Methods: Between January 2009 and December 2009, 161 patients in the ICU (Intensive Care Unit) and who were provided PN (Parenteral Nutrition) were included. SAS (Statistical Analysis System) was used for the analysis. Results: In this study, 161 patients were administered PN for 7.3±6.7 days and 8.7% of the patients were provided support by NST. The number of PN prescriptions was 235 and among them, only 7.2% were conducted according to the recommendations of the NST. Both of the groups (the NST and non-nst groups) had a marked malnutrition level of 1 when admitted to the ICU. During the hospitalization, the malnutrition level was increased (NST: P=0.03, non-nst: P<0.01). The NST group was provided macronutrients more often than the non-nst group under the guidelines. Supplement of micronutrients (vitamins) was significantly different between the two groups (82.4% vs. 32.6%, respectively). There was no statistical difference in the administration of amino acid by dialysis between the dialysis and non-dialysis patients. Also, hepatic dysfunction occurred in the NST group more often than that in the non-nst group (35.7% vs. 12.3%, respectively, P=0.03). Sepsis was associated with hepatic dysfunction and among the 5 patients with hepatic dysfunction in the NST group, 3 patients were sepsis. Conclusion: Regardless of NST consultation, most patients were provided PN under the guideline, but the administration of amino acids to the dialysis patients and the administration of micronutrients were inappropriate in the non-nst group. Therefore, it is necessary to make efforts for providing proper nutritional support to patients through more systematic NST activities and advertisement. (KJPEN 2010;3(1):54-60) Key Words: Critically ill patients, Parenteral nutrition, Nutrition support team 중심단어 : 중환자, 정맥영양, 영양집중지원팀 1. 연구배경 서 입원환자에서영양의중요성은간과할수없으며그관심은더욱커지고있다. 최근에는단순히열량공급을통한영양지원 (nutrition support) 뿐아니라영양치료 (nutrition therapy) 를목적으로 stress에대한 metabolic response, oxidative cellular injury를줄이고면역반응을조절하는것에중점을두고있다. 1 중환자의경우입원전부터식욕감퇴, 우울, 불안, 질병등으로음식섭취량이감소되어있고, 진단및치료과정에서음식섭취가제한된다. 또한설사, 구토, 배액, CRRT (Continuous Renal Replacement Therapy) 등으로인해비정상적인영양소손실이발생하며대사율증가와전해질이상, 감염및수술후외상으로인해에너지, 단백질, 미량원소의요구량이증가되어있다. 이러한원인으로중환자에서영양불량은매우흔히나타나며이로인해감염, 호흡부전, 상처치유지연등이발생하여사망률, 유병률, 재원기 론 54

