We report a patient with PCA, confirmed by brain magnetic resonance imaging (MRI) and F 18 -fluorodeoxyglucose positron emission tomography(fdg PET). A 58-year-old right-handed woman presented initially with visual dimness and difficulty finding things around her. She had partial Balint s syndrome, partial Gerstmann syndrome, and idiomotor apraxia. She also had a mild memory disturbance, but preserved insight of her disease. Neuropsychological evaluation showed decreased parietal and left temporal functions bilaterally. Brain MRI and F 18 -FDG PET revealed typical bilateral occipitoparietal atrophy and hypometabolism, which were slightly worse on the right side. Cholinesterase inhibitor administration for 6 months improved the memory impairment slightly, but not the cortical visual dysfunction. This is a typical case of PCA, confirmed by neurologic signs and imaging findings. 2 인증, 시각실조증, 안구실행증 ), Gerstmann 증후군 ( 손가락실인증, 실서증, 좌우지남력장애, 계산장애 ), 시각실인증 (visual agnosisa), 실독증, 초피질성감각성실어증 (transcortical sensory aphasia) 등다양한대뇌후두부의피질기능이상을초기부터관찰할수있으나, 알쯔하이머치매초기부터나타나는기억력장애나병식의소실은질병의말기까지상대적으로심하지않은특징을보인다. 1)3) 병리소견상노인반 (senile plaque) 과신경섬유농축체 (neurofibrillary tangle) 가주로나타나므로그동안알쯔하이머치매의한변형으로생각되어왔으며, posterior cortical atrophy, 4) Alzheimer s disease with posterior cortical atrophy, 5) visual variant of Alzheimer s disease, 6) progressive posterior cerebral dysfunction, 7) progress biparietal atrophy로다양하게불려왔다. 후부대뇌피질위축은특징적신경학적, 임상적증상과경과가있지만비전형적치매양상을보여이질병에대해숙지하지않으면임상현장에서놓치기쉬운질환이라생각된다. 이에저자들은후부대뇌피질위축의전형적인임상증상과경과를보인환자를뇌영상학적검사로확진하고콜린분해억제제로치료를한증례를문헌고찰과함께보고한다. 증례 우가간혹있었는데, 근래에는낮에도길을자주잃어버려혼자서는전혀외출을할수없었다. 그럼에도불구하고기억력의장애는심하지않다고환자의남편은보고하였다. 환자자신은상기한여러문제들에도불구하고비교적안정된모습이었고, 자신에게어떤문제가있는지정확히인식하여구체적으로불편한점을이야기하였다. 환자의어머니도사망전에길을잘찾지못하는증상이있었으나정확한검사와치료를하지는않았다. 신체검사상특이소견은없었고, 일반혈액검사, 간기능검사, 신장기능, 전해질, 매독, 비타민 B12 검사상모두정상이었다. 