Smoking, hypertension, and atherosclerotic vascular diseases can be major risk factors for ischemic gastritis. Acute gastric ischemia presents as an acute abdomen with diarrhea or hematemesis that rapidly progresses to acute peritonitis, irreversible septic shock, and death if untreated. We report a case of acute extensive ischemic gastritis combined with tetraplegia due to cervical myelopathy and extensive atherosclerotic changes of the celiac trunk and abdominal aorta. (Korean J Helicobacter Up Gastrointest Res 2017;17: ) Key Words: Ischemic gastritis; Stomach; Treatment 서 론 위는위벽내측부혈관이잘발달되어있어서급성허혈성손상이매우드물다. 1-3 허혈성위염은대개수술이나치료색전술 (therapeutic embolization) 후발생하거나담배, 고혈압, 동맥경화성혈관질환이있는환자에서발생할수있으며, 의심하지않고발견이늦어지는경우에치명적일수있다. 2-7 저자들은명치의통증과흑색변을주소로내원한환자에서내시경으로허혈성위염으로진단하고보존적인치료로호전된환자를경험하였기에문헌고찰과함께보고한다. 증 례 82세남자환자가명치의통증과흑색변을주소로내원하였다. 환자는내원 2일전흑색변이있었고이후무른변이지속되었으며, 명치의통증과오심, 구토가동반되어전원되었다. 내원당시혈압은 107/41 mmhg 였고, 맥박수는분당 97회, 호흡수는분당 20회, 체온은 37.0 o C였다. 환자는과거력에서 20년전에뇌경색이있었고, 16년전부터당뇨와고혈압으로치료받 Received: April 5, 2017 Revised: May 6, 2017 Accepted: May 7, 2017 Corresponding author: Young Hee Choi Department of Pathology, Hallym University Dongtan Sacred Heart Hospital, 7 Keunjaebong-gil, Hwaseong 18450, Korea Tel: , Fax: , yhchoi@hallym.or.kr 아왔으며, 5년전척추협착증으로수술받았고 2달전부터경추척수증 (cervical myelopathy) 에의한사지마비 (tetraplegia) 로요양병원에서입원중이었다. 아스피린, 실로스타졸 (cilostazol), H2 수용체차단제, 스테로이드제를포함하여당뇨, 고혈압과신경계약물을복용하고있었다. 내원당시신체검사에서만성병색을띄었고결막은약간창백하였으며, 심음과호흡음은정상이었다, 복부는평탄하였고간이나비장은촉지되지않았으며, 장음이전반적으로감소되어있었고복부잡음 (bruit) 은청진되지않았다. 복부에명치부위에압통이있었으나반발통은없었다. 일반혈액검사에서백혈구는 16,100/mm 3, 혈색소는 10.1 g/dl, hematocrit은 29.3%, 혈소판수치는 96,000/mm 3 였다. 생화학검사에서 AST 24 IU/L, ALT 13 IU/L, BUN 61.2 mg/dl, creatinine 1.63 mg/dl, amylase 174 IU/L, lipase 96 U/L였고, 혈액응고검사에서 PT, INR 1.09 (normal range: INR), 10.6 sec (normal range: sec), aptt 25.0 sec (normal range: sec) 였다. 응급내시경검사에서식도와십이지장은정상이었으나, 위저부의대만에서전정부까지출혈성점막탈락과자발적인출혈을보이는심한괴사성위염소견을보이는부분과정상소견으로보이는부분이함께존재하였고 (Fig. 1), 조직검사에서궤양과미란을동반한만성활동성염증소견을보이면서심한호중구와호산구의침윤과함께삼출물과섬유화조직소견을보였다 (Fig. 2). 복부 CT에서 광범위한 위 점막하층의 부종성 비대가 있었으 나 소장과 대장은 정상적으로 보였으며, 복강동맥의 기시부 (celiac trunk)와 복부 대동맥에 광범위한 동맥경화성 변화를 보였지만, 혈전이나 폐색은 없었다(Fig. 3).

출혈과 심한 허혈성 변화로 인해 수술을 고려하였으나 환자 와 보호자가 수술을 원하지 않고 활력징후가 안정되어 있으면 서 증상이 없고, 순환기검사에서 혈전이나 폐색은 없어 보존적 인 치료를 시행하면서 상태 악화시 수술을 고려하기로 하였다.

