Shoulder pain usually causes disability in activities of daily living, and therefore its proper diagnosis and management are essential. The prevalence of shoulder pain in Korea has been gradually increasing, and the differential diagnosis of shoulder pain has become more important. In Korea, the most common causes of shoulder pain are rotator cuff disease, including impingement syndrome (31%), adhesive capsulitis (35%), and calcific tendinitis (4%). This article presents the pathophysiology, clinical characteristics, typical physical examination findings and provocation tests, and radiologic findings of common shoulder diseases, including rotator cuff disease, adhesive capsulitis, and calcific tendinitis, to provide a guide to differential diagnosis. Common shoulder diseases can be easily diagnosed with careful history taking and precise physical examination; moreover, ultrasonography is a useful diagnostic tool for rotator cuff disease and can also assist with the differential diagnosis of shoulder diseases in the outpatient clinic. Key Words: Joint capsule; Rotator cuff; Shoulder pain; Shoulder impingement syndrome; Tendinopathy 서론 어깨는 해부학적으로 관절상완관절(glenohumeral joint), 견봉쇄골관절(acromioclavicular joint), 흉골-쇄골관절 외국의 어깨통증 유병률은 16-26%이며 주로 중년과 장년 층에서 발생하며[1] 국내는 2011년 국민건강보험공단 발표 자료에 의하면 어깨통증 진단(진단코드 M75)은 전체인구의 4.3%이며 최근 6년 동안 연평균 8.9% 증가하여 일선 현장에 서의 어깨통증 진단 및 치료의 중요성도 증가하고 있다. 연령 은 40대에서 60대 환자가 71.3%로 중년층이 대부분이다[2]. Seoul: Koonja Publishing; 2014, with permission from Koonja Publishing [3]. tendon rupture), 관절와순파열(labral tear) 등이 있으며, 후방어깨통증의 흔한 원인은 후방불안정성/탈구(posterior instability/dislocation), 견갑상신경포착(suprascapular nerve entrapment), 회전근개질환, 관절와순파열, 관절상 완 골관절염, 경추신경병증(cervical radiculopathy), 근위부 상완골절(proximal humerus fracture) 등이 있으며, 불특 정 위치의 어깨통증은 경추신경병증, 관절상완골관절염, 류 마티스관절염 및 내부장기질환의 전이통 등이 있다[4]. 