All rights reserved. Herpes simplex encephalitis (HSE) is rare, serious sporadic encephalitis associated with high mortality in untreated cases. Although cognitive impairment with fever could be the clue of diagnosis, various symptoms can make it difficult to be diagnosed. Dizziness is one of the most frequent symptoms ENT doctors can encounter. Authors experienced the HSE patient suffered rotatory sense of dizziness as a primary symptom and treated the patient successfully without major complication. We reported our experience and findings of vestibular function test of patient. It could be helpful to other ENT doctors who encounter similar patients. Research in Vestibular Science 2013;12(2):67-71 Key Words: Dizziness; Herpes simplex encephalitis 서론단순헤르페스뇌염 (herpes simplex encephalitis, HSE) 은중추신경계 (central nervous system, CNS) 에발생하는심각한감염성질환중하나로 250, ,000명당 1명꼴로발생하는굉장히드문질환이다. 1 임상증상으로발열 (fever), 두통, 혼돈 (confusion), 그리고발작 (seizure) 등이급속히발생할수있으며, 때때로 olfactory/gustatory hallucination, urinary/ fecal incontinence, 감정조절상실 (loss of emotional control), hypomania, psychosis 등이발생할수있다. 2 만약치료를하지않은경우사망률은 70% 로높은사망률을나타내는질환으로빠른진단과치료가가장중요하지만다양한임상 증상으로빠른진단이어려울수있다. 진단에전산화단층촬영 (computed tomography, CT), 자기공명영상 (magnetic resonance imaging, MRI) 등방사선학적검사들이도움이되지만가장정확한진단은뇌척수액 (cerebrospinal fluid, CSF) 에서중합효소반응 (polymerase chain reaction, PCR) 을통하여 viral DNA를검출하는것이다. 치료는 IV acyclovir를사용하며, 정확한치료율은나와있지않지만기존연구에의하면사망률을 20%-30% 까지낮추는것으로알려져있다. 3,4 HSE에서발생가능한증상중 ataxia 또는 dizziness는이비인후과에서 HSE환자를마주칠수있는가능성을시사한다. 어지럼을동반한 HSE의보고는 herpes simplex virus (HSV) 가전정신경을통해 CNS로진입하여초기에는전정 67

Res Vestibul Sci Vol. 12, No. 2, Jun. 2013 신경염이발생하였다가수일내동측측두엽뇌염으로진행한경우와, 5 뇌간에국한되어침입한 HSV로인해초기에어지럼이동반된경우 6 가있었다. 하지만 HSE환자에서어지럼양상이나 vestibular function에관한보고는아직까지없는실정이다.

환자는이후증세호전이없고, 내원 4일전후경부통증 (posterior neck pain) 과회전성어지럼, 오심, 구토등이발생하여 2차병원에재방문하였다. 추가적인복약치료를받았으나증세가지속되어내원 2일전뇌전산화단층촬영 (brain CT) 을시행하였고, 특이소견관찰되지않으나 (Figure 1) 지속되는증세로큰병원으로가기를권유받고본원응급실방문하였다.

2 Res Vestibul Sci Vol. 12, No. 2, Jun 신경염이발생하였다가수일내동측측두엽뇌염으로진행한경우와, 5 뇌간에국한되어침입한 HSV로인해초기에어지럼이동반된경우 6 가있었다. 하지만 HSE환자에서어지럼양상이나 vestibular function에관한보고는아직까지없는실정이다. 본논문에서저자들은어지럼을주소로방문한환자에서 HSE를진단하였고, vestibular examination을시행하였다. 따라서치료경험과 HSE에서 vestibular laboratory finding을보고하고자한다. 증례 44세남자환자가내원 7일전기침과발열, 오한으로외부의원방문하여복약처방받고귀가하였다. 환자는이후증세호전이없고, 내원 4일전후경부통증 (posterior neck pain) 과회전성어지럼, 오심, 구토등이발생하여 2차병원에재방문하였다. 추가적인복약치료를받았으나증세가지속되어내원 2일전뇌전산화단층촬영 (brain CT) 을시행하였고, 특이소견관찰되지않으나 (Figure 1) 지속되는증세로큰병원으로가기를권유받고본원응급실방문하였다. 응급실에서시행한문진에서환자는회전성의어지럼을호소하고있었으며, 하루에약 3-4차례반복적으로발생을호소하였다. 빙글빙글도는회전성어지럼이었으며, 한번발생시수분정도지속됨을호소하였다. 