신지훈 외: 기도 협착의 인터벤션 기도 협착이나 술 후 문합부 협착이다. 활동성 단계를 지나 섬 유화 단계의 결핵성 기도 협착과 같은 기도의 비염증성 섬유 화 협착이 염증 및 석회화를 동반한 기도 협착이나 기도 연화 증보다 풍선 확장술에 더 잘 반응하는 것으로 알려져 있다(19, 21).

2 신지훈 외: 기도 협착의 인터벤션 기도 협착이나 술 후 문합부 협착이다. 활동성 단계를 지나 섬 유화 단계의 결핵성 기도 협착과 같은 기도의 비염증성 섬유 화 협착이 염증 및 석회화를 동반한 기도 협착이나 기도 연화 증보다 풍선 확장술에 더 잘 반응하는 것으로 알려져 있다(19, 21). 또한 풍선 확장 시 풍선의 허리 형성이 보였다가 점진적 으로 또는 갑자기 없어지는 소견은 섬유화 단계의 결핵과 같 은 섬유화 기도 협착의 경우 보일 수 있는 것으로 대개 좋은 임상 결과를 보이는 것으로 알려져 있다(19, 20). 합병증 풍선 확장 시 흉통을 호소할 수 있고 기관지경련, 기도의 점 막 열상 또는 파열(Fig. 2), 기종격동(Fig. 2), 또는 심한 출혈 이 기도 풍선 확장술의 합병증으로 보고되었다(19, 20, 22). 기흉과 기도 열상 또는 파열 후의 종격동염은 이론적으로 가 능한 합병증이나 지금까지 보고된 바는 없다. 풍선의 과도한 팽창으로 인한 이러한 합병증을 줄이기 위해서는 협착의 정도 나 주변 정상 기도의 직경에 따라 풍선 카테타의 크기를 신중 히 결정해야 한다. 그러나 합병증을 예방하기 위해 충분치 못 한 팽창이 이루어진 경우 충분한 치료 효과를 기대하기 어려 Table 1. 스텐트 설치술(Stent Placement) 1980년대부터 폐암과 같은 악성 기도 협착을 치료하기 위 해 팽창형 금속 스텐트를 사용하기 시작하였으며 최근에는 스 텐트 및 관련 기구들의 발달과 함께 스텐트 설치술이 관강내 병변뿐만

악성 병변의 경우 관강외 및 관강내 병변을 갖는 폐암이 가장 흔한 적응증이고(Fig. 3) 특히 수술이 불가능한 관강외 병변으로 인한 기도 협착의 경우 즉각적인 치료 효과 를 보일 수 있는 유일한 방법이 된다.

기관 삽관 후 생긴 협착의 경우 환자가 수술을 거부하거나 또는 수술적 재건술을 받기에 병변의 길이가 너무 긴 경우 스텐트 치료의 적응증이 된다. 최 근에는 술 후 문합부 협착이 폐 이식과 같은 기도 수술이 증 E Fig. 3.