어깨통증에 대하여 국내에서는 오십견 이란 부정확한 용 어를 많이 사용하며 한가지 예로 국민건강보험공단 보고자 료의 제목이 오십견 등 어깨통증 이며 보고자료에 따르면 오십견으로 불리는 유착관절낭염이 어깨통증 진단명의 35% 로 가장 흔하다[2]. 외국의 가장 흔한 어깨통증의 원인은 회 전근개질환(65-70%)으로 국내 진단명 분포와 차이가 있다 [5]. 이러한 차이가 인종 차이일 수 있으나 1차 의료현장에 서 자세한 감별진단 없이 어깨통증을 단순히 유착관절낭염 으로 진단하는 경우가 많을 수도 있다. 어깨질환은 통증과 함께 어깨관절 운동범위의 제한이 발생하여 기본 일상생활 동작에 제한을 주는 장해를 유발할 수 있다. 따라서 1차의료 현장에서는 다양한 어깨통증의 원인 질환에 대하여 특성을 잘 알고 있어야 하며 감별진단을 통하여 적절한 치료가 시행 되어야 한다. 하지만 회전근개질환과 유착관절낭염이 동반 된 경우처럼 여러 개 질환 또는 문제가 동반될 수도 있다[3]. 본 논문에서는 국내에서 어깨통증의 흔한 원인질환인[2] 회전근개질환, 유착관절낭염과 석회화건염에 대한 임상적 특징과 감별을 위한 검사에 대한 소개를 하고자 한다. 회전근개질환 1. 병태생리 회전근개는 극상근(supraspinatus), 극하근(infraspinatus), 견갑하근(subscapularis)과 소원형근(teres minor) 등 관절상완관절을 감싸고 있는 근육들로서 관절의 안정 성과 움직임을 제공한다[6]. 회전근개 질환은 뚜렷한 파열 없이 힘줄이 붓는 건증(tendinosis)과 힘줄 단면의 일부에 서만 파열이 관찰되는 부분층파열(partial thickness tear) 및 힘줄 단면의 전층에 걸쳐 파열이 관찰되는 전층파열(full thickness tear) 등의 부분 파열과 힘줄 자체가 끊어지는 건 파열(tendon rupture)을 포함한다[3]. 고전적인 회전근개의 병태생리는 회전근개 건에 과부하, 미세손상 누적으로 미세 파열과 염증이 발생하여 파열이 나타나는 건염(tendinitis)의 개념이었으나 최근에는 염증반응의 증거가 나타나지 않으므 로 건증(tendinosis)과 건병증(tendinopathy)이란 용어가 사 용된다[7]. 건증은 콜라겐의 퇴행성 변화와 염증세포 없이 혈류(vascularity) 및 세포성(cellularity)의 증가로 부종이 있 654 대한의사협회지

3 Lee HJ Differential diagnosis of common shoulder pain 는 상태로 병리현상에 대한 명칭이며, 건병증은 건손상과 연 관된 통증과 기능 감소에 대한 임상적 개념의 명칭이다[8,9]. Neer [10]에 의한 고전적인 회전근개질환의 개념은 급성건 염이 발생하고 충돌에 의하여 퇴행성 변화와 함께 건증 및 부분층파열로 진행하여 결국 전층파열과 건파열로 진행되는 것이다. 최근의 병태생리는 내부적 요인, 외부적 요인 그리 고 두 가지 혼합에 의한 것으로 설명한다. 내부적 요인은 과 도한 신장으로 인한 손상, 혈관공급의 경계에서 발생하는 허 혈상태, 노화로 인한 퇴행성 변화 등의 내부 변화에 의한 건 병증를 의미한다. 외부적 요인은 견봉하 충돌(subacromial impingement)과 내부 충돌(internal impingement) 등에 의 한 건병증을 의미한다. 견봉하 충돌은 과사용과 더불어 견봉 하 골극(subacromial spur), 견봉 모양 및 견봉쇄골 관절 골 극(AC joint spur) 등에 의한 해부학적 또는 오구견봉아치 공간(coracoacromial arch space) 압력 증가 등의 생역학적 이상에 따라 견봉하 공간(subacromial space)에서 힘줄 압 박이 발생하는 것으로 고전적인 충돌 증후군의 개념에 해당 한다. 견봉성형술을 시행한 연구 결과는 건파열이 점액낭면 보다는 대부분 관절면에서 관찰되어 견봉하 충돌만으로 설 명이 부족하다. 