Antidepressants have been the first-line therapies due to their confirmed efficacy, however, considering high rate of poor treatment response to these therapies, distressing side effects, and delayed onset of their efficacy, there has been much effort to find alternative treatments for major depression. Recently, evidence regarding disturbed circadian rhythms involved in the pathophysiology of major depression has emerged, the interest on this area has been increasing. Agomelatine is an emerging antidepressant, with a unique profile of selective antagonist at serotonin 2C (5-HT2C) receptors and melatonin receptor agonist. Previous studies have shown its superior efficacy over placebo in treating major depression. Previous trials have shown comparable antidepressant efficacy of agomelatine compared to other standard antidepressants including venlafaxine, sertraline, and fluoxetine. Regarding safety profile of agomelatine, it seems to be not associated with sexual dysfunction and it has less potential for serotonin syndrome or discontinuation syndrome than standard antidepressants including selective serotonin reuptake inhibitors. Considering favorable results on the efficacy and safety of agomelatine in treating depression, it could be a good, safe treatment alternative in the treatment of depression. Received: November 18, 2013 Revised: December 2, 2013 Accepted: December 3, 2013 Correspondence author: Won-Myong Bahk, MD, PhD Department of Psychiatry, Yeouido St. Mary s Hospital, The Catholic University of Korea School of Medicine, ro, Yeongdeungpo-gu, Seoul , Korea Tel: , Fax: , wmbahk@catholic.ac.kr 최근 주요우울증 치료의 진전에도 불구하고 약 의 환자들이 이하 등의 항우울제 치료에 적절한 반응을 보이지 않는 등 아직까지 많은 어려움이 있다 또한 이들 항우울제들이 