Bone is the second most common site of metastasis after the lungs in cases of thyroid cancer. Here we describe a case in which a 57-year-old female patient had extremely severe pain on the posterior neck. After magnetic resonance imaging, 18F-FDG PET CT and ultrasonography guided biopsy, right papillary thyroid carcinoma with cervical spine metastasis was suspected; therefore, she underwent surgery for removal of thyroid carcinoma and seventh cervical spine metastasis. Pathologic diagnosis was confirmed as left thyroid follicular carcinoma with seventh cervical spine metastasis and synchronous right thyroid papillary carcinoma. The patient then underwent an additional spinal tumor removal operation, I 131 treatment and external radiation therapy. Complete removal of the cervical spine tumor could not be achieved due to intra-operative bleeding and the need to prevent damage to the spinal cord. Following treatment, the patient reported great relief from severe neck pain. This is a rare presentation of follicular thyroid carcinoma with cervical spine metastasis in Korea. Key Words: Thyroid follicular carcinoma, Cervical spine, Metastasis 중심단어 : 갑상선여포암, 경추, 전이 Dong Kun Lee, Kyu Sang Cho, Jong Chul Hong, Heon Soo Park Department of Otolaryngology-Head and Neck Surgery, Dong-A University College of Medicine, Busan, Korea Received August 27, 2013, Revised October 17, 2013, Accepted October 21, 2013 Correspondence: Heon Soo Park Department of Otolaryngology-Head and Neck Surgery, Dong-A University Hospital, 1 Dongdaesin-dong 3-ga, Seo-gu, Busan , Korea Tel: Fax: hspark1@dau.ac.kr 서론 증례 갑상선여포암의특징은갑상선유두암에비해국소경부림프절전이의빈도는낮은반면초기에원격전이의빈도가높으며, 주로폐와골전이가많다. 골전이의경우척추, 골반, 흉골, 장골그리고갈비뼈의빈도순으로보고되며, 국내에서는갑상선여포암의경추전이에대한보고는드물며경추, 흉추, 그리고요추전이가각각 1예씩보고되었다.(1-4) 저자들은경부통증을주소로내원한환자에서갑상선여포암에서전이한경추암과갑상선유두암이동시에진단 1예를경험하였기에문헌고찰과함께보고하는바이다. 약 1년전부터후경부통증이있었던 57세여자가 3 4개월전부터통증이참을수없을정도로심해지고진통제에도효과가없으며좌측팔로방사통이동반되어, 지역병원을방문하여촬영한경추자기공명영상에서 7번경추종양이발견되어본원신경외과로전원되었다. 