1 ISSN eissn Original Article Evaluation of Bonwill triangle using cone beam computerized tomography in Korean Hyun-Jun Kong, Sang-Chun Oh* Department of Prosthodontics, College of Dentistry, Wonkwang University, Iksan, Republic of Korea Purpose: The aim of this study was to evaluate the Bonwill triangle of Korean using the cone beam computerized tomography (Cone-beam CT). Materials and Methods: 120 Koreans (60 males and 60 females) who visited Daejeon Dental College Hospital of Wonkwang University and who underwent the Cone-beam CT were selected. The Cone-beam CT images were analysed with Invivo 5.1 (Anatomage, San Jose, USA). After reorientation of axis, the intercondylar distance was measured by clicking both middle points of condyle. And the condyle-incisor distance was measured by clicking the middle point of condyle and contact point of the mandibular central incisor s incisal edge. The collected data were analysed using the SPSS Version 23.0 (IBM Inc., Armonk, USA) and statistical significance was verified by gender using independent t-test. Results: The mean intercondylar distance of Korean was mm, and the male (108.3 mm) was statistically significantly larger than the female (103.4 mm). The mean condyle-incisor distance of Korean was mm, and the male (108.1 mm) was statistically significantly larger than the female (102.3 mm). Conclusion: The mean intercondylar distance of Korean in this study was mm that was smaller than well-known 110 mm of Caucasian and the male was statistically significantly larger than the female. Within the limitations of this study, it would be necessary to use the articulator which can adjust the intercondylar distance according to the individual for prosthodontic treatment of Korean. (J Dent Rehabil Appl Sci 2018;34(2):97-103) Key words: Bonwill triangle; articulator; intercondylar distance; cone-beam CT 서론 본윌삼각 (Bonwill triangle) 은하악중절치절치연의접촉점과양쪽과두를연결하는선에의해서경계되어지는 4인치의이등변삼각형으로 1858년에 Bonwill의해부학적교합기에처음소개되었다. 1 Bonwill의교합기는평균치교합기이며, 과두간거리및과두-절치간거리를 100 mm로고정하였다. 2 이중과두간거리는교두융선과구의방향및전치부설면형태에영향을미친다. 과두간거리가증가함에따라, 과두와치열궁의치아사이의거리를증가시켜내외 측방로사이각을더크게하지만회전과두와치열과의거리증가에비해치아가상대적으로정중시상면에가깝게위치되어이의영향을많이받기때문에내외측방로사이의각도를감소시키는경향이발생하게된다. 또한과두간거리가커질수록상악전치의설면함몰부는커지고과두간거리가작아질수록상악전치의설면함몰부는작아진다. 과두-절치간거리는교두융선과구의방향에영향을미치며, 하악회전축 ( 회전과두 ) 으로부터치아가멀리떨어져있을수록상악과하악에있어외측방로와내측방로가이루는사이각이커진다. 