4 신지훈 외: 기도 협착의 인터벤션 운 점이 있다. 저자들의 경우 충분한 치료 효과를 얻기 위해 서는 어느 정도의 기도 점막 열상을 의인성으로 주어야 한다 고 생각하기 때문에 풍선의 크기는 대략 주변 정상 기도 정도 의 직경과 같은 것으로 한다. 스텐트 설치술(Stent Placement) 1980년대부터 폐암과 같은 악성 기도 협착을 치료하기 위 해 팽창형 금속 스텐트를 사용하기 시작하였으며 최근에는 스 텐트 및 관련 기구들의 발달과 함께 스텐트 설치술이 관강내 병변뿐만 아니라 관강외 병변의 치료에 효과적이고 급성 호흡 곤란을 빠르게 해결할 수 있는 장점이 있어서 그 사용이 더 증 가하고 있다. 기도 스텐트 설치술의 구체적인 적응증은 Table 1에 정리 되어 있다. 악성 병변의 경우 관강외 및 관강내 병변을 갖는 폐암이 가장 흔한 적응증이고(Fig. 3) 특히 수술이 불가능한 관강외 병변으로 인한 기도 협착의 경우 즉각적인 치료 효과 를 보일 수 있는 유일한 방법이 된다. 양성 병변의 경우에는 기관 삽관 후 협착이 가장 흔한 적응증으로 알려져 있으나 몇 몇 아시아 지역에서는 결핵성 기도 협착도 비교적 흔한 기도 스텐트의 적응증이 된다(Fig. 4). 기관 삽관 후 생긴 협착의 경우 환자가 수술을 거부하거나 또는 수술적 재건술을 받기에 병변의 길이가 너무 긴 경우 스텐트 치료의 적응증이 된다. 최 근에는 술 후 문합부 협착이 폐 이식과 같은 기도 수술이 증 E Fig. 3. -E. 64-year-old man with bronchogenic carcinoma involving the left bronchus.,. chest radiograph () and 3-T image () show total collapse of the left lung and abrupt cut-off of the left bronchus.. 스텐트의 종류 스텐트는 재질에 따라 크게 팽창형 금속 스텐트와 비팽창형 실리콘 스텐트로 나눌 수 있다.

5 대한영상의학회지 2005;52: 가함에 따라 다소 증가하는 추세에 있다(12, 14, 17, 23). 기 도 연화증은 주변 기도 연골의 파괴로 인한 기능적인 기도 협 착을 일으키는 것으로 특히 병변이 다발성이거나 긴 경우 또 는 환자의 상황이 수술을 받기 어려운 경우 일시적인 스텐트 설치술의 적응증이 될 수 있다(3, 24, 25). 선천성 기관 협착 은 풍선 확장술로 협착된 기관을 둘러싸는 연골을 파열 시킨 후 팽창형 금속스텐트를 넣는 치료법이 보고되고 있다(Fig. 5) (24, 26-28). 심한 출혈 소인, 환자의 매우 짧은 기대 수명, 그리고 기도의 심한 염증 등을 상대적 금기증이라고 할 수 있 다. 스텐트의 종류 스텐트는 재질에 따라 크게 팽창형 금속 스텐트와 비팽창형 실리콘 스텐트로 나눌 수 있다. 듀몬 스텐트(umon stent)로 대별되는 실리콘 스텐트는 비교적 매끈매끈한 실리콘으로 되 어 있어서 금속 스텐트에 비해 상대적으로 스텐트의 이동, 가 래 정체(sputum retention) 등이 많고 실리콘 자체의 두께로 인해 실제 내경이 작아지는 단점을 가진다. 이는 전신 마취 하 에서 경성 기관지내시경(rigid bronchoscopy)을 이용해 삽입 하는 것으로 대부분 호흡기내과나 흉부외과의 영역이므로 여 기서는 자세히 다루지 않기로 한다. 위에 언급한 실리콘 스텐트의 단점을 극복하고자 팽창형 금 E F Fig year-old man with left main bronchial obstruction due to endobronchial tuberculosis.,. chest radiograph () and 3-T image () just prior to stent placement show total left lung collapse and total occlusion of the left main bronchus.. 신지훈 외: 기도 협착의 인터벤션 속 스텐트를 쓰게 된 것으로 이는 국소 인후두 마취 하에 투 시 조영 하에서 삽입할 수 있는 장점이 있고 자가팽창성 성질 로 인해 기도 벽에 잘 순응하는(conformability) 장점을 가진 다. 팽창형 금속 스텐트는 피복 여부에 따라 피복 스텐트와 비 피복 스텐트로 나누는데 그 장단점을 Table 2에 정리하였다.

최근 에는 피복형 스텐트가 스텐트 이동과 같은 합병증이 있거나 치료의 경과 중 뺄 수 있는 장점을 가지면 비피복형 스텐트에 비해 월등히 유리할 수 있다는 보고들이 있었다(31, 32).

