationagitation scale, RAS) 를이용하였다 (Table 1) [13]. 회복실에서는자가통증조절장치의부하용량을이용하지않고 morphine sulfate를 2 3 mg씩정주하고용량을기록하였다. 병실을방문하여수술후 3, 6, 12, 24, 36시간동안추가적으로투여된진통제양, VAS를측정하였다. 추가적인진통제종류는 fentanyl 50 μg 정주로한정하였다. 수술후오심과구토, 항오심제의요구를수술후 36시간동안관찰하였다. 술후 36시간동안악몽, 환청, 환각, 섬망과오심, 구토, 가려움증. 진정, 분당 8회미만의호흡횟수를기준으로한호흡억제등의부작용빈도를관찰하였다. VAS와부작용유무는환자에게질문하는시간대가새벽 1 5시사이인경우환자의불편감을감안하여환자를깨우지않고아침 6시에방문하여묻도록하였다. 48시간이지나환자에게서분리된자가통증조절장치의단추를누른횟수 (attempt), 환자에게실제로주입된 (injected) 횟수및진통제의총량을자가통증조절장치에저장된자료를이용하여얻었다. 모든측정치는평균 ± 표준편차로표시하였으며, 통계분석은 SPSS (version 12.0 SPSS Inc. RK군과 R군간의수술중 remifentanil 총사용량과농도, RK군과 K 군간의수술중 ketamine 사용량은독립표본검정을이용하여평가하였다. 수술후 VAS, 자가통증조절장치를통한morphine 소모량및자가통증조절장치의누른횟수는반복측정분산분석을이용하였고사후검정은 Bonferroni test를사용하였다. Riker 진정-흥분점수는 Kruskal-Wallis test를이용하였고수술의종류, 성별, 수술중 glycopyrrolate, ephedrine 사용, 항오심제사용및부작용의빈도 ( 오심, 호흡저하, 어지럼증, 두통등 ) 는카이제곱검정및 Fisher의정확한검정을이용하였다. 모든통계결과는 P 값이 0.05 미만인경우를통계적으로유의한것으로판정하였다. 연구대상군의크기는본연구의사전연구에서 24시간동안의평균 morphine 소모량이 36 ± 5.2 mg인것을기본으로하여각군간 morphine 소모량의차가 5 mg 이상인경우를임상적으로의의있는것으로고려하여 80% 검정력과 5% 의오차수준에서각군당 20명으로산출되어총 80명으로하였다. 결과환자의연령, 성별, 체중, 신장, ASA, 수술의종류, 수술시간등은네군간에유의한차이를보이지않았다 (Table 2). 수술중사용된 remifentanil의농도에는차이가없었으나총 remifentanil의소모량은 RK군보다 R군이많았고 (P < 0.05), ketamine 소모량은 RK, K군간차이가없었다 (Table 3). 마취중 2시간동안의 sevoflurane의흡기농도는 C, K군이 R, RK군에비해상대적으로높았고 (P < 0.05), BIS는 RK군이 C, R, K군보다상대적으로높았고 K군이 C군보다높았다 (P < 0.05). 수술중 ephedrine과 glycopyrrolate사용횟수는네군간차이가없었으나혈압하강제는 C, K군에서상대적으로많이사용되었다 (P < 0.05). 수술후오심, 구토의빈도에서는네군간에차이가없었으나항오심제의요구량은 R군에서많았다 (P < 0.05) (Table 4). 회복실에서측정한 Riker 진정-흥분점수는 R군이 C, K, RK군보다높았고 K군이 RK군보다점수가낮았다 (P < 0.05). 다른부작용 ( 어지러움증, 진정 ) 에서는네군간에차이를보이지않았다. 회복실에서의 morphine 투여량은 R군에서 C, K, RK군보다많았고 (P < 0.05), K군에서 RK군보다 morphine 투여량

