Methods : To investigate the binding of major gangliosides on senile plaques and neurofibrillary tangles of Alzheimer disease-specific pathology, we developed four monoclonal antibodies -- GM1, GD1a, GD1b, and GT1b -- employing the hydridoma technique, and applied them for immunohistochemical staining at the frontotemporal neocortex and hippocampus of Alzheimer disease brains and agematched control brains. Results : Moderate immunopositivity for GM1 and GD1a was noted on the senile plaques and neurofibrillary tangles. Mild immunopositivity for GD1b and GT1b on neurofibrillary tangles was noted. Strong GD1b immunopositivity was observed on a few neurons and neurites. Strong immunopositivity for GT1b, and moderate immunopositivity for GM1 and GD1a were noted on reactive astrocytes. Conclusions : These observations suggest that GM1 and GD1a may be involved in the formation of senile plaques as well as neurofibrillary tangles in Alzheimer disease brains. Key Words : Alzheimer Disease-Gangliosides-Immunohistochemistry Alzheimer병은 원발성 치매를 일으키는 가장 흔한 원인으로 서, 임상적으로 치매만으로는 진단이 어렵기 때문에 병리학적 소견에 의하여 확진된다. 병리학적으로 여러가지 형태학적 변화 가 관찰되지만, 그 중에서도 가장 중요한 두가지 형태학적 변화 는 신피질에 광범위하게 분포된 노인반(senile plaque)과 신경 섬유변성(neurofibrillary degeneration)이다. 1 신경섬유변성은 비정상적으로 인산화된 tau 필라멘트가 쌍으로 꼬여 있는 신경 섬유농축체(neurofibrillary tangle)와 신경망실(neuropil threads) 로 관찰된다. 2 노인반은 아밀로이드(amyloid)를 함유하고 있으며, 아밀로이 드베타단백질(A )의 침착은 Alzheimer병의 병인에 있어서 가 장 중요한 역할을 한다. 또한 최근에는 아밀로이드베타단백질이 GM1 ganglioside와 강하게 결합된 상태로(GM1/A ) 존재한 다는 사실이 밝혀졌다. 3,4 즉 아밀로이드베타단백질은 GM1 ganglioside와 결합함으로써 그 생화학적 구조가 변형되고 아밀 로이드섬유 형성을 조장하는 토양을 제공하여 Alzheimer병을 유발시킨다고 한다. 5 Gangliosides는 sialic acid를 함유한 glycosphingolipids로서 척추동물의 중추신경계에 주로 분포되어 있으며, 이곳에서 신경 영양효과, 세포막을 통한 신호전달, 세포증식 및 분화 등의 다양 한 생물학적 기능을 한다 이 물질은 Alzheimer병 환자와 정 상 대조군의 대뇌에서 생화학적, 면역조직화학적으로 차이가 있 음이 보고되었다. 12,13 그러나 중추신경계에서 gangliosides의 국 소적인 분포에 대한 연구는 개발된 항체의 특이성 때문에 어려 움이 있었다. 이를 극복하기 위하여 Kotani 등은 하이브리도마 법(hybridoma technique)을 이용하여 각종 gangliosides에 대 한 단세포군 항체를 제작하여 중추신경계에서 gangliosides의 세 포특이성 발현을 확인하였다 본 연구는 이들 gangliosides에 대한 단세포군 항체를 동일한 방법으로 제작하여, Alzheimer병 환자의 뇌에서 관찰되는 노인반과 신경섬유 농축체에 있는 어떤 종류의 gangliosides가 관여하는지를 규명하고자 시행되었다. 400

