Although this is not a rare disease, the etiology and effective treatment are not well established yet. Various drugs have been used in attempting to manage BMS, but there is insufficient evidence to show the effect of them. The goal of this article is to review about diagnosis, treatment, and updates current knowledge of BMS along with our experiences. Although randomized controlled studies are required to establish the treatment for patients suffering from this chronic and painful syndrome, the authors hope that this document will encourage otolaryngologist to approach to this challenging disease without fear and contribute to a better therapeutic management. Korean J Otorhinolaryngol-Head Neck Surg 2013;56:550-6 Key WordsZZBurning mouth syndrome (BMS) ㆍ Neuropathic pain ㆍ Pain management ㆍ Stomatodynia. 서론 구강 작열감 증후군 은 특별한 이유 없이 만성적으로 구강점막에 타는듯한 또는 따끔따끔 찌르는 듯한 통증을 호소하는 상태이다 일반적으로 뚜렷한 점막병변이 없으며 주로 혀 특히 혀의 끝부분과 옆부분 입술 경구개 또는 연구개를 침범하는 경우에 진단할 수 있다 구강의 작열감 이외에 건조감 맛의 변화 비특이적인 치아통증 등 다양한 구강 내 불편감을 호소할 수 있다 이는 이비인후과 의사로서 임상에서 드물지 않게 만나는 질환이고 최근 우리사회가 노령화되면서 유병률이 증가하고 있으나 명확한 기전이나 치료가 정립되어 있지 않아 대부분의 이비인후과 의사들에게 본 질환으로 분류할 수 있는 환자를 진료함에 있어서 정신적 부담을 안겨주는 질환이다 그렇지만 최근 연구들에서 본 질환 군을 일관되게 정의하고 병인기전 및 치료를 정립하고자 하는 노력이 활발하게 이루어지고 있으며 몇몇 약제에 대하여서는 임상시험도 진행 중에 있 다 이에 저자는 그 동안의 문헌 고찰과 임상 경험을 바탕으로 구강 작열감 증후군의 진단과 치료에 대해서 소개하여 추후 임상적으로 구강 작열감 증후군의 치료계획을 수립하는 데 도움이 되고자 한다 본론 정의구강 작열감 증후군은 임상적 구강이상 소견이나 전신적 질환을 동반하지 않는 상황에서 구강점막의 타는듯한 작열감을 호소하는 질환이다 다양한 그룹에서 구강 작열감 증후군을 일관적으로 정의하고자 하였다 - 에서는 가 제시한 바와 같이 구강점막에 임상적인 병변이 없고 혈액검사 상 이상이 없는 구강 작열감으로 정의하였으며 - 에서는 다음과 같은 가지의 진단기준을 제시하였다 매일 하루종일 지속되는 구강의 작열감 구강점막은 정 550 Copyright 2013 Korean Society of Otorhinolaryngology-Head and Neck Surgery

2 Burning Mouth Syndrome Chang JW, et al. 상소견 국소 또는 전신 질환을 배제할 것 또한 구강 작열감 증후군은 와 같이 여러가지로 표현되기도 한다 임상양상구강 작열감 증후군은 구강점막의 임상소견 및 혈액검사 소견은 정상이나 구강의 작열감 또는 이상감각을 호소하는 질환이다 혀의 불편감을 가장 흔하게 호소하지만 입안의 여러 곳이 영향 받을 수 있다 통증은 대부분 양측성이며 말초신경의 분포범위와는 상관관계가 없다 대부분 중등고도의 통증이 하루종일 지속되지만 약간 호전되거나 악화되기도 하며 아침에는 심하지 않다가 저녁이 되면 심해지는 경우가 많다 그렇지만 이 통증으로 인하여 수면에 방해를 받는 경우는 흔하지 않다 다른 관련 증상으로 맛감각 변화 - 냄새감각의 이상 구강건조 타액성분의 변화 등이 있다 자극이 덜한 음식이나 비탄산음료를 마시 면 증상이 호전되며 맛이 강한 음식이나 뜨거운 음식을 먹으면 심해지는 경향이 있다 원발성 구강 작열감 증후군의 진단에 있어서 환자가 호소하는 증상이 가장 중요하며 속발성 구강 작열감 증후군을 유발할 수 있는 국소 전신 질환의 가능성 갑상선 기능저하증 당뇨 구강 칸디다 타액분비의 감소 약물 부작용 영양결핍 등 을 배제한 뒤에 진단이 가능하다 역학구강 작열감 증후군의 유병률은 약 로 보고되며 대 미만은 거의 드문 것으로 알려져 있다 증상은 고령일수록 여성일수록 심한 경향이 있으며 폐경과 관련이 있을 것으로 생각된다 분류구강 작열감 증후군에 대한 널리 인정되고 유용한 분류는 원발성 또는 속발성 구강 작열감 증후 Table 1. 