This study was undertaken lo assess the value of CT in chronic or recurrenl paranasal sinusitis and to ascertain the theory of rhinogenic origin as lhe palhophysiologic basis of sinusilis. We retrospectively a nalyzed the PNS CT findings of 108 patients with chronic or recurrent sinusitis. The CT scans were oblained in thin-section (3mm or 5mm) coronal and axial planes after conlrast medium administration. With respect lo the localion of the sinusilis. the a nlerior ethmoid sinus was involved in 98 (91 %). maxillary sinus in 93 (86%). fronta l sinus in 60 (56%). poslerior ethmoid sinus in 35 (32%). and sphenoid sinus in 32 cases (30%). CT findings of the sinusilis consisled of sinus opacificalion (74%) mucoperiosteal thickening (69%) mucosal enhancem ent (36%). air-fluid level (1 8 %) and bony wall thickening (11 %). Complications included retention cysts (29%). mucoceles (8%) and orbital cellulitis (1 %). Obstruction of the ostiomeatal unit was noted in 104 cases (96%). which were composed of inflammatory process. nasal polyps. concha bullosae. inverting papillomas. paradoxic middle turbinates. severe septal devialions and large ethmoida l bulla. Distribulion of the Pathology 대상및방법 1987 년 10월부터 1990년 8월까지고려병원에내원한만성또는재발성부비통염환자중 CT 를촬영한 108 례를대상으로하였고, 악성종양이나외상이원인이었던경우는제외하였다. 남자가 62 명 (57% ) 여자가 46 명 (43%) 이었으며, 연령분포는 5세에서 76세로평균 39세이었다. 사용한 CT 촬영기기는 GE-CT /T Quick Hilight이었으 Location Anterior ethmoid Maxillary Frontal Posterior ethmoid Sphenoid Table 2_ CT Findings of the Sinusitis No. of cases (%) 98 (91) 93 (86) 60 (56) 35 (32) 32 (30) 며 부비동부위를횡단및관상주사하였다. 횡단주사는 앙와위상태에서하안와 - 외이도선에평행하게경구개로 부터전두동상단까지 5mm 절편두께및간격으로연속주 사하였다. 관상주사는가능한환자를복와위로하여목을 과신전시킨상태에서횡단주사면에수직이되게전두동 전연에서접형동후연까지 OM unit 주위는 3mm, 타부위는 5mm 절편두께및간격으로연속주사하였다. 특히관상주 사영상을통해 OM unit 를구성하고있는각부위, 상악통구 (maxillary ostium ), 사골누두 ( ethmoidal infundibulum ), 반월열공 ( hiatus semi 1 unaris ) 및중비도 (middle meatus ) 를세밀히관찰하였다 (Fig. 1). 금기의경우를제 외한전례에서조영제주입후주사를실시하였고연조직 algorithm 을사용하였으며, 관상주사는 raw data" 를이 용하여필요한경우 OM unit 주위를골조직 algorithm 으 로재구성하였다. 걸 과 즉 Findings Sinus opacification Mucoperiosteal thickening Mucosal enhancement Air. f1 uid level Thickening of bony wall No. of cases (%) 80 (74) 75 (69) 39 (36) 19 (18) 12 (11) 가장많았고, 그다음은상악동으로 93 례 ( 86%) 이었다. 그외전두통, 후사골동, 접형동의순서이었다 (Tab l e 1)- 부비통염의 CT 소견으로는부비동혼탁이 80 례 (74% ), 골점막비후가 75 례 ( 69% ) 로가장많았으며, 그 외접악의조영증강, 공기수면상, 부비통골벽의비후가 있었다 (Ta ble 2). 부비동혼탁의경우부비동내에차있 는물질의점액성내지화농화정도에따라그음영강도 는다양하게나타났다. 합병중으로는정체낭종 ( r ete ntion cyst) 또는용종 ( pol yp ) 이 31 례 ( 29% ) 로가장많았고점액류 ( mucocele ) 가 9 CT 상에나타난병변분포상전사골동이 98 례 (93% ) 로례 ( Fig.2 ), 안와봉소염 ( orbita l cellu li tis ) 이 1 례 (F i g. 3) 2 3 Fig. 1. Messerkli. ng e r 동에의해점막섬모성정화작용의양상과비내시경 방법이발표되면서만성재발성부비동염에있어중비도 전사골봉소의중요성이더욱강조되었다 (1 3 ). 즉두접 막의접촉으로인하여정화작용에국소적인차단이있는 경우분비물의정체로인한이차감염의기회가높아져서 부비통염의근본원인이되며, 이러한점막간의접촉은중 비도와전사골봉소의좁은점막층에서가장잘발생 한다. 