Diagnosis of MG is established by characteristic clinical symptoms, and serologic, electrophysiologic and pharmacologic tests. The clinical course and prognosis of MG may be different according to its classification. This study aimed to investigate the sensitivity of diagnostic tests for MG and to identify differences in clinical course and prognosis according to classification. Method: Medical records of 53 patients with MG who were followed for at least 12 months were retrospectively reviewed. Patients were classified based on age of onset, clinical symptoms and thymoma. We investigated clinical features and outcomes by MGFA classification and post-intervention status. Result: The neostigmine test was the most sensitive for diagnosis of MG and the sensitivity of all tests (anti-acetylcholine receptor antibody, repetitive nerve stimulation and neostigmine test) were higher in patients with generalized MG than patients with ocular MG. Anti-acetylcholine receptor antibody levels were significantly higher in late-onset MG than early-onset MG, and in generalized MG than in ocular MG. Clinical stage at maximal severity and post-intervention status was not different among classification groups. Conclusion: More caution is recommended for diagnosis of ocular MG because the sensitivity of tests and titers of antiacetylcholine antibody are lower in patients with ocular MG than in patients with generalized MG. Although the number of patients was small, prognosis was not different in MG groups classified based on age of onset, clinical symptoms and thymoma in this study. Key Words: Myasthenia gravis, Prognosis, Classification 서론 중증근무력증은반복되는근력약화와피로가특징이며신경 근육이음부의근육막구성요소에대한항체에의해발생하는 책임저자 : 조정희 경기도고양시일산동구일산로 100 국민건강보험일산병원신경과전화: (031) , 팩스: jhcho.neuro@gmail.com * 본연구는국민건강보험일산병원의연구비지원으로이루어졌음 (NHIMC 2017CR010). 자가면역질환이다. 1,2 아세틸콜린수용체에대한항체가가장대 표적이지만, Muscle-Specific Kinase(MuSK), Lipoprotein- Related Protein 4(LRP4), agrin 등다른표적에대한항체들도 밝혀졌다. 3 중증근무력증은특징적인임상증상, 혈청학적검사, 전기생리학적검사및약리학적검사로진단한다. 4 중증근무력증은임상증상에따라안구형과전신형, 발병연령 에따라조기발병과후기발병, 그리고발견되는항체에따라분류 할수있고이러한분류에따라예후와치료에대한반응이다를 수있다. 5-8 최근에는새로운면역억제제및흉선절제술로중증근 무력증환자가치료목표에도달하는것이더용이해졌다. 9 이연구의목적은중증근무력증의진단검사들의민감도를조 사하고, 분류에따라임상경과와예후의차이를확인하는것이다. Clinical Analysis of Patients with Myasthenia Gravis 1. 