눈뜨고있거나움직일때어지럼이더악화되었다. 응급실에서시행한이학적검사에서안진은관찰되지않았으며, 안구움직임, 동공반사모두정상소견이었다. 의식수준은각성상태 (alert) 였으며양측운동및감각모두정상소견이었다. 소뇌기능검사역시이상소견을보이지않았으나주관적어지럼강도가너무심해입원치료시행하였다. 입원후시행한이학적검사에서도특이변화소견은없었으나발열과지속적어지럼호소하며, 약간의과수면양상이관찰되었다. 전두부의압박성두통을호소하였으나, meningeal irritation sign, neck stiffness 등은관찰되지않았다. 지속적인문진중약간의둔화된감정상태 (stunning) 가의심되었으나보호자는입원전과큰차이없다고하였다. 인지기능역시시간, 장소, 사람에대한정상적인인지기능을나타내고있었다. 이경검사에서양측고막정상소견이었으며, 순음청력검사상우측 22 db, 좌측 20 db, 어음명료도검사상우측 84%, 좌측 90% 로측정되었다. 변조이음향방사검사 (distortion product otoacoustic emission, DPOAE) 에서양측고음역의반응이저하된소견관찰되었으나특이소견은없었으며, 청성뇌간반응검사 (auditory brainstem response, ABR) 역시특이소견관찰되지않았다. 전정기능검사상온도안진검사 (caloric test) 에서양측반응의차이는없었으며, 자발안진 (spontaneous nystagmus) 및두위변환에따른안진은관찰되지않았으나안구움직임이불안정하게빠르게움직이는양상기록되었다 (Figure 2). 단속운동검사 (saccade test) 에서명확한 overshoot, undershoot은관찰되지않았지만일정위치에안구를고정하지못하고불규칙적인안구움직임이기록되었다 (Figure 3). 완서조화가속력 (slow hormanic accelerate, SHA test) 에서편위소견은없었으나모든주파수영역에걸쳐서이득 (gain) 이감소한소견이관찰되었다 (Figure 3). 환자의지속적인발열과두통, 그리고명확하지는않지만불규칙한안구운동소견으로 central vertigo 또는 CNS infectious disease를의심하였으며, MRI 시행후 lumbar puncture를계획하였다. 내원 2일째시행한 brain MRI에서 T2WI에서좌측 insular cortex와 medial temporal lobe에 high signal intensity A B C D Figure 1. (D) Slow harmonic acceleration test showing decreased gain through whole frequency. 관찰되었으며, 좌측의 diffuse leptomeningeal enhancement 소견이관찰되어임상적으로 herpes encephalitis가의심되었다 (Figure 1). 환자의 MRI 확인후즉시 IV acyclovir 투여 를시작하였고신경과로전과후 lumbar puncture를시행하여 CSF analysis를시행하였다. CSF study에서 red blood cell (RBC) 5/μL, white blood cell (WBC) 90/μL (lymphocyte 91%), protein 87.0 mg/dl, glucose 70 mg/dl 소견으로소량의 RBC 가검출되었고, WBC, protein이증가된소견이었다. CSF에서미생물학적검사를시행하였으며 india ink, cryptococcal antigen, H. influenza type B antibody, Streptococcus group B antibody, Pneumococcal antibody, N. meningitides antibody, E. coli 모두음성으로검출되었으며, HSV type 1 PCR 양성, HSV type 2 PCR 음성, varicella zoster virus (VZV), PCR 음성소견으로 HSE을확진할수있었다. Acyclovir 치료 1일째간이의식수준검사 (mini mental state exam, MMSE) 를시행하였으며총점 30점에서 24점기록되었다. 언어능력검사에서이해력 (comprehension), 반복 (repetition), 유창함 (fluency) 모두정상소견으로관찰되었다. 환자 acyclovir 치료 5일째다시시행한 MMSE에서 30점 /30점기록하였으나, 약간의과수면양상이지속적으로관찰되었다. 치료 7일째두번째 CSF study를시행하였으며, RBC 200/μL, WBC 100/μL, glucose 54 mg/dl, protein 94.0 mg/dl으로측정되었다. 같은날시행한 electroencephalography (EEG) 에서좌측대뇌의 mild diffuse dysfunction 소견이기록되었다. 치료 13일째다시시행한 EEG에서좌측대뇌의 severe diffuse dysfunction 소견이관찰되었으며, 15일째시행한 CSF study에서 RBC 50/μL, WBC 20/μL, glucose 64 mg/dl, protein 74.0 mg/dl으로측정되었으며, HSV PCR 음성으로확인되었다. 환자 CSF study 결과호전소견과임상증상의특이소견이관찰되지않아치료 16일째퇴원하였다. 외래관찰중우울소견외에는특이소견은관찰되지않는상태로현 69

