However, there is inconvenience to patients due to long examination time and repeated blood sampling. Recent studies have shown that only one to two times of blood hormone test can distinguish these pubertal conditions. Methods: We retrospectively reviewed the medical records of patients who had GnRH stimulation test at NHIS Ilsan Hospital between Jan and Dec Results: During the study period, total 69 girls were tested for the evaluation of precocious puberty. 65 patients were acceptable for diagnostic criteria for central precocious puberty (CPP Group), and 4 patients had premature thelarche but did not meet the precocious puberty diagnostic criteria (PT Group). Although not statistically significant, peak luteinizing hormone (LH) was higher in the CPP group (12.81±10.53 vs. 3.68±0.37, p<0.09). In the CPP group, 54 (83.1%) and 9 (13.8%) of the 65 patients reached the peak LH level at 30 and 60 minutes after GnRH administration respectively, accounting for 96.9% of the all CPP patients. In addition, peak LH 5 IU/L or more, which is a diagnostic criterion for CPP, was satisfactory in all patients in the CPP group at 30 minutes. Conclusion: Measuring LH from 30 minutes to 60 minutes is useful for the diagnosis of central precocious puberty. Key Words: Gonadotropin-Releasing hormone, Precocious puberty, luteinizing hormone (LH) 서론 성조숙증이란사춘기이차성징의출현이여아에서 8세이전, 남아에서는 9세이전에나타나는경우로여아에서훨씬흔하다. 시상하부- 뇌하수체- 성선축이빨리성숙되어오는경우를진성 ( 또는생식샘자극호르몬의존성 ) 성조숙증, 그렇지않은경우를가성 ( 또는생식샘자극호르몬비의존성 ) 성조 책임저자 : 김기혁 경기도고양시일산동구일산로 100 국민건강보험일산병원소아청소년과전화 : (031) , 팩스 : (031) lohen@nhimc.or.kr * 본연구는국민건강보험일산병원의연구비지원으로이루어졌음 (NHIMC 2013CR042). 숙증이라고한다. 1,2 성조숙증이의심되는경우병력청취와진찰, 골연령검사및성호르몬검사를시행한다. 성호르몬의측정은생식샘자극호르몬인황체화호르몬 (luteinizing hormone, LH) 과여포자극호르몬 (follicle-stimulating hormone, FSH), 에스트라디올또는테스토스테론을측정한다. LH와 FSH는하루에도혈중농도의변화가크고에스트라디올은측정값의변이가심하여이들의기저농도측정만으로는성조숙증의진단이어렵다. 또한진성과가성성조숙증의감별이필요한데, 진성성조숙증환자에서는정상적인사춘기에서와같이시상하부- 뇌하수체- 성선축의활성화가시작되어인공적 LH-releasing hormone (LHRH) 을주사하게되면혈중 LH, FSH 농도의증가가나타나게되는데, 이를이용한생식샘자극호르몬방출호르몬 (gonadotropin releasing hormone, GnRH) 자극검사가 Volume 16 Number 2 December

