The calcification was detected in bilateral basal ganglia, dentate nucleus, and thalamus by brain imaging. Serum calcium and phosphorus levels were normal. The underlying diseases of calcification of basal ganglia such as parathyroid dysfunction and other infectious, toxic, or metabolic illness were excluded. The patient had memory impairment and frontal executive dysfunction without aphasia, agnosia, apraxia, and visuospatial impairment in neuropsychological test. It suggested that the cognitive impairment might be due to the dysfunction of frontal-subcortical circuit. IBGC 는기저핵등의운동조절에관여하는뇌부위에칼슘이비정상적으로침착되어, 운동장애와신경정신과적증상을보인다. 1) 소아또는청소년기에발병할수있지만, 30~60 대에주로발병한다. 2) IBGC 에대한효과적인치료는아직없는실정이다. 2) IBGC 는운동장애와더불어점진적인인지기능저하를초래할수있다. 치매는 IBGC 의경과중늦게나타나며, 집중력, 최근기억, 계산및추상적사고능력의장애, 구음장애 (dysarthria), 동어반복증 (palilalia) 등을보일수있다. 3) 인지기능저하가동반된 IBGC 증례가외국에서는여러차례보고되었지만, 3-6) 국내에서는아직보고된바없다. 저자들은기억력저하를호소하는 IBGC 환자를경험하였기에증례보고와함께이와관련된문헌을고찰하고자하였다

증례 환자는 66세여자이며, 서서히진행되는기억력저하를주소로치매평가를위해아들과함께 2004 년 11월본원정신과를방문하였다. 환자는무학으로온화하고내성적인성격이었다고하며, 젊어서남편을사별하였고현재는큰아들과함께살고있었다. 과거력상환자는약 10년전에안면마비가있었다가회복되었다고하며, 내원당시당뇨병, 고혈압및천식으로약물복용중이었다.

또한전에비해적적하고외로워하였고, 활동이줄어주로집에혼자지내면서하루종일 TV만본다고하였다. 정신상태검사상협조적이었으나, 피곤해보였고묻는말에자신없는태도를보였다. 의식상태는명료하였으나집중력, 시간지남력, 집중력및단기기억력이저하되어있었다. 두드러진사고나지각장애는보이지않았고, 약간 우울한기분이었다.