임상적으로 항우울 효과를 나타내는 데에 보통 주의 시간이 걸리며 성기능 장애나 과 같은 여러 부작용 및 약물 상호작용 등의 측면에서 여러 제한점들이 있어서 항우울 효과 측면에서도 보다 효과적이고 내약성 측면에서도 보다 나은 새로운 항우울제에 대한 관심이 늘어나고 있다 주요우울증의 병태생리에 대한 기존의 연구들에서 단가아민 계의 이상이 입증되었으나 단가아민계 이상만으로는 주요우울증의 발생 기전을 다 설명할 수 없다 최근의 연구들은 주요우울증을 경험하는 환자들이 일주기 리듬의 장애를 보인다는 증거들을 지속적으로 보고하고 있어 주요우울증 환자들에서 수면 체온 행동 등 일주기 조절의 변화가 관찰되고 뿐만 아니라 을 비롯한 여러 호르몬 등의 신경내분비 기능에 있어서도 일주기 조절의 변화가 나타난다고 한다 이러한 일주기 조절에는 여러 신경전달물질 과 신경조절물질 들이 관여하는 것으로 알려져 있다 주요우울증에서는 - 분비 주기가 위상 전진 혹은 후진 되어 있다는 보고들이 있으며 일주기 리듬의 정상화가 항우울 효과를 가져온다는 보고들이 나오면서 의 우울증 치료에 대한 잠재적 역할에 대해서 기대가 높아졌다 하지만 이전 연구들의 결과를 토대로 살펴보면 은 우울증 환자에서 수면 상태는 호전시켰지만 항우울 효과는 없는 것으로 나타났다 이후 합성 대체물인 이 개발되었고 년 유럽에서 주요우울증 치료 약제로 승 Copyright c 2014 대한정신약물학회 1

2 Agomelatine 의효능과안전 인 받은 이후 현재까지 여러 나라에서 사용되고 있다 국내에 서도 은 식품의약품안전처에서 허가를 받아 곧 도입될 예정이다 이에 본 고에서는 새로운 기전의 항우울제 인 의 작용 기전과 여러 문헌들을 고찰하였던 이 전 종설 에 추가적으로 새로이 발표된 연구 결과들을 바탕으 로 의 효과 및 내약성에 대해서 고찰하고자 한다 Agomelatine 의 유사물 인 은 의 화학 구조를 가지고 있다 세 종류의 수용체 중 수용체의 활성은 일주기 리듬 조 절에 중요한 역할을 담당하는 시교차상핵 이하 의 발화 를 억제함으로써 수면을 촉진하며 수용체는 일주기 리듬의 전환과 위상 변위에 관여하는 것으로 알려져 있으나 의 역할은 아직 분명하게 밝혀져 있지 않다 은 처럼 수용체에 작용제 로 작용하지만 수용체에 대한 결합력이 보다 강하여 더 긴 반감기 를 가지고 더 지속적인 작용을 하는 것으로 알려져 있다 한편 은 이하 수용체에 길항제 로 작용하고 수용체에는 친화성이 없으며 단가아민 재흡수에도 관여하지 않는다고 알려져 있다 이 항우울 효과를 보이는 기전에는 계와 단가아민계가 모두 관여할 것으로 생각되고 있으며 특 Melatonin MeO N H O 히 전두엽에서 과 작용의 강화 및 일주기성의 재동기화 가 주요 작용 기전으로 제시되고 있다 일주기성 재동기화를 통한 의 항우울 효과는 수용체 및 수용체가 모두 일주기 리듬 조절에 중요한 에 존재한다는 사실에 의해 지지되고 있다 이 - 와 마찬가지로 주요우울증의 발생에도 관여한다고 알려져 있기 때문이다 한편 일주기 리듬에 따라 발현이 변동되는 유일한 수용체인 수용체 의 길항작용 은 빛의 합성 방해 작용을 억제하여 의 생성을 증가시키고 수용체의 작용제는 생성을 억제시킨다 이상에서 살펴본 바와 같이 이 일주기 리듬을 조절하는 기전은 수용체에 대한 작용과 함께 수용체 길항작용을 통해서 의 신경 활성을 조절함으로써 일어나는 것으로 생각된다 은 일주기 리듬 조절뿐만 아니라 항우울 효과도 나타내는데 이는 수용체가 존재하는 신경전달물질계를 통해서인 것으로 생각된다 