3 *Correspondence to: Sang-Chun Oh Professor, Department of Prosthodontics, College of Dentistry, Wonkwang University, 77 Doonsan-ro, Seo-gu, Daejeon, 35233, Republic of Korea Tel: , Fax: , scoh@wku.ac.kr Received: February 6, 2018/Last Revision: March 3, 2018/Accepted: March 5, 2018 Copyright 2018 The Korean Academy of Stomatognathic Function and Occlusion. cc It is identical to Creative Commons Non-Commercial License. 97

2 Kong HJ, Oh SC 그간과두간거리에관한연구가진행되어왔는데, Keshvad 4 는백인을대상으로평균과두간거리를 mm라고보고하였고, Tradowsy 5 는미국인을대상으로남성 108 mm, 여성 102 mm로보고하였으며, Lazic 등 6 은크로아티아인을대상으로남성 mm, 여성 mm로보고하였다. 그러나서양인과한국인의치아및악궁의크기와형태, 연조직외형등에는차이가있으므로, 과두간거리및과두-절치간거리또한인종간에차이가있을것으로추측할수있다 그러므로본연구에서는최근에널리사용되고있는콘빔형전산화단층영상 (Cone-beam computerized tomography) 을이용하여한국인의과두간거리및과두- 절치간거리를측정하였으며이를성별에따라비교분석하여한국인의본윌삼각을평가하고자하였다. 연구재료및방법 1. 연구대상 원광대학교대전치과병원에서 Cone-beam CT (PaX- Zenith 3D; Vatech Korea Co., Ltd., Seoul, Korea) 를촬영한환자중에서, 본연구의기준에부합되는 120명 ( 남성 60명, 여성 60명 ) 을대상으로개인의과두간거리, 과두- 절치간거리를측정하였다. 본연구는원광대학교대전치과병원 IRB심사 (IRB No. W1721/ ) 를거쳐진행되었다. 연구대상선정의기준은 Ferrario 등 11,12 의연구에서사용한기준을참고하여다음과같이설정하였다. 포함기준 : 1 한국인 2 제3대구치를제외한치아결손이없는영구치열 3 Angle 분류 class 1 제외기준 : 1 교정치료및악교정수술기왕력 2 악관절장애로인한치료및하악과두를포함하는외과적수술기왕력 3 상악과하악의중절치에교두를피개하는수복물이있는경우 4 그밖의이유로계측포인트가 Cone-beam CT 상에매우불량하여계측이힘든경우연구대상에포함된 120명의성별, 연령별분포는다음과같다 (Table 1). 2. 연구방법 1) 악궁의디지털화 Cone-beam CT 로부터얻어진이미지를 digital imaging and communications in medicine (DICOM) format 으로추출하였다. Cone-beam CT scan 을분석하기위 해 Invivo 5.1 (Anatomage, San Jose, USA) 을사용하였 다. 축의방향설정 (reorientation of axis) 은 Bayome 13 등 의연구에서사용한방법을참고하였다. 먼저관상영상 (coronal view) 에서 X 축은좌측및우측 Orbitale 를지 나는수평선으로설정하였다 (Fig. 1A). 다음으로시상영 상 (sagittal view) 에서 Y 축은 Nasion 과 Anterior Nasal Spine (ANS) 를지나는수직선으로설정하였다 (Fig. 1B). 마지막으로축영상 (axial view) 에서 Y 축은 ANS 와 Posterior Nasal Spine (PNS) 을지나는수직선으로설정하였 다 (Fig. 1C). 2) 계측기준 과두간거리와과두 - 절치간거리를계측하기위해계 측점을설정하였다 (Fig. 2). 과두간거리의계측을위해관상영상에서영상조절 (view control) 을경조직 (bone) 으로설정하였다. 이후 5 mm 간격으로클리핑 (clipping) 하면서, 각과두의최대외 연이보이는상을선택한후, Nagi 등 14 의연구를참고하 여각과두의최대외연과내연의중심점을찾아, 각과두 의중심점을클릭하여과두간거리를측정하였다 (Fig. 3). 과두 - 절치간거리는각과두중심점에서하악양중절 치절단연의접촉점까지의거리를의미하는것이다. 이를 측정하기위해관상영상에서과두중심점을클릭하고영 상조절을치아 (teeth) 로변환한후, 하악양중절치절단 연의접촉점을클릭하여각과두 - 절치간거리를계측하 였다 (Fig. 4). 4 Kong HJ, Oh SC 3) 통계분석과두간거리와과두-절치간거리의성별차이의통계적유의성을검증하기위하여 independent t-test를시행하였다. 통계처리에는 SPSS Version 23.0 (IBM Inc., Armonk, USA) 을이용하였다. 결과 한국인의평균과두간거리는 mm였으며, 남성 (108.3 mm) 이여성 (103.4 mm) 에비해통계학적으로유의하게크게나타났다 (Table 2). 