In Severance hospital, clinicians used a lower dose of palonosetron than approved for most pediatric patients, because of the safety concerns. Herein, we compared the efficacy and safety of palonosetron with ondansetron for the prevention of chemotherapy-induced nausea and vomiting in children. Methods : From January 1st 2015 to December 31th 2015, we retrospectively reviewed the electronic medical records of pediatric patients whose chemotherapy were moderately or highly emetogenic and no longer than 4 days. We compared the complete response (CR) rate, no vomiting rate, and no rescue medication rate of the palonosetron and ondansetron groups; and between the 0.02 mg/kg palonosetron and ondansetron groups. 2 임은정 : 소아청소년의항암화학요법에의한오심, 구토예방에사용되는 Palonosetron 과 Ondansetron 의효과비교 CR rate of the palonosetron group (42%) was significantly lower than that of the ondansetron group (57%) (p 0.05); whereas, the 0.02 mg/kg palonosetron group showed 46 cycles and no significant difference in the CR rate (46%) (p=0171) compared with the ondansetron group. The palonosetron group and 0.02 mg/kg palonosetron group had a higher proportion of highly emetogenic chemotherapy, and longer regimen days, as compared to the ondansetron group, with significance (p 0.05). Conclusion : In this study, the CR rate of the palonosetron group was lower than that of the ondansetron group. Treatment with 0.02 mg/kg palonosetron resulted in a high antiemetic effect without complication. This study has limitations, which include its retrospective design and differences in patients baseline characteristics. Further study is needed to address these issues. [Key words] Chemotherapy-induced nausea and vomiting, CINV, Palonosetron, Ondansetron, Children 연구배경및목적항암화학요법에의한오심및구토 (chemotherapyinduced nausea and vomiting, 이하 CINV) 는구토유발중등도위험항암화학요법 (moderately emetogenic chemotherapy, 이하 MEC) 과구토유발고위험항암화학요법 (highly emetogenic chemotherapy, 이하 HEC) 시에흔하게발생하는부작용이며, 특히 multiple-day chemotherapy regimen일경우 CINV의발생률이더높아질수있다. 1)-3) CINV는항암화학요법을받는소아환자및보호자에게삶의질저하를일으키는주요부작용이며, 환자의활동능력을감소시키고영양결핍을야기할수있기때문에, 효과적인항구토제의사용은매우중요하다. 1)-3) 5-HT3 receptor antagonist( 이하 5-HT3 RA) 는 CINV 예방에흔하게사용되는항구토제이나, 2015년 6월전까지국내에서소아의 CINV에허가된 5-HT3 RA는 ondansetron뿐이었다. 하지만 ondansetron은 1세대 5-HT3 RA로반감기가 3~8시간이기때문에하루 2차례이상투여가필요하고, 항암화학요법종료후에도지연형오심과구토를조절하기위해추가적인투여가필요하다는단점이있다. 4),5) 이에반해 palonosetron은 2세대 5-HT3 RA로서, 1 세대 5-HT3 RA과비교했을때수용체에대한강한 친화력과긴반감기 ( 소아에서 21~37시간, 성인에서는 41시간 ) 를가지며, 안전성측면에서도 QT 간격연장등의심혈관계부작용이보고되지않았다는장점이있다. 