2 도윤정외 : 중환자실재원환자의정맥영양지원과영양집중지원팀활동에관한비교분석연구 55 간이증가되는것으로알려져있다. 2 따라서중환자에서영양불량을예방하기위해정상적인식이가불가능한경우경장또는정맥을통해요구량만큼의충분한영양지원이필요하며, 이를위해국내여러병원에서는 NST (Nutrition Support Team, 영양집중지원팀 ) 를구성하여입원환자의영양지원을위한노력을하고있다. 중환자의치료영역에서영양지원이차지하는중요성에대한의료진의인식이부족하고, 3 초기영양상태평가및요구량산정이어려워중환자에대한적절한영양지원이쉽지않다. 중환자의경우체내의수분상태변화로인체계측지표 (triceps skinfold, arm-circumference) 의정확도가떨어지고, 내장단백질 (albumin, prealbumin) 역시스트레스, 수분변화등으로인해정확도와민감도에영향을받으므로영양상태를평가하는데제한이있다. 4 중환자의에너지요구량산정시간접열량측정계가추천되는데, 4 가격이고가이기때문에국내에서는실제임상에서사용하는경우가드물다. 일반적으로 Harrison-Benedict 공식을통한에너지요구량산정이가장널리사용되지만중환자특성상키, 체중의측정이어렵고체중변화가심해기준체중을정하는것이어렵다. 또한중환자는기저질환이매우다양하여앞에서언급한이유와더불어중환자의적절한영양지원에많은어려움이따른다. 2009년 ASPEN (American Society of Parenteral and Enteral Nutrition) 과 SCCM (Society of Critical Care Medicine) 에서중환자의영양지원에대한지침을발표한것을비롯하여여러학회및연구에서근거중심지침을제시하고있어 1,2,5 실제임상에서이를활용해볼수있다. 중환자의경우 PN (Parenteral Nutrition) 을통한영양지원이많은부분을차지하는데이와관련한합병증이발생할수있다. 대표적으로 PN 투여와관련된합병증은카테터관련합병증 ( 카테터관련감염등 ) 과대사적합병증 ( 고혈당, 간기능장애등 ) 으로나눌수있다. 6 대부분의합병증들은예방가능하고현대의학의발전으로비교적쉽게치료될수있으므로 PN 투여시주의깊은관찰및관리가요구된다. 2. 연구목적본연구에서는경희대학교동서신의학병원내과계, 외과계중환자실입원환자의영양지원현황을조사하고중환자영양지원에대한지침 (Table 1) 과비교하고자하였다. 또한본원의 NST로영양지원에대해의뢰한환자군과그렇지않은군의영양지원을비교분석하여보다효율적인 NST 활동과적절한영양지원을위한방법을모색해보고자한다. Nutrition support practice: A clinical guide. 2nd ed 대상환자 대상및방법 2009년 1월부터 12월까지경희대학교동서신의학병원의내과계, 외과계중환자실에 3일이상입원한환자중 PN을공급받은환자를대상으로하였으며, 이중키와체중을전자의무기록상으로확인할수없는환자는제외하였다. 2. 자료수집및방법 대상환자의전자의무기록을바탕으로후향적으로조사하였으며그내용은다음과같다. 1) 환자기본정보 나이, 성별, 키, 체중, 이상체중, 체질량지수 (Body Mass Index), 중환자실재원기간, 중환자실입실시 APACHE II 점수 (Acute Physiology And Chronic Health Evaluation score), 진료과, 중환자실입실및퇴실사유, 투석여부, 인공호흡기적용여부, 중환자실입실과퇴실시의영양불량점수등을조사하였다. 7 2) PN 투여관련정보 투여한 PN 종류, 투여기간, IV multivitamin 투여여부, NST로영양자문의뢰여부등을조사하였다. 3) PN 투여와관련된합병증 고혈당여부, 간기능장애여부, PN 투여이후새로운

