최영우 외: 압박골절을 동반한 척추체의 갈색종 동통과 압통을 볼 수 있다. 신경학적인 증상은 약하고, 다양한 정도로 나타난다. 삼분의 일에 해당하는 환자가 무증상이며 고 칼슘 혈증이 유일한 우발적인 소견이다.

원발성 부갑상선 기능 항진증의 심한 형태의 하나로 발견되 는 골 용해성 병변이지만 근래에는 특히 말기 신부전에 의한 투석을 받는 환자에서 이차적인 부갑상선 기능 항진증에 의한 병변으로도 보고되고 있다(2).

원발성 부갑상선 기능 항진증의 생화학적 검사 소견은 고칼슘 혈증과 더불어 높거나 정상 수준의 PTH이고, 속발성 부갑상선 기능 항진증의 경우 는 저칼슘 혈증과, 높은 수준의 PTH이다.

2 최영우 외: 압박골절을 동반한 척추체의 갈색종 동통과 압통을 볼 수 있다. 신경학적인 증상은 약하고, 다양한 정도로 나타난다. 삼분의 일에 해당하는 환자가 무증상이며 고 칼슘 혈증이 유일한 우발적인 소견이다. 과거 갈색종은 부갑상선 기능 항진증의 심한 형태로 발견되 었으나 근래에 들어서는 조기에 부갑상선 기능 항진증의 진단 과 치료가 가능하여 매우 드물게 발견된다(1). 원발성 부갑상선 기능 항진증의 심한 형태의 하나로 발견되 는 골 용해성 병변이지만 근래에는 특히 말기 신부전에 의한 투석을 받는 환자에서 이차적인 부갑상선 기능 항진증에 의한 병변으로도 보고되고 있다(2). 부갑상선 기능 항진증이라 하고, 이 같은 경우는 신장 이식환 자에서 가장 흔하고 만성 신부전 환자에서도 올 수 있다. 이소 성 부갑상선 기능 항진증은 폐암과 같이 이소성으로 부갑상선 호르몬 분비하는 종양에 의한 경우이다. 원발성 부갑상선 기능 항진증의 생화학적 검사 소견은 고칼슘 혈증과 더불어 높거나 정상 수준의 PTH이고, 속발성 부갑상선 기능 항진증의 경우 는 저칼슘 혈증과, 높은 수준의 PTH이다. 부갑상선 기능 항진증의 임상적 증상은 대부분 고칼슘 혈증 에 의한 것과 골격계 이상, 신장 이상 등이다. 증상은 오심과 구토, 무력감, 변비, 근육과 인대의 긴장저하를 포함하며, 뼈의 A B C D Fig. 1. 조직학적으로는 골 소주의 손실과 다핵성 파골 세포(거대세 포)의 축적이 보이고 헤모시데린을 포함하는 섬유 조직를 동 반하는 소견으로 보인다(4).

3 대한영상의학회지 2006;54:33-37 갈색종은 방사선학적으로 비특이적 소견을 보인다(6). 단순 촬영 상 좋은 경계를 보이는 팽창성 골 용해성 병변으로 편심 성 또는 피질부에 주로 위치하고, 단일 혹은 복수의 병변으로 나타난다. CT상 골용해성 연조직 종괴의 밀도를 보인다. 조직학적으로는 골 소주의 손실과 다핵성 파골 세포(거대세 포)의 축적이 보이고 헤모시데린을 포함하는 섬유 조직를 동 반하는 소견으로 보인다(4). 거대세포종과는 조직학적으로 뚜 렷한 감별점이 없어서, 이 질환의 진단에는 부갑상선 기능 항 진증의 임상적, 방사선학적, 그리고 검사실소견이 없이는 불가 능하다. 척추에 발생한 갈색종의 대부분의 경우가 요통과 점진적인 Fig. 4. The main sites of involvement are the mandible, sternum, ribs, pelvis and femur. Involvement of the spine is less common. We report here a case of brown tumor of the spine that developed in a patient with right parathyroid adenoma.