자기공명영상에서는 T1과 T2 강조영상에서저신호강도를보이는불균질의종괴가 7번경추의체부와척수후기둥을따라서관찰되고, 주변골을파괴시키고연부조직을침범하고있었고, 경추체는높이가낮아져있었으며, 조영증강이잘되었다 (Fig. 1A). 7번경추의전이성종양진단하에양자방출단층촬영 (18F-FDG PET CT) 및전신골스캔 (Whole Copyright 2013 Korean Association of Thyroid and Endocrine Surgeons; KATES. Tumor embolization was successfully undergone by polyvinyl alcohol particle. minectomy of 7th cervical spine, posterior lateral screw fixation of 4th-5th cervical spine, 1st-2nd thoracic spine), 척수주위종양제거술 (removal of paraspinal tumor) 을시행하였다. 2차수술은근치적종괴제거를목표로하였으나동맥색전술에도불구하고수술중출혈이많아서시야확보가어려웠고, 병변이광범위하여최대한육안적으로보이는종괴만제거하였다. 술후 3주후경추에총 64 Gy (8 Gy 8회 ) 방사선치료를시행하였고, 술후 10주경에 200 mci의방사성요오드동위원소치료를시행하였다. 환자의후경부통증과좌측상완저림증상은수술전보다호전되어약물치료로조절될정도가되었다. 수술후 7개월째시행한갑상선글로불린 (thyroglobulin, Tg) 은 ng/ml으로상승되어있고, I-123 전신스캔, 자기공명영상, 양자방출단층촬영상에서잔존암은크기가작아져서 7번경추에만부분적으로남아있고갑상선자극호르몬억제요법 (TSH suppression) 을하면서수술후 9개월째외래경과관찰중이다. 고찰 갑상선여포암의발생빈도는저자마다차이는있으나전체갑상선암중약 10 20% 정도를차지하며갑상선유두암다음으로흔한암이다.(5) 국내에서는 2009년보건복지가족부중앙암등록본부의암발생통계에의하면여포암은전체갑상선암의 3 7% 정도를차지하고있다.(6) 여포암은조직병리학적으로 최소침습여포암과광범위침습여포암으로구분할수있는데최소침습여포암은종양세포가피막을침범하였으나갑상선실질로침범하지않은상태이고, 광범위침습여포암은갑상선실질또는갑상선외조직으로침범한경우이다.(7) 원격전이율은최소침습여포암이 16%, 광범위침습여포암은 79% 이며, 10년생존율은각각 97.8% 와 80% 로보고된다.(8,9) 본증례는조직병리검사에서갑상선실질을침범한광범위침습여포암으로사료된다. 갑상선여포암은유두암과는달리진단당시비교적높은병기에서발견되고, 국소경부림프절전이는낮지만초기에원격전이가더흔하기때문에임상경과가공격적인양상을나타내며예후가더나쁘다.(10) 암에의한척수 (spinal cord) 나신경근 (nerve root) 의압박은뇌전이다음으로흔한암의신경학적합병증의원인이며, 전체암중에서약 10% 의환자가척수압박증상을첫증상으로호소한다.(10-12) 경부척추전이증상의약 90% 가경부통증이며참을수없을정도로극심한통증을유발하는경우가흔하고점진적인신경증상을유발하여환자의삶의질을심각하게손상시킨다.(13,14) 특히갑상선에서경부로전이한암은이와같은증상을잘나타낸다.(15) 갑상선암환자가후경부통증을주소로내원하는경우는드물지만본증례는심한경부통증을호소하는환자에서전이성암을의심하였고원발암을찾기위해양자방출단층촬영, 전신골스캔등의전신검사에서갑상선암이발견된경우이다. 진단당시부터원격전이를동반한진행된병기로발견될수도있는갑상선여포상암의공격적인임상양상을알수있었다. Gerfo 등 (16) 은전이성갑성선암에서는혈중 Tg가 500 ng/ml 이상으로보고한다고하였는데, 혈중 Tg가비정상적으로높을때는전이성갑상선암을의심하고전신전이유무를확인할필요가있다. 본증례에서는수술전 Tg를측정하지못하였고, 수술후잔존암이존재하는상태에서측정된 Tg은 ng/ml으로상당히높았다. 유두암에비하여여포암은전신전이에따른증상이나징후로암이발견되는경우가적지않고, 골전이의비율이 7 20% 로높으며, 그중일부에서만전신전이병변의국소치료가가능하다.(17) 골전이를동반한갑상선여포암의치료에는수술, 방사성요오드치료, 외부방사선조사치료, 항암화학요법등이사용된다. 특히골전이에대한단독방사성요오드치료는저항성을보이는경우가많아가능하면골전이부위를수술적으로제거하는방법이추천된다. 분화성갑상선암의골전이에대한수술적치료의적응증은내과적치료에효과가없는지속적인통증이나방사성요오드치료에저항성을보이는경우, 신경압박증상유무에상관없이척추불안정이있는경우이다.(18) 골전이부위가 5군데이하인경우골전이에대한수술을시행하면생존율과삶