(1) 고혈당 : PN 투여초기혈당이 150 mg/dl 이상으로 3일이상유지된경우를조사하였다. 8 (2) 간기능장애 : 간기능수치 (AST, ALT, ALP) 가정상범위의 3배이상증가한경우, total bilirubin >5 mg/dl, direct bilirubin >2 mg/dl인경우를조사하였다. 9 (3) 감염 : 미국질병통제예방센터 (CDC) 지침의정의를기준으로 PN 투여기간중미생물학적으로입증된경우, PN 투여이전에있었던감염의경우는제외하였다. 3. 결과평가및분석방법대상환자의체중을기준으로 PN 투여내역을조사하였으며실제체중이이상체중 a 의 120% 이상인환자의경우보정체중 b 을사용하였다. 중환자실재원기간동안동일한환자가다른조성의 PN을투여받은경우를고려하기위해 NST 의뢰후회신반영률과 PN 투여내역을환자별과 PN 처방별로각각나누어분석하였다. 그리고 NST 의뢰를통해 PN을투여한처방과그렇지않은처방의 PN 조성차이를비교하기위해 NST군과 non-nst군으로나누어분석하였고, NST로의뢰하였으나 PN 투여에대한회신을반영하지않은처방은 non-nst군으로분류하였다. 또한중환자의 PN 을통한영양지원에대한지침과실제투여내역을비교하기위하여 PN 투여환자중중환자실재원기간동안전부금식인환자 (all-npo c 군 ) 를대상으로분석하였다. 1,2,5,10 통계프로그램은 SAS (Statistical Analysis System) 를이용하였고두군간의차이를비교하기위해 T-test, chi-square test 또는 fisher s exact test를하였으며, 통계적유의수준은 0.05 이하로하였다. 결과연구기간동안의대상환자는총 161명이었으며환자의기본정보는 Table 2와같다. 평균나이는 62.5세로비교적고령이었으며, 남녀비율은 52.8% vs. 47.2%, 입원병동은 56.5% vs. 43.5% ( 내과계중환자실 vs. 외과계중환자실 ) 로비슷하였다. 평균 BMI는 22.5±3.6 (kg/m 2 ) 으로정상범위였고, 11 평균 APACHE II 점수는 9.3±9.4점이었다. 중환자실재원기간은평균 8.6±8.6일이었고, 최소 3일에서최대 59일까지환자에따라차이가있었다. 중환자실입실사유는신경계질환, 심혈관계질환순으로 (36.6%, 19.9%), 신경외과와 Table 2. Malnutrition score (20 points): normal (0 3 point), malnutrition stage 1 (4 8 point), malnutrition stage 2 (9 20 point). 심장혈관내과환자 (37.3%, 20.5%) 가가장많았다. 중환자실재원기간동안 PN을투여받은환자는총 161 명이었으며, 평균 7.3±6.7일동안투여받았다 (Table 3). 이중 NST로의뢰된경우는 24명 (14.9%), 그렇지않은경우는 137명 (85.1%) 였고, 실제 NST의영양자문대로영양지원을실시한환자는 14명으로전체환자의 8.7% 였다. PN 처방별로는총 161명의환자에게 235건이처방되었고 NST로의뢰된처방은 30건 (12.8%) 이었다. 이중 NST의영양자문대로처방한경우는 17건으로전체처방의 7.2% 가 NST의권고에따른것으로조사되었다. 중환자실퇴실사유로는 NST군과 non-nst군모두병동전실 (71.4%, 90.5%) 이가장많았고, 사망률은 NST군이 21.4% 로 non-nst군 (7.5%) 에비해높게나타났다 (Table 4). 그러나사망을비롯한중환자실퇴실사유모두두군간에통계적인유의성은없었다 ( 사망 P=0.11; 병동전실 P=0.05; 퇴원 P=0.31). PN 관련합병증은 NST군이고혈당-간기능장애-감염순 (78.6%-35.7%-33.3%) 으로많이발생하였고, non-nst군은고혈당-감염-간기능장애순 (52.1%-20.9%-12.3%) 이었다. 두군모두고혈당이가장많았고 (78.6% vs. 52.1%, P=0.10), 간기능장애는 NST군 (35.7%) 이 non-nst군 (12.3%) 보다더많이나타났다 (P=0.03). 영양불량상태는중환자실입실시두군모두영양불량 Table 5. NST군은환자체중당투여된열량이 25.9±10.7 (kcal/kg) 로 non-nst군 (20.6±7.2) 보다많았으나, 두군간의차이없었다 (P=0.06). 미량영양소중포도당 (g/ day) 과아미노산 (g/kg) 은 NST군이 non-nst보다많이투여되었고통계적으로도유의한차이를보였다 (197.1±72.9 vs ±40.9, P=0.01; 1.0±0.5 vs. 0.7±0.2, P=0.01). 지방 (kcal/ day) 도 NST군이더많이투여되었으나 (348.5±183.3 vs ±159.8) 통계적인유의성은없었다 (P=0.71). 또한 NPC (non-protein calorie) 중차지하는포도당과지방의열량 (%) 도두군간의유의한차이는없었다 (P=0.07, P=0.07). 