This study examined the long-term impact of the outbreak of Middle East Respiratory Syndrome (MERS) in Korea, 2015 on the mental health of the survivors. Methods Sixty-three survivors from MERS were recruited from a prospective cohort study at six hospitals one year after the outbreak in The Korean-Symptom Check List 95 was administered to evaluate their psychiatric problems and analyzed according to the patient s characteristics and exposure to traumatic events during the outbreak. Results A total of 63.5% of survivors suffered from significant psychiatric problems: post-traumatic symptoms (36.5%), sleep problems (36.5%), anxiety (34.9%), and depression (30.2%). Survivors with a history of a ventilator treatment during the MERS epidemic, a family member who died from MERS, and a past psychiatric history showed higher post traumatic stress disorder, anxiety, depression, and suicidality than people who do not have those histories. Conclusion The study suggests that MERS survivors could have a high chance of adverse psychiatric consequences, even after their recovery from MERS. Exposure to traumatic events during the outbreak and premorbid individual vulnerability would affect the long-term mental health problems. J Korean Neuropsychiatr Assoc 2019;58(3): KEY WORDS Middle east respiratory syndrome Emerging infectious diseases Mental health Posttraumatic stress disorder Depression. 서론 년 메르스 사태는 명의 환자 명의 사망자 명의 격리자를 발생시킨 국가적인 재난 상황이었다 메르스는 코로나 바이러스가 원인인 바이러스로 주로 비말로 전파되고 심각한 경우에는 중 증호흡기 질환에까지 이르게 하는 신종감염병에 속한다 많은 사람들이 좁은 공간을 함께 쓰는 한국 차 병원의 특성으로 인해 사태 초기에 메르스는 빠른 속도로 전파되었다 빠른 전파 많은 수의 격리자들 높은 사망률 의사 소통의 실패 불확실성으로 인해 많은 한국 시민들과 환자들이 큰 공포와 불안을 경험했다 년 메르스 사태 당시 국립중앙의료원 Copyright 2019 Korean Neuropsychiatric Association 245