정해진통로, 더우기부비동내부에서의성모운동방향은항상 즉자연개구부쪽으로향하게되어있어서 (12, 13 ) 개구부및그이하의좁은통로의폐색은염증유 발의근본원인이된다. 이상과같이대부분의만성부비 동염, 특히전두동과상악동염은그원발병소가중비도 전사골봉소라는비성원인 (rhinogenic o rigin ) 설이강조 치료실패의원인을찾을수있다 (1, 12,1 3 ). 여기에서특히도움이되는관상영상은앙와위의경추 과신전상태로도얻을수있으나분비물퉁이중력에의해 OM unit 를막아서실제병변이없음에도불구하고가양 성소견이나타날수있으므로가능한복와위상태를취 하게해야한다. 또한종양성질환이의심되거나동반된 염증소견을관찰하기위해서는조영증강이목필요하다 (2, 18). Zinreich 퉁의보고에의하면만성부비동염환자의 72% 에서전사골동병소가있었는데 (1 ), 저자들의경우도 91 % 의높은빈도를보여전사골동이비성원인에의한부 비동감염의시발부위로서중요함을확인할수있었다. 만성부비동염의전형적인소견으로는부비동골벽의 비후, 골점막비후및부비동이나비강의동반된염증성 용종퉁이있고공기수연상이나점막조영증강이있는경 우에는만성부비동염에급성 염증성변화가동반되었음 을생각할수있다 (1 8,1 9 ). 저자들의경우에도골점막비 후및골벽비후소견이각각 69%, 11 % 에서나타났고, 그외급성염증의동반을의심할수있는공기수연상, 점 막조영증강, 부비동혼탁퉁의소견들이보였다. 합병증 되고있다 ( 9, ). 이상의비성원인설에서중요하게다 루고있는협소부위이자배설통로가 OM unit 이다. OM unit 는상악동구에서사골누두와반월열공을거쳐중 비도로통하는좁은통로이다 (1, 14,1 5 ). 그러나이부위 의병변은외래에서흔히사용하는전후비경검사나단 순방사선검사만으로는잘알수없다. 이 단순부비동촬 영은빠르고간단하나전사골동, 전두와 ( frontal recess ) 및비강의상 2/3 는관찰할수없으며여러 구조물들이 겹쳐서보인다는문제가있고 ( 7, 16,17), 앓은절편두께의 재래식단충촬영술 ( thin'section conventional tomogra phy ) 의경우에도 phantom artifact에의해 OM unit를포 함한작은구조물들이불명료해진다는단점이있다 (1 ) 이에비해 CT 는 OM unit 를평가할수있는가장좋은 방사선학적진단방법이다. CT 는골조직과연조직을같이볼수있다는점에서다 른영상검사보다유리하며특히관상주사는비내시경의 시야와유사하며 OM unit 관찰에많은도웅을준다 (1, 8 ). 부비동, 특히 OM unit 를검사하는데 CT 의역할로는첫 째, 이부위의정확한해부학적정보를제공하여준다. 즉전두동, 전두와, 구상돌기 ( uncinate process), 사골누 두, 사골포, sinus lateralis, 중비도, 후사골봉소, 접사항 요 (s phenoethmoidal recess), 접형통동을잘볼수 으로는크게국소점막성합병증과국부영역및두개강내합병증으로대별해볼수있으며, 전자에속하는것으로는정체낭종, 염증성용종, 점액류, 농류둥이있고, 후자에는안와봉소염, 농양, 경막하축농 (subd ur a l empy. ema), 대뇌농양, 뇌막염, 정맥동전색, 골수염퉁이포함된다 ( 1 8, 20, 21 ) 저자들의경우에는정체낭종또는용종, 점액류, 안와봉소염의합병증소견을보였다. 정체낭종은부비동내한개의점액선폐색이원인으로대개부비통점막쪽으로광저부 ( broad base ) 를두고내강쪽으로볼록한연조직덩어리의형태로나타나고인구 10% 정도에서임상적의의없이우연히발견된다 (1 8, 22, 23 ). 점액류는부비동구나중격으로구분된부위의폐색이후점액성분비물이차서생기는데, 타보고에따르면약 60-65% 가전두동에, 약 25-30% 가사골동에, 그리고약 10% 가상악동에나타나는것으로알려져있으나 ( 2, 24, 26 ), 저자들의경우사골동, 전두통, 상악동의순서로상이한결과를보였다. 그러나본연구는점액류의증레수자체가적으므로이결과가가지는의의는적다고본다. OM unit 페색의원인으로는크게염증성변화, 종양, 외상및해부학적변이둥이포함되며, 이중해부학적변이로는 concha bullosa, 비중격일측편위, paradoxic middle turbinate, Halle cell, 거대사골포, 구상돌기첨

5 이영욱외 : 만성및재발성부비동엽의전산화단층촬영 단 ( unci nate process ti p ) 의일측면위내지기포화퉁이 있다 (1, 9 ). 저자의경우전례의 96% 에서 OM unit 의폐 색올보였고, 그원인또한다양하였다. 또한이러한 CT 소견들은비내시경소견과도일치하였고, concha bullosa 의경우비내시경소견상중비도의국소돌출이있을때 C T 로그내부가기포화되어있거나염증성변화로농이 차있음을보여줌으로써확진이가능하였다. 이상과같이 부비동염의원인및그범위를정확히밝혀주는것이치 료방침설정에있어매우중요하다. 더우기과거 C-L 시 술을받았음에도불구하고재발한 17 명의환자중 1 5 례에 서 OM unit 의페색이있었음을생각할때치료전에정확 히그원인과병변상태를파악함으로써불필요한광범위 수술을막고실제예후또한향상시킬수있다. 비내시경술은실제육안적평가가가능하고접근가능 부위에있어서의민감도가어떤방법보다도우월하며조 직생검또한가능하다. 또한 CT 는내시경술로는접근하 기힘든부위의해부학적구조및변이그리고병변의진 행확산정도동을정확히보여준다는장점을가지고 있다. 며, 따라서이두가지방법을병용하는것이가장좋으 실제 Nass 둥은 90% 이상의높은적중률을보고한바 있다 ( 2 7). 결론적으로, CT 는만성내지재발성부비동염의경우 주요병소부위인 OM uni t 의해부학적상태를관찰하고 선행원인및범위, 한진단과적절한치료에 현재진행상태둥을밝혀줌으로써정확 해주는영상진단방법이라고사료된다. 있어매우유용한정보를제공 참고문헌 of the paranasa l sinuses.