미량영양소중비타민공급률은 NST군이 82.4%, non-nst군이 32.6% 로통계적으로유의한차이를나타내었다 (P<0.01). 중환자실재원기간내내 NPO인환자의 PN 투여내역을 PN 처방별로 NST군 (n=3) 과 non-nst군 (n=64) 으로나누어비교하였다 (Table 6). 하루동안공급된총열량과체중당투여된총열량모두 NST군 (1,768.0±231.3, 30.2±1.7) 이 non- NST군 (1,123.9±312.1, 22.0±7.8) 보다많았다 (P=0.03, P<0.01). 미량영양소중포도당의공급 (g/day) 은 NST군 (256.7±60.3 vs ±49.0) 이더많았으나큰차이는없었다 (P=0.09). 아미노산 (g/kg) 은 NST군이 1.2±0.1로공급되어 non-nst군

(0.8±0.2) 에비해유의한차이를보였고 (P=0.03), 지방 (kcal/ day) 공급도 NST군이더많았다 (500.0±0.0 vs ±17.3, P<0.01). NPC 중포도당과지방이차지하는열량은두군간에차이가없었고 (P=0.63, P=0.63), 미량영양소인비타민의공급은 NST군이더많았다 (P=0.03). 투석을받은환자중 NST의회신대로영양지원을시행한환자는없었으며투석여부에따른아미노산공급내역은다음과같다 (Table 7). 전체 PN 처방별아미노산공급량 (g/kg) 이투석받은환자와그렇지않은환자간에차이가없고 (0.7±0.2 vs. 0.7±0.3; P= 0.67), 재원기간동안 NPO인환자들역시통계적인차이가없는점 (0.7±0.3 vs. 0.8±0.3; P= 0.17) 으로미루어볼때투석여부와관계없이아미노산을투여한것으로판단된다. 고찰본원에서는 2006년 11월부터 NST를구성하여입원환자의영양상태평가및영양지원을위해활동하고있다. 보다적극적인영양지원활동을위해입원환자의영양상태를검색하며, 상업용중심정맥 PN을 3일이상투여시 NST로영양자문을의뢰하도록진료화면에알림창을띄우는방법을통해시스템적으로 NST 의뢰율을높이려는노력을하고있다. 또한일주일에한번씩중환자실회진을통해 NST 의뢰를권고하고있다. 그러나이러한노력에도불구하고본연구에서중환자실환자의 NST 의뢰율이 14.9% 로매우낮게조사되었는데, 이는키와체중을전자의무기록에서확인가능한환자만을대상으로조사하였기에실제보다적었던것으로생각한다. 또한 NST 회신반영률은 58.3% (14/24명) 로조사되었는데, 이는 PN에대한회신반영만을포함한것으로다른경로의영양지원 (Enteral Nutrition; EN, 식이등 ) 에대한반영률이제외되어실제전체회신율에비해낮게나타난것으로판단할수있다. NST에서는주된영양공급원이 PN인환자의경우영양요구량에가장근접하게투여될수있도록대부분원내조제 PN을투여하도록권고하고있다. 다른경로의영양지원여부와별개로원내조제 PN을투여환환자모두 NST 회신을반영한점을미루어보아, 주로 PN을통해영양공급이이루어지는환자의 NST 회신반영률은본연구의결과보다높을것으로생각한다. 외국의연구사례에서입원환자의 30 40% 가재원중요구량만큼의충분한영양지원을받지못하는것으로보고되었고, 국내의연구는아직드문실정이지만문성신등의연구에따르면, 15 전체중환자실재원환자의 81.3% 에서영양공급률이 80% 미만으로나타나영양불량위험률이높은것으로보고되었다. 또한이등의연구에서중환자의 87% 에서영양불량이발생하였고, 16 그정도가심각한환자가그렇지않은환자에비해최초병원감염발생이빠르고발생횟수또한많다고보고된것을미루어볼때우리나라도외국의경우와비슷하다고예상해볼수있다. 본연구에서도 NST군, non-nst군모두중환자실재원기간동안영양불량상태가악화되었음을알수있었다. 또한중환자실입실과퇴실당시모두 NST군이영양불량점수가더높았는데, 이는임상상태에따른차이로예상해볼수있다. APACHE II 점수에따른예상사망률 17 은 NST군이 16 25%, non- NST 군이 5 8% 로본연구에서조사된실제사망률 (21.4% vs. 7.5%) 과비슷하였다. 그러나사망률과 APACHE II 점수모두두군간의통계적인유의한차이가없었다 (P=0.11, P=