갑상선기능검사상 T3 가 217.4ng/dl ( 기준치 80~200ng/dl) 로약간상승되었으나 T4, TSH 모두정상범위로치료가필요한상태는아니었다. 아포지단백 E 유전자 (Apolipoprotein E genotype) 는 e3/3 였다. 신경학적검사를시행하였는데, 뇌신경기능검사상시야장애는관찰되지않았고, 논문에씌어진작은글씨도판별이가능한것으로보아시력에는문제가없어보였으나, 글씨를읽을때빠뜨리고읽거나줄을통째로건너뛰는양상을보였다. 환자에게 작은 8자들로이루어진 3자 (Navon figure 검사 ; 그림 1) 를보여주었을때, 커다란 3자는보지못하고 8자만보인다고이야기하였는데, 동일한검사를여러번반복하여도매번같은오류를반복하였다. 앞바퀴가빠진자전거옆에서도움 환자는 6년의교육을받은오른손잡이 58세여자로, 약 6년전시작된주변의물건을잘찾지못하는증상이점차심해져내원하였다. 환자는눈이침침해안과를방문하였지만시력에는문제가없다는진단을받기도했다. 그럼에도불구하고책을읽는데어려움이지속되었고, 읽고난후에도무슨이야기인지알수없다고자주호소하곤했다. 물건을찾는데도어려움이있어서, 남편이환자바로옆에있는물건을가져다달라고해도보지못하고엉뚱한곳을찾곤했고, 최근에는바로옆에놓인요리재료를찾을수가없어서환자의남편이곁에서도와주지않으면식사준비를할수없는지경에이르렀다. 식탁에음식을차릴때에도밥과국을식탁한가운데에놓고주위에반찬을놓는가하면, 수저와젓가락도제자리에놓지못하는경우가많았다. 문우측에손잡이가있음에도좌측을더듬거리며찾기도하고, 자가용문을찾지못해차주위를빙빙돌기도했다. 과거에는밤에외출하여길을잃어집을찾는데어려움을겪는경 Fig. 1. 3 을청하는우편배달부소년그림 (telegraph boy figure) 을보여주며보이는것을모두설명하라고하였을때, 환자는자전거바퀴를가리키며 마차 라고이야기할뿐, 그림의다른요소들은전혀인지하지못하는동시실인증 (symultanagnosia) 을관찰할수있었다. 하지만시각실조증 (optic ataxia) 이나안구실행증 (ocular ap-raxia) 은관찰되지않았다. 운동기능, 감각기능, 소뇌기능, 병적반사등의신경학적검사상특이소견은없었다. 환자의고위피질기능은신경심리검사 (SNSB;Seoul Neuropsychological Screening Battery) 로평가하였다 ( 표 1). 신경심리검사결과를보면, K-MMSE 상 20 점 ( 지남력 -3, 주의집중및계산 -2, 기억회상 -2, 언어 -1, 오각형그리기 -1) 으로, 나이와학력을보정하 더라도 1.5 표준편차보다낮았다. 환자의노인형우울척도검사 (Geriatric Depression Scale) 상 19점으로경도의우울증을보였으며, 신경정신행동검사 (Neuropsychiatric Inventory, NPI) 에서도우울, 불안, 무감동 / 무관심, 식습관과식욕변화가있었다. 환자의주의집중력은정상범주였으나, ᆨ, ᆫ, ᆮ, ᆯ, ᆷ, ᆸ, ᄏ을혼재해놓고 ᆨ 을찾아지우는글자지우기 (letter cancellation test) 에서 6번의오류를보였다. 언어와연관된기능에서보스턴이름대기검사는거의수행할수없었는데, 환자가장화를보여주었을때 머리를묶는끈, 거미줄을 양산, 저울을 시계, 하모니카를 계단 등으로명칭하는등의동시실인증을시사하는반응을보였다. 또한좌우지남력장애 (right-left disorientation) 와 Table 1. 환자는 2003년에어지럼증으로본원외래를방문하여뇌 MRI(magnetic resonance imaging) 를촬영하였는데, 당시양측대뇌피질의경한위축이외에는특이소견을관찰할수없었다 ( 그림 2).

F 18 -fluorodeoxyglucose positron emission tomography(f18-fdg PET) 검사상양측후두엽, 두정엽, 측두엽에서포도당대사감소가관찰되었는데, 특히후두엽과두정엽그리고우측에서보다현저하였다 ( 그림 4).