2 증례 환자는 66세여자이며, 서서히진행되는기억력저하를주소로치매평가를위해아들과함께 2004 년 11월본원정신과를방문하였다. 환자는무학으로온화하고내성적인성격이었다고하며, 젊어서남편을사별하였고현재는큰아들과함께살고있었다. 과거력상환자는약 10년전에안면마비가있었다가회복되었다고하며, 내원당시당뇨병, 고혈압및천식으로약물복용중이었다. 정신과적과거력이나치매가족력은없었다고하였다. 아들의보고에의하면환자는내원 1~2 년전부터물건을두고잘찾지못하고, 가스불을켜놓고잊는등의기억력저하가점차심해진다고하였다. 최근에는아파트문을잠그지않은채열고다니고, 당뇨가있는데도음식조절을하지않고많이드시고, 빨래를구분하지않고삶는등의변화가나타났다고한다. 또한전에비해적적하고외로워하였고, 활동이줄어주로집에혼자지내면서하루종일 TV만본다고하였다. 정신상태검사상협조적이었으나, 피곤해보였고묻는말에자신없는태도를보였다. 의식상태는명료하였으나집중력, 시간지남력, 집중력및단기기억력이저하되어있었다. 두드러진사고나지각장애는보이지않았고, 약간 우울한기분이었다. 이학적검사상특이소견은없었으며, 신경학적검사상운동장애등의이상소견은발견되지않았다. 검사소견상흉부 X선검사, 심전도및갑상선기능검사는정상이었고, 일반혈액검사, 간기능검사, 매독검사는정상범위였다. 당뇨로인한혈당증가 (209), 소변당 (1+) 이있었고, 혈청 BUN, 마그네슘이정상범위보다약간높았다. ApoE 유전자형은 ε2/ε3 였고, 비타민 B 12, 엽산, homocysteine 은정상범위였다. 뇌자기공명 T2 강조영상에서양측기저핵, 치아핵 (dentate nucleus), 시상 (thalamus) 부위에다발성으로광범위한저강도병변, 전반적대뇌와소뇌위축을보였다 ( 그림 1). 뇌자기공명영상소견상과석회증 (hypercalcinosis), 가성부갑상선기능저하증 (pseudohypoparathyroidism) 또는독성물질침착등이의심되어정확한진단적평가를위해내분비대사내과에의뢰하였다. 진단적평가를위해시행된뇌전산화단층촬영상양측기저핵, 치아핵, 시상부위에다발성으로광범위하게낮은감약 (low attenuated) 병변이있었다 ( 그림 2). 혈청칼슘과인 (phosphorus) 은정상범위로부갑상선기능저하증 (hypoparathyroidism) 과가성부갑상선기능저하증등의부갑상선기능이상은배제되었다. 인지기능검사상한국형간이정신상태검사 (Korean version of mini-mental sate examination) 가 14점이었고, 집중력, 언어기능, 비언어적기억력및시공간능력은 Fig. 1. 한국형노인우울검사 (geriatric depression scale) 상 22점으로우울증상을보고하였다. 상기검사결과를종합할때, 본증례는 IBGC 으로인한피질하 (sucortical) 치매로진단할수있었다. 환자와보호자에게고혈압, 당뇨등의혈관성위험요인에대한지속적인치료와함께일반적인치매관리에대해서도교육하였다.

Brain computed tomography showing multifocal diffuse low attenuated lesions in bilateral basal ganglia, dentate nucleus, and thalamus. 비교적적절하게유지되었다. 그러나언어적자극에대한정보의회상특히재인과정에서는심각한기억결함을보였으며, 전두엽의집행기능을평가하는 contrasting program, Go-No-Go 검사, fist-edge-palm 검사에서도심각한결함을보였다. 기본적일상생활을수행하는데장애는없었지만, 도구적일상생활을수행하는데몇가지장애를보였다. 한국형노인우울검사 (geriatric depression scale) 상 22점으로우울증상을보고하였다. 상기검사결과를종합할때, 본증례는 IBGC 으로인한피질하 (sucortical) 치매로진단할수있었다. 환자와보호자에게고혈압, 당뇨등의혈관성위험요인에대한지속적인치료와함께일반적인치매관리에대해서도교육하였다. IBGC 에대한효과적인치료방법이없었기때문에비타민 E와 tanamine 을처방하였으나, 환자는외래방문을중단하였다. 고찰 본증례는양측뇌기저핵석회화와함께치매가동반된경우이다. 뇌전산화단층촬영상기저핵석회화가약 0.3~ 12.5% 에서흔하게발견되지만, 대부분의석회화는임상증상과연관성이없다고보고되었다. 7-9) 반면에 IBGC 는다양한운동장애나신경정신과적증상이동반된다. 3)10) IBGC 는상염색체우성으로유전되는경향이있지만, 산발 적으로발병될수있다. 2)4) 유전되는경우염색체 14q 와의연관성이보고되었다. 1) 본증례는가족력이없이발병한 IBGC이다. IBGC 의진단을위해서는양측기저핵의비정상적석회화소견이있어야한다. 4) IBGC 에서보이는석회화는기저핵의담창구 (globus pallidus) 에서가장흔하며치아핵, 시상, 대뇌백색질 (cerebral white matter), 소뇌, 속섬유망 (internal capsule) 등에서도보일수있다. 1)3) 대부분의비정상적인기저핵석회화의원인은부갑상선기능이상 [ 부갑상선기능저하증, 가성부갑상선기능저하증, 가성가성부갑상선기능저하증 (pseudopseudohypothyroidism), 부갑상선기능항진증 (hyperparathyroidism)] 이다. 2)3) 그외의원인으로는선천성및조기발병유전장애 [Cockayne 증후군, 결정경화증 (tuberous sclerosis), 유전조로증양 (progeria-like) 증후군, Kearns-Sayre 증후군 ], 염증및감염상태 [ 후천성면역결핍증 (acquired immunodeficiency syndrome), 세포거대봉입체병 (cytomegalic inclusion disease), 뇌염 (encephalitis), 톡소포자충증 (toxoplamosis)], 독성및무산소 (anoxic) 장애 [ 일산화탄소중독, 납중독, 출생무산소증, 방사선치료, methotrexate 치료 ], 사립체병증 (mitochondriopathy) 등이있다. 2)3)11) IBGC 에서보이는석회화에대한많은병리적생화화적연구에도불구하고이에대한원인은아직밝혀지지않았다. 12)13) 진단을위해서는비정상적석회