6 도윤정외 : 중환자실재원환자의정맥영양지원과영양집중지원팀활동에관한비교분석연구 ). 본연구에서두군간에대상환자수의차이가크고 (14명 vs. 147명 ), 연구기간을중환자실재원기간으로제한한점을고려할때, 이후추가적인연구를통한 NST 활동평가가요구된다. PN 투여시약 50% 의환자에서고혈당이발생하고내과계중환자실과외과계중환자실환자들의유병률및사망률에영향을미친다고보고된바있다. 18,19 또한고혈당으로인해감염발생률이증가되고인공호흡기사용기간이연장되는등치료결과에영향을미치므로 20 PN 투여시적절한혈당관리가요구되는데, 현재까지혈당조절기준에대해서는많은논란이있다. 2009년 NICE-SUGAR 연구자의보고에따르면혈당조절정도에따라 intensive group ( mg/dl) 과 conventional group ( mg/dl) 으로나누어비교한연구에서전자의경우오히려저혈당의위험이높고, 사망률도통계적으로유의하게높았다. 21 또한 ASPEN 에서는영양치료를받는중환자의경우혈당을 mg/dl로조절할것을권고하고있다. 1 본연구에서 NST군과 non-nst군모두 50% 이상의환자에서고혈당이발생하였으므로지속적으로적절한혈당관리를통한사망률과유병률을감소시키려는노력이필요하리라생각한다. PN 투여와관련한간기능장애는과잉공급등다양한요인으로인해나타날수있는데, 특히과도한포도당위주열량공급및패혈증여부와관련이있다 일반적으로 PN 투여시간기능장애를예방하기위해서과잉공급과 (>35 kcal/kg/day) 과과도한포도당주입 (>5 g/kg/day) 이이루어지지않도록주의하고조기에경장영양을시작하는것이도움이된다. 26 본연구에서는 NST군이 non-nst군에비해간기능장애가더많이나타났는데, 투여된열량은두군간의차이가없었으나포도당의공급은 NST군이더많았다. 이결과는 NST군의 GIR (glucose infusion rate, mg/kg/ min) 이 2.7±1.0로기준이상으로과도하게공급되지않았으나, 간기능장애가나타난 NST군환자 5명모두 PN 투여이전부터감염이있었고, septic shock과패혈증소견환자가 3명으로이와관련이있을것으로생각한다. PN 처방전체를대상으로한투여내역은 PN을통한영양지원내역만을포함하여다른경로로투여 (EN, 식이등 ) 된양은고려되지않았다. 그러나전체 PN 처방중 NPO인경우가 NST군은 76.4%, non-nst군의경우 88% 로 PN 이외의영양지원이본연구결과에크게영향을미치지않았을것이라고판단된다. All-NPO군에서투여된열량 (kcal/kg) 은 NST-NPO군이 non- NST-NPO군에비해더많았으나모두지침 (Table 1) 범위내에서투여되었다. 아미노산 (g/kg) 또한 NST-NPO군이더많이투여되었는데신기능과투석여부에따른아미노산 공급권장량 10 에따르면, NST군은총 3건중 2건 (66.7%) 이기준에맞게투여되었고 1건 (33.3%) 은기준보다낮게투여되었다. 그러나 non-nst-npo군은총 64건중 15건 (23.4%) 이기준에맞게투여되었고, 기준보다낮게투여한경우는 45건 (70.3%) 이었으며기준이상으로투여한경우는 4건 (6.3%) 이었다. 특히 non-nst-npo군중투석을하면서 PN 을투여한경우는 8건이었는데이중 1건만기준에맞게아미노산이공급되었고나머지 7건은기준보다낮게투여되어, 투석시소실되는아미노산이고려되지않아 NST 의뢰를통한아미노산의공급이더적절했다고판단된다. 지방을통한열량공급은 NST-NPO군이 non-nst-npo군에비해절대적인공급량은많았으나두군모두 NPC의 60 70% 의비율로공급되어지침과비교시적절하였음을알수있다. ASPEN-SCCM 1 에서는항산화비타민 (vitamin C, E 포함 ) 과미량원소 (Se, Zn, Cu 등 ) 의공급이환자의임상양상을개선시키므로특정영양치료를받는모든중환자에게미량영양소인비타민과미량원소를함께공급할것을권고하고있다. 본원에서는 NST 회신을통해투여되는원내조제 PN 에비타민과미량원소 (Zn, Cu, Mn, Cr) 가포함된다. 반면상업용 PN제제에는비타민과일부미량원소가포함되어있지않으므로이들의공급을위해 PN과함께비타민제제를처방해야한다. 본연구에서 NST-NPO군에서는비타민이모든환자에게공급되었지만 non-nst-npo군은 30% 의환자에게만공급되어미량영양소의투여가간과되고있음을알수있었다. 중환자의경우입원전부터식사를제대로하지못한경우가많고재원기간동안 PN을통한영양지원율이일반병동에비해많으므로미량영양소공급에관심을가질수있는방법을 NST 차원에서모색해볼필요가있다. 결론본연구에서영양불량은중환자에게흔히발생하였으며, 중환자실재원기간동안영양상태가더욱악화됨을알수있었다. PN을통한영양지원은 NST로의뢰한환자와그렇지않은환자모두중환자영양지원에대한지침범위내에서투여되었으나, non-nst군에서투석여부에따른아미노산공급과미량영양소공급이부적절하였다. 따라서보다체계적이고시스템적인 NST 활동과홍보를통해환자에게적절한영양지원을위한노력이요구된다. REFERENCES 1.
