With the recent acceptance of first trimester and integrated screening as a part of routine clinical practice, there are now a variety of accepted screening protocols for Down syndrome. These choices can be confusing both to patients and clinicians. First trimester screening is at least as accurate as traditional second trimester screening and provides results earlier. Combining first and second trimester results increases the sensitivity and specificity of screening and reduces the number of women requiring invasive testing. However, the decision to screen and to perform invasive confirmative testing is a personal one that may be influenced by the presence or absence of a family history of aneuploidy or genetic disorders and by the woman s obstetrical and medical history, attitudes and beliefs about termination of pregnancy, education, religious beliefs, and economic concerns. The purpose of this article is to outline the various options in prenatal screening, and their individual advantages and disadvantages. Key Words: Down syndrome, Prenatal screening 다운증후군을포함한태아염색체이상의위험도는산모의나이가증가할수록높아지고특히 35세이후부터는급격히증가하게된다. 그래서 1980 년대중반까지태아염색체진단검사는분만시나이가 35세이상이거나다운증후군임신의과거력이나가족력이있는산모에게만적용되었다. 그러나실제다운증후군의 2/3 는젊은산모에서발생하며, 재발률도낮기때문에좀더진단율을높일수있는표지자가요구되었다 년 Merkatz 등 1 이다운증후군임신산모의 15~20 주사이혈액에서알파-태아단백 (alpha-fetoprotein, AFP) 이감소되어있다는것을보고한이후 35세미만의산모에서 접수일 : 채택일 : 교신저자 : 정의 eui2536@hotmail.com 도다운증후군의선별검사가시행되기시작하였다. 이후로여러혈청물질 (human chorionic gonadotropin, hcg; unconjugated estriol, ue3; inhibin A) 이추가되면서 ( 사중검사 ) 다운증후군의발견율은 80% 까지향상되었다. 최근에는임신초기산모혈청내 pregnancy associated plasma protein A (PAPP-A) 와 free βhcg 측정치및초음파검사로태아목덜미투명대 (nuchal translucency, NT) 를측정하여함께분석하는임신초기선별검사도사용되고있다. 이러한초음파검사및모체혈청검사는임신제1 삼분기, 제2삼분기, 또는두시기에통합하여선별검사로사용될수있다. 2 현재사용되고있는여러선별검사들은산모의나이만을기준으로한것보다좋은결과를얻을수있기때문에, 미국산부인과학회 (The American College of Obstetricians and Gynecologists) 에서는더이상 35세의나이만으로융모막검사나양수검사여부를결정하지말라