Rey figure 보고그리기검사는전혀수행하지못하였다. 언어기억검사에서도즉시회상, 지연회상, 재인회상에서모두저하된소견이었다. 환자는 2003년에어지럼증으로본원외래를방문하여뇌 MRI(magnetic resonance imaging) 를촬영하였는데, 당시양측대뇌피질의경한위축이외에는특이소견을관찰할수없었다 ( 그림 2). 4년이지난이번방문시다시시행한뇌 MRI 검사에서는전반적으로대뇌피질의위축이있었고, 특히양측후두엽과두정엽일부에서심한위축이관찰되었으나상대적으로해마의위축은심하지않았다 ( 그림 3). F 18 -fluorodeoxyglucose positron emission tomography(f18-fdg PET) 검사상양측후두엽, 두정엽, 측두엽에서포도당대사감소가관찰되었는데, 특히후두엽과두정엽그리고우측에서보다현저하였다 ( 그림 4). 환자에게콜린분해억제제 (cholinesterase inhibitor) 인 donepezil 5mg/ 일을 1개월간 사용한후증량하여 10mg/ 일을 5개월간투약하였다. 총 6개월의투약후신경심리검사를다시시행하였는데, 주관적인기억장애의호전을보였고, 추적시행한신경심리검사에서도미약하지만언어기억의호전을보였다. 그러나그림따라그리기를비롯한시공간영역의평가에서는변화가없었다 ( 표 1). 고찰 후부대뇌피질위축은피질-기저핵변성 (corticobasal degeneration), 일차성진행성실어증 (primary progressive aphasia) 과함께뇌의특정부분이국소적으로위축되어증상을나타내는국소대뇌피질증후군 (focal cortical atrophy syndrome) 중하나이다. 8) 1988년 Benson 등이실독증 (alexia), 실서증 (agraphia), 시각실인증 (visual agnosia), Balint 증후군, 초피질성감각성실어증 (transcortical sensory aphasia) 등을보이는진행성치매 5 례를보고하면서, 환자들의영상소견에서대뇌후두부피질의위축이현저한것을관찰하고, 처음으로 posterior cortical atrophy 라고명명하였다. 4) 환

T1-weighted image A) sagittal, B) axial, and C) coronal views of the brain MRI. 자는, 병명에서알수있듯이후두엽과두정엽을침범함으로서나타나는다양한증상들을보일수있다. 일반적으로시각경로를배측경로 (dorsal pathway;where pathway) 와복측경로 (ventral pathway;what pathway) 로나누는것과마찬가지로, 후부대뇌피질위축도뇌영상에서보이는위축소견과임상증상을바탕으로배측아형 (dorsal subtype) 과복측아형 (ventral subtype) 으로나눈다. 배측아형은후두엽과두정엽의위축과그 로인한증상이주가되는것으로, Balint 증후군, Gerstman 증후군, 옷입기실행증, 그리고실어증등을보일수있으며, 복측아형은후두엽과측두엽의위축으로인해, 실서증을동반하지않는실독증 (alexia without agraphia), 시각적물체실인증 (visual object agnosia), 그리고얼굴실인증등이관찰될수있다. 1) 하지만알쯔하이머치매와는다르게초기에기억력, 병식, 판단력이상대적으로잘유지되는것으로보고되고있고, 일반적

Proposed diagnostic criteria for posterior cortical atrophy Core features Insidious onset and gradual progression Presentation of visual complaints in the absence of significant primary ocular disease explaining the symptoms Relative preservation of anterograde memory and insight early in the disorder Disabling visual impairment throughout the disorder Absence of stroke or tumor Absence of early parkinsonism and hallucinations Any of the following findings: Simultanagnosia with or without optic ataxia or ocular apraxia Constructional dyspraxia Visual field defect Environmental disorientation Any of the elements of Gerstmann syndrome Supportive features Alexia Presenile onset Indeomotor or dressing apraxia Prosopagnosia Investigations Neuropsychological deficits referable to parietal and/ or occipital regions Focal or asymmetric atrophy in parietal and/or occipital regions on structural imaging Focal or asymmetric hypoperfusion/hypometabolism in parietal and/or occipital regions on functional imaging 으로질병의후기까지유지되는경향을보인다. 