4 화의알려진원인질환이배제되어야한다. 본증례에서도뇌석회화가기저핵뿐만아니라치아핵, 시상부위에서광범위하게발견되었다. 검사상혈청칼슘과인은정상범위로부갑상선기능저하증과가성부갑상선기능저하증등의부갑상선기능이상이배제되었다. 기저핵의비정상적석회화를초래할수있는다른기저원인들을검사상또는임상적으로배제하여 IBGC 로진단하였다. IBGC 는운동장애나신경정신과적증상을보이며, 임상증상은중추신경계통에국한되어나타난다. 핵심증상은운동장애로구음장애, 추체외로징후및운동실조등이다. 14)15) 그외근육긴장이상 (dystonia), 무정위운동 (athetosis), 무도병 (chorea), 운동이상증 (dyskinesia) 등의다양한증상이나타날수있다. 2)16)17) 신경정신과적증상으로정신증, 성격변화, 조울증, 우울증, 강박장애및치매등이보고되었다. 10)18-20) 정신증의경우환청, 피해망상, 관계망상등의정신증상이보고되었다. 15)21-23) 치매의경우기억력저하, 실행기능저하및인지과정의완만함이보고된반면, 언어상실증 (aphasia), 인식불능증 (agnosia) 및실행증 (apraxia) 등은보고되지않았다. 3)6)20) 이와같은인지기능장애는헌팅턴병이나파킨슨병에서와유사하였다. PET 상양측기저핵석회화병변환자들에서전전두엽 (prefrontal cortex) 의대사저하가보고되었다. 10) 이는해마 (hippocampus) 등의내측측두엽의이상보다는피질하의기능이상으로인한전두엽 -피질하회로의장애를시사한다. 본증례의경우운동장애는보이지않았지만, 경도의우울증과함께치매증상을보였다. 인지기능검사상기억력장애와전두엽집행기능의현저한장애를보였다. 신경영상검사상양측기저핵등의피질하부위의광범위한석회화병변을보였지만, 해마를포함한내측측두엽의위축이나다른병변은관찰되지않았다. 따라서본증례를 IBGC 으로인한피질하치매로진단할수있겠다. 치매는다양한뇌질환으로인해발생될수있으므로원인적평가를위해서는신경영상검사가필요하다. 신경영상검사는특히치매의조기발병, 비전형적인임상양상및인지기능의최근또는급격한악화등을보일때시행되어야한다. 또한뇌종양, 경막하출혈및수두증등과같은치료가능한원인질환을배제하기위해서도신경영상검사가중요하다. 24)25) 본증례에서는신경영상검사를통해치매의드문원인질환인 IBGC 을발견할수있었다. 신경영상검사를시행하지않았다면다른유형의치매로 오진하여불필요한치료를시행했을가능성이있었다. 저자들은본증례를통하여 IBGC 의진단과함께동반될수있는신경정신과적증상에대해살펴보았다. 중심단어 : 기저핵 석회화 치매 신경정신과적증상. 참고문헌 1.