2 대한산부회지제 53 권제 12 호, 2010 고권고하고있다. 2 그러나가족력이있거나비정상초음파소견을보이는등다운증후군의위험도가특별히높은산모에서는선별검사없이진단검사를할수도있다는것을알려주어야한다. 선별검사상고위험군산모는유전상담과진단검사를하게된다. 임신제1삼분기선별검사최근의연구들은임신초기다운증후군선별검사에초점이맞추어져있는데, 이는정상산모의경우에보다일찍불안감을해소시켜주고비정상결과의경우에는좀더안전한주수에임신중절술을선택할수있다는장점이있다. 조기선별검사는임신 10~14 주사이에모체혈청검사와초음파검사를각각또는병합하여시행한다. 다운증후군산모는임신제1 삼분기에 free βhcg는증가, PAPP-A 는감소된소견을보인다 ( 평균값 ; free βhcg 1.98 MoM, PAPP-A 0.43 MoM). 3,4 태반에서생성되는 hcg 는 α와 β subunit 으로이루어져있으며, 혈중에는 α와 β가결합되어진활성화형의 hcg 와각각으로분리되어있는 free αhcg, free βhcg 가있게된다. 연구기관및보고자에따라그중 free βhcg 만을검사에이용하는경우와 total β hcg (bound form hcg 와 free βhcg) 를이용하는경우가있을수있다. 다운증후군, 터너증후군, 삼배수체 (triploidy), 또는여러형태의태아수종에서 hcg 는증가하게된다. 5-7 NT는임신제1삼분기의태아에서관찰되는초음파소견 이다. 태아에게림프관형성부전이나심장기형에의한심부전, 다운증후군이나터너증후군같은염색체이상, 기타선천성기형이있는경우에는이 NT가증가된소견을보이게된다. NT 측정에대해서는이미정해진가이드라인이있는상태이지만, 이에추가해서 NT를절대값보다는 MoM 으로, 검사기관에따른 MoM 보다는검사자 (sonographer) 에따른 MoM 을사용하는것이, 그리고한번측정한값보다는여러번측정하여평균으로나타낸것이좋은결과를보였다. 8 또한모든검사에서임신주수를 last menstrual period 가아닌초음파로보정한값을사용했을때위양성률이낮았다. 8 NT 측정은단독검사만으로도다운증후군의발견율이 64~70% 를나타낸다고할만큼유용한임신초기검사가되고있다. 9 그러나 NT 측정단독으로는다태임신의경우와같이특정한경우를제외하고는선별검사로권고되지않고있다. 2,8 왜냐하면발견율을 85% 로했을때위양성률이 20% 에달해불필요하게침습적인검사를하게되는산모의수가많아지기때문이다 (Table 1). 반면, NT와혈청검사를병합한제1삼분기병합선별검사 (combined first trimester screen) 는 5% 의위양성률에서 82~87% 의발견율을보여사중검사와비슷한정도의높은정확도를보인다 (Tables 2, 3). 2,10 제1삼분기병합선별검사는주수에따라검사의정확도가변하는데, First And Second Trimester Evaluation of Risk 연구에의하면, 11주에비해 13주에검사를시행하면발견 Table 1. 4 대한산부회지제 53 권제 12 호, 2010 임신제 2 삼분기선별검사 다운증훈군산모는임신 15~20 주사이증가된 hcg, inhibin A, 감소된 AFP, ue3 치를갖는다 ( 평균값 ; AFP 0.7 MoM, hcg 2.06 MoM, ue MoM, inhibin A 1.77 MoM). 13,14 Inhibin 은성선과태반에서합성되는 dimeric glycoprotein 으로 α와 β, 두가지 subunit 가있고, β subunit 이 βa 및 βb로나누어져 inhibin A와 inhibin B 를형성한다. 15 사중검사라고불리는이선별검사는위양성률 5% 에서 81% 의발견율을보인다 (Table 3). 10 이전의이중검사 (AFP, hcg) 와삼중검사 (AFP, hcg, ue3) 는각각 66%, 74% 의발견율을보였고더이상다운증후군의선별검사로의미를갖지못한다. 에드워드증후군의경우 hcg, AFP, ue3 모두낮은값을보인다. 16 정확하고유용한검진이되기위해서는검진당시의정확한임신주수가매우중요하며, 초음파를통한정확한임신주수산정 ( 임신 8~11 주사이에측정한 crown rump length) 이가장효율적이다. 우리나라에서는 1994 년경부터삼중검사가보급되면서이용되기시작하였고, 2005 년부터는보험급여의대상에포함되었다. 최근에는대부분의기관에서위양성률이낮으면서발견율이높은사중검사를이용하고있다. 통합선별검사 (Integrated screening test) 다운증후군의발견율을높이고불필요한침습적검사를줄이기위한노력으로제1 삼분기선별검사를제2삼분기선별검사와통합해서사용하는연구들이계속되어왔다. 통합선별검사는제1 삼분기선별검사와제2 삼분기선별검사가모두끝난후, 하나의위험도를계산하여제시하는방법이다. 17 통합선별검사에서 NT와 PAPP-A 는임신 10주에측정하는것이, free βhcg와 inhibin A를포함하는사중검사는 14~20 주사이에측정하는것이가장높은변별력을보인다. Total hcg 보다는 free βhcg 가조금더변별력이높고, hcg 는임신제1삼분기나제2 삼분기어느때나측정이될수있지만, 임신제2삼분기때좀더차별화된수치를보이므로제2 삼분기때사중검사로측정하고있다. 이경우 5% 의 위양성률을기준으로 94~96% 의발견율을보였고, 위양성률을 1% 로하였을때의발견율도 85% 정도로매우높다 (Table 3). 