Vertebral osteomyelitis due to mucormycosis is very rare, with only three cases caused by mucormycosis since 1970 being reported, and none in Korea. Here, we present a case of vertebral osteomyelitis caused by mucormycosis in a 67-year-old woman, having type 2 diabetes mellitus for 10 years, who was in complete remission from acute leukemia after chemotherapy 3 years previously. (Korean J Med 2017;92:84-88) Keywords: Mucormycosis; Osteomyelitis; Vertebra; mphotericin 서론털곰팡이증 (mucormycosis) 은 Rhizopus spp, Mucor spp, Cunninghamella spp, pophysomyces spp, bsidia spp, Saksenaea spp. 등에의해발생하는감염으로공격적이고전격적인감염을일으키는것으로알려져있다. 대부분은부비동 (39%), 폐 (24%), 피부 (19%) 에서감염이나타나며, 기저질환으로는악성종양, 당뇨병, 장기이식등이있다 [1]. 척추골수염은전체골수염의 3-5% 정도를차지하는감염으로유럽지역에서매년 10만명당 명이발생한다. 원 인균으로 Staphylococcus aureus가약절반정도를차지하는가장흔한균이며, 그람음성간균 (Gram negative rods) 이 7-33%, coagulase-negative staphylococci가 5-16% 를차지한다. 또한유행지역에서 Mycobacterium tuberculosis 및 rucella 도원인균으로자주확인되고있다 [2]. 그러나, 털곰팡이에의한척추골수염은아주드문질환으로현재까지문헌검색상 3예만이보고되었다. 저자들은 67세여자에서털곰팡이에의한흉추부위의척추골수염을진단한사례를경험하였기에문헌고찰과함께보고하고자한다. Vertebral osteomyelitis caused by Mycormycosis 증 근력은 고관절 아래로 모두 grade 0이었다. 감각은 T5-6 level 례 아래로 떨어져 있었고, 심부건반사는 정상으로 병적 반사는 환 자: 67세 여자 없었다. 검사실 소견: 말초혈액 검사에서 백혈구 9,000/mm3, 혈색 주 소: 2주 전 발생한 양쪽 하지 근력 저하 현병력: 3년 전 급성 백혈병으로 유도 항암화학요법과 1 소 10.8 g/dl, 혈소판 155,000/mm3였다. Erythrocyte sed- 차례의 공고 항암화학요법 후 완전완화 상태로 이후 항암 치 imentation rate은 90 mm/hr였다. 혈청 생화학 검사에서 ST 료를 원치 않아 외래 추적 관찰만 하던 환자로 내원 3주 전 63 IU/L, LT 120 UI/L, 총 빌리루빈 0.4 mg/dl, blood urea ni- 우상엽의 괴사성 폐렴(Fig. 1)으로 입원하였다. 당시 기관지 trogen 14 mg/dl, Cr 0.63 mg/dl, Na 137 mmol/l, K 4.1 mmol/l, 내시경을 시행하였으나 동정된 균은 없었으며, 세균성 폐렴 Cl 103 mmol/l, C-reactive protein 3.93 mg/dl, 혈청 gal- 으로 생각하고 연고지 인근 병원에서 ceftriaxone, clindamycin actomannan assay는 0.26 (negative)였다. 을 투약하였다. 내원 2주 전부터 점차 양쪽 하지 근력 저하가 방사선 소견: 흉부 단순촬영에서 우측 중앙 폐야에 경화 발생하였고, 내원 1주 전부터 배뇨 장애가 동반되어 다시 전 가 있으며 우측편에 흉수가 있었다. T-spine X-ray에서 T2-5 원되었다. 에 골 파괴와 디스크 공간 협착이 있었다. 흉부 컴퓨터단층 과거력: 10년 전부터 당뇨병으로 metformin, linagliptin을 촬영(computed tomography, CT)에서 우상엽에 경화 및 공동 복용 중이고, 7년 전부터 고혈압으로 olmesartan을 복용 중이 성 병변이 있었고, 우측 가슴에 흉수가 확인되었다. 이는 3주 었다. 전 흉부 CT와 비교하여 큰 변화는 없는 상태이며, reverse 신체 검사 소견: 활력 징후는 혈압 149/89 mmhg, 맥박 수 halo sign을 보이고 있었다. 또한 T3, T5의 척추체가 파괴되 92회/분, 호흡 수 20회/분, 체온 37.4 였다. 