2 J Korean Neuropsychiatr Assoc 2019;58(3): 에 입원한 메르스 확진 환자 명에 대한 차트 리뷰에서는 가 불면 우울 긴장 공격성 환청 기억력 저하 지남력 저하 등의 정신과적인 문제가 있었고 는 이로 인해 약물치료가 필요한 수준으로 평가되었다 년 홍콩에서 발생한 급성중증호흡기증후군 이하 과 년 서아프리카 지역에서 발생한 에볼라 출혈열에 대한 기존 연구들에서는 신종감염병 생존자들은 감염 당시뿐만 아니라 회복 이후에도 우울 불안 외상 후 스트레스 증후군 불면 피로 집중력 저하 등의 다양한 정신과적 문제들을 겪는 것으로 나타났으며 이는 감염에서 회복된 이후 장 단기적 기간동안 지속된다고 보고되었다 년 한국 메르스 사태의 심각성을 고려할 때 메르스에서 회복된 이후의 환자들도 상당한 정신적 어려움을 겪을 수 있다고 추측할 수 있다 그러나 메르스 환자들의 정신건강에 대한 추적 관찰 연구는 매우 부족한 실정이다 이에 정신과적 문제가 장기화되기 이전인 단기 기간 기준에 해당하는 년의 시점 에서 생존자들의 정신과적 문제의 심각성을 평가를 하고자 하였다 회복 후 환자들이 겪는 정신과적 문제는 환자 특성과 사건 경험에 따라 그 종류와 심각도가 다를 수 있다 생존자 연구에서 장기적인 외상 후 스트레스 장애의 위험인자로 성별 기존 신체적 질환 이환 여부 스테로이드 치료의 부작용인 무혈관성 괴사 등이 있다고 밝혀졌고 우울과 불안 위험 위험도를 증가시키는 요인으로는 정신과적 자문을 받은 기왕력 의료계 종사 여부 감염자를 알고 있는지 여부가 보고되었다 에볼라 출혈열 생존자 연구에서도 해당 사태 동안 친척을 잃은 경험이 집중력 저하와 우울감을 예측하는 인자로 밝혀졌다 본 연구에서는 년 메르스 사태의 생존자들이 감염 년 후 시점에 어떠한 정신과적 문제를 갖고 있는지 조사하였다 또한 메르스 감염 당시 환자의 특징과 외상성 사건의 경험에 따라 정신과적 문제의 차이가 있는지 검증하고자 하였다 방법 연구설계및대상본 연구는 년 한국에서 발생한 메르스 사태 생존자들을 대상으로 한 전향적 전국적 코호트 연구의 일부로 진행되었다 한국질병관리본부로부터 메르스 바이러스 감염 확진을 받고 이후 회복한 사람들을 대상으로 하였다 메르스 사태 개월 후 명의 메르스 생존자들에게 연락하여 그 중 명이 코호트에 포함되었다 추적 관찰에서 누락된 대상자들을 제외한 명을 메르스 사태 개월 후에 평가하였고 그 중 명이 심리검사에 동의하였다 연구 데이터는 메르스 사태로부터 년 후인 년 월부터 월까지 총 개의 병원인 국립중앙의료원 서울대학교병원 서울의료원 단국대학교병원 충남대학교병원에서 수집하였다 연구 참가자에게 연구참여 전에 연구에 대한 설명을 하고 연구참여에 대한 서면동의서를 얻었다 본 연구는 개 병원 소속 연구윤리심의위원회로부터 연구 승인을 받았다 서울대병원 서울의료원 국립중앙의료원 단국대학교 충남대병원 측정도구연구대상자들은 사회인구학적 정보 임상 특성 메르스와 관련된 질병경험과 정신과적 증상들을 조사하는 자기보고 설문지를 작성하였다 사회인구학적 정보와 임상특성으로 나이 성별 결혼여부 월 수입 세 미만의 아이 유무 질환력 메르스 감염 이전 정신과적 과거력에 대한 정보를 수집하였다 정신과적 과거력은 정신과 방문 심리상담을 포함한 정신건강관련 서비스를 이용한 경험과 약물 복용 경험 여부와 진단을 함께 설문지로 파악하였다 메르스 관련 정보로 감염되었을 때의 상태인 기관삽관 여부 체외막산소공급 이하 치료 여부 메르스로 인한 가족 구성원의 사망 경험 유무 증상으로부터 확진까지 걸린 시간 입원 일수 진단 발생 후 완치까지의 일수를 조사했다 정신과적 결과는 간이정신진단검사 이하 를 이용하여 측정하였다 는 문항으로 구성된 자가보고식 척도로 정신과적 질환을 선별하는 도구로 고통을 받는 정도에 대해서 개의 점수 점 아니다 점 거의 항상 그렇다 로 구성된 리커트 척도로 평가하고 개 증상범주로 분류한다 그 중 메르스 생존자와 관련성이 있을 것으로 평가되는 개의 증상 범주를 이용하였다 우울 불안 공포증 외상후스트레스 증후군 신체화 공격성 자살성 중독 수면 문제 각 범주의 증상은 온라인 점수화 를 통해서 점수로 계산했고 이를 낮음 < 보통수준 < 주의 및 위험수준 < 문제수준 총 개의 수준으로 분류하였다 각 증상의 점수가 주의 및 위험수준 또는 문제수준에 해당할 경우 해당 증상을 가지고 있다고 평가하였다 척도의 α는 로 평가되었다 통계분석생존자들의 사회인구학적 정보 메르스 이환 당시 임상적 특징에 대해 빈도분석을 시행하였다 메르스 감염 당시 환자 246