대상 대상및방법 본연구는후향적연구로 2005 년 1월 1일부터 2016 년 12월 31 일까지국민건강보험일산병원에내원한중증근무력증환자를 대상으로하였다. 특징적인중증근무력증증상이있으면서아세 틸콜린수용체항체검사, 반복신경자극검사, 또는네오스티그민검 사중한가지이상에서중증근무력증에합당한소견이관찰될때 중증근무력증으로진단하였다. 반복신경자극검사에서복합근육 활동전위가 10% 이상감소할때양성으로판정하고네오스티그 민검사는네오스티그민근육주사후임상양상이뚜렷하게호전되 었을때양성으로판정한다. 모든환자는흉부 CT를시행하여흉선종의유무를확인하였 다. 중증근무력증진단후 1년이상추적관찰한환자의의무기록 을분석하였다. 본연구는국민건강보험일산병원기관생명윤리 위원회에서검토및승인되었다. 2. 환자군의분류 발병연령에따라 50 세미만의조기발병군 (early-onset myasthenia gravis, EOMG) 과 50 세이상의만기발병군 (late-onset myasthenia gravis, LOMG) 으로, 임상증상에따라안구형과전 신형으로, 흉선종의유무에따라환자군을분류하였다. 3. 예후척도 질병의중증도는가장악화되었을시기의 Myasthenia Gravis Foundation of America(MGFA) 등급으로측정하였다. 10 치 료에대한반응은마지막내원시 MGFA 중재후상태(post- intervention status) 에의거하여 complete stable remission (CSR), pharmacologic remission(pr), minimal manifesta- tion(mm) 로분류하였다. 10 CSR은치료를받지않고도 1년이 상중증근무력증증상이나징후가없는상태이고 PR은콜린에 스테라제억제제를제외한치료를받으면서중증근무력증증상 이나징후가없는상태이고 MM은근력은약하지만기능적인제 한은없는상태이다. 4. 통계분석 통계분석은 SPSS 23.0을이용하였고 p 값이 0.05 미만인것 을통계학적으로유의하다고판단하였다. 발병연령, 임상증상, 흉 선종의유무에따라환자군을분류하고임상경과의차이를카이 제곱이나피셔의정확도검정을이용하여비교분석하였다. 환자 군의분류에따른아세틸콜린수용체항체수치의비교는 t 검정을 이용하였다. 1. 인구통계학적구성 결과 53명의중증근무력증환자중남자는 25 명 (47.2%), 여자는 28 명(52.8%) 으로남녀비는 1 : 1.12 였다. 평균발병연령은 44세 (8 개월~84 세) 이고발병연령은 세가가장많았다 (Fig. 1). 평균추적기간은 71.4(12-201) 개월이었다. EOMG 와 LOMG 에 서남녀차이는없었다 (p =0.386) (Table 1). 중증근무력증환자중 35 명(66.0%) 은 EOMG, 18 명(34.0%) 은 LOMG 이고안구형은 30 명(56.6%), 전신형은 23 명(43.4%) 이고흉선종은 10 명(18.9%) 에서동반되었다. 가장악화되었을때 MGFA 등급은 I군이 30 명(56.6%), II군이 14 명(26.4%), III군이 8 명(15.1%), IV군이 1 명(1.9%) 이었다. 마지막내원시환자의 MGFA 중재후상태는 CSR 9 명(17.0%), PR 6 명(11.3%), MM 36 명(67.9%) 이었다 (Table 2). 2 명이추적관찰중사망하였는데, Number of patients Onset age (years) Female Male Fig. 1. 3 조정희. 한명은갑상선암, 당뇨병이동반된 82세남자로전신형중증근 무력증으로스테로이드복용중전신근력약화, 식욕저하등으로 입원하였고급성심장사로사망하였다. 다른한명은 45세남자로 전신형중증근무력증으로스테로이드복용중뇌농양으로와병 상태(bed-ridden state) 로욕창이심했고흉선암재발및흉막전 이로사망하였다. 2. 중증근무력증진단검사 아세틸콜린수용체항체검사는모든환자에서시행하였고 39명 (73.6%) 이양성, 14 명(26.4%) 이음성이었다. 반복신경자극검 사는 49명에서시행하였고 37 명(75.5%) 이양성, 12 명(24.5%) 이음성이었다. 네오스티그민검사는 44명에서시행하였고 39명 (88.6%) 이양성, 5 명(11.4%) 이음성이었다 (Table 3). 중증근무 력증의진단에대한민감도는네오스티그민검사가가장높고반 복신경자극검사, 아세틸콜린수용체항체검사순이었다. 아세틸콜 린수용체항체검사, 반복신경자극검사, 네오스티그민검사모두안 구형에비해전신형에서의양성률이높았다 (Table 3). 3. 발병연령에따른임상경과 아세틸콜린수용체항체검사는 EOMG 환자 27 명 (77.1%), LOMG 환자 12 명(66.7%) 에서양성으로검사의민감도는차이가 없었다 (p=0.413). 