전두엽과 - 등에 존재하는 - 이하 계의 에는 수용체가 풍부하게 존재한다 이들 계의 은 과 의 분비를 억제하는 것으로 알려져 있는데 이 수용체 길항작용을 통해서 이들 을 억제시킴으로써 전두엽에서 및 분비가 증가되어 항우울 효과를 가져오는 것으로 생각된다 결론적으로 의 항우울 효과는 계와 수용체를 포함한 단가아민계에 대한 복합적인 작용을 통해서 설명되며 이에 대한 보다 자세한 내용은 아래의 전임상 연구 결과들에서 추가로 논의될 예정이다 N H (preclinical studies) Agomelatine MeO Fig. 1. N H O 일주기리듬관련연구동물 모델 연구들에서 은 주행성 야행성 동물 모두에서 일주기 리듬의 재동기화를 촉진하는 것으로 나타났다 다만 일주기 리듬의 재동기화는 의 기능이 정상적인 경우에만 나타났다 과 그리고 의 급성 스트레스 및 위상 변위에 대한 효과를 비교하기 위하여 등 은 수용체 기능을 저하시킨 유전자 삽입 생쥐를 이용하여 연구를 시행하였다 이 연구에서 은 과 비슷 2 대한정신약물학회지 2014;25:1-10

3 우영섭등 한 항우울 효과를 나타내었고 보다 우월한 위상 변위 회복 효과를 보여주었다 의 위상 변위 회복 효과는 위상 변위가 일어나기 전 주 이전에 투여했던 경우에 보다 두드러졌다 을 투여했을 때 - 의 농도 및 과는 아무런 관련이 없는 것으로 나타나 - 의 작용이 의 기능 변화와는 관련이 없는 것으로 보여진다 동물우울증모델연구강제 수영 학습된 무기력 만성적인 경도 스트레스 등 다양한 동물 모델에서 의 항우울 효과에 대한 연구들이 진행되었고 대부분의 연구들에서 항우울 효과가 입증되었다 의 효과를 서로 비교한 만성 경도 스트레스 모델 연구 에서 - 은 에 상응하는 항우울 효과를 보여주었고 이들 약제보다 유의하게 빠른 항우울 효과를 나타냈다 하지만 은 항우울 효과를 나타내지 않았다 수용체 길항제에 의해서 의 항우울 효과가 부분적으로 억제된다는 사실에서 의 항우울 효과는 계에 대한 작용만으로는 설명되지 않음을 시사해주었다 학습된 무기력 우울증 동물 모델에서 - 길항제 과 의 효과를 비교한 연구 에서 을 투여한 쥐에서는 - 을 투여한 쥐와 비슷한 정도의 회피 학습의 장애가 호전되었지만 을 투여하지 않은 쥐에서는 회피 학습이 일어나지 않았다 하지만 수용체 길항제를 전처치하고 을 투여한 결과 회피 학습의 호전 효과가 관찰되지 않았다 한편 길항제나 을 단독 투여했을 때에는 쥐의 회피 학습에 아무런 영향을 미치지 못하였다 위의 결과들을 종합해보면 의 항우울 효과는 계에 대한 작용만으로는 설명되지 않으며 에 대한 길용작용만으로도 충분하지 않음을 시사한다 강제 수영 시험 이하 우울증 동물 모델 연구 에서는 을 급성 혹은 반복적으로 투여했을 때 에서 쥐의 비활동 시간이 감소하는 것으로 나타났고 이런 경향은 사전에 수용체 작용제 및 수용체 길항제를 전처치 한 후에 강화되는 것을 확인할 수 있었다 이러한 결과를 토대로 수용체에 대한 작용이 의 항우울 효과에 일부 관여함을 보여주었다 신경전달물질관련연구 은 상행성 단가아민계에 영향을 주는데 수용체에 의해 억제성 긴장성 통제를 받는다 설치류에 항우울제를 투여했을 때 전두엽에서 - 과 의 세포 외 농도가 증가된다 등 의 연구에서 쥐에게 을 투여했을 때 전두엽에서 용량 의존적으로 농도가 증가하였다 하지만 농도의 증가는 이나 - 에서는 관찰되지 않았다 또한 투여시 전두엽에서 의 농도가 증가하였으며 에서 세포 발화 도 동시에 증가되는 것이 관찰되었다 에 의한 과 - 의 농도의 증가는 