키는 152 cm, 몸 었고, T2-T5 level의 척추체 주변 및 척주관에 경계가 불분명 무게는 61 kg이었다. 만성 병색을 보였고 의식은 명료하였 한 연부조직 병변이 있었다(Fig. 2). 다. 양쪽 상지의 근력은 grade 5로 확인되었고, 양쪽 하지의 조직 생검: 오른쪽 T5 척추 옆의 농양 유사 병변에서 CT 유도하 조직 생검을 하였다. 조직 생검의 Hematoxylin and Eosin 염색에서 마디가 없으며 두께가 다양한 모양을 보이며 둔각으로 갈라져 나오는 균사가 있었고, 주변으로 호중구 침 윤 및 조직의 괴사가 있었다(Fig. 3). Gomori s methenamine silver 염색에서 흑색으로 염색된 균사가 뚜렷하게 관찰되었 다(Fig. 3). 조직 진균 배양에서는 진균이 자라지 않았지만, 곰팡이의 종류를 감별하기 위해 aspergillosis와 mucormycosis 면역화학(immunohistochemistry, IHC) 염색을 이전 연구에서 C D Figure 1. (C) These hyphae showed positive for anti-rhizopus immunohistochemical staining ( 200), (D) but negative for anti-spergillus immunohistochemical staining ( 200). 기술한것 [3] 과같이시행하였고, mucormycosis IHC stain에서양성을나타내었다 (Fig. 3C and 3D). 치료및임상경과 : 조직생검결과를바탕으로다시흉부 CT를리뷰하였을때 3주전에 reverse halo sign이관찰되었고, 이와인접한부위에연부조직과뼈감염이생긴것을고려할때 3주전에세균성폐렴으로간주되었던병변이폐에생긴털곰팡이증의가능성이높은것으로판단하였다. 그러나폐에추가적인침습적인검사는진행하지않았다. Liposomal amphotericin 5 mg/kg/day 투약하면서보존적인치료를하였으나 1개월째뚜렷한호전경과는없는상태로연고지병원에서 liposomal amphotericin 를유지하기원하여전원되었고, 치료를시작한지 3개월째호전, 악화없이비슷한경과를보이고있다 ( 스테로이드는투약하지않았다 ). 고 털곰팡이증은혈관침범을통해급속한괴사를일으키는특징을나타내며, 주로포자 (spore) 의흡입을통해코대뇌부위및폐에감염을일으키거나피부에직접적으로포자가접종되면서피부털곰팡이증을일으키게된다 [1]. 털곰팡이증의결정적인진단은조직형태학적진단과진균배양을조합하여이루어지지만, 특히털곰팡이증에서는 D 찰 배양이잘되지않는다. 따라서이러한경우에는조직형태학적진단에의존하게되지만, 모양자체가서로비슷할때도있어구분이불명확할수있다 [4]. 그러므로면역조직화학염색, 중합효소연쇄반응 (polymerase chain reaction, PCR) 등의다른보조적인진단방법이연구되고있으며, 최근면역조직화학염색을이용하여진단의정확성을높일수있다고보고하였다 [3]. 이번증례에서도 Gomori s methenamine sliver 염색을통한조직형태학적진단뿐만아니라 mucormycosis IHC stain 양성및 aspergillosis IHC stain 음성소견이확인되어, 비록배양에서음성으로확인되었지만털곰팡이증으로진단할수있었다. 침습성곰팡이감염에서아스페르길루스증 (aspergillosis) 과털곰팡이증을정확히구별하는것은치료약제가달라지기때문이다. 아스페르길루스증에서는일차치료로 voriconazole을투약하는것이 amphotericin deoxycholate를투약하는것에비해치료성공률이더높다고보고되었다 [5]. 이에비해털곰팡이증에서는 amphotericin deoxycholate의최소억제농도 (minimal inhibitory concentration) 가 0.33 µg/ml 임에비해서 voriconazole의최소억제농도는 µg/ml로높아 voriconazole이의미있는효과를나타내지못한다 [6]. 따라서저자들은확인된털곰팡이증에대하여 liposomal amphotericin 를투약하였다. 털곰팡이증에의한척추골수염은매우드문질병으로 1970년부터 2016년 1월까지전세계적으로 3예가보고되었다 (Table 1). uruma 등 [7] 은암으로인하여후두절제술및부분하인두절제술 (laryngectomy with partial hypophyaryngectomy) 및방사선치료를받았던적있는환자에서목통증이있으며천천히진행하는근력저하 ( 양쪽팔부터시작하여사지마비로진행 ) 환자를보고하였다. 이환자는추후광범위폐색전증으로인하여사망하였는데, 부검을통하여조직학적으로넓고 non-septate 된균사가보여털곰팡이증으로진단하였고 ( 배양검사는시행하지않았음 ), 이로인한척추골수염및경막외농양이신경학적결손의원인임을확인하였다. Chen 등 [8] 은요추천자및 radiofrequency nucleoplasty를시행받은이후털곰팡이증에의한척추골수염및척추추간판염을보고하였다. 털곰팡이증은농양에서시행한배양검사상 Rhizopus rhizopodoformis 가동정되었고, 조직학적으로털곰팡이증에합당한균사가확인되어진단되었다. 이환자는 liposomal amphotericin 투약과함께두차례의수술적변연절제술을시행하여완전히치료되었다. 마지막으로 2014년