4 이동근외 : 갑상선여포암의경추전이 1 예 265 의질개선을보이므로, 수술적접근이가능하고절제가능한골전이에대해서적극적인수술을시행하는것이 1차치료이며종양이큰경우에는수술로서크기를줄이는것이방사선요오드치료의효과도증가시킬수있다.(19,20) 증상을유발하는단일골전이병변에대한완전절제는생존율을증가시킬수있으므로 ( 특히 45세이하 ) 적극적인치료가필요하다.(21) 분화암이골전이만존재하고골격계이외의장기에전이가없는경우에는근치적수술이생존율을향상시킬수있다.(11) 본증례에서는경추에만단일골전이가있었고후경부통증및팔에방사통이있었던경우로, 골전이병변에대한적극적인수술적치료를시행하였지만척수신경손상위험성과과다출혈로근치적수술을시행할수는없었다. 방사성요오드를섭취하는골전이에대한방사성요오드치료는생존율을증가시키며, 진단당시골전이가있는분화갑상선암종의경우에는 45세이하, 특히 3개이하의골전이가있을때효과가있다.(11,20) 용량은경험적 ( mci) 으로하거나선량을측정하여용량을계산하여투여한다.(22) Durante 등 (23) 에따르면나이가젊고, 방사선요오드흡수성을가진크기가작은종괴환자는수술적치료보다방사선요오드치료와같은보존적치료를하는것이효과적이라고하였다. Petrich 등 (21) 의 107명을대상으로한연구에서도 45세이하와이상의두그룹간에방사선요오드치료의효과는 62.5% 와 49.0% 로젊은환자에서반응이더좋게보고되었다. 외부방사선조사치료는방사성요오드치료에반응이없는경우적용가능하며, 통증, 골절에대한위험그리고척수압박으로인한신경학적합병증이있을때에완화치료로써이용되며종양재발률과골전이부위의통증을낮춰준다. 분화갑상선암에서외부방사선치료와방사선요오드치료병합요법은암재발방지와통증완화에효과가있으며수술후병합요법을시행하면추가적인골파괴위험성을낮출수있다.(13) 본증례는잔존전이암으로수술후통증이남아있어서수술후방사성요오드치료와방사선치료를시행하였다. 항암화학요법은다양한약제가시도되었지만치료성과는좋지못하다.(11) 골전이암의색전술은근골격계통증을완화하고암종의성장을억제하며수술중출혈을줄이는효과가있다.(12) 본증례에서도첫번째술시경추암의출혈이심하여시야확보가어려웠기때문에두번째수술전에 Polyvinyl Alcohol Particle로성공적인색전술을시행하지만수술중출혈이많아서근치적절제술을시행할수없었다. 분화갑상선암은 10년생존율이 80 90% 에이르지만골격계원격전이가있을때는 15% 로아주낮아진다.(24-26) 본증례에서와같이경추골전이병변의경우수술, 방사성요오드치료, 외부방사선치료등다각적인치료방법을선택할수있다. 적극적인치료를통해환자의참을수없는극심한통증을완화시켜 줄수있고신경압박에의한추가적인신경증상발생을예방하여삶의질과생존율을향상시킬수있기때문에적극적인치료를시행해야한다. 수술적제거가불가능하고통증을유발하는병소에대해서는위에서언급한치료이외에고주파열치료, 주기적인 pamidronate 또는 zolendronate 주사, 척추성형술등의여러방법을개별혹은병용치료로사용할수있다.(27,28) 본증례처럼갑상선암과경추종괴가함께있다면갑상선에서기원한전이암의가능성도고려하여철저한문진과갑상선촉진을포함한이학적검사, 세침흡인검사, 초음파그리고경부 CT 등실시해야한다. 드물지만방사선조사력, MEN IIB, familial Gardner s disease, Cowden s disease 등이있는환자에서도갑상선암의가능성을의심할필요가있다.(15,29,30) 저자들은후경부통증을주소로내원한환자에서경추에전이된갑상선여포상암종을수술및외부방사선치료, 방사성요오드치료그리고동맥색전술로적극적으로치료한 1예를경험하였기에문헌고찰과함께보고하는바이다.