2 Alzheimer병과 Gangliosides 401 단세포군 항체 제조 재료와 방법 대뇌의 주요 gangliosides인 GM1, GD1a, GD1b, GT1a에 대한 4종류의 단세포군 항체를 하이브리도마법을 이용하여 제조 하여 각각을 GMB16, GMR17, GGR12, GMR5라고 명기하였 으며, 14 그 과정은 다음과 같다. 실험 동물은 여러 종류의 생쥐 에서 예비 실험을 한 결과 면역항체 생성이 가장 우수하였던 계 대사육된 순수 혈통의 C3H/HeN종 생쥐를 사용하였다. 항원으로 사용한 정제된 ganglioside는 박층 크로마토그라피 (thin-layer chromatography: TLC) 상 다른 ganglioside와의 혼합이 없음을 확인한 후 사용하였다. 항원인 주요 ganglioside 는 10 g씩을 산을 처리하여 항원성을 유지시킨 Salmonella minnesota에 흡착시켜 ganglioside-세균 복합체를 만든 후 사용 하였다. 항원은 7주 이상 된 건강한 생쥐에 생쥐 1마리당 ganglioside-세균 복합체 250 g을 실험 당일, 4, 7, 11, 21일째에 정 맥을 통하여 5회 주사하여 민감화하였다. 마지막 주사 후 3일째에 민감화된 생쥐로부터 얻은 비장의 림프구를 배양하여 BALB/ C종 생쥐로부터 유도된 골수종세포주(PAI)와 융합시켜 잡종 세포를 만들었다. 잡종 세포주의 배양액으로부터 희석을 하여 GM1, GD1a, GD1b, GT1b gangliosides에 대한 단세포군 항 체를 제조하였다. 이들 항체는 IgM(k) 또는 IgG(k)형이었다. 잡종 세포로부터 생성된 항체가 함유된 상층 배양액으로부터 효 소결합면역흡착법(ELISA)으로 항체의 역가를 산정하였다. 생성된 GM1, GD1b, GT1b 항체는 각각의 항원과 선택적 특이성이 있었으며, GD1a 항체는 GD1a 항원 특이성 외에 GM1b, GT1b 항원과도 약한 교차반응을 보였다(Table 1). 면역조직화학 염색 부검 후 신경병리학적 검색상 CERAD 기준에 17 의하여 Alzheimer병으로 진단된 5예(캐나다 British Columbia 대학 뇌 Table 1. No other glycolipids, such as other a-and b-pathway gangliosides, or neurtral glycolipids tested were detected; b: IgM ( ) type, c: IgG ( ) type, +++: strong, +: weak, -: negative. 조직은행으로부터 공여받았으며, 연령은 67-82세)에서 채취한 대뇌 조직과 대조군으로서 신경과적 질환이 아닌 질병으로 사망 한 환자 중 Bielschowsky 은염색상 노인반과 신경세사 농축체 가 관찰되지 않는 대뇌조직 5예(연령은 68-80세)를 각각 선택 하였다. 이들 대뇌의 반구는 전두엽, 측두엽, 해마 부위에서 3 mm 두께로 조직편을 채취한 후, 0.1 M 인산완충용액과 혼합 된 4% paraformaldehyde에서 2일 동안 4 에 고정한 후, 동 일 완충용액에 희석한 15% sucrose 용액에 냉장 보관하였다. 냉장 보관된 대뇌조직편은 냉온 박절기를 이용하여 30 m 두 께로 박절한 후 4 냉장 보관용액에 보관하였다. 이들 박절편 에 대한 면역조직화학 염색은 네모상자 시험관에서 부유시키면 서 시행하였다. 4 염색은 일차 항체와 biotin이 부착된 이차 항체에 반응시킨 후 avidin-hrp 복합체(ABC Elite, Vactor Lab, Burlingame, CA, U.S.A.)와 작용시켰다. 항원항체 결합은 0.05 M Tris- HCl 완충액에 0.01% DAB, 0.6% nickel ammonium sulfate, 0.05 M imidazole, % H 2 O 2 가 포함된 기질을 사용하여 발색시켰다. 이때 음성 대조염색은 일차 항체 대신에 희석된 정 상 토끼 혈청을 사용하였다. 검색된 조직편은 neutral red 대조 염색 후 관찰하였다. 항원인 주요 ganglioside 는 10 g씩을 산을 처리하여 항원성을 유지시킨 Salmonella minnesota에 흡착시켜 ganglioside-세균 복합체를 만든 후 사용 하였다.