내부 충돌은 후상방 충돌(posterior superior impingement) 또는 상방충돌(superior impingement)로 서 야구 선수의 코킹과 같이 머리위로 던지는 행위를 할 때 어깨 외회전, 외전, 신전이 동시에 일어나게 되어 상완골 (humerus)이 회전근 힘줄을 견관절와(glenoid rim)의 상방 또는 후상방으로 압박하는 충돌현상을 의미한다. 내부 충돌 에 의한 건손상은 힘줄의 관절면 손상에 대한 설명이 될 수 있다[8-10]. 회전근개질환의 명칭은 충돌증후군(shoulder impingement syndrome)과 동일한 개념으로 사용하기도 하지만[4], 회전근개질환과 충돌증후군은 다른 것이며 충돌증후군과 회 전근개질환을 별도의 질환으로 분류하였으며[11] 최근에는 충돌증후군을 포함하는 회전근개질환 또는 회전근개건병증 의 명칭이 더 많이 사용되고 있다. 2. 임상적 특징 회전근개질환에 대한 위험요인은 나이, 비만, 당뇨, 흡연, 이상지질혈증(dyslipidemia), 가족력 등이 있으며[4,12], 작 업환경의 위험요인은 힘을 주며 어깨 굴곡을 유지하는 경우, 어깨의 반복적인 움직임, 손을 어깨위로 올린 상태에서 작업 하는 경우다[1]. 자연 경과는 전층파열의 경우 연령에 따라 크기가 증가하며 65세 이상 50%에서 무증상의 전층파열이 있으며 이는 2-3년 후 크기가 증가하여 증상을 유발하는 경 우가 많다. 증상이 있는 전층파열의 50%는 2년 내에 크기가 증가한다. 부분층파열은 전층파열에 비하여 악화 속도가 느 리다. 전층파열이 있을 경우 주기적 검사가 권고된다[12]. 통 증은 손이 어깨 위로 가는 일을 할 때 악화되며 주로 외측으 로 둔한 양상으로 강도는 다양하게 나타난다. 통증으로 외전 및 내회전의 제한을 호소하는 경우가 있다[11,13,14]. 3. 신체검사 어깨통증에 대한 신체검사는 양측 어깨에 대한 시진 (inspection), 촉진을 통한 압통점(tender point) 확인, 능동 적 및 수동적 운동범위, 유발검사(provocation test) 및 근 력검사를 시행한다. 회전근개 질환은 극상근 건이 부착하는 대결절(greater tuberosity)에 압통이 나타난다. 수동적 운 동범위는 초기에는 제한이 없다. 외전의 능동적 운동범위 60도에서 120도사이에 통증을 호소하며 이를 통증궁 (painful arch) (Figure 1)이라 하며 극상근건파열(tendon rupture)의 경우 외전 후 팔이 갑자기 떨어지는 drop arm 징후가 관찰될 수 있다. 내회전 시 통증 유발로 운동범위 제한이 있을 수 있다. 유발검사는 충돌검사(impingement test)로서 Neer 검사(Figure 2)과 Hawkins-Kennedy 검사 (Figure 3)가 대표적이다. 근력측정은 극상근에 대하여 깡통 비우기 검사(empty can test, Jobe s test) (Figure 4)를 견갑 하근(subscapularis)에 대하여 lift-off 검사를 시행하며 양 성 소견은 통증이 유발되거나 건파열이 있을 경우 근력감소 로 나타난다[3,13,15,16]. 이러한 검사들은 민감도와 특이도 가 높은 것으로 알려져 있으며[15], 최근 메타분석은 Neer 검사와 Hawkins-Kennedy 검사는 민감도가 높지만 특이 도가 낮고 통증궁은 민감도가 낮고 특이도가 높으므로 여러 신체검사를 조합하여 정확도를 높이는 것이 필요하다고 보 고하였다[17]. 흔히 발생하는 어깨통증의 감별진단 655