저자들을 포함해 우리나라 인터벤션 영역에서 많이 쓰는 제 품은 covered retrievable expandable nitinol stent로 기존에 폴리우레탄(polyurethane)으로 피복한 제품을 써왔으나 폴리 우레탄의 경우 약 10% 정도에서 수개월 후 제거했을 때 피복 물질의 찢어짐이나 변성이 발견되어(33) 최근에는 화학 내성 Table 2.

6 신지훈 외: 기도 협착의 인터벤션 속 스텐트를 쓰게 된 것으로 이는 국소 인후두 마취 하에 투 시 조영 하에서 삽입할 수 있는 장점이 있고 자가팽창성 성질 로 인해 기도 벽에 잘 순응하는(conformability) 장점을 가진 다. 팽창형 금속 스텐트는 피복 여부에 따라 피복 스텐트와 비 피복 스텐트로 나누는데 그 장단점을 Table 2에 정리하였다. 이 중 어느 하나의 장점과 단점이 각각 상대방의 단점과 장점 이 된다. 정리하면 피복형 팽창형 금속 스텐트는 스텐트 관강 의 장기 개통성을 향상시킬 수 있으나 스텐트 이동이나 주변 소기관지 입구를 막을 수 있는 위험성이 있다(29, 30). 최근 에는 피복형 스텐트가 스텐트 이동과 같은 합병증이 있거나 치료의 경과 중 뺄 수 있는 장점을 가지면 비피복형 스텐트에 비해 월등히 유리할 수 있다는 보고들이 있었다(31, 32). 이 와 같은 개념의 제거가능 피복형 금속 스텐트의 경우 스텐트 의 제거를 위해 스텐트의 근위부 상단 내측으로 제거용 갈고 리가 잡아 끌 수 있도록 나일론 끈(drawstring)을 부착하였다. 저자들을 포함해 우리나라 인터벤션 영역에서 많이 쓰는 제 품은 covered retrievable expandable nitinol stent로 기존에 폴리우레탄(polyurethane)으로 피복한 제품을 써왔으나 폴리 우레탄의 경우 약 10% 정도에서 수개월 후 제거했을 때 피복 물질의 찢어짐이나 변성이 발견되어(33) 최근에는 화학 내성 Table 2. 팽창성 금속 스텐트의 경 우 투시 유도 하에서 인터벤션 영상의학과 의사가 기관지내시 경의 도움 없이 삽입이 가능하다.

저자들은 호흡 기내과 의사와의 협진으로 혈관조영실에서 투시 유도와 기관 지내시경의 도움으로 모든 기도 인터벤션 시술을 하고 있다. 스텐트 삽입술의 방법도 유도철사를 넣고 선택적 기도조영 술을 하기까지는 풍선 확장술과 같다.

7 대한영상의학회지 2005;52: 이 매우 강한 PTFE(polytetrafluoroethylene)가 기도 스텐트 의 피복 물질로 사용되기 시작하였다(Fig. 4). 사전 검사 및 시술 풍선 확장술 전에 협착의 위치, 길이, 정도, 양상을 평가하 고 무기폐 여부를 확인하기 위해 단순 흉부 방사선 촬영과 기 관지내시경을 시행하고 3차원 재구성 영상이 포함된 T 영상 을 얻는 것은 풍선 확장술과 같다. 팽창성 금속 스텐트의 경 우 투시 유도 하에서 인터벤션 영상의학과 의사가 기관지내시 경의 도움 없이 삽입이 가능하다. 그러나 스텐트 전후의 기도 상태를 평가할 수 있고 쉽게 유도철사를 넣을 수가 있으며 응 급 상황에 적절하게 대처할 수 있는 장점이 있으므로 기관지 내시경의 도움이 매우 중요하다고 할 수 있다. 저자들은 호흡 기내과 의사와의 협진으로 혈관조영실에서 투시 유도와 기관 지내시경의 도움으로 모든 기도 인터벤션 시술을 하고 있다. 스텐트 삽입술의 방법도 유도철사를 넣고 선택적 기도조영 술을 하기까지는 풍선 확장술과 같다. 선택적 기도조영술로 협 착 부위를 확인한 후 최소한 스텐트가 포함하여야 할 부위의 Fig. 6.