3 메르스생존자의정신건강 J Shin, et al. 의 특징과 사건 경험에 따라 정신과적 문제를 비교하기 위하여 문헌리뷰를 통하여 정신과적 문제와 관련이 높을 가능성이 있는 가지 인자 세 이상 이전 정신과적 병력 감염 당시 의료인 여부 기관삽관 유무 메르스로 인한 가족 사망 유무 를 선정하였다 모든 사망자가 세 이상으로 조사된 메르스 환자 연구를 토대로 연령을 세 기준으로 나누어 비교하였다 각 인자 별 두 군의 사회인구학적 정보 비교에는 를 사용하였고 항목별 점수를 비교에는 검정을 사용하였다 유의수준은 < 로 하여 를 사용하여 분석하였다 결과 연구참여자들의사회인구학적특성과임상적특성참여자는 명 여성 명 으로 평균 연령은 ± 세였다 연령이 세 이상인 참여자는 명 이었다 명은 메르스 사태 이전에 신체적 질환 병력이 있으며 명은 이전 정신과적 병력이 있었다 명 은 폐렴에 이환되었고 명 은 기관삽관 명 은 치료를 시행받았다 표 1년추적관찰후정신과적증상유병률메르스 사태로부터 년 이후 로 평가하였을 때 한 가지 이상의 증상을 갖고 있는 생존자는 명 이었다 그림 가장 높은 이환율을 보인 것은 외상 후 스트레스 증상 과 수면문제 였으며 두 번째로 빈도가 높은 문제는 불안 이었다 다음으로 빈도가 높은 순서대로 우울 자살성 신체화 증상 공포증 공격성 과 중독 이 뒤를 이었다 생존자특성과사건경험에따른정신과적문제비교메르스 생존자들의 사건 경험에 따라 군을 나누어 사회인구학적 임상적 특성을 비교했을 때 정신과적 과거력이 있는 경우가 없는 경우에 비하여 여성 과 배우자가 없는 비율이 높았다 표 인공호흡기 치료 경험이 있던 군은 그렇지 않는 군에 비해서 외상 후 스트레스 증상 과 공포 점수가 유의미하게 높았다 표 가족이 메르스로 사망한 경험이 있는 군도 그러한 경험이 없는 군에 비해서 외상후스트레스 증상 과 자살성 점수가 높았다 정신과적 과거력이 있는 경우는 없는 경우에 비해서 불안 우울 자살성 점수가 높았다 연령과 의료진 여부에 따른 정신건강 문제에는 차이 가 없었다 Table 1. Psychiatric problems of survivors based on the score of the Korean-Symptom Check List 95 one year after Middle East respiratory syndrome outbreak in South Korea, PTSD : Post traumatic stress disorder. 고찰 본 연구를 통해서 메르스에서 회복된 지 년 이후에도 의 생존자들이 상당한 수준의 정신적 문제를 겪고 있음을 알 수 있었다 주요 정신과적 증상은 외상 후 스트레스 증상 수면문제 불안 등이었다 또한 생존자의 메르스 이환 당시 환자요인 메르스 사태 경험 임상적 특성 차이에 따라 정신과적 문제를 비교했을 때 주목할 만한 결과를 보였다 인공호흡기 치료 경험 가족 사망 경험 이전 정신과적 병력이 있는 경우에는 정신과적 문제 수준에 차이를 보였다 인공호흡기 치료 경험이 있는 군은 없는 군에 비하여 외상 후 스트레스 증상과 공포 문제가 많았다 이는 중환자실에 입실한 만큼 메르스 심각도가 높았다는 것을 의미하기 때문에 질환 심각도 자체가 회복에 부정적인 영향을 미쳤을 가능성이 있다 메르스의 치사율이 높았던 만큼 자신의 질병이 심각하다는 인식 역시 불안과 공포를 악화시켰을 수 있다 중환자실을 경험한 환자들에 관한 연구에서 고립된 환경이 외상 후 스트레스 장애 불안 장애 