The examiner assessed the deltoid with the arm at 90 of abduction and neutral rotation. And then the patient places the arm into internal rotation with forward flexion 30 degree and the thumbs toward the floor. During holding this posture the examiner pushes down at the distal forearm to test the strength of supraspinatus muscle with resistance. When there is weakness or pain provocation, the test is positive. 4. 영상검사 영상검사는 단순방사선촬영, 초음파검사, 자기공명영상검 사 및 자기공명관절조영술 등을 사용할 수 있다. 단순방사 선검사는 다른 부위의 퇴행성 변화와 회전근 파열의 간접 적인 소견을 확인할 수 있으며 다음과 같다. 대결절의 골피 질 불규칙(cortical irregularity)소견, 견봉쇄골 관절의 골극, 회전근개의 석회화 및 상완골두(humeral head)의 상방이동 (superior migration) 등이며 대결절의 골피질 불규칙 소견 의 회전근개 파열에 대한 민감도는 90%이며 상완골두 상방 이동은 심한 회전근개 파열을 의미하며 견봉과 상완골두 거 리가 6 mm 이하인 경우 전층파열과 연관성이 높다. 초음 파 검사는 검사가 편하고 비침습적으로 최근 어깨통증에 대 하여 많이 시행되고 있다(Figure 5). 하지만 초음파는 골조 직에서 반사가 많이 되는 특성으로 관절낭, 관절와순, 연골, 골 조직 등 관절내부의 이상여부는 확인하기 어렵다. 자기공 명영상검사는 초음파에 비하여 연골등 관절내부 조직 및 주 변 근육의 위축과 지방침윤에 대한 평가가 가능하다. 자기 공명관절조영술은 조영제를 관절강 내 주사함으로써 관절 와순 등 관절강 내 구조물 평가에 적합하며 결손충전(filling defect) 소견으로 회전근개파열을 진단할 수 있다[18]. 회전 근개파열의 전층파열에 대한 민감도와 특이도는 초음파, 자 기공명영상검사, 자기공명관절조영술 모두 비슷하게 높은 656 대한의사협회지

5 Lee HJ Differential diagnosis of common shoulder pain 절낭염이라고 명명하였다[7,20]. 유착관절낭염은 특발성인 일차성 유착관절낭염과 회전근개 파열, 석회성건염, 견봉 쇄골관절염 등의 기저질환에 의하거나 수술 및 외상에 의 한 이차성 유착관절낭염으로 분류할 수 있으며, 예후와 치 료방법이 다르므로 기저질환을 확인하여 일차성과 이차성 것으로 보고되었으나, 부분층파열에 대한 민감도는 초음파 와 자기공명영상검사가 각각 66.7%와 63.6%로 낮은 편이며 자기공명관절조영술이 85.9%로 가장 높은 것으로 보고되었 다[18,19]. 회전근개질환이 의심될 경우 40세 이상에서는 우 선 단순방사선검사 시행 후 이상 소견 없으면 초음파검사로 진행하며 40세 이하에서는 바로 초음파검사를 시행하는 것 을 권유하고 있다[18]. 유착관절낭염 1. 분류, 병태생리 A C Figure 5. Long axis view shows linear fluid filled defect (arrow) throughout the articular side to bursa side of supraspinatus tendon. 유착관절낭염은 통증과 함께 서서히 증가하는 여러 방 향의 어깨 운동범위의 소실에 의한 강직이 특징적이며 처 음에는 관절주위염(periarthritis)으로 불리다가 1934년 Codman이 동결견(frozen shoulder)으로 명명하였으며 1945년 Neviaser가 만성염증의 의한 관절낭의 섬유화와 비대, 이로 인한 관절낭 구축의 병리현상을 근거로 유착관 B D 을 구분하는 것이 필요하다. 