성인에서는 palonosetron이 ondansetron보다우월하다는여러보고가있었으나, 1),2) 소아에서는 palonosetron이 ondansetron에비해비열등하다는임상시험결과 1) 를바탕으로 2015년도에한국식품의약품안전처 ( 이하 MFDS) 의허가를받았다. 보편적으로소아에서약물을사용하는경우체중당용량을계산하더라도, 성인의최대용량을넘지않는한도내에서사용한다. 6) 그러나소아의 CINV에대한 palonosetron 허가용량은 0.02 mg/kg(max. 1.5 mg) 로, 성인에게허가된 0.25 mg보다고용량이될수있어이에따른안전성을우려할수있다. 따라서세브란스병원에서는 2015년 7월부터 2016년 6월까지 palonosetron의소아최대용량을 0.25 mg으로사용하였고, 이용량에서도 ondansetron과비슷한효과가나타나는지의문이있어확인할필요성이대두되었다. 따라서, 본연구에서는 palonosetron을최대용량 0.25 mg으로제한하여사용한경우와 ondansetron 을 CINV 예방에대한효과와안전성측면에서비교해보고, 추가적으로 0.02 mg/kg 용량을투여한 palonosetron 군과 ondansetron군을비교하여소아 CINV 예방을위한효과적인항구토제사용방안을알

3 JKSHP, VOL.34, NO.2 (2017) 아보고자하였다. 에시행받았던경우는제외하였다. 연구방법 3. 연구방법 1. 연구대상약제본연구에서 palonosetron은항암화학요법첫째날 1회정맥투여되었다. 투여용량은 0.02 mg/kg, 최대 0.25 mg였다. Ondansetron은 MFDS 허가사항및국내보험기준에따라항암화학요법시 5 mg/m 2 를1회정맥투여하고 2 mg 또는 4 mg(bsa 0.6 m 2 : 2 mg, BSA 0.6 m 2 : 4 mg) 를추가로 1회경구투여하였다. 또한, MEC 일경우항암화학요법투여종료후 2일까지, HEC일경우항암화학요법투여종료후 5일까지 2 mg 또는 4 mg를하루 2번경구투여하였다. 7),8) 2. 연구대상 1) 선정기준본연구는 2015년 1월부터 2015년 12 월까지, 세브란스병원에서 CINV 예방을위해 palonosetron 또는 ondansetron을사용한 6개월이상, 만 17 세이하소아를대상으로선정하였다. 다만, MEC 또는 HEC에해당하는항암화학요법이연속적으로 3일이내로투여되는경우만을포함하였으며, 환자연령은항암화학요법시작일을기준으로하였다. MEC, HEC의분류는국내보험기준에따라시행하였다. 9) 2) 제외기준 Ondansetron군에서항암화학요법시에 ondansetron의정맥또는경구투여처방중하나라도누락된경우는본연구에서제외하였다. Palonosetron이나 ondansetron 외 metoclopramide, aprepitant 등다른항구토제를항암화학요법시작전에미리투여받았던경우, 오심 / 구토예방외목적으로 steroid가투여된경우도연구대상에서제외하였다. 장기부전등의문제로오심 / 구토가있었던경우, 항암화학요법시작전 24 시간이내에오심 / 구토증상이있었던경우, 뇌또는복부에방사선요법을항암화학요법 1주일이내 1) 자료수집대상환자의의무기록을바탕으로후향적으로수집하여성별, 연령, 체중, 진단명, 항암화학요법의투여기간, MEC 및 HEC 여부를조사하였다. 또한예방적으로사용된 palonosetron 및 ondansetron의용량, 용법, 투여기간, 구제약물여부도조사하였다. 그리고 palonosetron 또는 ondansetron과연관되었다고언급된부작용에대해, 발현된부작용의종류와처치방법, 진행결과에대해정보를수집하였다. 2) 유효성가. 전체 palonosetron군과 ondansetron군의비교 1차평가지표 (primary endpoint) 로 overall phase 에서의완전반응률 (complete response rate) 을비교분석하였다. 2차평가지표 (secondary endpoint) 로는 overall phase에서구토가없었던비율, 구제약물이없었던비율을비교하였으며, 구제약물은 palonosetron 및 ondansetron을제외한추가적인항구토제로정의하였다. 또한 palonosetron이반감기가상대적으로더길기때문에 acute와 delayed phase 각각에서완전반응률, 구토가없었던비율, 구제약물이없었던비율을비교하였다. 여기서 overall phase는항암화학요법시작부터종료후 3일째까지, acute phase는항암화학요법시작부터종료후 1일째까지, delayed phase는항암화학요법종료후 2일째부터 3일째까지로정의하였으며, 완전반응률은해당기간에서구토와구제약물이모두없었던경우로정의하였다. 1),2) 나. 연령별 palonosetron군과 ondansetron군의비교 Palonosetron이연령별로 pharmacokinetics(pk) 가상이하다는기존연구결과를토대로 10) 총4개의연령군으로나눠 overall phase에서의완전반응률을 ondansetron군과비교하였다. 4개의연령군은생후 6개월 ~ 만 2세미만, 만 2세 ~ 만 6세미만, 만 6세 ~ 만

