Recently, we experienced a 40-year-old male patient diagnosed with thyroid storm with severe respiratory failure and lower extremity weakness. At first, he was diagnosed with Graves disease and critical illness neuropathy. His thyroid function was improved by anti-thyroid drug, but his neurologic function aggravated at the same time. He accordingly diagnosed with myasthenia gravis. We studied that when we should examine the neurologic function from this case. Our case highlights the importance of investigating muscle weakness or other neurologic problems in the thyrotoxic patient during their management. Key Words: Graves, Myasthenia gravis, Neuropathy 서론 중증근무력증환자의대략 5-10% 는자가면역성갑상선질환에의해기능항진증이발생하는데, 이중그레이브스갑상선염에의한경우가약 0.2% 미만을차지한다. 1) 그레이브스갑상선염과중증근무력증의동반은보통아시아지역과 50세미만여성에서관찰되었는데, 2) McEachern 등 3) 은이두질환의경과사이에시소현상 (see-saw relationship) 이있다고제안했다. 최근, 저자들은중증그레이브스병에의한갑상선기능항진증환자가중환자실에입원하여항갑상선제치료로호전되던중, 기계호흡이탈의어려움과지속적인하지위약감, 안검하수및삼킴장애가발생하여중증근무 력증으로뒤늦게진단된 40세남자환자를경험하여, 과연언제이러한환자에서신경학적기능을평가해야하는지고찰해보고자한다. 증례 40세남자가호흡곤란과의식저하소견으로응급실에내원하였다. 환자는과거력상당뇨, 고혈압, 간염및기타자가면역질환등특이기왕력이없었다. 신체검사상혈압 120/80 mmhg, 맥박수 210회 / 분 ( 심방세동 ), 호흡수 24회 / 분, 체온 38.5 o C이었고, 시진상목부위의갑상선비대소견이관찰되었다. 환자의의식상태는 Glasgow Coma Scale 상 3점으로 deep coma상태였다. 의식상태의저하로다른신경학적검사는시행하지못 Received January 8, 2014 / Revised 1st March 1, 2014, 2nd March 24, 2014 / Accepted April 1, 2014 Correspondence: Heung Yong Jin, MD, PhD, Division of Endocrinology and Metabolism, Department of Internal Medicine, Chonbuk National University Medical School, 20 Gungi-ro, Deokjin-gu, Jeonju , Korea Tel: , Fax: , mdjinhy@jbnu.ac.kr Copyright c 2014, the Korean Thyroid Association. Changes of neurologic function by his thyroid function Neurologic examinations Admission Hospital day: 10 day Hospital day: 5 weeks At first day (FT4> pmol/l) At weaning of ventilator (FT pmol/l) At aggravation of MG (FT pmol/l) Hospital day: 2 months At improvement of MG (FT pmol/l) Mental status Cranial nerve function Motor nerve Sensory nerve DTR Stupor Abnormality of eve movement (1grade), difficulty of swallowing Lower extremity weakness (IV) Lower extremity weakness (loss of DTR) Abnormality of eve movement (2 grade), difficulty of swallowing Lower extremity weakness (IV) Lower extremity weakness (loss of DTR and ankle jerk) DTR: deep tendon reflex, FT4: free T4, MG: myasthenia gravis 하였다 (Table 1). 흉부사진상심비대및양측흉수가있었고, 동맥혈액검사상 PaO mmhg, PaCO mmhg, ph 7.1, 산소포화도 91.5% 소견으로, (A-a) DO2 가 20.6 mmhg로증가되어있었다. 갑상선관련검사상 free T4>154.8 pmol/l ( 참고치 : ), TSH<0.01 μu/ml ( 참고치 : ), TSH-receptor antibody 40.0 IU/L ( 참고치 : ), anti-thyroglobulin antibody of IU/mL ( 참고치 : 0-115), anti-microsomal antibody 600 IU/mL ( 참고치 : 0-34) 이었다. 