Gonadotropin-Releasing Hormone Stimulation Test in Patients with Precocious Puberty 시상하부-뇌하수체-성선축활성의평가와사춘기질환의진단에널리이용되고있다. 3 일반적으로 LH과 FSH의혈중농도는 GnRH 를주입한후 5회이상의채혈을통해최고반응농도를검사하게된다. 채혈의횟수가많을수록검사의신뢰도는높아지지만, 두시간이상의긴검사시간과여러번의혈액검사로인해환자및보호자들의불편감또한증가한다. 최근 GnRH 를투여한후 1-2회검사한 LH 농도만으로도사춘기유무를판별할수있다는연구보고들이있었지만, 4-8 검사의진단신뢰도및장단점에대해서는아직까지는더많은연구와검토가필요하다. 9 따라서본연구에서는성조숙증으로진단받은환아들에서 GnRH 자극검사의결과를후향적으로분석하여, 진성성조숙증검사를더쉽고빠르게시행할수있을지알아보고, 성호르몬기저치의진단유용성에대해알아보고자한다. 대상및방법 1. 대상 2008년 1월부터 2013년 12월까지일산병원소아청소년과에 8세전발생한유방발달을주소로내원하여병력청취와진찰, 골연령검사및기저성호르몬검사에서성조숙증이의심되어 GnRH 자극검사를시행한여아들을대상으로하였다. 진성성조숙증의기준은 8세이전의이차성징의출현, 골연령의증가, GnRH 자극검사에서 LH의반응이 5IU/L 이상인경우를대상으로하였다. 1-3,9 2. 방법의무기록을후향적으로분석하여환아들의임상자료를조사하였으며, 골연령은 Greulich-Pyle 법을이용하여측정하였다. GnRH 자극검사는 LHRH (100 μg) 을정주하고, 기저치와 30분간격으로 120분까지연속적으로 LH와 FSH를측정하였다. LH와 FSH는 immunoradiometric assay 를이용하여측정하였다 통계분석검사와측정결과는평균 ± 표준편차로나타내었으며, 통계분석은 SPSS version 18.0 (SPSS Inc, Chicago, Illinois, USA) 을이용하여그룹간의비교는 Student's t-test와 Mann-Whitney U-test 를, 상관관계의분석에는 Spearman's correlation 을사용하였다. p값이 0.05 미만인경우에통계학적의미를두었다. 결과 연구대상기간중 69명의여아에서 GnRH 자극검사를시행하였다. 이중 65명이진성성조숙증의진단기준에합당하였으며, 4명은조기유방발육은있었으나진성성조숙증의진단기준에는합당하지않았다. 이들을진성성조숙증군 (CPP group) 과조기유방발육군 (PT group) 으로분류하였다. LH 기저치는두군간의차이가없었으며, GnRH 자극검사결과 LH 최고치는진성성조숙증군에서 12.81±10.53 IU/L 로조기유방발육군 (3.68±0.37 IU/L) 에비하여높았으나, 조기유방발육군의수가상대적으로적어통계적차이는없었다 (Table 1, Fig. 1B). 최고 LH/FSH 비는두군간에차이가없었다 (Table 1). 진성성조숙증환자 65명에서 GnRH 자극후 30, 60, 90, 120분에 LH가최고치에도달한환아는각각 54명 (83.1%), 9명 (13.8%), 1명 (1.5%), 1명 (1.5%) 이며, 진성성조숙증의진단기준인 LH 5.0 IU/L 를만족한것은 30분에이미 100% 에도달하였다 (Fig. 2). 진성성조숙증군과조기유방발육군모두에서 GnRH 자극 Table 1. There was statistically significant difference between groups in the change of LH concentration by time (A; p<0.01), but there was no difference in FSH (B). Data are presented as median and 95% CI. 후 LH와 FSH 치가증가하였다. LH의증가는진성성조숙증군에서훨씬급격히일어나며, FSH는양군에서비슷하게증가하는양상을보였다 (Fig. 2, p<0.05). 진성성조숙증환자들에서 LH 최고치와 LH, FSH, LH/FSH 비등의기저치와비교해보았을때 LH 기저치만유의한상관관계를보였다 (Fig. 3). LH 기저치를이용하여성조숙증을예측하기위하여 ROC curve 를작성하였으나, 진성성조숙증군에비하여조기유방발달군의숫자가적어서의미있는 cut off value를구하지는못하였다 (Fig. 4, p=0.143). 고찰 여아에서만 8세이전에유방발육이나타나거나, 남아에서만 9세이전에고환의크기가증가하는것을성조숙이라고하며, 일반적으로조기사춘기는사춘기가여자에서 세, 남자에서 세사이에시작되는것을말한다. 