3 402 이민철 우영종 김승업 외 1인 결과 Alzheimer병 환자와 정상 대조군의 대뇌 조직에서 주요 gangliosides에 대한 면역조직화학 염색결과 Alzheimer 병소와 정상 신경세포에서 각각 다른 면역조직화학 소견을 관찰할 수 있었다. Alzheimer병 환자의 전측두엽 신피질과 해마에서 관찰되는 노 인반에서는 GM1과 GD1a gangliosides가 중등도로 발현되었으 며(Fig. 1A), 신경섬유농축체에서는 GM1과 GD1a gangliosides 가 중등도로 발현되었고, 또한 GD1b와 GT1b gangliosides도 경 도로 발현되었다(Fig. 1B). 소수의 피라미드형 신경원세포와 돌 기에서는 GD1b gangliosides가 강하게 발현되었다(Fig. 1C). 반 응별아교세포에서는 GT1b gangliosides가 강하게, GM1과 GD1a gangliosides가 중등도로 발현되었다(Fig. 1D). 주요 gangliosides 에 대한 단일 항체 중에서 GM1과 GD1a는 아밀로이드 베타 단 백질로 구성된 노인반과 신경세사 농축체에 강한 양성을 보였다. 소수의 피라미드형 신경원세포와 돌 기에서는 GD1b gangliosides가 강하게 발현되었다(Fig. 1C). 반 응별아교세포에서는 GT1b gangliosides가 강하게, GM1과 GD1a gangliosides가 중등도로 발현되었다(Fig. 1D).

Immunohistochemistry of monoclonal antibodies for major gangliosides in age-matched control brains disclosed strong GM1-positivity on neuronal cytoplasm at stratum oriens of the hippocampus, and moderate GD1a-positivity along cytoplasm and cytoplasmic membranes of each external pyramidal neurons in the frontal cortex. 따라서 Alzheimer병의 노인반 생성에 있어서 아밀로이드베타단 백질에 GM1과 GD1a gangliosides가 결합됨을 관찰할 수 있었다. 대조군과 Alzheimer병 특이 병소 이외의 대뇌와 해마 조직에 서 공통적인 면역조직화학 양성소견이 관찰되었다. GM1 ganglioside는 전측두엽 신피질의 백질부와 해마의 stratum oriens에 있는 신경원세포질을 따라 발현되었다(Fig. 2A). GD1a ganglioside는 대뇌 전측두엽 신피질의 외측 피라미드층의 신경원세포 질에서 발현되었다(Fig. 2B). 이러한 GM1과 GD1a ganglioside 의 면역조직화학적 발현 강도는 Alzheimer병과 정상 대조군의 대 뇌에서 유의한 차이가 없었다. 이들 ganglioside 발현은 주로 신 경세포의 세포질과 세포막을 따라 분포하고 있음을 보여 주었다. 이상의 면역조직화학적 소견은 Table 2에 요약하였다. 고찰 중추신경계에 존재하는 주요 gangliosides는 GM1, GD1a, GD1b, GT1b의 4종류이며, 이는 전체 gangliosides의 80-90% 를 구성하고 있다. 18 Alzheimer병 환자의 대뇌 조직의 gangliosides 변화에 대한 연구는 생화학적 분석 또는 면역조직화학 염 색법을 통하여 시행되고 있다. 이 중에서 생화학적 정량 분석 결과 Alzheimer병 환자의 대뇌에서는 위에 기술한 주요 gangliosides의 함량이 감소되고, 단순 gangliosides인 GM3, GM2, GD3 등의 함량이 증가된다고 하였다. 또한 본 환자의 뇌척수액 을 생화학적으로 분석한 결과 GM1과 GD1a gangliosides의 함 량이 증가되어 있고, GD1b와 GT1b gangliosides의 함량이 감 소되어 있음이 보고되었다. 12,13 이러한 생화학 변화는 여러 가지 손상에 의한 신경원세포사와 반응별아교세포 증식 과정 중에 gangliosides의 용해성 파괴와 관련이 있을 것으로 사료되었다. 