3 Korean J Otorhinolaryngol-Head Neck Surg 2013;56:550-6 군으로 나누는 것이다 에서 제시한 것처럼 속발성 구강 작열감 증후군은 다양한 구강점막상태 점막손상 궤양 구강 칸디다증 등 와 전신상태를 가진 환자에서 생기는 구강 작열 통증을 지칭하며 관련된 다양한 선행요인을 확인할 수 있고 이를 적절히 치료하면 증상 또한 호전될 수 있다 발병기전원발성 구강 작열감 증후군의 발병기전은 아직 완전히 규명되지 않았으나 최근 연구들에 의하면 신경병적 상태 - 로 인한 것으로 추정되고 있다 은 현재까지 이루어진 연구들에서 구강 작열감 증후군과 관련된 감각신경의 병적인 변화를 정리한 것이다 속발성 구강 작열감 증후군은 에서 언급한 것처럼 국소 전신 질환과 관련되어 있다 특히 진균과 구강 작열감 사이의 관련성 미각이상의 발병률 및 치료와 관련성 아연 및 비타민 의 결핍이 구강 작열감을 유발할 수 있다는 연구도 발표되어 있다 그렇지만 가능한 모든 원인을 하나하나 배제하는 방식으로 원발성 구강 작 열감 증후군을 진단하는 것은 효율적이지 않은 것이며 진단을 위한 객관적인 을 정립하는 것은 쉽지 않다 한 연구에서는 구강의 작열감을 호소하는 모든 환자에서 전혈구검사 를 시행하였으나 실제로 혈액검사에서 어떠한 이상을 보인다고 하여 이것이 구강 작열감과 관련되어 있을 가능성을 알려주는 것은 확립되어 있지 않다 심리적요인구강 작열 증후군 환자는 우울증 불안증 신체화 등의 병적인 심리적 상태를 지닌다는 연구결과가 있다 또한 구강 작열 증후군 환자에서는 일상생활의 스트레스와 직접적으로 관련되지 않아도 정신적으로 스트레스에 취약하다는 보고가 있다 그러나 구강 작열감의 강도와 정신적 스트레스와의 상관관계는 명확하지 않으며 심리적 요인이 구강 작열 증후군에만 연관되어 있는 것인지 아니면 만성통증 증후군에서도 관련되어 있는지는 정확하게 알려져 있지 않다 그렇지만 치료의 결과에 심리적 요인이 중요한 영향을 미치기 때문에 구강 작열 증 A B Fig. 1. Mean reduction in VAS with placebo 0.6 Odds ratio for possibility of improvement or resolution: 7 times higher for α-lipoic acid 5.7 times higher for gabapentin 13.2 times higher for combination α-lipoic acid and gabapentin Carbone et al., ) López-Jornet et al., ) Cavalcanti and da Silveira, ) Femiano et al., ) Silvestre et al., ) Marino et al., ) Petruzzi et al, ) Heckmann et al., ) Gremeau-Richard et al., ) López-D alessandro and Escovich, ) 후군을 치료할 때는 심리적 요인을 고려하는 것이 효과적인 치료의 결과에 도움이 된다고 할 수 있다 그러므로 구강 작열 증후군의 치료계획을 수립함에 있어서 심리상담을 고려하는 것이 좋겠다 치료원발성 구강 작열 증후군의 치료에 대한 광범위한 무작위 연구는 아직 없으며 와 비교했을 때 효과가 명확하지는 않으나 현재까지 보고된 문헌고찰에 따르면 이 시행되고 임상에서 응용되고 있는 치료를 정리해보면 와 같다 또한 병인기전이 명확히 밝혀져 있지 않고 치료원칙이 아직 정립되어 있지 않은 질환이기 때문에 본 질환의 치료는 증상에 대한 완화요법이 주된 목표일 것이다 그러므로 치료 은 가장 부작용이 적은 치료에서 시작하여 단계적으로 올라가면서 병합요법을 고려하는 것이 타당할 것이다 Alpha-lipoic acid(ala) 