3-6,9,10 성조숙증은시상하부- 뇌하수체- 성선축이일찍활성화되면서생기는진성성조숙증과활성화없이 2차성징을보이는가성성조숙증으로구분한다. There was no statistical significance because the number of PT groups was small (p=0.143). 환자를평가하는첫단계는정상소아에서단순히 2차성징처럼나타났다가소실되거나빠른변화가없는양성 (Benign) 인경우와실제로진행하는심각한성조숙증을구분하는것이다. 2차성징이언제나타났고, 얼마나빠르게진행하는지알아보고출생전후에문제가있었는지, 성선스테로이드노출유무, 가족력, 연간성장속도등의병력을청취하고키와체중을측정하여 Tanner stage 로평가한다. 이후이른아침에혈중 FSH, LH, 에스트라디올, 테스토스테론농도를검사사고성선종양등이의심되면인간융모성생식선자극호르몬 (human chorionic gonadotropin, hcg), 알파태아단백질 (alphafetoprotein, αfp) 등도측정해야한다. 또한, 골연령을측정 (Tanner-Whitehouse 3 방법이나 Gleurich-Pyle 방법 ) 하여실 제역연령과비교한다. 일반적으로진성성조숙증여아에서기저에스트라디올농도는 10 pg/ml 이상이며, 기저 LH 농도는 0.6 miu/ml( 평균 1.6 miu/ml) 이상이다. 11 진성성조숙증의확진을위해서는 GnRH 자극검사가필요하다. GnRH 를투여한후시간별 LH 농도를검사하여기저 LH 농도에비해 2-3배증가하거나, 최고 LH 농도가남아에서 9.6 miu/ml, 여아에서 6.9 miu/ml 이상이면진성성조숙증으로확진한다. 12,13 현재우리나라의료보험에서는 GnRH 자극검사에서최대 LH 농도가 5.0 IU/L 이상인경우에만성조숙증으로인정하고있다. 특히 6세이전에이차성징을보인중추성성조숙증으로진단된남아나여아는뇌하수체종양등을감별하기위해반드시뇌자기공명영상촬영을추가적으로진행하여야한다. 14 시상하부의억제가풀렸더라도시상하부 -뇌하수체 -성선축이활성화된것을확인하여야한다. 보통골연령과, 연간키성장속도를검사하고, 여아는유방발달상태로남아는고환크기의증가를평가지표로이용한다. 선별진단검사로여아는에스트라디올이, 남아는테스토스테론이이용되기도하지만, 에스트라디올검사는측정값의변이가커서사춘기유무를판단하기에는민감도가낮다. 테스토스테론검사는이보다이용가치가높다. 최근 3세대화학발광면역측정 (Chemiluminescent Immunoassay) 검사에서기저 LH 농도가 IU/L 이상이면사춘기가능성이높지만, 15,16 농도가매우높지않으면 GnRH 자극검사로확진하는것이바람직하다. 9 기저 LH가 0.2 IU/L 이상인경우시상하부- 뇌하수체- 성선축의성숙을의심할수있지만, 16 Tanner stage II 이상을보이는소녀의 53.8% 에서이보다낮은범위를보이므로정확한진단을위하여 GnRH 자극검사가필요하다. 17 현재시행되는 GnRH 자극검사방법은 GnRH 를투여한후 30분마다 5번이상의채혈을하고있어환자및보호자에게도상당한시간적, 경제적부담을주고있다. 본연구에서진성성조숙증환자 65명에서 GnRH 자극후 30, 60, 90, 120분에 LH가최고치에도달한환아는각각 54명 (83.1%), 9명 (13.8%), 1명 (1.5%), 1명 (1.5%) 이어서, 약 97% 의여아들에서이미 30분과 60분의측정에서최고 LH 반응이나타나고있었으며, 진성성조숙증의진단기준인 LH 5.0 IU/L 이상을만족한것은 30분에이미모든환아에서 100% 에도달하였다. 이결과로보아 30분과 60분에측정한 LH 결과만으로진성성조숙증진단에충분할것으로생각된다. 이전의연구들에서도 GnRH 검사에서 30분에서 60분사이에 LH를측정하는것으로도진성성조숙증의진단에충분하다는보고들이있었다. 4,7,8, Korean Journal of National Health Insurance Service Ilsan Hospital

5 윤봉식외. 성조숙증환자에서생식샘자극호르몬방출호르몬검사에대한고찰 결론적으로성조숙증을확진하기위해시행하는 GnRH 자극검사시현재 GnRH 투여후 30분간격으로 120 분까지 4회혈액을채취하여혈중 LH를측정한다. 본연구의결과에서진성성조숙증환자에서 GnRH 자극후 30분과 60분에 LH가최고치에도달한환아가 96.9% 이며, 진성성조숙증의진단기준인 LH 5.0 IU/L 이상을만족한것은 30분에이미 100% 이므로 GnRH 자극검사시혈액채취를 30분과 60분 2회만시행하여도진단에유용할것으로사료된다.