12 한편 면역조직화학적 기법을 이용한 결과 신경섬유농축체에서 는 GQ1c, GQ1b, GD3, GT1b gangliosides 등이 혼합된 A2B5 항체에 대하여 강한 양성 발현이, 노인반에서는 GM1과 GD1a gangliosides의 양성 발현이 보고되었다. 22,23 이러한 연구 보고들 은 gangliosides 일부가 Alzheimer병의 발병 과정에 관여됨을 시 사해 준다. 본 연구에서는 Alzheimer병 환자의 사망 후 부검을 통하여 채취된 뇌 조직을 냉장 formaldehyde에 고정한 후 냉동박절편 을 제작해서 새로 개발한 gangliosides 항체를 이용하여 면역조 직화학 염색을 시행하였다. 그 결과 신경섬유농축체에서 GM1 과 GD1a gangliosides에 중등도, GD1b와 GT1b gangliosides 에 경도의 양성 발현이 관찰되었다. 이러한 소견은 과거에 신경 섬유 농축체에서 발현된 GM1과 A2B5 외에 19,21 GD1a와 GD1b gangliosides가 추가로 발현됨을 보여주었다. 또한 본 연구팀이 실온에서 포르마린에 2주 동안 고정된 부검조직에서 a-pathway gangliosides인 GM3, GM2, GM1, GD1a 및 GT1a에 대 한 단세포군 항체를 이용하여 면역조직화학적으로 관찰한 결과, 노인반에서는 GM3, GM2, GD1a가, 신경섬유 농축체에서는 GM2와 GD1a가 양성을 보였으며, 정상 신경원에서는 GM2가 소뇌의 Bergmamn 아교세포 이외에 특이적 반응을 보이지 않 았다. 4 이러한 면역조직화학 소견상의 차이점은 본 연구에서 개 발한 항체의 특이성과 부검한 대뇌를 짧은 기간 동안 냉장 고정 하고, 냉동박절편을 사용함으로써 면역조직화학 염색의 반응도

5 404 이민철 우영종 김승업 외 1인 가 더욱 더 예민해졌기 때문이라 생각된다. 노인반에서는 GM1 과 GD1a gangliosides가 중등도의 양성을 보였는데, 이것은 다 른 연구 결과와 일치된 소견이었다. 12,23 Alzheimer병의 병소에서 관찰되는 반응별아교세포에서는 GM1, GD1a, GT1b gangliosides의 양성 발현이 관찰되었는데, 이러한 소견은 Alzheimer병 이외의 반응별아교세포증에서의 소견과 일치하였다. Gangliosides는 일반적으로 신경영양물질로 알려져 있으며, 특 히 신경원세포에서 축삭돌기의 왕성한 성장 및 신경연접 형성 시 에 GM1과 GD1a gangliosides의 함량이 증가한다고 알려져 있 다. 6,7 그러나 한편으로는 GM1이 A 와 결합함으로써 Alzheimer 병의 병인으로 작용한다는 연구가 다음과 같이 보고되고 있다. 첫째, GM1/A 결합체는 Alzheimer병의 발병과정 중에 중 요한 아밀로이드 섬유를 형성하는 토양 역할을 한다. 3,5 둘째, GM1/A 형성은 과량의 콜레스테롤에 의해 증폭된다. 24 A 분절과 GM1 ganglioside 간의 특이적 결합이 보고된바 있으며, 특히 A 1-40 분절은 생체외 시험관에서도 주요 gangliosides와 결합할 수 있다. 25 셋째, GM1/A 는 신경원 세포막을 직접적으 로 손상을 일으킨다. 26 이러한 보고들과 본 연구 결과를 종합하 면 gangliosides/a 복합체 형성은 Alzheimer병의 병리학적 구조를 이루는 노인반과 신경섬유농축체를 생성하는 시발점으로 판단된다. 본 연구에서 생성한 gangliosides에 대한 단세포군 항 체는 Alzheimer병에서 관찰되는 신경섬유농축체와 노인반에서 의 주요 gangliosides가 관여되어 있음을 보여 주고 있다. 27 특히 배양된 대뇌피질 신경원세포에 M의 GM1 ganglioside 를 처치하면 흥분독성이나 산소라디칼에 의한 신경원세포의 세 포자멸사와 괴사를 억제하지만, M 농도에서는 신경 원세포의 손상이 더욱 더 증폭되었다. 이러한 보고와 본 연구 결과는 대뇌에서 적정 농도 이상의 gangliosides가 침착되면 신 경원세포에 대한 영양 또는 보호 효과보다는 손상에도 관여한다 는 사실을 말해 준다. 참고문헌 1.