4 임은정 : 소아청소년의항암화학요법에의한오심, 구토예방에사용되는 Palonosetron 과 Ondansetron 의효과비교 12세미만, 만 12세 ~ 만 17 세이하로설정하였다. 1) 다 mg/kg 용량을투여한 palonosetron군과 ondansetron군비교 0.02 mg/kg 용량을투여한 palonosetron군과 ondansetron군의완전반응률, 구토가없었던비율, 구제약물이없었던비율을각각 overall, acute, delayed phase측면에서비교하였다. 3) 안전성안전성은전자의무기록상 palonosetron 및 ondansetron과연관되어있다고언급된부작용을조사하여, 부작용의종류와처치방법, 진행결과를확인하였다. 4. 통계분석수집한자료중연속형변수에해당하는환자연령, 체중, 항암화학요법투여기간등은 student s t-test 를이용하여평균 ± 표준편차로표기하였고, 범주형변수에해당하는성별, 완전반응률등은 chi-squared test 또는 fisher s exact test를사용하여빈도 (N, %) 로나타내었다. 본연구에서는 p-value가 0.05 미만일경우통계학적으로유의한차이가있는것으로간주 하였으며, 자료분석은 SPSS 23.0 version(ibm Co., Armonk, NY, USA) 을사용하였다. 5. 연구대상자보호본연구는연구대상자의안전및보호에관한사항등을포함한연구계획을연세대학교세브란스병원기관윤리심의위원회 (Institutional Review Board, IRB) 로부터승인받았으며 (IRB number : ), 후향적연구로연구대상자의동의는생략하였다. 연구결과 1. 대상환자의특성 2015년 1월부터 2015년 12 월까지해당선정기준을만족하여, ondansetron 또는 palonosetron을예방적으로사용한경우는 437 cycle(134명 ) 이었고제외기준에따라이중 100 cycle(33명 ) 이제외되었다. 최종적으로 337 cycle(101명 ) 을대상으로연구하였으며, 이중 palonosetron군은 147 cycle(52명 ), ondansetron 군은 190 cycle(49명 ) 이었다 (Fig 1). 5 JKSHP, VOL.34, NO.2 (2017) 량으로 palonosetron을사용하지않은군에서는평균 mg/kg였다. 대상환자군의평균연령은 palonosetron군 6.95세, ondansetron군 6.79세로차이가없었으나 (p=0.770), 평균체중은 palonosetron군이 kg로 ondansetron군 kg보다낮았다 (p=0.028). 성별은남성이 palonosetron군 95 cycle(65%), ondansetron 군 114 cycle(60%) 로두군모두남성이더많았으나두군간유의한차이는없었다 (p=0.081). HEC에해당하는항암화학요법은 palonosetron군이 52 cycle(35%) 로 ondansetron군 47 cycle(25%) 보다많았으며 (p=0.010), 평균항암화학요법일수도 palonosetron 군이 2.33일로 ondansetron군의 1.88일보다약 0.45 일높았다 (p 0.001, Table 1). 2. 효과비교 1차평가지표였던 overall phase에서의완전반응은 palonosetron군이 62 cycle(42%) 로 ondansetron군 108 cycle(57%) 보다적었다 (p=0.008). 구토가없었던경우는 palonosetron군이 113 cycle(77%) 로 ondansetron군 124 cycle(65%) 에비해적었고 (p=0.021), 구제약물이없었던경우도 palonosetron군이 70 cycle(48%) 로 ondansetron군 114 cycle(60%) 보다적었다 (p=0.024). Acute phase에서비교하였을때, 완전반응은 palonosetron군이 72 cycle(49%) 로 ondansetron군 113 cycle(59%) 보다적었다 (p=0.030). 그러나구토가없었던경우는 palonosetron군 116 cycle(79%), Table 1. 6 임은정 : 소아청소년의항암화학요법에의한오심, 구토예방에사용되는 Palonosetron 과 Ondansetron 의효과비교 ondansetron군 151 cycle(79%) 로유의한차이가없었고 (p=0.900), 구제약물이없었던경우도 palonosetron군 82 cycle(56%), ondansetron군 115 cycle(61%) 로유의한차이가없었다 (p=0.381). Delayed phase에서비교하였을때, 완전반응은 palonosetron군 108 cycle(73%), ondansetron군 151 cycle(79%) 로두군간유의한차이는없었다 (P=0.195). 구토가없었던경우에서 palonosetron군 139 cycle(95%), ondansetron군 185 cycle(97%) 로유의한차이가없었고 (p=0.184), 구제약물이없었던경우도 palonosetron군 109 cycle(74%), ondansetron군 152 cycle(80%) 로유의한차이가없었다 (p=0.203, Table 5). 