일부는 특발성인 경우만 유 착관절낭염 으로 이차성은 통증성 견부강직(painful stiff shoulder) 으로 분류하기도 한다[7,20,21]. 특발성은 관절 낭의 비후와 섬유화, 액와주름(axillary fold)의 유착, 관절 낭의 상완골 경부로 유착 등으로 관절 구축이 발생하며, 구 축위치에 따라 특정방향으로 운동범위 제한이 나타날 수 있 다[21,22]. 유착관절낭염은 흔히 오십견으로 불리는데, 오 십견은 50세 이상에서의 어깨통증을 의미하는 모호한 용어 로서 진단명으로 적합하지 않다. 2. 임상양상, 자연경과 및 역학 통증은 서서히 수개월에 걸쳐 증가하며 삼각근 부착점의 전이통으로 나타나며, 수면에 지장을 줄 정도로 야간통증 이 흔하다. 반복적인 머리위로의 활동(overhead activity) 때 유발되는 통증은 전형적인 양상이 아니므로 다른 질환 을 의심해야 한다. 운동범위소실은 단계가 진행하면서 점 점 악화된다. 일부는 유착관절낭염이 저절로 호전되는 질환 으로 치료가 필요 없다고 하지만 간단한 치료만 시행할 경 우 증상호전에 4년 이상 소요되거나 40%에서 증상이 지속 된다는 보고가 있다[22]. 통증 및 운동범위소실의 경과는 Hannafin 등[23]과 Neviaser과 Hannafin[24]이 임상양상 과 마취 하 수동적 운동범위와 관절경 소견을 기준으로 제 안한 4단계로 구분한다. 1단계는 염증기(0-3개월)로 움직 일 때 통증과 약간의 운동범위소실이 있으며, 2단계는 동 결진행기(freezing stage, 3-9개월)로 통증 증가와 운동범 위 소실의 증가가 나타나며, 3단계는 동결기(frozen stage, 9-15개월)로 통증은 감소하고 운동범위 소실이 더 심해지 며, 4단계는 해동기(thawing stage, 15-24개월)로 통증은 미약하게 남아 있으며 운동범위가 조금씩 회복되는 상태다. 유병률은 2-2.4%로 알려져 있으며 호발 연령은 40대와 70대사이다. 위험요인으로는 성별과 전신질환(당뇨, 뇌혈관 흔히 발생하는 어깨통증의 감별진단 657

B C [7,21] 병력 청취와 운동범위 측정 으로 진단을 하는데 진단기준은 통 일된 것이 없다. 공통적으로 최소 한 1개월 이상의 견관절부 통증이 있어야 하며, 여러 방향으로 능동 적/수동적 관절가동범위의 장애가 있어야 하고, 단순방사선 소견이 정 상이어야 한다. 의 내용이 있다[3]. 질환, 갑상선 기능이상, 유방암 환자)등이 있다. 당뇨는 가 장 많이 연관되는 질환으로 예후가 가장 안 좋다. 여성들에 서 유병률이 높다. 비우성수 쪽에서 더 빈번하며, 같은 쪽에 서는 재발이 드물고 20-30%는 반대편에서 증상이 나타나며 14% 정도 양측에서 증상이 나타난다[21,22,24]. 3. 진단 신체검사는 관절운동범위의 제한을 확인하며 영상검사는 다른 질환 감별하기 위하여 시행한다. 신체검사는 어깨 관절 의 모든 방향에서의 수동적 및 능동적 운동범위를 측정하며 양측에서 시행하여 건측과 비교한다(Figure 6). 운동범위 감소는 특징적으로 수동적 외회전에서 많이 관찰되지만 여 러 방향에서 운동범위 감소가 발생하여 옷입기, 빗질하기, 브레지어 착용 등의 일상생활동작에 지장을 준다[21,22]. 능동적 운동범위 측정할 때 견갑상완리듬(scapulohumeral rhythm)의 감소에 대하여 견갑골 움직임이 보상을 해 주기 때문에 견갑골 고정이 중요하다[13]. 단순방사선검사는 퇴 행성, 관절염 및 골절 유무를 확인하여 관절염, 석회성건염, 골절 등을 감별할 수 있다. 초음파검사는 회전근개 등의 이 상 유무를 확인할 수 있지만 유착관절낭염 진단에는 큰 도 움이 안 된다. 