수용체 길항제에 의해서 아무런 영향을 받지 않았고 투여시에는 전두엽에서 과 의 농도의 변화가 관찰되지 않아 이상의 결과를 토대로 - 에 대한 수용체 차단에 의한 효과인 것으로 생각된다 신경가소성관련연구 의 신경가소성과 항우울 효과에 대한 연구에서 쥐에게 만성적으로 을 투여한 경우 - 의 에서 신경 발생 - 과 세포증식이 증가되는 것이 관찰되었다 - 가 정서 반응에 중요한 역할을 감당하므로 - 의 이러한 작용이 항우울 및 항불안 효과와 연관이 있을 것으로 시사되었다 등 의 연구에서는 - 이 쥐의 성숙 비성숙 의 비율을 증가시키고 에서 의 성장을 촉진시켜 의 성숙을 촉진하는 것으로 나타났다 이러한 의 효과는 의 농도 증가를 수반하였으나 과 - 의 농도는 변하지 않았다 한편 은 세포의 생존과 증식을 통제하는 세포 신호들 세포 외 을 활성화시켰다 하지만 과 길항제를 투여하였을 때는 다른 결과가 나타났는데 의 경우 이러한 효과들이 전혀 나타나지 않았고 길항제의 경우에는 에서 투여시와 비슷한 세포 증식이 나타났지만 세포 생존에는 아무런 영향을 주지 못하였다 이러한 결과를 토대로 은 신경 발생의 전 단계에 작용하며 신경가소성과 관련하여 에 대한 작용 및 에 대한 작용이 모두 필요하다는 3

4 Agomelatine 의효능과안전 것을 보여주었다 (clinical studies) 주요우울장애 상 연구에서 의 용량을 시험한 결과 넓은 용량 범위에서 전반적으로 안전성과 내약성에는 큰 문제가 보고되지 않았다 주요우울증 환자를 대상으로 개의 이중 맹검 위약 대조 연구가 시행되었으며 이들 연구에서 은 우울 증상의 개선에 효과적이고 부작용 발현은 비교적 적은 편이었다 등 의 연구에서는 의 과 그리고 위약의 효과를 여 명의 주요우울증 환자들에서 서로 비교하였다 을 투여했을 때에는 위약과 유의한 차이가 관찰되지 않았으나 을 를 투여했을 때에는 위약보다 유의하게 우월한 항우울 효과가 관찰되었다 반응률에서 - 투여군은 위약군은 였고 관해율은 투여군에서 위약군에서 였다 와 는 명의 주요우울증 환자를 대상으로 주간의 과 위약의 효과를 비교하였다 투여군은 위약군과 비교하여 이하 총점의 변화가 유의하게 컸다 와 는 총점이 점 이상인 주요우울증 환자를 대상으로 의 변동 용량과 위약의 효과를 서로 비교하였다 주째 위약과 투여군 간의 총점의 차이는 점이었고 반응률에 있어서는 투여군의 경우 위약군의 경우 였다 등 은 주 동안 총점 및 중 불안 정신운동지체 수면 우울감 등의 하위 척도 점수에 대한 효과를 의 고정 용량 과 위약 간에 서로 비교하였다 이 연구에서 흥미로운 점은 을 투여한 군에서 위약과 유의한 차이를 보인 하위 척도의 수가 투여군보다 많았다는 점이다 반응률은 군에서 군에서 로 위약군의 보다 유의하게 높았다 관해율은 군에서 군에서 로 위약군 와 유의한 차이가 관찰되지 않았다 의 부작용은 전반적으로 적었고 두통이 가장 흔한 부작용이었다 투여군 모두에서 수면 개시 및 수면의 질 측면에서 위약보다 유의하게 나은 효과를 보여 주었는데 특히 투여군에서 수면의 질이 더 나아지는 것으로 나타났다 하지만 등 의 연구에서는 총점이 점 이상인 주요우울증 환자에서 의 과 위약의 효과를 주간 비교하였을 때 투여군에서만 위약보다 유의하게 우월한 효과가 나타났고 투여군에서는 위약과 유의한 차이가 관찰되지 않았다 등 은 세 이상의 노인 주요우울증 환자를 대상으로 주간의 과 위약의 효과를 비교하였다 투여군에서 위약군과 비교하여 로 평가한 우울 증상의 호전이 더 유의하게 컸다 반응률은 투여군에서 