본환자는부분적인 Balint 증후군과 Gerstman 증후군, 그리고실행증을보이므로배측아형으로판단하였는데, 이는영상소견과도잘일치되었다. 또한, 영상소견으로확인되는대뇌위축은일반적으로좌측보다우측이더심한것으로알려져있는데, 2)9) 이는본환자의 MRI 의 T1강조영상두정단면 (coronal section ; 그림 3C) 의우측측두엽에서도잘관찰할수있다. 병리학적인소견상대부분의경우에서노인반과신경섬유성농축체가주소견으로관찰되므로일반적으로알쯔하이머치매의변형이라고여겨져왔다. 3) 하지만 Mario 등은임상적으로확진된 15명의후부대뇌피질위축환자를알쯔하이머치매와비교하면서, 후부대뇌피질위축환자에서는후두부피질증상이현저하게나타나는반면알쯔하이머치매의주증상인언어유창성, 기억력, 및 병식의소실은상대적으로덜하다고보고하였다. 또한우울증이상대적으로심하게나타나므로조기에항우울제를보다적극적으로사용하는등알쯔하이머치매와는다른치료전략을사용해야한다고주장하며이러한차이로인해후부대뇌피질위축후부대뇌피질위축은알쯔하이머치매의변형이라기보다는새로운질환군일가능성을제시하였다. 10) 본환자에서도신경정신행동검사상, 우울, 불안, 무감동 / 무관심, 식습관과식욕변화를보인점과, 환자의기억력과병식이상대적으로보존되었다는점에서상기한특징에잘부합하였다. 후부대뇌피질위축과알쯔하이머치매에서보이는시각피질증상에대한임상양상을비교한다른연구에서도, 전형적인알쯔하이머치매에서는후두- 측두엽의침범으로나타나는복측경로증후군이우세한반면, 후부대뇌피질위축에서는후두엽과두정엽을침범하는배측경로증후군에서보이는시각증세가우세하므로알쯔하이머치매에서보이는시각증상과는양상이다르다고하였다. 11) 이러한주장은두질환간의병리학적인비교연구가발표되면서더욱힘을얻고있는데, Tang Wai 등은 40명의후부대뇌피질위축환자에대한임상소견의분석과함께 9명의병리소견을발표하였다. 9명중 2명은피질기저핵변성이었고, 나머지 7명에서알쯔하이머치매에서와같은소견이보였으나, 차이점으로는신경섬유농축체가해마에서는거의관찰되지않고 Broadmann 영역 17, 18에서통계적으로의미있게증가되어있음을보고하였다. 이에후부대뇌피질위축이알쯔하이머치매와는다른새로운치매질환군이라고주장하였고, 임상증상에따른새로운진단기준을제시하였다 ( 표 2). 1) 치료반응과관련해서는김등이아세틸콜린분해억제제를 6개월사용한후에시공간지각력의호전을보인후부대뇌피질위축환자를보고하였고, 이를토대로후부대뇌피질위축은알쯔하이머치매의한변형일가능성이있다고주장하였다. 12) 하지만본환자의경우언어기억의미약한호전을보일뿐, 시공간지각력은차이가없어치료경과와관련해서도향후연구가필요할것으로생각한다 ( 표 1). 결론적으로현재까지의여러연구결과를토대로후부대뇌피질위축을알쯔하이머치매의한변형으로보는견해가아직은우세하지만, 고찰한바와같이알쯔하이머치매와구별되는특징으로인해치매의또다른질환군으로분류하려는연구또한활발하게진행되고있다. 국내에서도후부대뇌피질위축환자에대한두증례가

8 신경과영역에서보고된바있다. 13)14) 하지만본증례는, 약 40개월의간격을두고두정엽및후두엽의피질위축이진행되는과정을 MRI 소견으로관찰하였고, FDG -PET 을사용하여이들부위, 특히우측부위의대사감소를확인하였다. 또한 6개월간아세틸콜린대사억제제를사용한뒤신경심리검사를추적하여약물에대한반응을관찰하였다는점에서의의가있다고하겠다. 후부대뇌피질위축환자는정신의학적증상을수반하는경우가많기때문에이들환자의정신의학적특징에대한숙지도함께필요하다고할수있다. 후부대뇌피질위축환자의 25% 에서환시가동반된다. 15) 환시가동반되는후부대뇌피질위축환자는파킨슨증상, REM 수면장애, 간대성근경력반사 (myoclonic jerk) 가나타날수있고, 뇌영상소견상일차시각피질, 렌즈핵, 시상, 중뇌등에위축이보다심한것으로보고된다. 13) 알쯔하이머치매환자에서보다우울증상의공존가능성이높고, 일부에서는우울증이선행하여나타나기도해비전형적인지기능손상에대한세심한주의를기울이지않으면진단이지연될수있다. 10)16) 특히이들환자들의인지기능저하는전형적인노인성치매양상으로나타나지않으므로, 자세한신경학적검사가없이는정확한진단이어렵다. 따라서, 비전형적인인지기능저하와피질시각장애를보이는노인환자들은본증례에서제시한여러신경학적특징을확인하고영상학적검사를시행하여후부대뇌피질위축을배제할필요가있다. 중심단어 : 후부대뇌피질위축 피질성시야장애 발린트증후군 거스트만증후군 관념운동실행증 후두 -두정부대사저하증. 참고문헌 1.