전해질검사상 Na 139 mmol/l ( 참고치 : ), K 3.8 mmol/l ( 참고치 : ) 이었다. 초음파상양측갑상선비대와실질의불균일에코및혈류량증가소견이보였으며, 갑상선스캔검사상양엽에증가된 uptake가관찰되었다. 환자는갑상선중독증점수 4) 상 125점 (45점이상시중증갑상선중독증 ) 으로증가되어있었고, 중환자실에입원하여치료를받게되었다. 신경학적이상및폐부종, 흉수에의한저환기증과환기 / 관류불균형에의한호흡부전그리고갑상선중독증이초기환자의의식장애를일으킨것으로생각되었다. 이에기도삽관및기계호흡 (assisted control mechanical ventilation) 이이루어지고, 고용량의 propylthiouracil (PTU 1200 mg) 과스테로이드, 베타차단제및루골용액으로항갑상선약물치료를시작하였다. 환자의 aspartate aminotransferase (AST) 와 alanine aminotransferase (ALT) 수치가내원시 34 IU/L와 52 IU/L이었으나, 입원후 2일째각각 1250 IU/L과 2269 IU/L로증가하였으며, total bilirubin 또한 3.2 mg/dl에서 7.3 mg/dl으로증가되었다. 내원시보다악화된환자의간기능은갑상선중독증및간울혈에의한소견으로생각되어서항갑상선약물치료 (PTU) 를지속하였고그결과, free T4 수치는 89 pmol/l까지감 소하였고, AST/ALT 수치는 22/55 IU/L, total bilirubin 은 2.0 mg/dl까지감소하면서환자의전반적인심폐기능과의식도호전을보였다. 흉부단순촬영사진에서도폐실질의이상은관찰되지않고, 맥박수도 80회 / 분으로안정화되었다. 이렇게갑상선기능의안정화 ( 입원 10일 ) 와전신상태의호전에도불구하고, 환자는여전히기계호흡이탈에어려움이있었고, 삼킴장애와하지위약감이유지되었다. 이에우리는신경학적이상에대해재평가를하였고, 신경전도검사 (NCS), 근전도 (EMG) 와 acetylcholine receptor antibody (Ach R Ab) 등관련혈액검사를시행하였다. 신경학적검사상의식상태와감각은양호하나, 안구운동장애 (1단계) 와하지근력의약화 (IV), 하지발목반사의소실이관찰되었다 (Table 1). Ach R 항체는 4.8 nmol/l ( 참고치 : nmol/l) 로약간증가되었지만, neostigmine 검사는음성소견이었고, EMG와 NCS상축삭타입의다발성신경병증소견이관찰되었다. 이러한검사소견과환자의상태를고려할때, 환자의신경기능이상은 critical illness neuropathy로사료되었다. 2주후기관절개술과 synchronized intermittent mandatory ventilation으로기계호흡보조를하였고, 항갑상선약물치료를유지하면서 free T4 수치는 51.3 pmol/l로감소하였고, 이후환자는기계호흡을멈추고일반병실로전동할수있었다. 3주후 free T4 수치는 34.0 pmol/l까지감소하고갑상선기능항진증도호전추세를보여환자는항갑상선약제를 PTU에서 methimazole로변경하여투여하였고, 이전투여하던스테로이드는감량하여투약을중지하였다. 입원 5주후 free T4 수치는 21.0 pmol/l까지감소하였고환자의의식상태및감각도양호하나, 환자는지속적으로오른쪽안검하수및복시증, 삼킴장애, 하 Vol. 7, No. 2,

stimulation. 지위약감을호소하였고, 특히나안검하수증상은저녁에악화되는경과를보였다 (Table 1).

These findings demonstrate a significant decrement in the muscle action potential amplitude of about 30.2% at the abductor digiti minimi muscle and 15.6% at orbicularis oculi muscle (positive test result: more than 15% decrement) at low/high frequency stimulation. 지위약감을호소하였고, 특히나안검하수증상은저녁에악화되는경과를보였다 (Table 1). 신경학적검사상무릎반사는관찰되지않았고, 하지의근육파워도 IV로상지 (V) 에비해약화되어있었다. Ach R 항체는 2.3 nmol/l로여전히약간높은수치를보였으며, EMG와 NCS상퇴행성의운동신경병증이관찰되었다. 다시시행한 neostigmine 검사상 (Fig. 1) 양성소견을보였고, ice 검사에서도양성소견을보였으며, repetitive neurostimulation (RNS) 검사 (Fig. 2) 또한신경근접합부의전도이상소견을보였다. Visual fluoroscopy swallowing study (VFSS) 상구강기의심한지연으로위에음식물의잔류량이유지되었고, 흉부전산화단층촬영상비대된흉선이관찰되었다. 이러한결과를토대로환자는비대된흉선이있는중증근무력증으로진단되었고, 고용량 187 Clin Exp Thyroidol