위약군에서 로 유의하게 차이가 났으며 관해율은 투여군 위약군 로 양 군 간에 유의한 차이가 없었다 위약 및 활성 대조 디자인으로 주요우울증에서 시행된 연구는 개의 연구가 있다 이들 연구에서 이 위약과 비교하여 우월한 항우울 효과를 보여주었고 비교 대상인 항우울제와는 최소 동등한 효과를 보여주었다 또한 의 측면에서도 이 안전하다는 것이 밝혀졌다 와 를 주간 이중 맹검으로 투여하여 효과를 비교한 등 의 연구에서는 이 과 비교하여 일주기성 휴식 활동 주기를 주째부터 유의하게 개선시켰고 수면 잠복기 및 수면 효율에 대한 개선도 에서 우월하였다 또한 - 로 평가한 항우울 효과에서도 이 우월한 항우울 효과를 보여주었다 등 은 명의 우울증 환자에서 과 를 투여하며 성기능에 대한 영향을 서로 비교하였다 연구 결과 약물 투여 후 발생한 성기능 장애는 투여군에서 투여군에서 로 군에서 적었다 항우울 효과는 두 군에서 비슷하였으나 중도탈락률에 있어서는 투여군에서 투여군에서 으로 군이 보다 낮았다 과 활성 대조군 비교 연구는 총 개의 연구가 시행된 바 있다 과 - 의 효과를 비교한 등 의 연구에서 주간 두 치료군 사이의 총점의 변화는 비슷한 것으로 나타났다 이 연구에서 두 치료약제 간에 수면에 미치는 영향을 비교하기 위하여 - 를 이용하였다 수면의 질 수면의 유도 수면 중 각성 등의 측면에서 이 보다 우월하였고 이하 로 평가한 전반적 4 대한정신약물학회지 2014;25:1-10

(2013) 52) Agomelatine mg/day 8 weeks HAMD (p=0.013) Depression rating scale score difference: difference in final score for agomelatine-treated patients versus final score for comparator patients, HAMD-17: 17-item Hamilton Rating Scale for Depression, NS: not significant 인 호전 정도에서도 이 보다 더 나은 효과를 보여주었다 약물 부작용으로 인한 중도탈락률에 있어서도 군에서 로 나타나 군의 보다 낮았다 등 은 중증의 주요우울증 환자 총점 점 이상 점수가 점 이상 들을 대상으로 주간 과 의 효과를 서로 비교하였다 그 결과 점수의 감소는 - 군에서 군과 비교하여 유의하게 컸고 반응률에서도 투여군에서 으로 투여군의 에 비해 유의하게 높은 것으로 나타났다 등 의 연구에서 주요우울증 환자에서 과 이 수면 및 각성 상태에 미치는 영향 및 항우울 효과에 대하여 비교하였다 은 수면 유도 수면의 질 주간 상태 등의 측면에서 보다 유의하게 우월한 효과를 보였으며 항우울 효과는 서로 비슷하였다 등 은 주간의 연구를 통해 주요우울증에서 과 의 효과를 비교하였다 로 평가한 항우울 효과를 비교한 결과 은 과 비교하여 열등하지 않은 것으로 나타났다 반응률에 있어서 군은 주째 주째 였고 군은 주째 주째 로 양 군 간에 비슷한 결과를 보여주었다 또한 정서의 둔마 등 정서적 경험 측면에서는 - 이 보다 더 우월한 효과를 보여주었다 치료와 관련된 부작용은 군에서 군보다 적게 보고되어 의 더 나은 내약성을 보여주었다 우울증 증상의 심각도에 따른 의 효과를 확인하기 위하여 등 은 메타분석을 시행하였다 메타분석 결과 치료 초기 우울 증상이 심할수록 위약과 효과의 차이가 크게 나타났다 이는 다른 항우울제들 연구에서와 비슷한 결과이며 이 심한 우울 증상에도 효과적임을 보여준다 의 재발 예방 효과를 확인하기 위한 최초의 연구인 등 의 연구에서 위약군의 재발률이 매우 낮아 위약과 간에 재발 예방에 있어서 유의한 차이가 관찰되지 않았다 이후의 등 은 주간의 치료에 충분한 치료 반응을 보인 환자들을 대상으로 주간의 무작위 배정 위약 대조 연구를 진행한 결과 군의 재발률 이 위약군 보다 유의하게 낮고 재발까지의 기간도 군에서 유의하게 긴 것을 확인하였다 이와 같이 위약보다 우월한 의 재발 예방 효과는 총점이 5