Group C: control, Group R: remifentanil, Group K: ketamine, Group RK: remifentanil-ketamine, PAR: postanesthetic recovery room. *P < 0.05 compared with C, P < 0.05 compared with K, P < 0.05 compared with RK. 의감소를보였다 (P < 0.05) (Table 5). 병실에서돌발통을위해투여된 fentanyl의총투여량에서는네군간차이가없었다. 병실에서자가통증조절장치를누른횟수는 R군이수술후 3시간에 C, K군보다많았고 6, 12, 24, 36시간에는 K군보다많았다 (P < 0.05) (Fig. 1). 병실에서자가통증조절장치로투여된누적된 morphine의소모량은네군간에통계적인차이가없었다 (Fig. 2). 수술후 36시간동안의 VAS에서수술후 15분, 30분, 6 시간, 36시간에 K군의 VAS가 C군보다통계적으로의미있게낮았고 (P < 0.05), 수술후 30분, 45분, 1시간, 6시간에 K군의 VAS가 R군의 VAS보다낮았다 (P < 0.05) (Fig. 3). 수술후 36시간에 R군의 VAS가 RK군보다통계적으로의 의있게높았다 (P < 0.05). 고찰본연구에서는 sevoflurane을이용한전신마취시 remifentanil을보조적으로사용하는경우혈역학의변화감소와 sevoflurane 소모량감소의이점이있었다. 그러나회복실에서환자가흥분을보이는경우가많았고 morphine의요구량도증가하였으며병실에서자가통증조절기를누른횟수가증가하였다. Ketamine을수술중과자가통증조절장치를통해투여하는경우회복실이나병실에서의진통제소모량이나통증점수에서감소를보였으며 remifentanil과같이

R: remifentanil group, RK: remifentanil-ketamine group, K: ketamine group, C: control group. 투여하는경우 remifentanil의소모량을감소시켰으며 remifentanil만투여하는경우보다는회복실에서의통증으로인한흥분이나 morphine의요구량증가를감소시키는현상을볼수있었다. Remifentanil은수술중흡입마취제의사용량감소와혈역학조절의용이성때문에점차그사용이증가되어가는추세이다 [2-5]. 이러한수술중 remifentanil 사용시의많은장점때문에수술후급성내성이나통각과민을유발하는지의여부는매우중요한관점이되어이와관련한많은연구들이보고되었다 [8-11]. Remifentanil이통각과민과급성내성을유발하지않는다는논문들도보고되었다 [10,11]. Gustorff 등의 [10] 연구나 Cortinez 등의 [11] 연구에서임상적으로 remifentanil을투여한군에서통각과민현상을찾아볼수없었다고보고하고있다. 그러므로 remifentanil의내성과통각과민을차단하는방법에대한연구에서는그모델에서실제적으로내성과통각과민이나타났는지확인하는단계가필요하다. 통각과민이발생하지않은경우차단약제의양성적효과는아편유사제에의한통각과민차단효과보다는다른효과에의한결과일가능성이있다. 지금까지의 remifentanil 의통각과민의차단에관련한임상논문대부분이단순히 remifentanil 투여군과차단효과가있는약제투여군만을비교한모델들이었다 [14,15]. 본연구는 ketamine과같은약제를투여하는군뿐만아니라대조군, remifentanil만투여하는군, ketamine만투여한군과 remifentanil과 ketamine을병용투여하는군을연구에포함하여실제로모델에서통각과민이발생하는지를확인하고자하였다. 일부연구에서는 remifentanil을지속투여한경우수술후통각과민을일으키거나수술후 morphine의요구량이증가한다고결론짓고있다 [14]. 부인과수술환자에서 sevoflurane 마취시사용된 remifentanil 1, 3 ng/ml의지속투여시회복실에서의통증점수가 3 ng/ml에서높았고진통제투여에서는큰차이는없었다고한다 [16]. 본연구에서도환자가회복직후라서자가통증조절장치사용에대한어려움이있어자가통증조절장치에의한부하용량은사용하지않고환자의통증호소에따라 morphine을투여하였는데회복실에서의 morphine 소모량이 remifentanil 투여군에서많았고수술후병실에서의진통제소모량에는큰차이가없었다. 하지만 remifentanil군에서병실에서자가통증조절장치를누른횟수가다른군에비해많았는데주입회수 / 누