The gastric biopsy shows focal fibrosis and infiltration with neutrophils and eosinophils (H&E; A: 100, B: 400). 복부 CT에서 광범위한 위 점막하층의 부종성 비대가 있었으 나 소장과 대장은 정상적으로 보였으며, 복강동맥의 기시부 (celiac trunk)와 복부 대동맥에 광범위한 동맥경화성 변화를 보였지만, 혈전이나 폐색은 없었다(Fig. 3). 심장초음파검사와 말초 동맥에 대한 초음파검사에서 심방, 심실과 말초 혈관에 혈 전은 없었고, 정상 좌심실 수축 기능과 함께 경한 삼첨판 역류 와 경한 안정기 폐동맥 고혈압을 보였다. 출혈과 심한 허혈성 변화로 인해 수술을 고려하였으나 환자 와 보호자가 수술을 원하지 않고 활력징후가 안정되어 있으면 서 증상이 없고, 순환기검사에서 혈전이나 폐색은 없어 보존적 인 치료를 시행하면서 상태 악화시 수술을 고려하기로 하였다. 정맥 영양과 경험적인 광범위 항생제와 함께 양전자펌프억제 104 제(proton pump inhibitors), 아스피린, 실로스타졸을 투여하 면서 보존적으로 치료하였고 입원 8일째 시행한 추적 내시경에 서 점막의 부종과 발적은 있었으나, 궤양 및 출혈 등의 소견은 보이지 않았다(Fig. 4A). 내시경에서 호전 소견을 보여 식이를 시작하였으나 입원 11일째 흑색변이 있어 시행한 내시경에서 다시 점막의 자발적 출혈 소견을 보여(Fig. 4B) 식이를 중단하 고 보존적인 치료를 시행하였다. 이후 더 이상의 출혈이 없어 다시 식이를 시작하였고. 입원 3주째 시행한 내시경과 조직검 사에서 이전보다 염증 소견이 호전되었다(Fig. 4C, D). 환자는 이후 증상 없이 안정되어 퇴원하였고 이후 외래에서 경과관찰 중이다.

4 Korean J Helicobacter Up Gastrointest Res: Vol 17, No 2, June 2017 고 찰 위와십이지장은췌십이지장동맥 (pancreaticoduodenal artery) 과위십이지장동맥 (gastroduodenalartery) 으로부터이중혈관공급을받고풍부한점막하혈관얼기를가지고있기때문에허혈성위염은매우드물게발생한다. 1-3 위장관허혈은주로심폐기능질환을가지는고령의환자에서발생하며, 허혈성위염의주요위험인자로는담배, 고혈압, 동맥경화성혈관질환이있다. 추가적인위장관허혈의위험인자로는 50세이상, 울혈성심부전, 부정맥, 최근의급성심근경색, 혈량저하증, 저혈압또는패혈증, 이전에동맥색전증을앓았던경우, 혈관염, 심부정맥혈전증, 과응고상태 ( 단백 C와 S 부족증, 항트롬빈 III 부족증, 활성형단백저항증 ) 등이있고, 수술이나혈관시술후에도발생할수있다. 3,4,8 허혈성위염은매우드물어서여러개의증례만보고되어있으며, 외국에서이뇨제와강심제로치료중이던만성허혈성심질환환자, 내장동맥에심한동맥경화성질환을가진환자, 고혈압과당뇨병으로치료중이던환자에서발생한보고등이있고, 국내에서는말기신부전으로투석중인환자에서골반내농양과함께발생한증례 1예가있었다. 5,6,9-11 본증례에서는고혈압, 당뇨와뇌경색의병력이있었으며, 혈전이나폐색은없었으나, 복강동맥의기시부와복부대동맥에광범위한동맥경화성변화를보였다. 급성위허혈은급성복통, 설사, 토혈등의증상을시작으로급성복막염, 중증패혈성쇼크 (septic shock), 사망에이를수있기때문에신속하고정확한진단이필요하다. 3,4,8-13 급성허혈성위염의진단에조직검사는크게도움이되지않는것으로알려져있지만, 복부 CT와내시경이도움이될수있다. 허헐성위염의내시경소견은불규칙한점막의비후와삼출물과미란이동반된미만성의색조변화등이있을수있고, 만성허혈성위염의경우점막의결절성변화, 홍반, 미란, 다발성의표재성위궤양등이있다고보고되었다. 3-7,9-15 기종성위염 (emphysematous gastritis) 은내시경소견에서점막하부종에동반된화농성삼출물과괴사성위조직등이보이면서복부 CT에서두꺼워진위벽과위벽내농양과함께불규칙한모양을보이는위벽내의공기가있으면다른위염과감별할수있다. 16 이증례에서는내시경에서정상적인부분과명확하게구분되는광범위한점막의출혈성변화와부종과함께미란과궤양이보이고복부 CT에서위벽이심하게비후되어있으나위벽내농양이나공기가없고식이시작후악화와호전을보여급성허혈성위염으로진단하였다. 급성허혈성위염의표준적인치료는응급개복술을통한절제이지만, 복부 CT에서주요내장동맥에폐색이없으면정맥영 양과광범위항생제등의보존적인치료로호전되기도한다. 6,9-13 이증례에서처음내원했을때와입원 11일째악화되었을때수술을권하였으나보호자가수술을원하지않아보존적인치료를시행하였고수술없이호전되었다. 향후재발한다면수술적인치료가필요할수있다고생각한다. 저자들은복통과흑색변을보여내원한환자에서광범위한급성허혈성위염을내시경으로진단하고보존적인치료를통해호전된증례를경험하여문헌고찰과함께보고하는바이다.