6 Agomelatine 의효능과안전 점 이상인 환자들을 따로 분석한 결과에서도 관찰되었다 이 주요우울증 환자들에서 동반된 불안 증상을 줄이는 데 효과적이라는 연구들이 있었다 과 의 효과를 비교한 연구 에서 이하 로 평가한 불안 증상의 호전이 - 과 비교하여 에서 우월한 것으로 보고되었고 과 비교 연구 에서는 두 약물 사이에 항불안 효과가 비슷한 것으로 관찰되었다 의 항우울 효과뿐만 아니라 항불안 효과가 이전 연구들에서 보고되면서 불안장애에서 의 효과를 살펴본 연구 들이 발표되기 시작하였다 불안장애 중에서도 의 효과에 대해서 가장 많은 연구가 이루어진 것은 범불안장애이다 등 은 범불안장애 환자에서 주간 - 의 효과를 위약과 비교하였으며 투여군에서 점수 감소가 위약군과 비교하여 유의하게 큰 것으로 관찰되었다 또한 두 투여군 사이의 유의한 차이는 주 시점부터 관찰되었다 등 은 개월간 범불안장애 환자들을 대상으로 재발 예방에 대한 의 효과를 확인하였다 주간의 개방 치료기 동안 임상적으로 치료 반응을 보인 환자들이 총 주간 위약 혹은 에 무작위 배정되었다 이 연구에서 투여군이 위약군과 비교하여 유의하게 더 낮은 재발률 Table 2. 7 우영섭등 을 보여 의 급성기 및 유지기 치료에 있어서의 항불안 효과가 위의 두 연구를 통해 입증된 바 있다 이외에도 강박장애 사회불안장애 광장공포증을 동반한 공황장애 에서 의 효과를 보고한 증례 보고들이 있다 형 형 양극성장애 우울삽화에서 병합요법 의 효과가 두 개방 연구 에서 보고된 바 있으나 샘플 수가 적고 개방 연구라는 점을 고려할 때 향후 이중 맹검 위약 대조 연구를 통해 양극성장애에서의 - 의 효과가 확인되어야 할 것으로 생각된다 기존의 연구 결과들을 바탕으로 은 비교적 성인에서 안전하고 내약성이 높은 것으로 나타났고 다른 기존의 항우울제에 비해서도 더 나은 내약성을 보이는 것으로 관찰되었다 기존의 연구들에서 을 투여한 집단에서 약물 부작용의 발생률은 위약 투여 집단과 비슷하였고 부작용으로 인한 치료 중단율에서도 위약과 비교하여 유의한 차이가 관찰되지 않았다 또한 사용과 관련하여 체중 활력 징후 심전도의 변화를 비롯한 어떠한 심혈관계 지표의 이상도 관찰되지 않았다 - 투여 동안 보고된 가장 흔한 약물 부작용으로 두통 오심 어지럼 피로감 등이 있었고 이나 체중 변화는 보고되지 않았다 일부 연구들에서 와 비교하여 의 투여에서 부작용이 더 흔하다고 보고되기도 하였으나 대체적으로 용량 의존적이지는 않다고 생각되고 있으며 대부분의 약물 부작용은 심각도 면에서 중등도 이하였던 것으로 보고되었다 한편 기존의 항우울제에서 관찰되는 측면에서 - 은 상대적으로 안전한 것으로 보고되고 있다 - 등 의 연구에서는 혹은 으로 주간 치료한 뒤 관해에 도달한 환자들을 대상으로 무작위 배정을 실시하여 기존 치료 약물을 지속한 군과 위약으로 변경하여 투여 받은 군으로 나누어 주간 더 관찰하였다 이 연구에서 을 유지한 군과 을 중단한 군 사이에는 의 차이가 관찰되지 않았다 하지만 의 경우에는 을 중단한 군에서 근육통 불면 