4 Res Vestibul Sci Vol. 12, No. 2, Jun 재외래추적관찰중이다. 고찰 HSE는산발적으로발생하는치명적인바이러스성뇌염 (fatal viral encephalitis) 의가장많은원인으로백만명당 1-3명정도에서발생한다고알려져있으며, 계절, 성별에발생차이가없는것으로알려져있다. 7 전혀치료를시행하지않는경우 70% 정도의치사율 (mortality) 을보이며, 치료를시행하였을때도영구적인심각한신경학적합병증이발생할확률이높다. 8 일반적인 Herpes simplex viral disease와다르게 HSE는 viral reactivation으로인한것인지 primary infection에의한것인지정확하지않다. 또한 HSV의 brain으로침입기전역시정확하게알려져있지않다. 몇몇 HSE 환자들에서 olfactory tract에서 herpes viral particle이검출되었고, animal model 에서 olfactory tract이 CNS로 viral access가가능한점이확인되어 olfactory nerve와 trigeminal nerve를통해 virus가 brain으로전파되는것으로추측되고있다. 9 HSE는전형적으로발열과함께다음중하나이상의신경학적이상을나타낸다. 10 1) 의식수준의변화 (altered conscious state), 2) 인격의변화 (personality change), 3) 국소뇌신경장애 (focal cranial nerve deficits), 4) 편마비 (hemiparesis), 5) 발음장애 (dysphasia), 6) 보행실조 (ataxia), 7) 국소성간질 (focal seizure). 하지만 HSE의임상증상은굉장히다양하기때문에쉽게의심할수없는경우가많다. 과거 brain biopsy를통해서확진하였으나현재는검사의침습성과 MRI, PCR의발달로인하여특별한경우를제외하고는사용되지않고있다. 현재가장중요한검사는뇌척수액검사 (CSF analysis) 이다. HSE에서 CSF biochemistry는일반적으로 pleocytosis, high protein, high RBC, 그리고 normal glucose 소견이관찰된다. CSF에서 PCR을통해 viral DNA 검출은높은민감도 (98%) 와특이도 (94%) 를보여 HSE 진단에 gold standard로현재알려져있다. 11 하지만굉장히드문경우 PCR이음성으로나타날수있으며, 질병발생 2-3주이후면 PCR이음성이될수있음을염두에두어야한다. 12 MRI는증상이발생한환자에서거의모든경우이상소견이관찰된다. 주로 fluid-attenuated inversion recovery (FLAIR) sequence, diffusion restrict image, 그리고 contrast-enhanced image에서가장먼저이상소견이관찰되며, 주로 medial temporal lobe와 insula에병변이관찰된다. 13 CT은 MRI에비해진단적가치가높지않으며, 오히려질병초기에정상 CT 소견으로인하여진단을느리게할수있다. HSE는내과적응급질환으로빠른치료의시작이질병의예후에가장중요한요소로작용한다. 따라서 HSE가확진되지않았다하더라도임상적으로 HSE가의심되는상황에서는 HSE에준하여치료를시작한다. HSE의치료는 IV acyclovir를 1회용량 10 mg/kg을하루 3번 (8시간간격 ) 으로사용한다. 두개의 randomized trials에서상기용량의 IV acyclovir가 HSE의치사율을 20%-30% 미만으로까지줄였음을보고하였다. 3,4 하지만고연령환자, 의식상실이진행된상태 (coma state), 입원후치료시작까지 48시간이상지연된경우 acyclovir 치료를하였다하더라도결과가나쁘다고보고되었다. 14 CSF study 또는 imaging study에서 HSE 가의심되는경우 PCR에의한확진이되지않더라도지체없이 IV acyclovir 사용을권한다. 또한최근 guideline에의하면 CSF study 또는 imaging study가없는상황에서도 HSE 가의심된다면입원 6시간이내 acyclovir 사용을권장하고있으며, CSF study, imaging study가음성이다하더라고 HSE 가의심되면확진될때까지 acyclovir를입원 6시간이내사용을권장하고있다. 3 Bacterial meningitis의경우경험적인항생제의사용은원인균주발견을지연시켜진단을지연시킬수있다. 하지만 HSV의경우 acyclovir 치료를시작하고수일이지나도 CSF에서 PCR로인하여검출이가능하기때문에경험적치료를시작하여도 CSF sampling으로확진할수있다. 15 Acyclovir 는비교적안전한약물에속하지만주로신장을통해배설되므로신장기능장애 (renal impairment) 를일으킬수있다. 주로치료 4일이후가역적신질환 (reversible nephropathy) 이발생할수있으나충분한 hydration, renal function monitoring 등을통하여예방할수있으며, 기존신장질환환자에서는용량을감량해서사용을권한다. 총 14-21일간 IV acyclovir를사용하며, CSF study를반복하여 HSV PCR 음성을확인한다. 계속 HSV PCR 양성인경우 IV acyclovir 사용을지속하면서 1주일간격으로 PCR 을재시행, 음성으로확인될때까지치료하는것으로알려져있다. 14 본증례에서환자는지속적인회전성어지럼을호소하는상태였지만전정기능검사상특이소견은발견되지않았으며발열및과다수면등 encephalitis를의심할수있는다른증상들도동반되었다.