는 세포내의 글루타티온 - 을 증가시켜 활성산소를 제거함으로써 항산화작용을 나타내는 물질로 신경손상을 회복하는 데 도움을 주는 것으로 알려져 있다 또한 비타민 와 같은 다른 항산화물질을 활성화 시켜 신경성장인자 의 생성을 증가시킨다 이러한 신경보호작용으로 인하여 수년 동안 당뇨병성 신경병에 사용되었으며 약 명 이상의 구강 작열감 증후군 환자에서 시행된 에서 증상의 완화에 효과가 있음이 제시되었으며 하루 번 을 투여한 이중맹검 임상 시험에서도 증상의 호전을 보였다 또한 심리치료와 를 병합한 경우 단일요법을 시행한 경우에 비하여 의미있는 부가효과가 있음이 보고되었으며 또 다른 연구에서 와 를 병합하였을 경우 좋은 결과가 보고된 바 있다 Capsaicin 은 고추의 매운 성분으로 인체에서 - 인 에 작용하여 자극에 대한 신경 반응을 억제하고 수용체를 감소시켜 에 대한 반응을 감소시킴으로써 구강 작열감 증후군의 증상을 호전시키는 것으로 알려져 있다 을 전신적으로 투여할 경우에 일부 증상완화에 효과가 보고되긴 하였으나 심한 을 유발하였으며 구강에 국소적으로 투여할 경우 이러한 부작용은 거의 보고되지 553

6 Burning Mouth Syndrome Chang JW, et al. 억제적 효과를 증가시키는 물질로 와 및 다양한 신경병적 통증에 사용되는 약물로 구강 작열감 증후군에서는 의 증상호전이 관찰되었으며 또 다른 연구에서는 과 를 병합하였을 때 각각의 단일요법에서 증상의 호전이 관찰된 것에 비하여 약 의 증상 호전이 관찰되었다 Steroid gargle 의 강력한 항염증작용은 잘 알려져 있으며 광범위한 염증질환에 사용되어 왔다 구강 작열감 증후군이 구강 내 신경말단의 염증성 반응에 의한 과민감각으로 생각되므로 스테로이드 가글 의 효과가 연구되었으며 약 에서 증상의 완화가 보고되었다 국내에서도 등 이 보고한 연구에 따르면 스테로이드 단독으로 치료한 경우 다른 치료와 스테로이드 가글을 병용한 경우 의 증상호전이 관찰되었다 위에서 언급한 약물치료 외에 저자는 점막의 궤양이 동반된 속발성 구강 작열감 증후군일 경우 보조적인 병합요법으로 의 을 억제하며 의 - 와 를 감소시켜 항염작용을 나타내며 베쳇병의 치료제로 사용되고 있는 점막방어인자 증강제인 - 등을 사용하고 있으며 이 외에도 약물요법에 반응이 크지 않은 환자에서 작열감을 호소하는 부위에 - 을 시행 뒤에 작열감 증상의 완화를 경험하였다 또한 구강 작열감을 호소하는 환자 중 절반 이상 환자가 속발성 구강 작열이었고 최근 등 이 보고한 연구결과에 따르면 철분과 진균의 부족이 비교적 흔하기 때문에 충분한 병력 청취 이학적 및 혈액학적 검사를 통해 이러한 소견이 발견될 경우 적절한 보충요법이 필요할 것으로 생각된다 추척관찰대부분 문헌의 구강 작열감 증후군 환자에서 특히 일반적인 약물치료에 반응이 없는 경우 주에 한 번 외래에서 이학적 검사 통증강도 심리적 상태에 대한 문진을 포함한 증상의 관찰이 필요하다고 보고하고 있으며 저자의 경우에도 치료에 반응이 있는 경우 개월에 한 번 반응이 비교적 적은 경우 주에 한 번 외래에서 추적관찰을 진행하도록 하였다 결론 최근까지 이루어진 연구들에서 원발성 구강 작열감 증후군 을 신경병적 질환으로 추정하고 있으며 이에 대한 치료 또한 신경물질의 전달에 관여하는 약물치료로 시도되고 있다 그렇 지만 그럼에도 불구하여 신경병적인 질환에 효과가 있는 물질 이 모두 구강 작열감 증후군에 효과가 있는 것은 아니다 대부 분의 환자에서 단일치료로 증상이 완전히 호전되는 경우는 없 으며 치료가 매우 어려운 질환이라고 할 수 있다 현재 구강 작열감 증후군에 대한 가장 적절한 치료는 구강 작열감에 대한 완화 요법이라고 할 수 있다 적극적인 완화 요 법과 함께 병인과 관련되어 있다고 생각되는 일상적인 스트레 스 뿐만 아니라 만성적인 질병의 경과로 인한 심리적 문제에 대해서도 임상에서 관심을 가지고 접근할 때 구강 작열감 증 후군 환자가 증상의 완화 뿐만 아니라 삶의 질의 향상에 다다 를 수 있다 여러 기관에서 질병의 병인기전 및 치료에 대하여 많은 시 도가 이루어지고 있으나 원발성 구강 작열감 증후군은 비교 적 흔하지 않기 때문에 향후 연구에서는 표준적 진단기준에 맞춘 다기관 협력 연구가 필요하며 이중맹검 무작위 임상시험 을 통해 적절한 임상진료지침을 마련할 수 있을 것으로 기대 한다 REFERENCES 1) Crow HC, Gonzalez Y.