오심 어지럼 등 의 증상들이 나타났고 - 점수도 증가하는 것을 관찰할 수 있었다 한편 은 와 이하 의 흔한 부작용인 성기능 장애 측면에서도 안전한 것으로 이전 연구들에서 보고되고 있다 위에서 살펴본 바와 같이 의 높은 안전성과 내약성에도 불구하고 간 질환이 있는 환자에서는 사용이 제한되며 신장 질환이 있는 경우에는 아직까지 충분한 안전성 자료가 없어서 그 사용에 주의가 필요할 것으로 생각된다 등 의 연구에서 가 증가했던 환자의 비율은 투여군에서 위약 투여군에서 투여군에서 로 나타났다 또 다른 연구 에서 투여시 가 증가했던 환자의 비율은 였다 이렇듯 투여에 있어 가장 주의해야 할 부작용 중 하나는 의 상승이다 독일에서 시행된 - 사용과 관련된 간독성에 관한 조사에서 간독성과 관련된 약물 부작용 중에서 가장 흔한 것은 무증상 간효소 수치의 상승 이었고 독성 간염 은 이 중 약 를 차지하였다 또한 사용과 관련하여 간독성 부작용을 보고한 환자들 중 여성 고령 다약제 사용 - 심혈관계 위험요인이 있는 경우가 흔하게 보고되어 이러한 특징들이 간독성 부작용 발현의 위험요인일 가능성을 보고한 바 있다 이상의 임상 결과를 토대로 을 투여 받는 모든 환자들은 간 기능 검사를 받아야 하며 치료 시작 시점 이후 약 주 주 주 시점 및 임상적으로 필요하다고 판단되는 시점에 정기적으로 간 기능 검사가 시행되어야 한다 의 수치가 정상 상한치의 배 이상으로 증가된 환자의 경우 시간 이내에 재검사를 받아야 하며 재검사시에도 수치가 정상 상한치의 배를 초과하는 경우에는 의 투여를 중단하고 - 수치가 정상으로 회복될 때까지 간 기능 검사를 정기적으로 해야 한다 하지만 투여와 관련된 - 수치의 상승은 대부분 의 투여를 중단한 경우에 정상으로 회복되었다 주요우울증 치료의 차 선택 약제인 등은 우울증의 병태생리가 단가아민계의 이상과 관련 있다는 가설에 근거한 약물들로서 그 임상적 효과는 제한적인 것으로 알려져 있다 우울증의 병태생리와 관련하여 새로운 가설들이 나오고 있으며 그 중에서 일주기 리듬의 장애가 주요우울증의 발생 기전에 중요한 역할을 한다는 증거들이 제시되면서 일주기 리듬을 정상화시키는 의 항우울 효과가 점 7

8 Agomelatine 의효능과안전 차 임상적 관심을 모으고 있다 은 수용체에 선택적 촉진작용과 세 로토닌 수용체에 길항작용을 함으로써 항우울 효과 와 일주기 리듬의 개선 효과를 보이는 것으로 생각되고 있다 기존의 여러 임상 연구들에서 주요우울증 환자의 치료에 있 어서 이 기존의 다른 항우울제들과 비교하여 최 소 동등한 항우울 효과를 가진다는 것이 밝혀졌고 심한 우울 증상과 불안 증상의 호전에도 효과적인 것으로 밝혀졌다 또 한 부작용의 측면에서 은 높은 내약성과 안전성 을 보였고 특히 성기능 장애와 의 위험성이 기존의 항우울제들보다 낮은 것으로 보고되었다 이러한 사용과 관련된 임상 자료의 축적에도 불구하고 의 작용 기전은 명확하게 밝혀져 있지 않다 이에 의 작용 기전을 규명하는 향후 연구들 이 진행되어야 할 것이며 이러한 연구들을 통해 주요우울증 의 병태생리를 좀 더 명확하게 규명할 수 있을 것으로 기대되 고 있다 또한 특수 인구 집단인 소아 및 노인 등에서의 - 의 효과 인지 기능에 미치는 의 영향 등 에 대한 연구가 보강되어야 할 것이며 계절성 기분장애 양 극성장애 우울삽화 등 일주기 리듬의 장애와 관련이 있는 다 른 정신 질환에서의 의 연구들도 보다 활발히 이 루어져야 할 것으로 보인다 중심단어 주요우울증 효과 안전성 REFERENCES 1.