6 96 Anesth Pain Med Vol. 8, No. 2, 2013 른회수의비율이자가통증조절장치에대한환자의만족도와일치할수있다는이론에따르면 remifentanil군에서자가통증조절장치에의한통증조절에대해환자가만족하지못하고있다는것을간접적으로시사한다고하겠다 [17]. 아편유사제유도성통각과민은장시간작용성아편유사제보다는 remifentanil같은강력하고단시간작용성아편유사제의투여에서보다흔하고빠르게형성된다고한다 [18]. 본연구에서는 remifentanil만수술중에투여한경우회복실에서흥분상태를나타내는환자가많았는데이것이통각과민이나급성내성으로인한증가된통증때문인지혹은 remifentanil의빠른작용소실에의한일시적인현상인지는감별할수없었다. 본연구의경우 R군과 RK군에서수술중 remifentanil의투여농도가약 3 ng/ml이였는데 Ryu 등의 [19] 연구에서는보다낮은농도인 remifentanil 1 ng/ml를투여한경우에도회복실에서통증에의한흥분을보이는경우가많았다. 따라서복부수술을시행하는환자에서 sevoflurane과함께투여한 remifetnanil이회복과정에서흥분증가와함께술후통증을증가시키는요인이되므로 sevoflurane을이용한마취시 remifentanil의지속투여는신중하게결정해야한다고결론짓고있다. 그러므로만일수술중에어떠한목적으로 remifentanil을사용하게되는경우수술후발생할가능성있는통각과민과통증조절의어려움에대한대안이필요할수있겠다. 수술후통각과민의치료를위해침해유도성통각과민의경우아편유사제, 비스테로이드성항염증제제, ketamine 과같은 N-methyl-D-aspartate (NMDA) 수용체길항제. gabapentin과 pregabalin 같은전압-관문성칼슘통로단백질에결합하는물질들이시도되어왔다. 특히수술후통증관리에대한 균형진통 의개념으로 NMDA 수용체의길항제중하나인 ketamine에대한다양한연구들이이루어져왔다 [20,21]. Ketamine을주술기에사용시수술후아편유사제에대한보존효과가기대되어임상적에서수술후통증치료에사용되어왔는데이 ketamine의효과와관련된논문 37개를검토한결과 27개의연구에서 ketamine이진통제요구량이나통증강도를감소시켰다고보고하고있다 [21]. 본연구에서도 ketamine을사용한군에서는대조군에비해수술후회복실에서아편유사제소모량이감소하였고병실에서의통증점수가낮았다. 항통각과민효과를위한 Ketamine의투여시기에대해서는다양한보고들이있다. Dahl 등은 [22] 피부절개전투여한경우가절개후투여보다통증점수가낮았다고보고하였다. Bilgin 등은 [23] 수술전부터수술이끝난이후에도계속투여하는경우가가장효과적이였다고보고하였다. 저자들은이연구에서 ketamine의적절한투여시기를밝히기보다는 remifentanil에의한통각과민반응을줄이려는노력 에초점이맞추어효과가있을수있는수술전후시기전체에 ketamine을투여하여최대한의효과를얻어보고자하였다. 1,026명을대상으로한연구에서 ketamine과 morphine의가장적절한병용투여는 1 : 1 (mg) 이고잠금시간은 8분이라는결과를보고하였다 [24]. 저자들의연구에서도상부복부수술이수술후통증이심한수술인점을고려하여지금까지가장적절한병용투여로알려진 morphine과 ketamine을 1 : 1로투여하였고잠금시간은 7분으로정하였다. 