3. palonosetron 군과 ondansetron 군의연령별비교총 4개의연령군으로나누어 overall phase에서 palonosetron과 ondansetron의완전반응률을비교하였다. 생후 6개월 ~ 만 2세미만군은 palonosetron 군이 5 cycle(31%) 로 ondansetron군의 15 cycle (60%) 보다완전반응이적은경향을나타내었으나통계적으로유의하지는않았고 (p=0.072), 만 2세 ~ 만 6세미만군에서는 palonosetron군 40 cycle(54%), ondansetron군 26 cycle(45%) 로두군간통계적으로유의한차이가없었다 (p=0.293). 그러나만 6세 ~ 만 12 세미만군은 palonosetron군이 4 cycle(18%) 로 ondansetron군 50 cycle(69%) 보다완전반응이유의하게적었으며 (p=0.001), 만 12세 ~ 만 17세이하군도 palonosetron군이 13 cycle(37%) 로 ondansetron군이 17 cycle(74%) 보다완전반응이유의하게적었다 (p=0.006). 대상군모두에서연령에따른특이적인경향성을발견할수없었다 (Table 3) mg/kg 용량을투여한 palonosetron 군과 ondansetron 비교 0.02 mg/kg 용량을투여한 palonosetron군은 46 cycle(16명 ) 로전체 palonosetron 147 cycle 중 31% 에해당하였다 (Fig 1) mg/kg 용량을투여한 palonosetron군은모두만 6세미만이었으며, 0.02 mg/kg 용량을투여한 palonosetron군의평균연령은 2세로 ondansetron군의 6세보다적었고 (p 0.001), 0.02 mg/kg 용량을투여한 palonosetron군의평균체중도 12 kg으로 ondansetron군의 27 kg보다적었다 (p 0.001). 8 임은정 : 소아청소년의항암화학요법에의한오심, 구토예방에사용되는 Palonosetron 과 Ondansetron 의효과비교 (59%) 로 ondansetron군 47 cycle(25%) 보다많았고 (p 0.001), 평균항암화학요법일수도 0.02 mg/kg 용량을투여한 palonosetron군이 2.41일로 ondansetron군 1.88일보다약 0.53일많았다 (p 0.001, Table 4). Overall phase에서의완전반응은 0.02 mg/kg 용량을투여한 palonosetron군이 21 cycle(46%) 로 ondansetron군 108 cycle(57%) 과유의한차이는보이지않았다 (p=0.171). Acute phase에서의완전반응도 0.02 mg/kg 용량을투여한 palonosetron군이 22 cycle(48%) 로, ondansetron군 113 cycle(59%) 보다적었으나유의한차이가없었고 (p=0.152), delayed phase에서의완전반응도 0.02 mg/kg 용량을투여한 palonosetron군이 38 cycle(83%) 로 ondansetron군의 151 cycle(79%) 보다적었으나유의한차이가없었다 (p=0.633). 구토가없었던비율은 overall, acute, delayed phase에서모두두군간유의한차이가없었고구제약물이없었던비율은 overall phase를제외한 acute, delayed phase에서는 0.02 mg/kg palonosetron군이유의하게높았다 (Table 5). 5. 안전성 항구토제와관련되었다고언급된부작용은 palonosetron군, ondansetron군에서각각 1건있었으나, 2 건모두경증으로처치후회복되었다. palonosetron 투여후나타난부작용 1건 (0.7%) 은소양증이었고, ondansetron군에서의보고된부작용 1건 (0.5%) 은수면장애였다. 고찰 CINV는환자의삶의질에큰영향을주기때문에, 1),2),11) 효과적인항구토제의사용이중요하다. 소아의 CINV 예방에 0.02 mg/kg 용량을투여한 palonosetron군이 ondansetron군보다비열등하다는결과를바탕으로, 1),2) 본연구에서는 overall phase에서의완전반응률을 1차평가지표로비교분석하였다. 선행연구와는다르게본연구에서는 overall phase 에서의완전반응률과구제약물이없었던비율측면에서모두 palonosetron군이유의하게낮은결과를보였다 (Table 2). 이는첫째로, 오심 / 구토의위험성을증가시키는요인인, 항암화학요법중 HEC 비율과평균항암화학요법투여일수가 palonosetron군이 ondansetron군보다유의하게높았기때문으로해석 Table 5. Ondansetron군에서는 palonosetron이허가되기전이므로, 구제약물로사용가능한 5-HT3 RA는 ondansetron뿐이었으나, palonosetron군에서는구제약물로사용가능한 5-HT3 RA 가 ondansetron과 palonosetron 2종류였기때문이다. 셋째로, 본연구에서사용된 palonosetron의용량이적었기때문으로사료된다. 