한국인의평균좌측과두-절치간거리는 mm였으며, 남성 (108.6 mm) 이여성 (102.4 mm) 에비해통계학적으로유의하게크게나타났다. 한국인의평균우측과두-절치간거리는 mm였으며, 남성 (107.6 mm) 이여성 (102.2 mm) 에비해통계학적으로유의하게크게나타났다. 한국인의평균과두-절치간거리는 mm였으며, 남성 (108.1 mm) 이여성 (102.3 mm) 에비해통계학적으로유의하게크게나타났다 (Table 3). 한국인의평균좌측과두-절치간거리 (105.5 mm) 와우측과두-절치간거리 (104.9 mm) 는통계학적으로유의한차이가없었다 (Table 4). 고찰 교합기는측두하악관절과하악의움직임을재현할수있는기계적장치로서악골운동을구강외에서재현함으로써인체의기능에조화된보철물을제작하기위하여사용한다. 15 하악골의모든운동은일정한한계내에서일어나며, 교합기가이러한한계운동을더유사하게재현할수록정확한교합과하악운동을분석할수있으며구강내에서장애를일으키지않는좋은보철물을제작할수있게된다. 교합기의설정된과두간거리가환자의그것에비하여크거나작을경우, 교두의움직임을재현하는데에미세한오차가생길수있다. 교합기의과두간거리가하악보다작을경우교합기운동의축이환자의구강운동에비해미세하게근심으로위치하게되며, 하악보다클경우환자의구강운동에비해원심에있게된다. 16 고정성보철물제작을위한반조절성교합기의사용은치료기간중에임상적인조절을최소로하는최대한의진단정보를제공하며, 인체의해부학적구조와거의같은크기이기때문에오차가최소이다. 현재한국에서사용되는많은반조절성교합기의과두간거리는 110 mm 로고정되어있다. 5 Evaluation of Bonwill triangle using cone beam computerized tomography in Korean Louisville, USA) 는 110 mm로고정되어있으며, Whip Mix articulator (Whipmix Corporation, Louisville, USA) 는 96 mm, 110 mm, 124 mm로조절이가능하다. 그러나이렇게서양인을대상으로제시된 110 mm의과두간거리를한국인에적용시치아및악궁의크기와형태의차이로인한오차를예상해볼수있다. 따라서한국인에게적합한교합기의개발을위해서는한국인의악구강계관련치수를분류및정리하는것이필요하다. Cone-beam CT는근래에널리사용되고있으며, 악구강계관련치수를 3 차원적으로신속하고정확하게분석할수있는유용한영상진단수단이다. 따라서본연구에서도 Cone-beam CT를사용하여지금까지보고된적이없는한국인의과두간거리와과두-절치간거리를분석하였다. 한국인의과두간거리를분석하였을때한국인의평균과두간거리는 mm로 Keshvad 4 에의해보고된서양인의평균과두간거리인 mm에비해작은수치를나타냈다. 이러한결과는한국인과서양인의과두간거리에는차이가있으며, 한국인의구강재건을위한반조절성교합기사용에있어서표준화된 110 mm의적용을재고할필요성을의미한다. 평균과두간거리의성별에따른차이를보면, 남성 mm 여성 mm로남성의과두간거리가통계학적으로유의하게더큰것으로나타났다. 과두간거리에관한 Tradowsy 5 와 Lazic 등 6 의연구에서도남성이여성보다큰거리를보였으며, 이는한국인의스피만곡반경의성별에따른차이를보고한 Oh 등 17 의연구결과와유사하다. 또한양측과두에서의절치간거리를조사한결과남성의평균좌측과두-절치간거리는 mm 우측과두-절치간거리는 mm로통계학적으로유의할만한차이는없었다. 여성의평균좌측과두-절치간거리는 mm 우측과두-절치간거리는 mm로통계학적으로유의한차이는없었다. Lundström 18 과 Williamson과 Simmons 19 는치열과두개골의성장발육이대칭적으로이루어지기는어렵다고보고하였는데, 본연구에서나타난개인의좌우과두-절치간거리의차이가이러한비대칭적인성장에기인한것이라고생각할수있다. 본연구에서측정된한국인의평균과두간거리는 mm로 110 mm 보다작은값을보였다. 또한남성이여성에비해통계학적으로유의하게크게나타난것을고려했을때한국인에게특정수치를일괄적으로적 용하는것은바람직하지않다고판단되었다. 과두간거리는교합면형태에영향을미치는요소이므로한국인의보철적수복에있어서절대적인수치가적용된반조절성교합기의사용보다는, 각개인에맞는과두간거리로조절가능한교합기의사용이필요할것이다. 향후한국인에게적합한반조절성교합기의제작을위하여, 한국인의과두경사로와전방경사로등에대한추가적인연구가필요할것으로사료된다. 결론 한국인의본윌삼각을평가하기위해 120명의 Conebeam CT 영상으로과두간거리와과두-절치간거리를분석한결과, 다음과같은결론을내릴수있었다. 1. 한국인의과두간거리는 mm였으며, 과두-절치간거리는 105.2mm였다. 2. 성별에따른과두간거리는남성 (108.3 mm) 이여성 (103.4 mm) 에비해통계학적으로유의하게크게나타났다. 3. 성별에따른과두-절치간거리는남성 (108.1 mm) 이여성 (102.3 mm) 에비해통계학적으로유의하게크게나타났다. 본연구에서측정된한국인의평균과두간거리는 mm로서양인을기준으로제시된 110 mm에비해작았다. 이것은한국인의구강재건에있어서표준화된과두간거리의적용을다시고려해야할필요성을의미한다.