등의 후유증을 남긴다는 보고들이 있다 메르스 환자의 경우 일반적인 중환자실 환경보다도 더욱 고립된 비인간적인 환경 의료진들의 낯선 보호장구 가족 면회 차단 등 에 놓여 있었기에 공포와 죽음에 대한 불안은 더욱 컸으리라 예상된다 메르스로 인한 가족 사망경험 유무에 따라서 외상 후 스트레스 증상 자살성에서 차이가 있었다 일반적으로 가족의 사망은 큰 심리적인 부담을 남기고 오랜 애도 과정을 거친다 특히 메르스 환자들은 사망한 가족에게 자신이 질병을 전파했을 가능성이 있다는 점에서 가족의 죽음은 죄책감 공포 분노 등 더욱 복잡한 감정을 일으켰을 것이다 이들은 메르스에 감염된 피해자 임에도 자신들을 전파자 가해자 로 보는 시선들과 편견 또한 이들의 정서적 어려움에 기여했을 것으로 추측된다 격리된 상태로 임종을 지키지 못하고 장례식에 참석하지 못하는 등 상실로 인한 슬픔을 나누고 표현할 수 있는 기회마저 차단당하였기에 년 후에도 외상 후 스트레스 증상 자살성 등이 높았다고 추정할 수 있다 정신과적 과거력이 있는 군은 없는 군에 비하여 우울과 불안 자살성 수준이 높았다 정신과적 과거 진단 중 우울증과 불안장애의 비율이 를 차지하고 있다는 점을 고려하면 이에 대한 취약성을 가졌던 환자라고 볼 수 있다 이들이 높은 치사율의 메르스 감염을 겪으면서 불안과 우울 등 심리적 어려움을 보다 심하게 겪었고 이후에도 충분히 회복되지 않고 남아 있을 수 있겠다 높은 연령은 메르스 사망률에 영향을 미치는 위험인자로 248

MERS : Middle East respiratory syndrome, PTSD : Post traumatic stress disorder 알려져 있으나 연령에 따른 정신과적 문제 차이는 없었다 정 신과적 문제에는 메르스를 경험하는 질적인 차이나 개인적인 소인이 더 중요하게 작용하고 있음을 시사한다 또한 의료진 은 질병과 격리의 상황에 보다 익숙하여 정신과적 문제가 적 을 것이라고 추측할 수 있겠지만 비의료진과 비교 시 정신과 적 문제 수준의 차이는 없었다 메르스 사태 당시 의료진의 외상 후 스트레스 증상 우울 불안 증상도 상당한 수준에 이 르렀다 이는 의료진들의 정신건강에도 관심이 필요함을 시 사한다 본 연구는 메르스 생존자의 만 참여했기 때문에 일반 화하기에 한계가 있다 그러나 우리나라에 큰 파장을 일으킨 메르스 사태를 경험한 생존자들이라는 점에서 희소한 가치가 있다 또한 환자의 인구학적 정보와 메르스 이환 경험에 따른 정신과적 문제의 차이를 본 기술적 연구이기 때문에 이를 통 하여 인과관계를 파악하기에는 어려움이 있다 정신과적 평 가에 객관적인 증상평가 없이 자가 보고식 평가 도구만 사용 한 점은 연구의 제한점으로 남는다 정신과적 과거력 메르스 중 중환자실 치료나 가족 사망경험이 있던 메르스 생존자의 경우에 정신건강 측면에 특별한 주의가 필요할 수 있겠다 추후 메르스 생존자들이 겪는 정신과적 문제들과 관련된 요 인과 효과적인 개입에 관한 후속 연구가 필요하다 결 론 본 연구는 메르스 환자들이 메르스 감염으로부터 회복 이 후에도 높은 수준의 정신과적 문제를 보인다는 것을 보여준 다 메르스 사태 동안의 외상적 경험 감염 이전 개인의 취약성 이 장기적인 정신과적 문제에 영향을 줄 수 있음을 시사한다 중심단어 메르스 신종감염병 정신건강 외상 후 스트레 스 장애 우울증 Acknowledgments 본 연구는 한국 보건복지부에서 지원하는 한국보건산업진흥원 한국보건기술 프로젝트 와 보건복지부 한국정신건강기술 프로젝트 의 지원을 받았음 Supplementary Materials The online-only Data Supplement is available with this article at Conflicts of Interest The authors have no financial conflicts of interest.