선행연구에서 0.01 mg/kg과 0.02 mg/kg 용량을투여한 palonosetron 과 ondansetron을각각비교하였을때, 0.02 mg/kg 용량을투여한 palonosetron군만이 ondansetron과비열등하다는결과를만족시켜 0.02 mg/kg로허가를받았다. 본연구에서사용된 palonosetron군의평균용량은허가용량의 70% 인 mg/kg였기때문에, 이로인해완전반응률이낮았을수도있다고사료된다. 선행연구에서는 acute phase보다 delayed phase에서 palonosetron이더좋은효과를보였고이를 palonosetron이 ondansetron보다반감기가더길기때문으로해석하였다. 1) 하지만본연구에서는 acute phase에서의완전반응률은 palonosetron군이 49% 로 ondansetron군 59% 보다유의하게낮았고, delayed phase에서의완전반응률도 palonosetron군이 73% 로 ondansetron군 79% 보다낮은결과를보였으나유의한차이는없었다 (Table 2). 또한, 소아연령이증가할수록 palonosetron의 clearance가증가하는기존 PK연구결과 10) 와만6세미만의그룹에서 6세이상의그룹보다완전반응률이증가했다는연구결과 13) 를바탕으로, 4개의연령군으로나누어 overall phase에서의완전반응률추가적으로비교하였으나, 두군간차이가없었고, 연령에따른특정한경향성또한발견할수없었다 (Table 3) mg/kg 용량을투여한 palonosetron군과 ondansetron군의 overall phase에서의비교에서도마찬가지로 0.02 mg/kg 용량을투여한 palonosetron 군에서항암화학요법중 HEC 비율이더높고항암화학요법투여일수가더길었기때문에 (Table 4), 오심 / 구토의위험성이더높았을것으로사료된다. 1)-3) 그러나, overall phase에서의완전반응률은 0.02 mg/kg 용량을투여한 palonosetron군과 ondansetron군간 에유의한차이가없어 (Table 5), palonosetron군과 ondansetron군을비교하였을때와는다른결과를보였다. Gabor Kovacs의연구에서는 ondansetron 및 palonosetron 관련부작용발생비율이 4% 로보고되었으나, 1) 본연구는 palonosetron 0.7%, ondansetron 0.5% 으로기존연구보다낮았다. 이는 palonosetron 의최대용량을 0.25 mg으로제한하다보니허가용량인 0.02 mg/kg으로사용한환자가 31% 였고, 후향적연구로 ondansetron 및 palonosetron 관련부작용으로확실히명시되지않은경우는제외하였기때문에본연구에서부작용발생비율이적은것으로생각된다. Ondansetron의허가사항에서최소연령을 6개월이상으로제한하였기때문에, 본연구에서도대상환자의최소연령을 6개월이상으로한정하여 1개월 ~6개월소아에대한평가는이뤄지지않았다. 또한 palonosetron의반감기를고려하여, 3일이내의항암화학요법으로한정하였고고용량의 steroid를투여하는경우를제외하였기때문에 steroid로인한변수를줄일수있었으나, 급성백혈병및림프종환자가제외되어소아암전체를대변하지못했다는한계가있다. 그리고후향적으로진행되어오심 / 구토의정확한횟수및심각도파악이어려웠고, 각환자군의특성이동일하지않아정확한비교가어려웠다는한계가있다. 그러나, 소아암환자를대상으로한국내최초 palonosetron 연구이며, 본연구를통해 palonosetron의적절한용량에대해논의할기회가되었다는점에의의가있다. 본연구결과에서, 최대 0.25 mg으로 paolnosetron 을사용한군은 ondansetron군보다 overall phase의완전반응률이유의하게낮았으나 0.02 mg/kg 용량을투여한 palonosetron에서는 ondansetron과유의한차이를보이지않아, palonosetron의용량을증량했을때항구토효과가증가할것으로예측해볼수있었다. 이에세브란스병원에서는 2015년 7월경부터는최대용량을 0.25 mg에서 0.75 mg으로변경하여사용중으로, 이에대한추가연구를통해 palonosetron의적절한용량검토가이루어지기를기대한다. 13) 결론

10 임은정 : 소아청소년의항암화학요법에의한오심, 구토예방에사용되는 Palonosetron 과 Ondansetron 의효과비교 Overall phase에서의완전반응률을비교한결과 palonosetron군이 42% 로 ondansetron군의 57% 에비해유의하게낮았다. 그러나 0.02 mg/kg 용량을투여한 palonosetron군에서는 overall phase의완전반응률이 46% 로 ondansetron보다는낮은경향은있었으나유의한차이는보이지않았고역시관련부작용은보고되지않았다. 따라서허가사항에맞는 palonosetron 용량을사용한다면항구토효과가증가할것으로사료되며부작용우려는높지않다고사료된다. 추후본연구의한계점을보완하여대규모환자를대상으로전향적연구를시행하는등보다효과적인항구토요법을찾는노력이지속되어야할것이다. 참고문헌 1) Kovacs G., Wachtel A.E., Basharova E.V., Spinelli T., Nicolas P., Kabickova E.