또한수술의종류를위절제술로한정시키고단일외과의의집도에의한수술에대한자료만수집하였으므로발생가능한편향 (bias) 를줄여결과에대한신뢰도를높였다. Ketamine이침해손상에의한통각과민뿐만아니라아편유사제에의한급성내성과통각과민현상에도효과가있다는논문들이보고되었는데이연구에서사용된 remifentanil에의한통각과민을예방하였다는보고들이있다 [14,15]. Remifentanil의고용량사용후회복실에서즉각적인심각한수술후통증으로인해 morphine의부가적인투여가필요할수있는데 [19] 이는심한호흡저하를유발시킬수있으므로통증조절을위한 morphine의단독사용은부적당할수있다. 이런경우진통의상승작용을나타내는약제들을혼용하는균형진통이적절할수있으며 remifentanil 기본마취에 ketamine의첨가가회복의지연없이수술후통증증가를예비할가능성이될수있다. Remifentanil에대한 ketamine의통각과민현상의차단에대한다른설명으로는 ketamine이수술중 remifentanil의사용을감소시키는능력에기초를둘수있다. 아편유사제유도성통각과민의발생이아편유사제의사용용량과밀접한관계가있으므로이러한 remifentanil의용량감소가아편유사제의통각과민발생을감소시킬수있겠다 [16]. 저자들의연구에서는수술중 remifentanil의농도가 RK군과 R군간큰차이가없었으나 remifentanil의총사용량은 remifentanil만투여한군이증가되어있어 remifentanil군에서의회복실의진통제소모량이나환자의흥분의증가가 remifentanil과 ketamine을투여한군에서적은현상의이유가될수있다. Ketamine의 remifentanil 유도성통각과민의차단에대한효과를입증하기위해서 RK군과 R군간 remifentanil의사용량을동일하게유지하는것이이상적이다. 하지만본연구에서 RK군에서 R군과같은양의 remifentanil을사용하면서 BIS를 40 60으로유지하기위해서혈압상승제가불필요하게소모될수있고이는임상윤리에어긋나는것으로고려하여 RK군에서 remifentanil의용량을변화시켰으므로이점은이모델의임상적한계점이라고하겠다. 그러므로이연구모델에서수술중 ketamine이 remifentanil의통각과민현상을직접적으로차단하였는지 remifentanil의사용량감소에의한차단효과였는지정확히밝히기는어렵겠다. 소량의 ketamine을수술중에사용하는경우발관이나각

7 이지희외 3 인 :Effect of ketamine and remifentanil on postoperative pain 97 성의연장을유발하지않았으나첫진통제요구시간이지연되었고낮은통증점수를보이며진정점수또한높아진통제요구를지연시킨것으로보인다고하였다 [14]. 본연구에서도회복실에서 ketamine만투여한군에서진정도가증가되어있었고 remifentanil과같이투여한경우는차이가없었다. 본연구에서는 ketamine을투여한 RK군과 K군의BIS 가증가되어있었는데 Hans 등의 sevoflurane으로마취시 ketamine 0.5 mg/kg을정주한후 BIS가증가한다고보고한논문과일치하는결과를얻을수있었다 [25]. Ketamine은환각, 악몽과같은정신병유사성부작용, 어지럼증, 오심과구토를나타낼수있으며자가통증조절기에 ketamine을병용투여하는경우아편유사제의보존효과로오심과구토가감소될수있다는보고가있으나 [21], ketamine의용량증가에따라오심, 어지럼증의빈도가증가할수있다는보고도있다 [26]. 본연구에서는 remifentanil만투여한군에서수술후항오심제요구량이증가되었는데이는 remifentanil 소모가수술후오심, 구토의강력한중계인자라고할수있겠다 [27]. 그러므로본연구에서수술중 remifentanil의사용이흡입마취제에의한전신마취시혈역학안정성및흡입마취제의사용량감소를이끄나수술후특히회복실에서의통증및마약진통제의요구량중가와환자의흥분초래의관점에서볼때 ketamine을함께투여하는것이수술후환자의통증관리에바람직할것으로본다. 참고문헌 1.