8,10 이는실제로 100,000명의산모를선별검사했을때진단검사시술에의한정상태아손실이 9건있을수있다는결과로사중검사의경우는 45건이다. 8 의료비용측면으로도모든산모가검사를여러번시행해야하지만, 융모막검사나양수검사같은고비용의진단검사를받는건수는줄기때문에전체적으로는가장높은경제적효율성을보였다. 8 즉통합선별검사는현재까지의검사중가장높은발견율과낮은위양성률을보이지만제1삼분기선별검사를공개하지않음으로인해더일찍진단을받을수있는환자를제2삼분기까지기다리게한다는점과두단계를모두거쳐야하는환자순응도의문제가단점이다. 이문제를해결하기위해두가지의순차선별검사 (sequential screening test) 가소개됐다. 이검사들에서는제1삼분기선별검사의결과를환자에게알려줘고위험군산모가융모막검사등의진단검사를빨리시행할수있게해준다. 단계적 (stepwise) 순차선별검사에서는제1삼분기선별검사에서고위험군으로나오면그결과를알려주어융모막검사를고려할수있는선택권을준다. 고위험이아닌나머지산모에게는결과를알려주지않은채제2삼분기선별검사를다시받게하고두검사를병합한최종결과를알려준다. 18 조건부 (contingent) 순차선별검사의경우는제1삼분기선별검사의결과에따라고위험군, 중간위험군, 저위험군산모로나눈다. 고위험군산모는검사결과를듣고융모막검사등을결정할수있으며, 중간위험도를가진산모 (1/30~1/1,500) 만이제2삼분기선별검사를받고, 저위험도를갖는산모는더이상검사를진행하지않는다. 19 이경우약 23% 의산모만이제2삼분기선별검사를받게된다. 위험도에대한결정은기관에따라다른데예를들어고위험군을 1/8.9 이상의위험도라고결정하면전체검사환자의 0.1% 가여기에속하게되고이산모들이융모막검사를하게되면전체다운증후군의 30% 가임신제1 삼분기검사후발견되게되는것이다. 만약 1/65 이상을고위험군이라고한다면전체검사환자의 1% 가여기에속하고다운증후군의 70% 가이단계에서발견되게된다. NT 측정이불가능하거나신뢰도가떨어지는기관에서는제1삼분기의 PAPP-A 와제2 삼분기의사중검사를병합한

5 정의. 다운증후군의산전선별검사 혈청통합선별검사를통해하나의위험도를제시할수있다. 20 이경우 5% 의위양성률기준으로 88% 의발견율을보인다. 즉, 통합선별검사에비해부족한점은있으나사중검사보다는높은발견율을보인다. 제1삼분기선별검사와제2삼분기선별검사를각각독립적으로시행하는것은높은위양성률을초래하기때문에권장되지않는다. 10,21 겠다. 어떤산모에서는 NT 측정을통해융모막검사를결정하고그결과에따른임신초기선택유산술이요구되기도한다. 삼태이상의다태에서는모체혈청선별검사의자료가더부족하기때문에현재까지는 NT 측정이적합한선별검사가되겠다. 22 결론 다태아에서의선별검사다태아에서의혈청선별검사는단태아만큼정확하지못하다. 그이유는다운증후군을포함한다태임신의자료가많지않기때문이다. 융모막성 (chorionicity) 과접합체성 (zygocity) 에따라서도혈청검사는다른결과를보이는데특히 PAPP-A 의경우단일융모쌍태아에서는 10% 낮은값을보인다. 22 즉, 쌍태아의분류에따른연구도여전히필요한상황이다. 또한정상아와비정상아에서유래된표지자가모체의혈청에서평균효과를내게됨으로써다운증후군에의한비정상혈청검사소견을감추게된다. 쌍태아에서임신제2 삼분기혈청선별검사는 5% 의위양성율기준으로 50% 의발견율을보인다. 23 역시 5% 의위양성율에서임신제1삼분기병합선별검사는 70% 의발견율을보이는데이는 NT 측정과비슷한결과이다. 또한 NT는각태아에특정한위험도를나타낼수있기때문에다태임신의선별검사로선호되고있다. 상담역시간단하지않은데혈청검사에서고위험군으로나왔을때어느태아가고위험군에해당하는지알수가없어모든태아에서침습적인진단검사를해야하고, 만약한태아에서다운증후군이나왔을때과연선택적유산술을시행할건지에대해서도충분한상담과고민이필요하 다운증후군선별검사는정확성, 안전성, 경제성을모두고려해야한다. 여러보고들을종합해보면제1삼분기병합선별검사는제2 삼분기사중검사와비슷한결과를보이며, 통합선별검사가가장좋은결과와효율성을보여준다. 여러선별검사의장점과단점뿐만아니라검사를시행하는주수에따라정확도에차이가생긴다는것을알고, 각의료기관의사정과산모에따라적합한검사를선택해야하겠다. 즉, NT 측정이나융모막검사가불가능한기관에서는제2삼분기선별검사나혈청통합선별검사를시행해야하겠다. 현재우리나라에서가장많이사용되고있는다운증후군선별검사는제2삼분기의사중검사이며, 점점초기검진에대한관심이놓아지고있다. 최근에는 NT의측정이보편화되면서임신부들마저임신초기에목둘레를측정한다는것을알고요구하고있으며, 그에따라임신초기에진단검사를해야하는경우도늘고있는추세이다. 그러면서융모막검사가가능한의료기관부터점차조기선별검사가늘어가고있다. 그러나원칙적으로우리나라현행법상모자보건법에서태아가염색체이상등기형이있을때인공임신중절을허용하고있지않아진단검사또는선별검사의시행자체에모순이포함되어있는상황이다. 1.