(2014) [9] Present case (2016) Spine ge/ Sex Underlying disease or condition region affected 60/M Laryngectomy with partial hypophyaryngectomy and radiotherapy due to hypopharyngeal carcinoma 57/F L4 5 disc puncture and radiofrequency nucleoplasty Diagnostic methods C3-4 Histomorphology (autopsy) L4-5 bscess culture, histomorphology 42/F Kidney transplantation S1 L culture, abscess culture, PCR 67/F ML, DM T4-5 Histomorphology, IHC Surgery ntifungal agents Treatment duration Prognosis No No. Death Yes Yes CD, followed by itraconazole 8 months Survival LM, followed 3 months Survival by posaconazole No LM On treatment On treatment M, male; F, female; CD, amphotericin deoxycholate; L, bronchoalveolar lavage; PCR, polymerase chain reaction; LM, liposomal amphotericin ; ML, acute myeloid leukemia; DM, diabetes mellitus; IHC, immunohistochemistry. 에 Navanukroh 등 [9] 은신장이식을받은지 1개월이지난환자에서털곰팡이증에의한폐감염및엉치뼈 (sacral spine) 의골수염, 허리엉치뼈 (lumbosacral spine) 의경막외농양을보고하였다. 털곰팡이증은농양및기관지폐포세척액배양검사에서 Cunninghamella spp. 이동정되었고, 중합효소연쇄반응검사를통해 Cunninghamella bertholletiae로확인되어진단되었으며, 수술적변연절제술및 liposomal amphotericin 투약으로호전되었다. 앞서기술한 3예와본증례를종합하여보면, 암, 신장이식과같은면역저하환자이거나척추에침습적인시술을받은환자에서척추골수염이발생하였다. 척추골수염의발생부위는다양하였고, 진단방법은주로조직형태학적진단또는배양검사를하였고, PCR 검사를시행한경우도있었다. 3예중 2예에서수술적치료를시행한후항진균제병용을통하여성공적으로치료되었다. 털곰팡이증에대한가장좋은치료방법에대해서는정립된바가없으나, Roden 등 [1] 이시행한연구에서 amphotericin deoxycholate 투약과함께수술적절제를시행한경우생존율이가장높았으며 (70%), amphotericin deoxycholate 단독 (61%), 수술단독 (57%) 으로치료한경우생존율이낮았다. 또한장기간의투약이필요한데 liposomal amphotericin 의투약이기존의 amphotericin deoxycholate보다신독성의발생빈도가낮아선호된다 [10]. 본증례에서도 liposomal amphotericin 를투약하였으나, 이미근력이 grade 0으로발생한지 2주이상지난시점으로감압술은수술로얻을수있 는이득과위험을고려해서시행하지않았다. 본증례는당뇨및급성백혈병병력이있는환자에서털곰팡이에의해척추골수염이발생하였다. 이러한경험을바탕으로척추골수염환자에서악성종양, 당뇨등의곰팡이감염위험성이있는경우세균및결핵이외에다른원인에대한가능성을고려하여조직배양및조직검사가반드시필요하다고생각한다. 요 털곰팡이에의한척추골수염증례보고가국내에서는아직없었으며, 전세계적으로도매우드물다. 저자들은급성백혈병으로 3년전항암치료를받고재발이없는상태로지내던환자에서발생한털곰팡이에의한척추골수염을조직형태학적방법및면역조직화학염색방법으로진단하였기에보고하는바이다. 중심단어 : 털곰팡이증 ; 골수염 ; 척추 ; mphotericin 약 REFERENCES 1.