자기공명영상 및 자기공명관절조영술은 관절 낭의 비후, 부리위팔뼈인대(coracohumeral ligament), 하 방관절상완인대(inferior glenohumeral ligament)와 관절 낭을 포함하고 있는 회전근개 간격의 조영증가 및 반흔, 액 와주름의 감소 등의 특징적 소견이 관찰되지만 일차적 진단 으로서 가치는 떨어지며 다른 질환 감별에 큰 도움이 된다 석회성건염 석회성건염은 건의 내부 또는 부착점에 석회침착이 발생 하는 것으로 어깨에는 회전근개의 극상근과 극하근 건에서 흔히 관찰된다. 이러한 석회침착은 증상 없이 영상검사에 서 관찰되는 경우가 많지만 어깨통증의 7%를 차치한다. 호 발 연령은 40대에서 60대 사이로 여성에서 더 흔하며 우성 수 어깨에서 조금 더 흔하다. 어깨 힘줄내부의 석회침착의 병태생리는 명확히 밝혀진 것이 없지만 다른 부위의 석회 성건염에서 언급되는 괴사조직에서 발생한다는 퇴행성 변 화로 보는 시각이 있지만, 다른 부위와 달리 살아있는 힘줄 에서 세포에 의한 매개로 발생하는 것으로 추정되고 있다. 이러한 개념은 퇴행성이 크게 관여하는 회전근개질환의 호 발 연령보다 석회성건염의 호발 연령대가 더 이른 시기이거 나 비슷한 시기라는 점에서 일치한다고 생각할 수 있다. 임 상적으로 칼슘이 힘줄내부에 침착을 시작하여 초크 같은 성 상으로 석회 침착이 관찰되는 형성기(formative phase), 석 회침착이 마무리되어 석회화 초점(focus of calcification)이 더 뚜렷해지는 휴지기(resting phase)와 석회침착이 흡수되 어 치약처럼 강도가 연해지는 흡수기(resorptive phase)의 3단계로 구분할 수 있다. 통증 강도는 형성기와 휴지기에는 다양한 강도로 발생하지만 흡수기에는 심한 통증과 운동범 위 제한이 나타나는 급성 염증성 통증으로 발현된다. 임상 적 양상과 신체검사 소견은 회전근개 질환의 충돌 증후군과 비슷하다. 하지만 통증이 심한 경우 전 범위에 걸쳐 운동범 위 제한이 나타날 수 있다. 형성기와 휴식기는 수년에 걸쳐 658 대한의사협회지

B C 진행되며 이후 저절로 흡수기가 진행되어 통증은 시간 경 과에 따라 자연적으로 소멸되거나 보존적 치료로 좋은 결 과를 보이는 경우가 많다. 하지만 흡수기에 심한 통증을 호 소하거나 형성기 또는 휴식기의 통증이 일상생활에 지장을 주는 정도라면 적극적 치료를 시행해야 한다[25,26]. 영상 검사는 단순방사선검사, 초음파검사를 통해 진단할 수 있 수 있지만 통증양상의 정확한 파악과 통증유발검사 또는 운 동범위 측정 등의 신체검사를 통하여 임상적 진단의 범위를 좁힐 수 있으며 쉽게 회전근개를 잘 평가할 수 있는 초음파 검사와 다른 동반질환을 감별하기 위한 단순방사선검사를 이용하여 진단을 내릴 수 있는 경우가 많으므로 이러한 내용 에 대하여 잘 숙지하여 활용하는 것이 중요하다. 다. 단순방사선검사는 석회침착이 주로 대결절 주변에 관찰 되며, 뚜렷하고 균일하며 경계가 있는 경우 증상이 약한 형 성기 또는 휴지기의 소견일 가능성이 높으며 경계가 분명하 지 않고 흐릿하며 분명하지 않은 경계로 나타나는 경우 통 증이 심한 흡수기의 소견일 가능성이 높다. 초음파는 석회 침착의 특징적 소견과 위치를 평가할 수 있지만 병태생리적 단계를 평가하기는 어렵다(Figure 7). 하지만 흡수기의 경 우 우방음영이 없는 엉성하게 보이는 석회덩어리 소견이 관 찰되기도 한다. 컴퓨터단층촬영은 골조직 평가에 우수하므 로 시술 또는 수술 전 석회침착의 위치와 성상을 평가할 때 유용하다[7,25]. 결론 국내에서도 어깨통증이 증가추세에 있으며 일상생활에 지 장을 주므로 적극적인 감별진단과 치료가 필요하다. 국내에 서 흔한 어깨통증의 원인질환은 충돌 증후군을 포함한 회전 근개질환, 오십견으로 대표되는 유착관절낭염과 석회화건염 이 대표적이다. 이러한 질환들은 임상양상이 비슷하게 보일 찾아보기말: 관절낭; 회전근개; 어깨통증; 어깨충돌증후군; ORCID 건병증 Ho Jun Lee, REFERENCES 1.