4 Sun Hee Kim, et al 의스테로이드와 pyridostigmine으로치료한후, 이전의신경학적이상소견들 ( 안구운동장애및삼킴장애, 하지운동장애 ) 이호전되었다 (Table 1). 퇴원후환자는 methimazole (2.5 mg), 유지용량의 steroid (10 mg), pyridostigmine (240 mg), cyclosporine (50 mg) 을투약하면서, 정상갑상선기능과신경학적소견을유지하면서경과관찰중에있다. 고찰 중증근무력증은자가면역기전에의해보통자가면역성갑상선질환과종종동반되는것으로알려져있다. 그중그레이브스갑상선염과하시모토갑상선염이보통중증근무력증과잘동반된다. 5) 이를테면 TSH와 acetylcholine receptor에대한자가항체간에관련성이이두질환의임상양상에영향을주는것으로사료된다. 6) 기존의중증근무력증과그레이브스갑상선염이동반된사례들은보통가벼운임상양상과안구침범, 흉선질환의낮은발생, acetylcholine receptor 항체양성과관계되어있었다. 7) 그런데이러한두질환이동반된경우, 신경병증발생시임상에서감별하기가쉽지않다. 그렇지만임상상황에맞추어적절한검사를통해서두질환을감별하여치료하는것이향후환자의좋은임상경과를가져오는데있어필요하다. 이전에보고된문헌들에따르면, 먼저임상양상면에서중증근무력증의경우신경병증의임상양상이일중변화를보이는데비해, 갑상선기능항진증에의한근육병증의경우에는진전및무도병양상, 전해질변화에따른주기성마비형태로증상이나타나며, 갑상선기능이호전될경우이러한증상들이호전되는경과를취하게된다. 또한, 두질환이동시에발생하는경우, 각질환이병의경과도중서로에게영향을주는현상도나타난다. 이를테면갑상선질환의호전은중증근무력증의악화를가져오기도한다. 이러한현상을소위시소현상 (see-saw relationship) 이라고한다. 3) 그래서일부에서는갑상선이정상기능으로회복되는데비해, 중증근무력증은악화되어 crisis까지진행된경우도있다. 이러한시소현상이갑상선기능의변화이외에, 항갑상선약제인 methimazole 치료이후에발생한경우도있었다. 6) Baretic 등 6) 은 methimazole이일부 T 림프구클론에영향을주고, 이것이 immune self-tolerances를유발하여이러한현상이발생할수있다고하였다. 우리증례는초기신경학적이상소견은 critical illness neuropathy에의해발생된것으로생각되었다. 실제임상에서 critical illness neuropathy와갑상선기능항진증에의한근육병증, 중증근무력증을구분하기는쉽지않다. 특히 critical illness neuropathy의경우임상양상이다양하게나타나며, 환자의질환이중환자실에입원할정도의중한상태에서호전되면그진단이확진되는경우가많아서특히그러하다. 우리환자의경우, 갑상선기능이호전되면서뒤늦게중증근무력증이진단된경우로, 시소현상에의해질환의진단이늦어진사례이기도하여, 갑상선기능항진증치료과정에서과연언제신경기능이상에대한평가를시행해야하는가고민하게한다. 먼저이증례에서처럼갑상선기능이안정화되는시점에신경학적변화를유의하여살펴볼필요가있다. 치료경과중신경기능에영향을줄수있는약제 ( 스테로이드등 ) 의투여및용량이변화되는시점도고려해야한다. 또한, 신경학적기능의임상양상이변화되는시점도고려해야한다. 이를테면, 안검하수가오후에더심해진다는증상들이그러하다. 일반화하기는이르지만항갑상선약제의변경도유의할필요가있다. 아직까지어떠한항갑상선약제가중증근무력증이동반된갑상선기능항진증환자에적합한지에대해서는정립되지는않았다. 단지 methimazole 사용후중증근무력증이악화된한증례만보고되었을뿐이다. 6) 따라서추후이두질환이시소현상과관련하여신경병증과어떠한관계가있는지정확한기전과영향을줄수있는인자들에대한연구가더필요할것으로보인다. 아직까지어떠한문헌에서도중증의그레이브스병에동반된중증근무력증환자에서신경학적변화를정확히알수있는검사항목과시기에대한가이드라인은부족한편이다. 단지이와유사하게몇가지케이스들만보고되었고, 두질환이동시에의심되는상황에서유의할필요가있다고제시하고있다. 1,8-10) 그렇지만이증례를토대로환자증상의일중변동성과치료경과의변화시점을토대로갑상선기능항진증환자에서특히중증근무력증이의심되는경우에는신경학적증상에대해 Ach R 항체검사 (ocular type MG의경우민감도는 45-60% 이나 generalized type MG의경우 90% 의민감도를보임 ) 와 EMG, NCS, tensilon 검사 (ocular type MG의경우 86% 의민감도, generalized type MG의경우 95% 의민감도 ) 및 neostigmine 검사, RNS 검사 (ocular type MG 의경우 48% 의민감도, generalized type MG의경우 ocular type MG보다높은민감도 ), ice 검사 (ocular type MG의경우 95% 민감도 ), single fiber EMG (ocular type MG의경우 95% 민감도, generalized type MG의경우 Vol. 7, No. 2,

5 A Severe Thyrotoxicosis of Graves Disease Presenting with Myasthenic Crisis 100% 민감도 11) ) 등을시행할것을권유해볼수있겠다. 또한, 추후이에대한예측인자를비롯한많은대규모연구들이필요할것으로보인다. 요약 갑상선기능항진증과중증근무력증은자가면역질환으로심심치않게동반되어보고된다. 그런데이두질환이동반된경우, 신경병증이발생하였을때두질환을적절히진단하고치료하는것이쉽지않다. 이에저자들은 40세남성증례를통해과연언제신경학적기능을평가해야할지고민해보았고, 이를통해질환의적절한진단을놓치지말아야겠다. 중심단어 : 그레이브스병, 중증근무력증, 신경병증.