그러나항체가양성인환자들의항체역가는 LOMG 환자군이 EOMG 환자군에비해유의하게높았다 (11.11 vs 6.90 nmol/l, p=0.046). EOMG 환자군과 LOMG 환자군사 이에가장악화되었을때의 MGFA 등급, 안구형과전신형의빈 도, 흉선종의유무, MGFA 중재후상태의차이는관찰되지않았 다 (Table 4). 4. 안구형과전신형에따른임상경과 아세틸콜린수용체항체검사는안구형 20 명(66.7%), 전신형 19 명(82.6%) 에서양성으로검사의민감도는차이가없었다 (p =0.192). 그러나항체가양성인환자들의항체역가는전신형에서안구형 에비해유의하게높았다 (11.42 vs 5.13 nmol/l, p =0.001). 안 구형과전신형사이에흉선종의유무, MGFA 중재후상태의차 이는관찰되지않았다 (Table 5). 5. 흉선종유무에따른임상경과 흉선종의유무에따른그룹간의비교에서아세틸콜린수용체항 Table 2. Clinical features of 53 patients with myasthenia gravis 체검사의민감도및항체의역가, 가장악화되었을때의 MGFA 등 급, MGFA 중재후상태의차이는관찰되지않았다 (Table 6). 6. 치료 중증근무력증환자에대한치료로피리도스티그민은 28명 (52.8%), 스테로이드는 23 명(43.4%) 에서사용되었다. 스테로이 드외사용된면역억제제는아자티오프린이가장흔했고그외 시클로스포린, mycophenolate mofetil, tacrolimus 가있다 (Table 7). Treatments provided for myasthenia gravis Treatment at the last visit 고찰 중증근무력증은모든연령에서발생하지만, 대체적으로두봉 우리분포 (bimodal distribution) 를보인다. 기존에는발병연령 이 대에는여자가더많고 대에는남자가더많다고 알려졌으나 대에여자가더많고 대에남자가더많 다는연구도있다. 2 50세이전발병에서는여자가더많지만 50 세이후발병에서는남녀비의차이가없다는연구도있다. 3 본연 구에서남녀비는 1 : 1.12였고 EOMG 와 LOMG 에서남녀차이는 없었다 (p=0.386). 그리고중증근무력증의발병연령은 세 가가장많았다. 중증근무력증의진단에대한검사의민감도는임상양상에 따라차이가있는데텐실론검사는안구형에서 60-95%, 전신형 에서 (n=53) No treatment 10 (18.9%) Pyridostigmine 15 (28.3%) Prednisolone 8 (15.1%) Prednisolone + pyridostigmine 5 (9.4%) Other immunotherapy 4 (7.5%) Other immunotherapy + pyridostigmine 1 (1.9%) Prednisolone + other immunotherapy 3 (5.7%) Prednisolone + other immunotherapy + pyridostigmine Other immunotherapy: azathioprine, cyclosporine, mycophenolate mofetil, tacrolimus 7 (13.2%) % 이고, 아세틸콜린수용체항체검사는안구형에서 50-75%, 전신형에서 70-95% 이며, 반복신경자극검사는안구형 에서 10-17% 이고, 전신형에서 % 이다. 4 전신형중증근무 력증에서아세틸콜린수용체항체가음성일경우 MuSK 항체검사 를시행하고반복신경자극검사가음성일경우단일섬유근전도를 시행하도록권고되고있다. 9 본연구에서는중증근무력증의진단에 대한민감도는네오스티그민검사가가장높고반복신경자극검사, 아세틸콜린수용체항체검사순으로, 민감도는네오스티그민검사는 안구형에서 83.3%, 전신형에서 95.0% 이고, 반복신경자극검사는 안구형에서 61.5% 이고, 전신형에서 91.3% 이고, 아세틸콜린수용 체항체검사는안구형에서 66.7%, 전신형에서 82.6% 였다. 그리고 아세틸콜린수용체항체검사는안구형과전신형에서진단의민감 도는차이가없었으나항체가양성인환자들의항체역가는전신 형에서안구형에비해유의하게높았다 (11.42 vs 5.13 nmol/l, p =0.001). 기존의연구와검사들의민감도는비슷하였다. 중증근무력증의발병연령은 세가가장많았고남녀비의 차이는없었다. 중증근무력증의진단에대한민감도는네오스티 그민검사가가장높고반복신경자극검사, 아세틸콜린수용체항체 검사순이고모든검사는안구형에비해전신형에서의양성률이 높았다. 환자수가작은한계가있지만본연구에서 EOMG 와 LOMG, 안구형과전신형, 흉선종의유무에따른예후의차이는 관찰되지않았다.
